Peculiarities of epidemiology, etiology and clinical picture of allergy disease in different regions of the Russian Federation taking into account changing environmental factors.

AIMS: To study the peculiarities of epidemiology, etiology and clinical course of allergic diseases in different regions of the Russian Federation. MATERIALS AND METHODS: To achieve this goal, a questionnaire survey of chief specialists was conducted in 2019. Questionnaires were sent to all regional, republican centers and cities of federal significance. The received data were analyzed and structured. RESULTS: Responses to the questionnaires were received from 49 regions of the Russian Federation, where a total of 1468105 patients with allergic diseases were registered during the study period. Significant discrepancies in the structure of allergic diseases and the spectrum of etiologically significant allergens were revealed in different regions of the Russian Federation, reflecting their climatogeographical peculiarities. There is an increase in respiratory forms of allergy, multysensitization and multi-organ allergic lesions. Low availability of specialized care and insufficient prescription of pathogenetic allergen-specific immunotherapy (ASIT) were found in all federal districts of the Russian Federation. CONCLUSIONS: Different climatogeographic regions of the Russian Federation differ from each other in the structure of allergic diseases and sensitization spectrum, which should be taken into account in the development of preventive, diagnostic and therapeutic measures in these territories.

Автоматизация процесса разработки метеорологических моделей

Introduction. The chief specialist in working with information resources and geoinformation systems (GIS) of the crisis management centers of EMERCOM of Russia for managing forces and means in responding to natural and man-made crisis situations or their threat develops models of the situation and sends them to cooperating organizations that are part of the Unified state emergency prevention and response system situations (RSChS). With the simultaneous occurrence of several crisis situations of different nature, the time for effective management may be missed. Along with other crisis situations, a complex of adverse meteorological phenomena, is a threat of emergency situations. For successful and timely processing of meteorological information to be provided to responding units, it is necessary to constantly improve the methods of working with it. A software application can automate routine actions and reduce the processing time of weather data, the use of which will minimize the threat of an emergency situation. Goals and objectives. The purpose of the study is to model the process of preparing documentation for probable scenarios of the development of the situation in connection with adverse meteorological conditions, which helps to reduce labor costs and increase the validity of decisions made by officials managing forces and means in crisis situations. To achieve this goal, the following tasks have been solved: the analysis of data, methods and tools that are used in the daily activities of the chief specialist in working with information resources and GIS technologies has been carried out; the existing problems were identified; a computer software application was developed; a study of the developed software application using meteorological data for the Irkutsk region for 2023 was conducted. Methods. To achieve the goal and solve the tasks of the study, methods of system analysis and data systematization were used. Results and discussions. An algorithm for collecting meteorological data and a functional model of the software application is proposed, which made it possible to automate the process of developing models of the development of the meteorological situation for interacting organizations that are part of Unified state emergency prevention and response system situations (RSChS). This allows you to prepare documentation in the shortest possible time, carry out preventive measures, and also contributes to the adoption of operational management decisions in crisis situations. Conclusion. A software application was developed in the object-oriented programming language Visual Basic using the "Atlas of Hazards and Risks" system. Using meteorological data for the Irkutsk region for 2023 as an example, the effectiveness of its use is shown. Scientific Laboratory “Moscow Polyclinic”: Experience of Project Implementation

Background. In 2021, a new educational project “Scientific Laboratory ‘Moscow Polyclinic’” was launched. Its aim is to develop research competencies among primary health care workers and to boost activities such as journal publication and the organization of specialized scientific and practical conferences. The article briefly describes several characteristics and features of this project for two seasons (2021–2022). Materials and methods. The author collected, aggregated, statistically processed, and analyzed the comparative data for two project seasons. Results and discussion. The analysis included information on the project design (topic-based tracks, blocks “Transformation” and “Accelerator of Ideas”, sections “Science” and “Service Design”, Scientific Club), the results of sociological surveys and their interpretation, and the brief scientometric characteristics of participants’ publications. Between 2021 and 2022, 9818 specialists took part in the Scientific Laboratory; they were outpatient polyclinic care providers in the Moscow healthcare system. In addition, the number of speakers nearly doubled, and a community of mentors was formed from the active researchers in Moscow healthcare. Conclusions. The project speakers included leading foreign and Russian scientists, representatives of scientific journals and peer-reviewed research databases, and Moscow healthcare specialists. They shared with the participants their experience of planning, their methodology for conducting a study; algorithms for selecting a study topic, and the features of research data publication, including possible sources. The project mentors were the chief specialists of the Moscow Healthcare Department, and the heads of medical organizations experienced in carrying out the studies. As a result, the participants successfully published their articles in the peer-reviewed journals from the list of the Higher Attestation Commission (VAK). The project demonstrates that the employees of Moscow polyclinics are not only active professional physicians but also researchers with critical thinking and deep knowledge in clinical medicine.

Integrated digitalization at road transport enterprises: prospects for implementation

Modern opportunities for the introduction of digital technologies for enterprises of all industries in the Russian Federation are constrained by the fact that most software products are designed for certain areas of functional work blocks: corporate document management systems, accounting, finance and credit, personnel records management. With the displacement of management units by digital collection points, analysis, evaluation and solving problems of route optimization, cost of services, calculation of the level of logistics services, followed by the search for the most effective alternatives, the management structure is not just rationalized, it undergoes transformation, both regulations and the complete elimination of the structural management units themselves. In the future, only the chief specialist can remain from specialized management services with widespread digital support of management functions. Therefore, the most important condition for digital transformation at ATP enterprises should be integrated control systems based on automation, robotization and digitalization of most processes, the execution of which can be delegated to the plane of computerized execution on the ground. Rural areas’ factors of sustainable socio- economic development: estimates of agricultural producers

The article presents the results of an expert survey of managers, chief specialists and heads of structural divisions of agricultural enterprises of Stavropol Krai (Russia). We carried out a factor analysis of the expert assessment of socio-economic contradictions negatively affecting the development of rural areas in the South of Russia. The opinions of the expert community represented by agricultural producers are important, since agricultural enterprises are the backbone economic units of rural settlements. A brief analysis of the publications of the science-intensive Scopus database confirms the relevance of the research topic and allows us to formulate a theoretical model of the problem field affecting the sustainable socio-economic development of rural areas. In the empirical part of the study, a survey was conducted of 205 experts – representatives of agricultural enterprises of Stavropol Krai, holding the positions of managers, chief specialists, and heads of structural divisions. The obtained research results provide an information and analytical basis for the development and improvement of programs for the strategic development of rural areas of Stavropol Krai. The methodology for studying the factors of sustainable socio-economic improvement of rural areas based on the assessment of agricultural producers can be used to analyze the territorial development of rural regions of Russia.

Possibilities and prospects for the development of integrated digitalization at the motor transport enterprise

Modern opportunities for introducing digital technologies for enterprises of all industries in the Russian Federation are constrained by the fact that most software products have been developed for certain areas of functional blocks of work: corporate document management systems (with weak industry-specific support in relation to vehicles), accounting, finance and credit, personnel records management. With the displacement of management links by digital points for collecting, analyzing, evaluating and solving problems of optimizing routes, cost of services, calculating the level of logistics services with the subsequent search for the most effective alternatives, the management structure is not only rationalized, it is undergoing some transformation, both of regulations and the complete elimination of direct structural departments of management. In the future, when ubiquitous digital support for management functions only one chief specialist can remain from specialized management services. Therefore, the most important condition for digital transformation at motor transport (MT) enterprises should be integrated control systems based on automation, robotization and digitalization of most processes, which can be delegated to the plane of computerized execution in the field.

Systemic analysis of the AST results in medical organizations of the Russian Federation

Objective. To analyse aggregated AST results for key microorganisms collected through the 2022 reports of chief specialists in clinical microbiology and antimicrobial resistance. Materials and Methods. The study included an analysis of the interpretation criteria used in the laboratories and an evaluation of the AST reports. Data were obtained from the clinical microbiology and antimicrobial resistance annual reporting system. Reports were analyzed using EUCAST guidelines for expected resistance phenotypes and expected susceptible phenotypes. Data processing and analysis were realized using the «R» programming language. The 95% CI for the percentages of inaccuracies/errors distributed by federal districts was calculated using the Wilson method. Results. A combination of several interpretation standards was used in 27.78% of laboratories, MUK 4.2.1890-04 was noted as one of the options in 57.6% of laboratories. Irrelevant standards of interpretation with a lag of 1 year or more were used in a significant number of cases. The highest percentage of errors/ inaccuracies by the type «expected resistance» was observed for A. baumannii – 14,06% (N = 9163), E. faecium – 8,05% (N = 3451) and S. pneumoniae – 6,18% (N = 2779). «Susceptibility categorization in the absence of interpretive breakpoints» was highest for S. aureus – 13.24% (N = 19784) and S. pneumoniae – 8.76% (N = 3942). Rare phenotype was determined in the highest percentage in relation to S. pneumoniae and antimicrobials: vancomycin – 54.04% and linezolid – 64.6%. Conclusions. The study revealed a significant number of errors/inaccuracies in the data reported. The use of irrelevant interpretation criteria, the exclusion of situations with rare phenotypes and expected resistance, may contribute to a significant increase in the likelihood of inappropriate antibacterial prescribing.

Management of the activity of the Moscow pathology and anatomical service in the context of a pandemic of a new coronavirus infection in 2020–2022

The management of the activity of the pathology and anatomical service (PAS) of the Moscow Healthcare Department (DZM) in the context of the COVID-19 pandemic and the provision of timely and accurate pathological diagnostics, as well as the receipt of reliable statistics on mortality rate, made it possible to provide timely management decisions to combat the pandemic. Purpose. To present the successful experience of managing the activities of the Moscow Pathoanatomic Service of DZM in the conditions of a pandemic of a new coronavirus infection. Methods. The paper analyzes and summarizes the features of the activity of PAS of DZM in the conditions of the COVID-19 pandemic, presents the main activities carried out to ensure timely and accurate diagnostics in anatomical pathology, and obtaining reliable statistical data on mortality, demonstrates the most important management decisions for organizing work aimed at combating the pandemic. Analytical, organizational, and statistical research methods are used in the work. Results. In order to improve the organization of the work of PAS of DZM in the context of the COVID-19 pandemic and ensure timely and accurate pathological diagnostics, and obtaining reliable statistics on mortality from it, a number of important activities were carried out including reprofilation of the PAS and reorganization of the work of PAS of DZM, reorganization and optimization of the work of the organizational and methodological department for pathological anatomy (OMO PA) of the Research Institute for Healthcare Organization and Medical Management of Moscow Healthcare Department, carrying out activities to establish interaction between the chief specialist in pathological anatomy and OMO PA with the DZM, the Rosstat and the egistry Office in Multiple functional Centres, connection of all Departments of Pathology by the new laboratory information system «LABFORS», development of continuing medical education. Limitations. Limited time period for description; there is no comparison with other subjects; conditions of the COVID-19 epidemic. Conclusions. Thus, the PAS of DZM, under the guidance of the chief specialist in pathological anatomy of DZM and OMO PA, showed its efficiency and effectiveness in the context of a pandemic of a new coronavirus infection, successfully implemented new technologies and organizational principles, improved the organization of its work, developed both post-mortem and intravital diagnostics of diseases in anatomical pathology.

Experience of anticipatory therapy with IL-6 receptor inhibitors and perspectives for its use in the evolution of COVID-19

The Advisory Board chaired by the chief specialist in infectious diseases of the Ministry of Health of Russian Federation, Professor V.P. Chulanov was held on June 18, 2022 in Saint Petersburg. Aim. The main purpose of the Board was following discussion: the analysis of the real-world data of levilimab as an anticipatory therapy for COVID-19 in hospitalized patients; the review of the experience and perspectives of levilimab as an anticipatory anti-inflammatory option for outpatient patients who meet defined clinical and laboratory criteria. Results. The analyzed data on clinical efficacy and safety formed the basis of recommendations proposed by experts for the use of levilimab in the inpatient and outpatient medical care for COVID-19.

Features of the organization of coloproctological care to the population taking into account regional indicators of incidence

Rationale. The article provides an analysis of the features of the organization of coloproctological care for the population of the Perm Territory, taking into account regional morbidity rates, which made it possible to identify a number of significant shortcomings. There is no connection between the volume of specialized coloproctological care provided and the real need of the population. It was revealed that 32.0 % of coloproctological patients are being treated by doctors of other specialties. In 2018, 47,172 cases of coloproctological diseases were registered, and only 30,900 specialized outpatient appointments were made in the region (11.7 appointments per 1,000 population). More than half of them (57.4 %) were held in Perm.The aim of the study. Determination of the features of the organization of coloproctological care for the population of the Perm Territory, considering the incidence rates.Materials and methods. An analysis was made of the distribution among the adult population of the Perm Territory of pathologies related to the profile of coloproctology according to official statistical reports and the report of the chief specialist of the region for 2018.Results. In 2018, 3.9 sigmoidoscopies per 1,000 population were performed and 1.7 fibrocolonoscopies per 1,000 population, which led to an insufficient level of diagnosis of coloproctological pathologies. At the same time, identified for the period 2004–2018, a significant increase in the incidence of colorectal cancer (by 48.1 %), with an average annual growth rate over 15 years of 7.8 %, which is 3 times higher than the national figure, requires a change in approaches to the organization of coloproctological care, incl. for early diagnosis.Conclusion. Thus, in order to improve the quality of early diagnosis of coloproctological, and especially onco-coloproctological diseases, it is required to optimize the outpatient specialized coloproctological care in the Perm region. It is proposed to reorganize the service with the aim of organizing a primary link on the basis of an outpatient coloproctology center with a daily stay hospital, separate or at the coloproctology department of the City Clinical Hospital.

Management technology for groundwater resources in in-situ leach of weakly watered uranium reservoirs

The ore bodies in the Vitim uranium ore province are nonuniformly encroached with groundwater. The weakly watered ore-bearing areas are unsuitable for the in-situ leaching at admissible economic performances. In this connection, as a case-study of Khiagda deposit, a geo-permeation model has been constructed to identify the highly and weakly watered areas in the local ore bodies. Using a digital hydrodynamic model, the technology is developed for re-distributing groundwater between the highly and weakly watered ore-bearing sites. The groundwater management technology based on the geopermeation model in combination with the model of ore body X4 (insert model) has helped finding solutions to some specific problems of concern: the source of flooding is detected in a highly watered area at the bottom of ore body X3 treated by in-situ leaching (ISL); the gradual flooding strategy is selected for ore body X3; the volume of the flooding solutions is determined; the flooding process duration is calculated. The calculations prove economic profitability of the developed technology. The profitability is achieved owing to the scenario of series flooding of operating blocks through wells to be producing later on (the cost of the well construction is not included in the finance-and-economic model), as well as due to the enhanced productivity of operating ISL cells up to the project level and recycling of sulfuric acid and oxidizer (FeIII) taken from residual sulfuric solutions from depleted operating blocks. It is proposed to continue testing the technology after retrofitting of a testing ground with instrumentation. The authors appreciate participation of the Chief Specialist of Promtekhnologii JSC I. Residual Professional Knowledge in the System of Quality Assurance of Higher Education

The article discusses the process of accumulation and verification of residual professional knowledge of higher education students. To achieve the goals of the article: the memory system is analyzed; the legislative status of milestones and procedures for testing the knowledge of students is systematized; it is established to what extent the assessed knowledge can be understood as residual knowledge; the social causes of low residual knowledge are investigated. The main idea of the article: the effective accumulation and testing of residual professional knowledge in the system of quality assurance of higher education and, in general, the approximation of the content of vocational education to the requirements of production are most successfully able to be provided by teachers with at least three to five years of professional experience in industry organizations and the level of achieved qualifications that meet the qualification requirements for the positions of chief specialists.

Expert-analytical Support of Foreign Policy Activities

The article presents the final results of the project studying the system of expert and analytical support for foreign policy decisions. The project is devoted to conducting a survey and a set of interviews with employees of government bodies (leaders and chief specialists) who are involved in the process of preparing and making foreign policy decisions. The article contains the officials’ assessment of the main challenges when interacting with the scientific and expert community, as well as a "desired image" of such interaction.The study was held in two stages (stage I in February-October 2020, stage II-in January-August 2021) in the form of a survey with a set of follow-up interviews for a more detailed interpretation of the data obtained. The interviewees included 24 employees of federal executive bodies (the Ministry of Foreign Affairs of Russia and Rossotrudnichestvo; departments of international cooperation of line ministries, agencies and services); offices of the chambers of the Federal Assembly of the Russian Federation (departments providing international and inter-parliamentary cooperation). All participants of the study are in positions of the "managers" category or are part of the group of "chief specialists", that is, they have experience in decision-making process and/or in elaborating them. The survey and interviews were anonymous. The research is within structural-functional, institutional and regulatory approaches. The authors note that informational materials with low level of generalization and lacking recommendations and forecasts will never replace genuine analytics and expertise. Given the development of artificial intelligence, generalizing information will be automatized. As a result, expertise has to be of better quality.

Analysis of the demand for human resources management specialists on the Moscow labor market

The article presents the results of an analysis of the demand for labor of personnel management specialists in Moscow according to the state as of July 2021. It provides a brief overview of the leading world and Russian employment sites. The correctness of using the HeadHunter website (hh.ru) for analysing the demand for HR specialists in the Moscow labor market has been substantiated. Based on the results of a preliminary analysis of positions offered to HR specialists in Moscow, a sample of 250 vacancies has been formed, on which the requirements of employers to applicants have been assessed. The analysis has been carried out for three large groups: for managers (directors) for personnel, chief (leading) specialists and specialists (assistants). A Box Plot diagram has been constructed and analysed based on the salary proposals.

This is almost the author of materials concerning the biography of our ancestor al-Farabi. Al-Farabi’s participation in the music of his ancestor, his writings are deeply interpreted, indicate that the work of scientists who saw his first teacher in this field is a recognized reality in that he is the chief specialist in the field of medicine

СОВРЕМЕННОЕ СОСТОЯНИЕ КАДРОВОГО ПОТЕНЦИАЛА СЕЛЬСКОГО ХОЗЯЙСТВА РЕСПУБЛИКИ ТАТАРСТАН

The research was carried out in order to study the state of human resource development in agriculture of the Republic of Tatarstan, the current causes and factors affecting the training of personnel and the provision of them in the industry. The work was carried out on the basis of data from the state statistics bodies, the Ministry of Agriculture and Food of the Republic of Tatarstan, as well as the results of their own observations and surveys. In 2015-2019, the number of employees in agricultural enterprises of the republic decreased by 5.9 %. The largest decrease during this period occurred in such professions as tractor drivers (by 24.3%), machine milking operators (25.5%), pig workers (26.3 %) and sheep workers (57.5 %). In general, the number of people employed in agriculture decreased by 34.5 thousand people, which, on the one hand, was the result of the processes of consolidation and automation of production, on the other, due to the ongoing urbanization. In the last 5 years alone, the city's population has grown by 3.6 %, while the rural population has decreased by 0.6 %. There are also problems in the higher education system. Today, about 25 % of the chief specialists (agronomists, veterinarians, accountants) do not have a higher education. There is an increase in the number of bachelor's degree students in the humanities and social areas to 37.5 and 32.7%, respectively, with the share of graduates in agricultural areas of training about 3 % of all graduates. According to the authors ' personal opinion, the increase in the active load on university teachers, as well as their distraction from the main educational classroom work, as well as the lack of professional standards in agricultural specialties that take into account the specifics of the industry, do not contribute to the training of competent specialists

The purpose of this research is to substantiate the need to develop a professional standard “Management specialist in industries and at enterprises of the agro-industrial complex” to improve the provision of the Russian agro-industrial complex with management personnel. Methods. To achieve this purpose, it is rational to apply analytical and statistical research methods: to analyze the provision of the Russian agro-industrial complex with specialists with economic and managerial competencies, to carry out a comparative analysis of the job responsibilities of managers and chief specialists of agricultural organizations performing the functions of personnel management and production processes, and the content and volume of economic-management training for the agro-industrial complex. Results. The heads of agricultural organizations, their deputies and chief specialists do not always meet the requirements for educational level and possession of economic and managerial competencies. On average, in the system of the Ministry of Agriculture of Russia, only 66 % of heads of agricultural enterprises have higher education, among the main specialists – 71 %. In the agricultural sector, there is an obvious shortage of managerial personnel, a high proportion of them are without professional education, a large proportion is occupied by employees of retirement age. Training of personnel for the agro-industrial complex is carried out in 54 universities subordinate to the Ministry of Agriculture of Russia. The number of graduates in the main agricultural areas and specialties trained internally at the expense of the federal budget is more than 19 000 people, but there are only about 6 000 graduates of the specialist and magistracy who have the right to occupy leadership positions. In addition, in the main professional educational programs of personnel for the agro-industrial complex, little study time is allocated for the study of economic and management disciplines, which does not allow the formation of the necessary competencies in graduates to perform the functions of organizing agricultural production. To provide agricultural organizations with personnel with modern economic and management competencies, it is proposed to recommend that agricultural universities, when teaching students in the main specialties of the agricultural profile, determine the list of relevant disciplines, their volume and content in the main professional educational programs in accordance with the labor functions, knowledge and skills specified in the draft professional standard “Specialist in management in industries and enterprises of the agro-industrial complex”.

In this issue, the founder of the Donetsk regional branch of the Ukrainian Association of Religious Studies (UAR), theologian, candidate of historical sciences, senior researcher of the Department of Religious Studies of the GS Skovoroda Institute of Philosophy of the National Academy of Sciences of Ukraine, President of the Center for Religious Studies member of the Expert Council on Freedom of Conscience and Religious Organizations under the Ministry of Culture of Ukraine, member of the Strategic Council under the Minister of Culture and Information Policy of Ukraine. I. Kozlovsky worked for more than 25 years in the Donetsk Regional State Administration as a chief specialist and head of the department for religions, in the early 1990s he established the College of Religious Studies at Donetsk Open University, which was later transformed into the Faculty of Philosophy and Religious Studies of the State University of Information. intelligence (DUiISHI), and has done much to promote interfaith and interreligious dialogue and cooperation in the Donetsk region, whose religious map is very diverse. At the beginning of the Russian-Ukrainian war, I. Kozlovsky became one of the organizers of the interfaith prayer marathon "Prayer for Ukraine", which took place in Donetsk from early March to November 2014, which was attended by members of the Donetsk branch of the UAR. Until 2016, some members of the UAR, led by I. Kozlovsky, who still remained in the occupied territories, collected data on the state of religious freedom in Donetsk and the region. In 2016, I. Kozlovsky was imprisoned by a terrorist group of the so-called "Donetsk People's Republic" and was held captive for almost 2 years (700 days).

Modeling the regional agro-elite’s social position in rural areas’ sustainable development system

The article concretizes the social position of the regional agrarian elite in the system of sustainable development of rural areas on the basis of factor analysis carried out by the method of principal components. A survey of 73 leaders and chief specialists of successful agricultural organizations in the South of Russia, traditionally represented by rural areas, was carried out in October-November, 2020 using Google Forms. The data was processed in SPSS Statistics (version 21) and used the Sustainable Development Goals proclaimed by the UN in 2015 as the 2030 Agenda for Sustainable Development. Seventeen Sustainable Development Goals were assessed on a five-point scale of significance in terms of relevance for agricultural organizations in rural areas of southern Russia. The assessments show the social position of the regional agrarian elite regarding the goal-setting in the field of sustainable development, both in rural areas and agricultural organizations. Factor analysis was carried out by the method of separation of the main components with Varimax Rotation with Kaiser Normalization. We present the models of the regional agrarian elite’s social position in the system of sustainable development of territories.
