Involving health professionals in the development of an advanced symptom management system for young people: The ASyMS ©-YG study

Purpose of the study ASyMS © is an advanced symptom management system utilising mobile phone technology for patients to report cancer chemotherapy-related symptoms. The aim of this paper is to present health professionals involvement in the development of ASyMS © for use with young people (YG) and evaluate their perceptions of the system. Stage 1 Health professionals reviewed the symptoms chosen by young people to be included on the personal digital assistant questionnaire to confirm they would have universal relevance. These included: mouth sores, nausea, diarrhoea, vomiting and weight loss. Stage 2 Health professionals completed a questionnaire prior to the study commencing, which suggested they felt ASyMS ©-YG would give young people control and facilitate timely interventions when symptoms occur. Post-study perceptions were sought through semi-structured interviews and consultation sessions with 23 health professionals. These showed two over-arching themes: young people's symptoms and perceptions of ASyMS ©-YG; and six sub-themes: increased control for young people; enhances communication between young people and health professionals; and helps with professional early intervention; increased support for young people; improve knowledge and understanding; technology appealing to young people. Conclusions This early development work indicates that ASyMS ©-YG is acceptable to health professionals and their perceptions of the system were overall very positive. Health professionals will continue to be involved in the study through developing self-care guidelines and alert system, which will be tested in an exploratory trial (stage 3) and randomised controlled trial (stage 4) in the future.

Evaluation of a mobile phone-based, advanced symptom management system (ASyMS©) in the management of chemotherapy-related toxicity

To evaluate the impact of a mobile phone-based, remote monitoring, advanced symptom management system (ASyMS) on the incidence, severity and distress of six chemotherapy-related symptoms (nausea, vomiting, fatigue, mucositis, hand-foot syndrome and diarrhoea) in patients with lung, breast or colorectal cancer. A two group (intervention and control) by five time points (baseline, pre-cycle 2, pre-cycle 3, pre-cycle 4 and pre-cycle 5) randomised controlled trial. Seven clinical sites in the UK; five specialist cancer centres and two local district hospitals. One hundred and twelve people with breast, lung or colorectal cancer receiving outpatient chemotherapy. A mobile phone-based, remote monitoring, advanced symptom management system (ASyMS). Chemotherapy-related morbidity of six common chemotherapy-related symptoms (nausea, vomiting, fatigue, mucositis, hand-foot syndrome and diarrhoea). There were significantly higher reports of fatigue in the control group compared to the intervention group (odds ratio = 2.29, 95%CI = 1.04 to 5.05, P = 0.040) and reports of hand-foot syndrome were on average lower in the control group (odds ratio control/intervention = 0.39, 95%CI = 0.17 to 0.92, P = 0.031). The study demonstrates that ASyMS can support the management of symptoms in patients with lung, breast and colorectal cancer receiving chemotherapy.

WISECARE+: Results of a European study of a nursing intervention for the management of chemotherapy-related symptoms

While the use of chemotherapy has significantly improved survival rates, the symptoms associated with chemotherapy remain a major burden for patients. Preventing or appropriately managing side effects significantly improves patients' functional status and quality of life, ultimately leading to greater patient acceptance of chemotherapy. However, symptom assessment and management are fraught with difficulties such as poor patient recall, retrospective assessment conducted by clinicians and lack of appropriate, clinically relevant and patient friendly symptom assessment and management tools. Furthermore the differences between clinician and patient perceptions of stresses and distress during chemotherapy are well recognised. This study aimed to evaluate the impact of a nursing intervention incorporating structured symptom assessment and management, facilitated by information technology, on chemotherapy-related symptoms, nausea, vomiting, fatigue and mucositis. This pan-European study, involved 8 clinical sites from Belgium, Denmark, England, Ireland and Scotland. Adults ( n = 249) receiving first line chemotherapy for breast, lung, ovarian or colorectal cancer, osteosarcoma, acute myeloid leukaemia (AML), acute lymphoblastic leukaemia (ALL) or lymphoma were recruited to the study. Patients completed daily symptom assessment questionnaires for 14 days following consecutive cycles of chemotherapy. Symptom outcomes were compared before and after the introduction of the intervention with positive impact on patients' experiences of nausea, vomiting and oral problems. Fatigue was not significantly improved.

The Benefits and Challenges of Using Computer-Assisted Symptom Assessments in Oncology Clinics: Results of a Qualitative Assessment

Developed for clinical use in oncology settings, the Patient Assessment, Care & Education (PACE) System is a computer technology tool designed to address the under-identification and treatment of chemotherapy-related symptoms. This system includes general core questions together with the Patient Care Monitor (PCM), a validated questionnaire that assesses patient-reported problems, six symptom burden indices, and one global quality of life index. The system automatically scores the PCM and generates a written report. The objective of this study was to assess the manner in which clinicians use this system and identify the benefits and challenges that oncology clinics may face when adopting this system. The study was part of a larger evaluation of the system that included standardized surveys and chart review. Sixteen providers (physicians, nurses, and physician assistants) at 13 community oncology clinics participated in a 30-minute interview. Responses were coded according to common phrases or concepts. Clinicians indicated that they use the system mainly for symptom assessment or review of systems. The most common benefits identified included the improved ability to identify under-reported symptoms, enhanced communication with patients; increased efficiency; and its ability to highlight patients' most bothersome symptoms. Challenges included patient burden from the frequent need to answer the questionnaires, issues with the wording and formatting of the screening questionnaire, and technical difficulties. In sum, these interviews suggest that electronic symptom assessments offer potential advantages in terms improving the integration of routine assessment of patients' symptoms and health-related quality of life into the daily flow of an oncology clinic. The approach should receive additional research and development attention.

Age-Related Differences in Quality of Life in Cancer Patients

The cancer care dialogues model emphasizes daily telehealth interactions between patients and a care coordinator (a registered nurse serving as a liaison to the oncologist) to assist patients in the management of common chemotherapy-related symptoms at home. We examined the impact of the dialogues on age-related differences in health-related quality of life (HRQOL) among newly diagnosed cancer patients receiving chemotherapy. We assessed HRQOL among 34 patients, including 15 older adults (65 years or older) and 19 younger adults who were followed for 6 months. Older patients consistently reported better HRQOL scores over the treatment period. In multivariate analysis, older patients reported 10.35 points higher in HRQOL (P = .007). In addition, patients who reported no nervousness while undergoing chemotherapy had an 8.60-point increase in HRQOL scores (P = .012). The dialogues model can make important improvement in symptom management and HRQOL, especially in older adults receiving chemotherapy. Older and younger adults with cancer may benefit equally in cancer treatment in a setting with appropriately managed symptoms. The dialogues model offers promising potential for promoting nurses' better understanding of both the patient needs as the patient receives treatment and innovative technologies in patient management.

Baseline Evaluation of the AIM Higher Initiative: Establishing the Mark From Which to Measure

To collect baseline measurements before the implementation of interventions associated with the AIM (Assessment Information Management) Higher Initiative--a quality improvement program intended to improve symptom assessment, management, and information distribution for five chemotherapy-related symptom groups: anemia, neutropenia, diarrhea and constipation, nausea and vomiting, and depression and anxiety. Subject telephone interviews and chart reviews. 15 community oncology clinics in the United States. 376 adult patients with cancer who visited a healthcare provider before the start of a chemotherapy cycle; patients were enrolled in the study after the initiation of chemotherapy, with at least one chemotherapy cycle remaining. Subject interviews and chart reviews to determine the frequency, assessment, and management of and information about target symptoms. The frequency of target chemotherapy-related symptoms and occurrence of symptom-specific assessment, information provided, and management. The five target symptoms had occurred in a considerable proportion of patients with cancer receiving chemotherapy during their most recent chemotherapy cycles. At a substantial number of clinic visits, no documentation of cancer-related symptom assessment, information distribution, or management occurred. Chemotherapy-related symptoms occur frequently but often are not assessed, managed, or handled with appropriate patient information. Findings in the baseline evaluation illustrate the need to improve supportive care--a key responsibility of oncology nurses.

The Cancer Chemotherapy Drug Etoposide (VP-16) Induces Proinflammatory Cytokine Production and Sickness Behavior–like Symptoms in a Mouse Model of Cancer Chemotherapy–Related Symptoms

Cancer chemotherapy-related symptoms such as fatigue, malaise, loss of interest in social activities, difficulty concentrating, and changes in sleep patterns can lead to treatment delays, dose reductions, or termination and have a profound effect on the physical, psychosocial, and economic aspects of quality of life. Clinicians have long suspected that these symptoms are similar to those associated with "sickness behavior," which is triggered by the production of the inflammatory cytokines IL-1beta, TNF-alpha, and IL-6 by macrophages and other cells of the innate immune system in response to immune challenge. The p38 mitogen-activated protein kinase (p38 MAPK) plays a central role in the production of these cytokines and consequently the induction of sickness behavior. Several cancer chemotherapy drugs have been shown to activate p38 MAPK, but whether these drugs can also induce the production of inflammatory cytokines to cause sickness behavior is unknown. The aim of this study was to determine whether the cancer chemotherapy drug etoposide (VP-16), which is known to activate p38 MAPK, could induce inflammatory cytokine production by murine macrophages and sickness-like behaviors when injected into mice. VP-16 activated p38 MAPK and induced IL-6 production in murine macrophages in a p38 MAPK- dependent manner. VP-16 administration rapidly increased serum levels of IL-6 in healthy mice and induced sickness-like behaviors as evidenced by a decrease in food intake, body weight, hemoglobin level, and voluntary wheel-running activity. These findings support the idea that the induction of IL-1beta, TNF-alpha, and IL-6 by cancer chemotherapy drugs underlies the fatigue and associated symptoms experienced by people undergoing cancer chemotherapy.

Cancer Chemotherapy-Related Symptoms: Evidence to Suggest a Role for Proinflammatory Cytokines

To provide an overview of the evidence that supports a role for the proinflammatory cytokines interleukin-1beta (IL-1beta), tumor necrosis factor-alpha (TNF-alpha), and interleukin-6 (IL-6) in the etiology of cancer chemotherapy-related symptoms. Electronic nursing, psychology, and medicine databases; online meeting abstracts; and personal experimental observations. Substantial evidence implicates the proinflammatory cytokines IL-1beta, TNF-alpha, and IL-6 in the etiology of chemotherapy-related anorexia, cachexia, anemia, pain, sleep disturbance, fatigue, and depression. Further investigation into the role of these cytokines in the genesis of chemotherapy-related symptoms is warranted. The development of appropriate animal models likely will be key to understanding the relationship among cancer chemotherapy, proinflammatory cytokines, and symptoms. Nurses traditionally have been leaders in symptom management. The symptoms experienced by patients undergoing chemotherapy have a profound negative impact on quality of life and patients' ability to receive prescribed treatments. An understanding of potential mechanisms underlying the physiologic and behavioral consequences of chemotherapy administration will aid nurses in the development of interventions to effectively manage chemotherapy-related symptoms.

Darbepoetin ?? as treatment for anemia in patients receiving chemotherapy: a single-center experience

We evaluated Darbepoetin alpha (Aranesp; Amgen), a novel erythropoietic protein, in patients who developed anemia while receiving chemotherapy. Seventy-five patients (median age 62 years, range 40-81 years) undergoing different cancer chemotherapy regimens were treated with darbepoetin alpha. Therapy was started if hemoglobin (Hb) levels fell below 10 g/dl or if symptomatic anemia developed. Treatment effect was evaluated after 4 weeks, 8 weeks and at the end of therapy (up to 12 weeks). If no increase in Hb was seen after 4 weeks, the dose of darbepoetin alpha was increased to 300 microg. Patients were questioned about fatigue and any change during treatment, with evaluation according to a four-point scale, where 0 = no fatigue and 3 = severe fatigue. We observed a treatment response in 54 of 75 patients (72%). Dose escalation was necessary in 30 of 75 patients (40%) and blood transfusions were required in 13 of 75 patients (17.3%). Response was observed in 32 of 43 patients (74.4%) who had a baseline Hb < 10 g/dl and in 22 of 32 (68.8%) patients who had a baseline Hb > or =10 g/dl. At baseline, 60 of 75 patients (93.3%) reported fatigue of grade 2 or 3, but at the end of the 12-week follow-up period, only 26 of 68 patients (38.3%) reported fatigue at these levels. We conclude that darbepoetin alpha is a highly effective and well-tolerated drug in the treatment of chemotherapy-associated anemia. Patients benefited both in terms of Hb levels and control of chemotherapy-related symptoms.

Telephone-linked care for cancer symptom monitoring: a pilot study.

The purpose of this pilot project was to explore the feasibility of using a telephone-based computerized system to monitor postchemotherapy symptoms and to test the mechanism of generating alert communications to healthcare providers about symptoms that are poorly controlled. Twenty-seven patients with cancer participated in the study by calling the telephone-linked care (TLC) system daily during a single cycle of chemotherapy and reporting on seven common chemotherapy-related symptoms. Using selected symptom data, symptoms that met a preset threshold for severity generated a fax notification of the patient's symptom pattern to the physician. Patients then were interviewed about their satisfaction with TLC and about suggestions for improvement. The study demonstrated that TLC is easy to learn and use and that it captures daily symptom information from patients in their homes. A majority of patients experienced symptoms that were severe enough to generate symptom-alert faxes. Patient satisfaction with TLC was high. The technique, TLC voice, and the duration of the calls were acceptable to patients. There were few technical problems. The TLC system has the potential to improve dramatically symptom monitoring and symptom care of patients with cancer at home. Further testing is needed, but the TLC chemotherapy monitoring application shows promise for improving supportive-care service delivery for cancer patients. In its initial test, the TLC chemotherapy monitoring application has been shown to be highly acceptable to patients, able to generate useful symptom data, and able to generate faxed alerts to healthcare providers, thus improving communication about poorly controlled symptoms.

Nausea, vomiting, and retching: complex problems in palliative care.

Patients with advanced cancer commonly experience nausea, vomiting, and/or retching (NVR) as a result of the malignant process and its treatment. Recently, increasing attention is being focused on end-of-life care, which includes relief or reduction of symptoms such as NVR. Pre-chemotherapy preparation and patient education in the palliative care setting are essential to preventing acute and delayed distress from NVR, as well as anticipatory symptoms. Careful assessment of chemotherapy-related symptoms should distinguish between the three phenomena rather than taking a global approach. Strategies for preventing anticipatory nausea, for instance, may differ significantly from those designed to reduce frequency of vomiting. Management of anticancer treatment-related NVR should incorporate both pharmacologic and nonpharmacologic approaches, whenever appropriate, with the overall goal of improving and/or maintaining the patient's quality of life.

Effects of Virtual Reality on Symptom Distress in Children Receiving Chemotherapy

This study tested the premise that virtual reality (VR) as a distraction intervention could mitigate chemotherapy-related symptom distress in children with cancer aged 10-17 years. Cancer treatments are intensive and difficult to endure. Distraction interventions are effective because the individual concentrates on pleasant or interesting stimuli instead of focusing on unpleasant symptoms. VR as a distraction intervention is both immersive and interactive. For this study the individual wore a Virtual IO(R) headset during a single intravenous chemotherapy treatment. Participants chose one of three commercially available, CD ROM-based scenarios: Magic Carpet, Sherlock Holmes Mystery, and Seventh Guest(R). An interrupted time series design with removed treatment was used to answer these research questions: (1) Is VR an effective distraction intervention for reducing chemotherapy-related symptom distress in children? and (2) Does VR have a lasting effect? The convenience sample consisted of 11 children receiving outpatient chemotherapy. The Symptom Distress Scale (SDS) and the State-Trait Anxiety Inventory for Children (STAIC-1) were used to measure the dependent variable of symptom distress. Repeated-measures ANOVA were used for data analysis. Data analysis of the SDS suggested that the VR intervention was effective at reducing the level of symptom distress immediately following the chemotherapy treatment (p <.10), but did not have a lasting effect. Analysis of the STAIC-1 demonstrated high levels of anxiety during the initial chemotherapy treatment that decreased during subsequent treatments. State anxiety levels were not influenced by the VR intervention. This study supports the application of VR as a distraction intervention.

5-HT3 receptor antagonists, a new approach in emesis: a review of ondansetron, granisetron and tropisetron.

In recent years a new class of agents, the serotonin type 3 receptor antagonists, has been identified. This article reviews the preclinical, pharmacological and clinical data of ondansetron, granisetron and tropisetron, the first representatives of this group. Preclinical work showed that the drugs interfere with a variety of physiological processes, and hold promise for clinical utility in a wide range of areas. To date, these agents have proven, both in early clinical and comparative studies, to be potent antiemetic agents in patients receiving cisplatin and non-cisplatin chemotherapy as well as radiotherapy. In comparative studies the antiemetic efficacy mostly has been superior to conventional antiemetic drugs with regard to the acute chemotherapy-related symptoms; whereas their role in delayed emesis needs further investigation. This also applies for their role as an antiemetic in other types of nausea and vomiting (post-operative). Toxic effects have been modest, no extrapyramidal reactions have been reported. Potential clinical use in psychiatric disorders has been suggested, and the results of clinical trials are awaited.

A RANDOMIZED, CONTROLLED STUDY OF HYPNOTIC AND NONHYPNOTIC BEHAVIORAL INTERVENTION FOR CHEMOTHERAPY-RELATED NAUSEA AND VOMITING

Hypnosis and nonhypnotic support were compared for effectiveness in reducing chemotherapy related nausea and vomiting for 60 chemotherapy combinations in 42 children with cancer. Patients were randomized to a hypnosis, support, or control group, and two matched baseline courses of chemotherapy were compared with two courses with intervention. The nausea and vomiting were reduced for the hypnosis group, with the largest reduction occuring for nausea duration (p<.03) and bother (p<.01). A reduction of > 8 hours in the duration of both nausea and vomiting occurred more than 3 times as often in the hypnosis group as in the control group. Unlike either of the intervention groups, 75% of the patients in the control group reported a 20% increase in the extent to which chemotherapy bothered them. In summary, nausea and vomiting, improved with hypnosis but did not improve with nonhypnotic support. The control group demonstrated a consistent worsening of all symptoms over time, indicating that children do not “get used to” chemotherapy. The findings indicate that behavioral intervention can modify chemotherapy related symptoms and reduce the extent to which treatment bothers children with cancer.
