The level of incidence of rhinosporidiosis in nasal mass in ear, nose and throat.

According to Swindle (1935) in the mucous membrane of the nose the arrangement of the blood vessels consists of a superficial venous plexus and a deeper arteriolar. System, arranged parallel to the long axis of the nose. The relationship of taking bath in infected pond and agriculture work is explained by the above observation. There is no specific occupational incidence in other of nasal mass in the present series. Out of 8 cases (6.15%) of malignant tumours, sq. cell carcinoma is the most common (66%), in which three cases (37.5%) were sq. cell carcinoma of maxillary sinus, 2 cases of sq. cell carcinoma of nose & 1 case of sq. cell carcinoma of ethmoid sinus. Nasal masses are more common on 2nd decade (33.33%) followed by 3rd decade (26%). Malignancy of nose /PNS are found in 4-6th decade of life. Nasal masses are more common in males (71%)

The external nose is pyramidal in shape with its root above and base directed downwards. The framework is composed of bones and cartilages. The bones are the two nasal bones articulating medially. Laterally they articulate with the frontal process of maxilla, superiorly to the medial part of nasal notch of frontal bone and inferiorly to the lateral cartilage of nose and posteriorly with the perpendicular plate of ethmoid bone. Considering the malignant conditions of nose & PNS, the most common issq. cell carcinoma of maxillary sinus (37.S°lc>), followed by S. q cell carcinoma of nose (25%), and then Adenocarcinoma of maxillary sinus (1 2.5%), Sq. cell carcinoma of Ethmoid sinus (12.5%) & adenoid cystic carcinoma of maxillary sinus (12.5°/o) These findings are comparable with reports of Cheesman (1957) , who had noted that Sq. cell carcinoma comprised of 80% of cases & Bahadur et al (1984), who reported that most common was Sq. cell carcinoma (65%).Followed by adenocysticcarcinoma (12%), Adinocarcinoma (12%) Meliginate lanoma (2%) olfactory neuroblastoma(2%).

Locally Advanced Squamous Cells Carcinoma of the Maxillary Sinus: A Case Showing Complete Response to Exclusive Chemoradiotherapy

Maxillary sinus squamous cell carcinoma is the most frequent type of Maxillary sinus carcinoma, and over 80% of Maxillary sinus squamous cell carcinomas are detected at an advanced stage because of a lack of typical symptoms, and making curative surgery sometimes impossible. We report a case of advanced squamous cell carcinoma of the left maxillary sinus, which had an aggressive clinical course but a complete response to chemoradiation. A discussion of the clinical course and treatment of squamous cell carcinoma is presented, along with a review of the pertinent literature.

ANALYSIS OF EXPRESSION OF P63 AND CASPASE-3 AND THEIR PREDICTIVE VALUE IN PATIENTS WITH SQUAMOUS CELL CARCINOMA OF MAXILLARY SINUS

Introduction: Maxillary sinus cancer is a malignant neoplasm with a prevalence of 3 to 5% of tumors in the head and neck and less than 1% of all malignant neoplasms. The role of p63 and caspase-3 and their predictive value in squamous cell carcinoma of maxillary sinus (SCCMS) remains an underdeveloped and controversial issue that determined the relevance of the study. The aim: To analyze the relationship between the level of expression of p63 and caspase-3 and clinical and morphological characteristics of squamous cell carcinoma of maxillary sinus and to determine their prognostic significance as immunohistochemical markers of tumor progression (relapse and metastases). Materials and methods: A comprehensive investigation with immunohistochemical study of squamous cell carcinoma of maxillary sinus of 103 patients with maxillary sinus cancer II - IV stage (T2-4 N0-3 M0) of the second clinical group was conducted. Results: The moderate and high expression rate of caspase-3 in the overall sample was 50.49% and 16.50% respectively, and one third of cases (33.01%) was characterized by a weak reaction. In the group without metastases (p <0.001), moderate expression of the marker (56.25%) prevailed; in the SCCMS group with metastases it was (60.87%). The mean level of expression of p63 in the examined patients with squamous cell carcinoma of maxillary sinus was 46.0%. The number of p63-positive specimens was statistically significantly (p = 0.034) higher in the SCCMS group with metastases. In patients with the development of squamous cell carcinoma of maxilla on the background of papilloma, a significantly higher (p <0.001) mean level of p63 expression was only moderate (90.91%) and high (9.09%). Conclusions: There was observed a relatively strong associate of the increase in the expression of caspase-3 with SCCMS metastases (p <0.001) and no correlation of immunohistochemical reaction of this marker with relapses. There was determined that the probability of metastases at high expression level of p63 is higher than in tumors with low and moderate level of the oncoprotein (p = 0.003). Based on the level of expression of p63, it is possible to predict the likelihood of development of SCCMS with papillomas (p <0.001); and the development of relapses of squamous cell carcinoma of maxillary sinus (p <0.001).

A case of maxillary sinus carcinoma: An occult aggressive entity

Carcinoma of the maxillary sinus is a rare aggressive entity. Squamous cell carcinoma of maxillary sinus has the highest incidence among the tumors developing within the sinonasal compartment and has one of the worst outcomes in comparison with other head and neck tumors. Management of these tumors often presents an uphill task to clinicians. In the present case, the patient presented with a swelling in the left cheek region with a history of trauma and altered sensation and no other signs and symptoms. Based on the history and clinical examination, a provisional diagnosis of infraorbital fracture was given. Further, radiographic and histopathological evaluation was done which leads to the final diagnosis of squamous cell carcinoma of the left maxillary sinus. As oral physicians, we come across diseases of the maxillary sinus; thereby, complete assessment of any pathology of the sinuses may help in early diagnosis of this deadly disease.

&lt;p class="abstract"&gt;&lt;strong&gt;Background:&lt;/strong&gt; The nasal cavity and paranasal sinuses are the site of origin of more complex, histologically diverse group of tumors which include neoplasms derived from mucosal epithelium, seromucinous glands and haematolymphoid cells. This study aimed to assess the incidence and prognosis of maxillary sinus malignancy.&lt;/p&gt;&lt;p class="abstract"&gt;&lt;strong&gt;Methods:&lt;/strong&gt; A total 25 (16 males, 09 females) malignancy cases between age group 11-70 years. All the cases were undergone with detailed clinical examination. The TNM staging system was followed to detect and standardize the disease stage. CT scan was used to identify the growth and extent of tumor. &lt;/p&gt;&lt;p class="abstract"&gt;&lt;strong&gt;Results:&lt;/strong&gt; Majority cases were between the age group 41-50 years (40%). Epistaxis was the commonest clinical symptom in 72% cases followed by nasal obstruction (68%), oral related symptoms (56%), oedema in cheeks (44%) and auditory disturbances in 16% cases. The TNM Staging system showed that 76% cases had T3 lesions, 16% had T4 lesions and 8% had T2 lesions. Nasal symptoms are commonly associated with malignancies and were belongs to T3 stage and few under T4.&lt;/p&gt;&lt;p class="abstract"&gt;&lt;strong&gt;Conclusions:&lt;/strong&gt; Maxillary si­nus malignancy is an aggressive tumor normally diagnosed at the advanced stage, with minimal prognosis and reduced survival rate. Surgery in combination with radiotherapy followed by chemotherapy remains the best treatment modality.&lt;/p&gt;

Introduction: Spindle cell carcinoma (SpCC) is an unusual subtype of squamous cell carcinoma with an aggressive behavior. The occurrence of this tumor in maxillary sinus has rarely been reported. Case Presentation: This case report describes the diagnosis, management, and follow-up of a 35-year-old female, who presented with Spindle cell carcinoma of maxillary sinus. Conclusions: The behavior of maxillary sinus SpCC seems to be more aggressive than Spindle cell carcinoma of other sites and shows a poor prognosis.

Result of Multimodal Treatment Including Radiation Therapy for Squamous Cell Carcinomas of Maxillary Sinus: A Retrospective Study

(26%) had adenoid cystic type maxillary sinus carcinoma. Nineteen patients (35%) were treated with IMRT, and 35 patients (65%) received 3DCRT. The median radiation doses for 3D-CRT and IMRTwere 62.1 and 63 Gy, respectively. Results: Over a median follow-up of 60 months (97 months for 3D-CRT and 34 months for IMRT), the 5-year locoregional recurrence-free survival (LRRFS), distant metastasis-free survival (DMFS), and overall survival (OS) rates were 66, 60.9, 50.1, and 67.2%, respectively. IMRT provided better 3-year LRRFS (89.2% versus 59.5%, pZ 0.035) and DMFS (94.7% versus 55.3%, p Z 0.042) rates than 3D-CRT. Out of total 13 local recurrences, the orbital wall (n Z 5) and infratemporal fossa (n Z 3) were the most common sites of local recurrence, and most local failures were marginal (n Z 8). Conclusions: Postoperative IMRT for patients with maxillary sinus carcinoma resulted in excellent disease control, and should be considered as the first treatment option in these cases. Superselective intra-arterial cisplatin infusion and concomitant radiotherapy for maxillary sinus cancer

Background:The purpose of this study was to evaluate the efficacy of superselective cisplatin infusion with concomitant radiotherapy (RADPLAT) for previously untreated patients with the squamous cell carcinoma of maxillary sinus (SCC-MS).Methods:Between 1999 and 2010, 54 patients were given superselective intra-arterial infusions of cisplatin (100–120 mg m−2 per week) with simultaneous intra-venous infusions of thiosulfate to neutralise cisplatin toxicity and conventional radiotherapy (65–70 Gy).Results:One patient (1.9%) was diagnosed with T2, 14 (25.9%) with T3, 27 (50%) with T4a, and 12 (22.2%) with T4b disease. Lymph-node involvement was present in 12 patients (22.2%). During the median follow-up period of 6.4 years, the 5-year local progression-free and overall survival rates were 65.8 and 67.9% for all patients, respectively. No patient died as a result of treatment toxicity or experienced a cerebrovascular accident. Osteonecrosis (n=5), brain necrosis (n=1), and ocular/visual problems (n=14) were observed as late adverse reactions.Conclusion:We have shown excellent overall survival and local progression-free rate in SCC-MS patients treated by RADPLAT with acceptable rates of acute and late toxicity. A multi-institutional trial is needed to prove that this strategy is a feasible and effective approach for the treatment of SCC-MS.

Predictive markers for neoadjuvant chemotherapy in advanced squamous cell carcinoma of maxillary sinus: Preliminary report

Conclusion: ERCC 1 seems to be promising as a predictive marker for response to neoadjuvant chemotherapy (NAC) and early decision for surgery in advanced squamous cell carcinoma (SCC) of the maxillary sinus. Objectives: This study aimed to find a possible relation of ERCC1 or XRCC1 expression with response to NAC and prognosis in advanced SCC of the maxillary sinus. Methods: From 1998 to 2006, 17 patients with advanced SCC of the maxillary sinus received NAC at the Seoul National University Hospital. The expression of ERCC1 and XRCC1 was assessed by immunohistochemistry. Complete and partial remissions were categorized as the chemo-sensitive group. On the other hand, stable and progressive diseases were categorized as the chemo-resistant group. Results: Of these 17 patients, 1 had complete remission, 6 had partial remission, 4 had stable disease, and 6 had progression of disease. The 5-year survival rate was 40% for all 17 patients. The expression of ERCC1 and XRCC1 was not correlated with nodal or distant metastasis. With a cut-off value of 65%, patients with higher ERCC1 scores showed chemo-resistance and survival disadvantage over those with lower ERCC1 scores. However, XRCC1 did not show a significant effect on the response to NAC.

Paranasal sinus malignancies are exceedingly rare. Chronic respiratory tract infections, nasal congestive symptoms and rhinosinusitis are much more prevalent in recent days and manifest many symptoms that overlap with those of sinus malignancy. Symptoms may be nonspecific and indolent for months or even years, leading to delay in diagnosis and consequent advanced diseased stage at presentation. The majority of maxillary sinus tumors in the literature have presented with advanced stage, resulting in generally poor survival outcomes. Hence, it is imperative that maxillofacial stomatologist should be aware of sinus pathologies, to diagnose early and it should alert them to include in differential diagnosis.
