First experiences of ultrasound vector flow imaging at the femoropopliteal artery in peripheral arterial disease.

Background: The femoropopliteal artery (FPA) plays a central role in diagnosing and treating peripheral arterial disease (PAD). FPA lesions are the most frequent cause of intermittent claudication, and no other artery of the lower extremities is recanalised more frequently. Generally, ultrasound is the primary imaging tool in PAD, particularly FPA. With the development of high-frame-rate ultrasound technology in addition to traditional ultrasound modes, vector flow imaging (VFI) has provided deeper haemodynamic insights when used in the carotid artery. Here, we report the use of VFI at the FPA level in routine PAD examinations. Patients and methods: In this single-centre prospective study, we evaluated consecutive patients with PAD using B-mode imaging, colour Doppler, pulsed wave Doppler (PW) and vector flow. Hemodynamic parameters at predefined locations at the carotid artery and FPA were compared. Results: Qualitatively adequate VFI at all sites was possible in 76% of the patients with PAD. With decreasing volume flow from the common carotid artery to the internal carotid artery and from the common femoral artery via the superficial femoral artery to the popliteal artery, the correlation between VFI- and PW-derived-volume flow was high at every site. Based on different techniques, the VFI-derived values were significantly lower than the PW-derived values. The mean wall shear stress was significantly lower at all femoropopliteal sites than at the carotid sites, whereas the oscillatory shear index at the femoral site was higher than that at the carotid sites rather than at the popliteal location. Conclusions: Our findings suggest that vector flow data acquisition in the FPA is feasible in most patients with PAD. Therefore, with knowledge of the method and its limitations, VFI provides haemodynamic information beyond traditional ultrasound techniques and is a promising new tool for flow analysis in PAD.

Claudication due to adventitial cystic degeneration: A differential diagnosis of peripheral artery disease.

Cystic adventitial disease (CAD) is an important and rare non-atherosclerotic cause of intermittent claudication and critical limb ischemia. Since the first case of CAD involving the external iliac artery was described by Atkins and Key in 1947, approximately 300 additional cases have been reported. The aim of this article is to report a rare vascular disorder, predominantly seen in young healthy men with minimal cardiovascular risk factors. We report a rare case of cystic adventitial disease of a young policeman. To confirm the diagnosis, an ultrasonography and a conventional angiography were performed. The therapeutic approach was surgical first. The procedure was successful without any complication, and the patient was discharged to home 4 days after procedure. While CAD is rare, the diagnosis should be suspected in a young patient who presents with arterial insufficiency and no risk factors for atherosclerosis. Catheter angiography is the investigation of choice in the absence of multislice CT and good MRA. It seems that the treatment that assures the best long-term results is reconstructive arterial bypass surgery.

Run-Off Computed Tomography Angiography (CTA) for Discriminating the Underlying Causes of Intermittent Claudication.

AimTo evaluate run-off computed tomography angiography (CTA) of abdominal aorta and lower extremities for detecting musculoskeletal pathologies and clinically relevant extravascular incidental findings in patients with intermittent claudication (IC) and suspected peripheral arterial disease (PAD). Does run-off CTA allow image-based therapeutic decision making by discriminating the causes of intermittent claudication in patients with suspected peripheral arterial disease PAD?Material and MethodsRetrospective re-evaluation of CTAs performed in patients with acute or chronic intermittent claudication (i.e., Fontaine stages I to IIB) between January 2005 and October 2013. Allocation to one of three categories of underlying causes of IC symptoms: vascular, musculoskeletal (MSK) or both. Clinically relevant extravascular incidental findings were evaluated. Medical records were reviewed to verify specific therapies as well as main and incidental findings.ResultsWhile focused on vascular imaging, CTA image quality was sufficient for evaluation of the MSK system in all cases. The underlying cause of IC was diagnosed in run-off CTA as vascular, MSK and a combination in n = 138 (65%), n = 10 (4%), and n = 66 (31%) cases, respectively. Specific vascular or MSK therapy was recorded in n = 123 and n = 9 cases. In n = 82, no follow-up was possible. Clinically relevant extravascular incidental findings were detected in n = 65 patients (30%) with neoplasia, ascites and pleural effusion being the most common findings.DiscussionRun-off CTA allows identification of vascular, MSK, and combined causes of IC in patients with suspected PAD and can guide specific therapy. CTA also allowed confident detection of crEVIF although detection did not necessarily trigger workup or treatment.

Nonatherosclerotic PAD: Approach to Exertional Pain in the Lower Extremities.

Atherosclerotic peripheral artery disease is the most common cause of intermittent claudication. Nonatherosclerotic peripheral artery disease is a heterogeneous collection of diseases affecting the extracoronary arteries which is not due to atherosclerosis. These diseases include, but are not limited to, popliteal artery entrapment syndrome, cystic adventitial disease, external iliac endofibrosis, and thromboangiitis obliterans. Due to its relatively low prevalence, nonatherosclerotic peripheral artery disease may be misdiagnosed leading to the mismanagement of potentially treatable conditions. The proper and timely diagnosis of these conditions is paramount to the prevention of adverse outcomes as treatments widely vary. The diagnostic approach to patients presenting with intermittent claudication must take into account both atherosclerotic as well as nonatherosclerotic causes of peripheral artery disease making the differential vital to clinical practice.

Clinical feathers and treatment of peroneal nerve entrapment neuropathy

Peroneal nerve entrapment neuropathy (PEN) is generally known as a drop foot with sensory disturbance. However, some patients experience numbness and pain in the affected area without severe paresis due to PEN. We report the clinical features and our surgical results of PEN cases. We encountered 17 cases of PEN. The patients were 7 females and 10 males and their ages ranged from 30 to 78 years(average 56.1 years). In these cases, conservative therapy was unsuccessful;therefore, we performed surgical treatment for PEN. Among the 17 cases, 4 were of bilateral and 13 were of unilateral PEN. There was no severe paresis, as in drop foot;however, mild paresis (4/5, manual muscle test, MMT) was noted in 15 cases. In all cases, intermittent claudication presented, which ranged from 10 to 800 m (average 150 m). In 13 cases, radiological abnormality of the lumbar region was noted and 8 cases had a history of lumbar surgery (they had failed back surgery syndrome). In all the cases, we performed neurolysis of the peroneal nerve under local anesthesia;there was no surgical complication. After the surgery, symptoms improved, and the numerical rating of the lower limb improved from 8.6/10 to 0.8/10. Intermittent claudication also improved in all of the cases. We successfully treated 17 cases of PEN, which had lower limb pain without severe paresis, as in drop foot. Our results indicate that PEN should be recognized as a cause of intermittent claudication. Neurolysis for PEN under local anesthesia is less invasive and is useful for the treatment of lower limb pain.

Surgical Treatment of Cystic Adventitial Disease of the Popliteal Artery: Five Case Reports

Cystic adventitial disease (CAD) is a rare cause of intermittent claudication and nonatherosclerotic conditions in middle-aged men without cardiovascular risk factors. The etiology of CAD is unclear; however, the direct communication between a cyst and a joint is presumed to be a cause. We herein report a case series of CAD of the popliteal artery (CADPA), in which patients were treated with surgical resection and vascular reconstruction. Although less invasive treatment modalities, including percutaneous cyst aspiration and percutaneous transluminal angioplasty, have been the subject of recent reports, these treatments have had a higher recurrence rate. Therefore, all of the CAPDA cases in the present series were treated surgically, which lead to good outcomes.

A Review of Superficial Femoral Artery Angioplasty and Stenting

Introduction: Peripheral arterial disease (PAD) of the superficial femoral artery (SFA) is the most common cause of intermittent claudication. Several endovascular treatment options exist; the most frequent are angioplasty or angioplasty with stent placement. The aim of this review is to clarify the role of angioplasty alone vs. angioplasty with stent placement. Methods: A literature review was conducted reviewing the topic of SFA angioplasty and SFA angioplasty with stenting. Three notable randomized trials were included in addition to other available studies on the topic. Results: When a residual stenosis or flow limiting dissection is visualized, stenting is clearly necessary. Current literature favors the use angioplasty for short (~4 cm) SFA stenoses or occlusions due to comparable patency and results. Primary stenting using Nitinol stents may be a superior initial treatment for intermediate (6-8 cm) and long (>10 cm) length lesions. In these intermediate and long length lesions, the use of stents appears to reduce the restenosis rates and improves patency. Conclusion: Angioplasty works best for short SFA stenoses or occlusions. However, angioplasty with stenting using is likely superior initial treatment for intermediate and long length lesions.

Endovascular Treatment of Cystic Adventitial Disease of the Popliteal Artery

Cystic adventitial disease of the popliteal artery is an uncommon cause of intermittent claudication in young patients. Several treatment options are available, oriented to either drainage of the cyst and/or arterial reconstruction. Endovascular techniques have been used in exceptional cases to treat this condition, mostly balloon angioplasty and only 2 cases of stenting, with mixed to poor results. We report the case of a 36-year-old female patient with claudication who was treated with primary stenting with long-term symptomatic relief and arterial patency.

A rare cause of intermittent claudication: recurrence of popliteal artery entrapment syndrome in a 16-years-old boy

Popliteal artery entrapment syndrome (PAES) is a rare clinical entity which is predominantly seen in young athletic men. The primary symptom of PAES is intermittent claudication. Surgery is the primary treatment of choice. After surgery in adolescence period particularly, these patients should be monitored closely because of risk of recurrence.

Vasculitides as a rare cause of intermittent claudication

Intermittent claudication of the lower extremities is a common symptom described in older patients with atherosclerotic peripheral arterial disease. Peripheral arterial disease due to atherosclerosis is known to be associated with a higher risk of myocardial infarction, stroke and all-cause mortality. However, if intermittent claudication appears in a younger group of patients or older patients in absence of traditional risk factors for atherosclerosis such as smoking, dyslipidemia, arterial hypertension and diabetes mellitus other causes than atherosclerosis must be considered. These conditions include vasculitides, fibromuscular dysplasia, cystic adventitial disease, excentric vascular compression by tumor, popliteal artery entrapment syndrome, trauma or dissection. Vasculitides present a heterogenous group of disorders characterized by inflammatory destruction of blood vessels. Although often not a leading symptom intermittent claudication could be a part of a clinical picture in giant-cell arteritis, Takayasu´s arteritis, Buerger´s disease, polyarteritis nodosa or Behçet disease. Limb claudication is usually of rapid onset, progressive and bilateral. Each of the mentioned vasculitides is specific in ethiology and clinical manifestation with a variable prognosis for the patient. Increased awareness of the presence of different causes of limb claudication and their early diagnosis with a prompt initiation of appropriate treatment may help to avoid clinical progression that can lead to vascular surgery or even limb loss (Ref. 37).

Intermittent claudication caused by a giant atypical lipoma of the thigh

The main cause of intermittent claudication in lower limbs is peripheral vascular disease. Less commonly, the etiology can be extrinsic to vascular structures, as in the cases of tumors that, due to their rapid growth, can reduce the blood supply and produce intermittent claudication during gait. We report the case of a 49-year-old patient with intermittent claudication in the left lower limb, reporting the presence of a tumor in the inner side of the left thigh with rapid growth. Doppler and angiography magnetic resonance imaging examinations demonstrated the presence of an adipose tumor that was producing deep and superficial extrinsic compression of the femoral arteries.

A síndrome do aprisionamento da artéria poplítea caracteriza-se pela compressão desta artéria sendo a principal causa de claudicação intermitente em jovens. Homem, 18 anos, branco, apresentava parestesia, frialdade e palidez do pé direito, iniciada 24 horas após exercício físico. Em membro inferior direito, ausência de pulsos tibial posterior e dorsal do pé. À flexão dorsal e flexão plantar forçadas, houve diminuição dos pulsos tibial posterior e dorsal do pé à esquerda. Tratado cirurgicamente, o paciente apresentou pulso em ambas as artérias. A síndrome é mais frequente em homens e a prevalência varia entre 0,16 e 3,5%. O aprisionamento da artéria poplítea tipo III é mais comum. A falta de tratamento pode levar à embolia, trombose e aneurismas pós-estenóticos. Esta síndrome deve ser lembrada como causa de dor na perna, especialmente em homens jovens e de prática esportiva intensa.

Unusual causes of intermittent claudication: Popliteal artery entrapment syndrome, cystic adventitial disease, fibromuscular dysplasia, and endofibrosis

In the general population, vascular causes of exercise-induced limb discomfort are most often the result of peripheral artery disease (PAD) due to atherosclerosis. However, several other clinical entities can often mimic the symptoms of atherosclerotic PAD of the lower extremities, particularly among younger patients with fewer risk factors for atherosclerosis, who often are more athletically fit than patients with PAD. Treatment for these entities often requires percutaneous or surgical intervention. This article reviews four uncommon vascular causes of exercise-induced limb discomfort: popliteal artery entrapment syndrome, cystic adventitial disease of the popliteal artery, fibromuscular dysplasia of the lower-extremity arteries, and endofibrosis of the iliac artery.

Bilateral popliteal artery entrapment syndrome: case report

Popliteal artery entrapment syndrome occurs due to an extrinsic compression of the popliteal vessels that results in vascular damage. It is one of the most frequent causes of intermittent claudication in young patients. The authors describe a case of bilateral syndrome by anomalous position of the gastrocnemius muscle, with abnormal slip of its medial head (Rich's type III). During the operation the occluded right side was reconstructed by autologous saphenous vein bypass from femoral superficial to peroneal artery and on the left side the slip muscle was transected by posterior approach. Popliteal artery entrapment syndrome should be treated by surgery despite the degree of symptoms. Surgical treatment technique has released the vessel by extracting the muscle that caused entrapment, and reconstructing the narrow lumen bypass grafting.

Bilateral functional popliteal artery entrapment in a young athlete

Popliteal artery entrapment syndrome is a frequent cause of intermittent claudication in young patients. We present a case of a bilateral functional entrapment, where static imaging did not demonstrate the occlusion until the patient's feet were placed in forced plantar flexion. A high index of clinical suspicion and dynamic tests with provocative manoeuvres are needed to diagnose this condition.

Adventitial cystic disease of the popliteal artery: a potential cause of intermittent claudication

Adventitial cystic disease (ACD) is an uncommon and rare vascular entity characterized by a highly viscous and mucinous cyst located in the adventitia of an artery. Although first described in the external iliac artery, the disease is more frequently sited in the popliteal artery. ACD characteristically occurs in young adults, mostly male, with intermittent claudication as the initial presenting symptom. As the cyst enlarges, it leads to vascular compression with stenosis or occlusion. Because cystic formation of the popliteal artery is uncommon, the disease is often unrecognized. We report a case of popliteal artery ACD in a 54-year-old woman who was successfully treated surgically.

Case Study: Cystic Adventitial Disease of the Popliteal Artery

Introduction —Adventitial cystic disease is an unusual cause of lower-extremity ischemia. It affects large arteries, typically where the artery is near a joint space, and the popliteal artery is most commonly involved. Methods —A 60-year-old man with venous insufficiency and prior deep vein thrombosis was referred for a lower-extremity venous evaluation. He had chronic degenerative joint disease, with bilateral knee osteoarthritis, but a history of exertional right calf pain was elicited. His right femoral and popliteal pulses were normal, but the posterior tibial pulse was diminished and the dorsal pedal artery pulse was absent. A lower-extremity duplex ultrasound examination was performed, including conventional and panoramic B-mode imaging, color Doppler and power Doppler imaging, and pulsed flow assessment. Results —Chronic deep and superficial venous thrombosis was identified in the right lower extremity. During the examination, the right popliteal artery was noted to be abnormal. Imaging showed the flow lumen in the popliteal artery to be visibly narrowed by more than 50%. Peak systolic velocities (PSV) in the proximal popliteal artery were 82 cm/sec, with a normal triphasic waveform, but the PSV increased to 221 cm/sec through the stenotic segment. The luminal narrowing was limited to an approximately 3 cm segment of the popliteal artery. There was no visible plaque or evidence of vascular calcification. The lumen appeared compressed by hypoechoic, septated cystic material. There was no flow within the well-visualized cystic structure. The findings were subsequently correlated with arteriography. Adventitial cystic disease of the popliteal artery was diagnosed. The patient underwent successful right popliteal artery cystectomy. Intra-operative and post-operative duplex evaluations demonstrated normal findings. Conclusion —Cystic adventitial disease is a rare disorder that can compromise arterial perfusion to the leg and it is one nonatherosclerotic cause of intermittent claudication. In some cases, the popliteal artery may occlude. Incidental arterial disease may be encountered frequently with lower extremity venous duplex scanning. With a careful examination by a thorough technologist, rare and interesting pathology, such as cystic adventitial disease, is possible.
