Immunologic monitoring and aspergillosis in renal transplant patients

Three cases of pulmonary aspergillosis in a “high risk” population of renal transplant recipients are presented. The source of infection was traced to the forced air exhaust system of the Transplantation Unit. Early definitive diagnosis of the infection was very important for effective management. Immunologic monitoring was demonstrated to be instrumental in indicating the early presence of infection, and as a guideline to reduced immunosuppression during therapy. Bronchoscopy with brushings and endobronchial cavitary biopsy were valuable methods for obtaining the infected tissue. Amphotericin B was effective when therapy was started early. Adequate levels of the drug were obtained by varying the dose and frequency of administration according to serum inhibitory titers. Control of infection was aided by immunologic monitoring at regular intervals.

Surgical aspects of pulmonary aspergillosis

Twenty-one cases of pulmonary aspergillosis managed surgically over a 13 year period in the Missouri State Chest Hospital were reviewed. Postoperative complications occurred in 11 patients (52 per cent) but were mostly manageable by conservative measures. There was one postoperative hospital death due to respiratory failure. With the lack of an effective and safe antifungal drug, resection is the treatment of choice for suitable candidates. In conclusion, some guidelines were drawn for the management of patients with this disease.

Pulmonary Aspergillosis Caused byAspergillus fischeri var. spinosus: Report of a Case and Value of Serologic Studies

This paper presents a case of pulmonary aspergillosis caused by Aspergillus fischeri var. spinosus, a rarely encountered organism. Serologic testing using complement fixation, immunodiffusion, and Immunoelectrophoresis showed a high degree of specificity for A. fischeri compared with other Aspergillus species.

Comparative study of quantitative serological reactions with a metabolic antigen of Aspergillus fumigatus

The following quantitative serologic reactions: agar-gel immunodiffusion, complement-fixation, opposite electrophoresis and latex particle agglutination tests have been performed in 38 sera from mycologically proved pulmonary aspergillosis cases. A metabolic antigen from a strain ofAspergillus fumigatus according toAjello et al technic modified by us, has been employed. Sera from 120 subjects suffering from non-mycotic lung conditions, as well as 10 sera from histoplasmosis cases, 10 sera from S. A. blastomycosis and 2 sera from patients with lung aspergillosis produced byA. niger, gave negative results with the above mentioned seroligic reactions. One hundred per cent of positive results were obtained with the complement-fixation test (titre ranging from 1/20 to 1/1280), agar-gel immunodiffusion test (titre up to 1/64) and the opposite immunoelectrophoresis (titre ranging from 1/2 to 1/256). Twenty five per cent negative and 4 non-specific results were registered with the latex particle agglutination test. A correlation of the number of serum precipition bands obtained by the electrophoresis technic with the titre of the quantitative serologic reactions, as well as a correlation of the titre of the circulating antibodies with the severity of the clinical form of aspergillosis seems to be present. Electrophoretic motility of the specific antibody performed in 10 sera showed results like the IgM in 1 instance and an intermediate position between IgA and IgG in 9 samples.

Sulfonamides and aspergillosis.

ABSTRACT To the Editor:— In The Journal (209:1191, 1969) a case of pulmonary aspergillosis is reported. A number of articles on systemic aspergillosis cite the various types of therapy during the past few years. None has mentioned the use of sulfonamide drugs.The studies and observations, made in the bacteriology laboratory of the New England Baptist Hospital, on a strain of Aspergillus isolated from a case of pulmonary aspergillosis noted that the organism was very sensitive to sulfonamides. A number of strains of Aspergillus were obtained and most of them were found to be sensitive to sulfonamides. We had hoped to test the clinical success of this therapy but have not had any cases of aspergillosis for treatment.We should like to report this experience in the hope that this type of therapy might be found useful. We realize that sensitivities on an agar plate might not reflect in vivo

Pulmonary aspergillosis in two families.

REPORTS of more than one case of pulmonary aspergillosis occurring in a family are rare. Hertzog, Smith & Goblin (1949) described a brother and sister aged 5 and 11 who died of pneumonia within a month of each other, necropsy in one of them showing multiple granulomata from which A. fumigatus was isolated. More recently, Strelling et al. (1966) reported two fatal cases of the invasive type of aspergillosis in two sisters, aged 4 years and 14 months respectively. In both cases the lungs were full of nodular lesions tending to become confluent, and A. funmigatus was cultured from lungs, and tracheobronchial lymph glands, which were enlarged. Because familial incidence of the allergic eosinophilic infiltration type of aspergillosis has not previously been reported, four such cases, occurring in two families are described.

Pulmonary aspergillosis occurring in a patient receiving large doses of prednisolone

A case of pulmonary aspergillosis developing in a woman on high doses of steroids is described. Its successful treatment is attributed to a course of amphotericin B and rapid reduction in steroid dosage. On décrit un cas d'aspergillose pulmonaire qui s'est développé chez une femme recevant un taux important de stéroïdes. Le succès thérapeutique est attribué à une cure d'amphotéricine B et à la diminution rapide des doses de stéroïdes. Se describe un caso de aspergilosis pulmonar en una mujer sometida a altas dosis de esteroides. Se atribuye el éxito terapéutico a la amfotericina B y a la rápida reducción de la dosis de esteroide.

Coexistence of pulmonary mycosis and amebic abscess of the liver.

Summary Autopsy and clinical findings on six cases of pulmonary aspergillosis associated with hepatic amebic abscess are reviewed. In no case was aspergillosis suspected on clinical grounds. The possible mechanisms involved in this uncommon association are discussed. A brief review of the literature concerning pulmonary aspergillosis is given.

SummaryA case of pulmonary aspergillosis is described. Clinical and pathological findings are reported. Besides the pulmonary lesions, endocarditis of the aortic valves had been diagnosed in this patient, a man 43 years old. Post mortem examination showed perforation of one of the aortic valves which was responsible of severe cardiac failure observed just, before death.The pulmonary disease looked as pseudo tuberculosis of the lungs.The authors discuss the primary origin of the pulmonary disease. Microscopic findings are described and illustrated abundantly.

A Case of Aspergillosis of the Lung

A case of pulmonary aspergillosis treated as pulmonary tuberculosis for many years has been discussed. The character of the sputum and the persistantly negative examination for tubercle bacilli led to repeated cultures of the sputum with the aspergillus fumigatus finally being recovered. Treatment with sulfathiazole and potassium iodide has produced complete cure within 5 months.
