Reliability and Construct Validity of the Nurses' Intention to Use Deep Vein Thrombosis Preventive Measures Questionnaire.

Valid and reliable measurements are paramount to advance the science of intensive care unit (ICU) nursing. This study aimed to evaluate the reliability and construct validity of the Nurses' Intention to Use Deep Vein Thrombosis Preventive Measures Questionnaire among critically ill patients in an Egyptian ICU. A cross-sectional, predictive design pilot-tested the psychometric properties of the questionnaire in a convenience sample of 78 RNs working in ICU. Construct validity was examined using factor analysis. The Kaiser-Meyer-Olkin measure of sampling was 0.71, and Bartlett's test of sphericity was significant (χ2 = 410.16, p < .05). Four factors were identified through factor analysis. Cronbach's α assessed the internal consistency reliability with a value of .82. Our analyses showed that the Nurses' Intention to Use Deep Vein Thrombosis Preventive Measures Questionnaire has good validity and reliability; however, further investigations will be completed.

Effects of Customer Orientation, Belief of Patient Activation and Professional Self-concept on Caring Behaviors of Nurses in Comprehensive Nursing Care Units

Objectives: The purpose of this study is to investigate the effects of customer orientation, professional self-concept and belief of patient activation on caring behaviors of nurses in comprehensive nursing care units.Methods: The data were collected from 141 nurses in the comprehensive nursing care units of the general hospital in D-City using structured questionnaire from August 1 to August 30, 2019. Statistical analysis was performed using the IBM SPSS Statistics 25.0 program.Results: In regression analysis, the higher customer orientation (β=0.49) and belief of patient activation (β=0.33), the more the work period in clinical careers in current unit (β=0.17) and the higher professional self-concept (β=0.15), the more the caring behaviors are affected. This analysis showed 81.3% explanatory power.Conclusions: This study reveals that caring behaviors of nurses in the comprehensive nursing care units can be enhanced through the improvement of the customer orientation, belief of patient activation and professional self-concept. Therefore, this study suggests that the development and application of education programs to enhance customer orientation, professional self-concept and belief of patient activation is required for increasing the quality of nursing care and patient satisfaction with caring behaviors.

Developing a Nursing Needs Assessment Scale and Patient Classification System based on Nursing Activities in Comprehensive Nursing Care Units

Purpose: This study was done to develop and validate a scale for assessing nursing needs on comprehensive nursing care units and to derive a patient classification system based on nursing needs.Methods: In this methodological study, the initial items were identified through a review of the literature and surveys from nursing staff regarding the nursing needs on comprehensive nursing care units. Content validity was evaluated by nine nursing staff members from comprehensive nursing care units. To evaluate the concurrent validity and derive a patient classification system, nursing needs scores, perceived nursing needs and perceived patient severity for 216 patient cases were evaluated by five nurses. These data were analyzed using Pearson‘s correlation coefficients, one-way ANOVA with Scheffépost hoc tests and K-means clustering.Results: After evaluating content validity, the developed scale contained 64 activities in two domains: nursing intervention and assistance of daily living. Concurrent validity was verified by analyzing the differences in the nursing needs scores according to each group of perceived nursing needs and severity (p&lt;.001) and by analyzing the correlation between the score of the developed scale and the National Health Insurance Service nursing need assessment scale (r=.68, p&lt;.001). Based on the score of the developed scale, a patient classification system that classified nursing needs into four stages was derived.Conclusion: The developed scale represented nursing activities in comprehensive nursing care units. It also provided specific data regarding the time spent on nursing activities. Therefore, it is expected to contribute toward establishing appropriate nurse staffing strategies to provide quality patient care.

간호간병통합서비스 병동 입원환자가 인지한 빠뜨린 간호와 간호서비스 만족도

본 연구는 간호간병통합서비스 병동에 입원한 환자가 인지한 빠뜨린 간호의 유형과 빠뜨린 간호에 따른 간호서비스 만족도의 차이를 파악하기 위해 시도하였다. 자료수집은 B광역시에 소재한 3개 종합병원의 간호간병통합서비스 병동에 입원한 환자로 2020년 10월 1일부터 2020년 11월 30일까지 142부를 수집하였다. 자료분석은 SPSS/WIN 21.0프로그램을 사용하였고, 본 연구의 결과는 다음과 같다. 간호서비스 만족도 평균은 3.39(±0.58)점이었고, 환자가 인지한 빠뜨린 간호는 ‘치료 관련 의논(28.9%)’, ‘구강간호(26.8%)’, ‘목욕 및 청결(26.7%)’, ‘담당간호사에 대한 정보(25.7%)’이었고 환자가 인지한 위해사건은 ‘피부손상/욕창(9.3%)’, ‘정맥수액이 피부로 새어 들어가 부어오름(6.2%)’, ‘투약오류(3.1%)’이었다. 환자가 경험한 빠뜨린 간호에 따른 간호서비스 만족도 정도는 ‘담당간호사에 대한 정보(t=2.28, p=.025)’, ‘치료에 대한 의논(t=3.43, p=.001)’, ‘검사나 시술에 대한 정보제공(t=4.42, p<.001)’, ‘치료나 질병에 대한 질문이나 걱정 잘 들어주기(t=3.29, p=.001)’, ‘구강간호(t=3.22, p=.002)’, ‘목욕 및 청결(t=4.41, p<.001)’, ‘침대에서 이동하기(t=4.79, p<.001)’, ‘보행보조(t=3.98, p<.001)’, ‘소리에 대한 간호사의 반응(t=2.99, p=.004)’이 통계적으로 유의한 차이가 있었다. 본 연구결과를 토대로 간호간병통합서비스 병동에서 환자가 인지한 빠뜨린 간호를 줄이고 간호서비스 만족도를 높이기 위한 다양한 교육과 프로그램의 개발 및 적용이 필요하다.

Covid-19 Effects on the Mental Workload and Quality of Work Life in Iranian Nurses.

Introduction:The mental health of people working in Covid-19 wards (nurses, doctors, etc.) may be compromised due to the specific conditions of the workplace and patients. Therefore, the aim of this study was to investigate the relationship between mental burden and quality of work life in nurses in intensive care units of Covid-19 patients.Method:In this cross-sectional study, a sample of 200 people—100 nurses in care units for patients with COVID-19 (group 1) and 100 nurses in non-COVID-19 patient care units (group 2—in three university hospitals were obtained. These 200 samples were randomly extracted from the list of employees and selected. Data were collected using three questionnaires, including (1) a demographic, (2) the NASA-Task Load Index (1988) (Hart & Staveland, 1988) and (3) National Institute for Occupational Safety and Health (NIOSH) Quality of Life. Data were analyzed using SPSS-24 software and descriptive and analytical statistical methods.Results:The overall mean scores of nurses’ quality of work life were significantly different between the two groups (P < 0.05). The average score of quality of life in nurses caring for patients with COVID-19 is 92.57, more than nurses caring for patients without COVID-19, 79.43. Among the dimensions of mental workload: Performance and efficiency, with an average score of 77.32 ± 15.85, had the highest score, while discouragement and failure, with an average score of 58.04 ± 26.72, had the lowest score of mental workload. There is a significant difference between the mental load of work in the two groups (P = 0.001). There is a significant inverse relationship between total quality of work life and total mental workload (r = –14 and P = 0.01).Conclusion:In this study, it was observed that nurses caring for Covid-19 patients are in a more unfavorable situation in terms of the studied characteristics. Due to the work period, these nurses have a high workload and a low quality of work life to compensate for the mental and physical deficiencies required by a long presence in the work environment.

ATUAÇÃO DO ENFERMEIRO NA PREVENÇÃO DE LESÃO POR PRESSÃO EM UNIDADE DE TERAPIA INTENSIVA

Pressure injury is caused by extensive exposure to intense pressure that can lead to a cracked skin. The aim of this study was to carry out an integrative review about the care of nurses in the prevention of Pressure Injury in Intensive Care Units. Twenty-seven scientific articles were evaluated in the databases: PubMed, Academic Google and LILACS, with descriptors: Pressure Injury, Nursing and Intensive Care Unit, in Portuguese, between 2017 and 2020. Pressure injuries represent challenges to the multidisciplinary team of health care in integrative care. Pressure injury damages the skin over bony prominences under pressure, friction, or shear. Intensive Care Unit patients are more likely due to physical and emotional weakness, poor mobility, enteral nutrition, use of sedatives and drugs. It was concluded that the nurse is the gold standard health professional in the management of clinical practices, developing strategies for the prevention and treatment of these injuries, playing a fundamental role in the care of patients with safety, protection and health recovery.

Delirium in adult patients in intensive care: nursing interventions.

Delirium is a neuropsychiatric syndrome of high incidence in the critically ill patient. It is characterised by changes in acute attention and cognition, has a multifactorial aetiology and has a negative impact on the patient's clinical situation and future quality of life. Prevention of delirium and early identification can reduce associated morbidity and mortality. Consequently, it is vital that intensive care unit (ICU) nurses perform targeted patient monitoring to identify acute cognitive changes. To identify nursing interventions directed at the prevention and management of delirium in adult patients in ICU. A scoping review was undertaken based on the principles recommended by the Joanna Briggs Institute. Seven studies were selected for inclusion. Non-pharmacological and pharmacological nursing interventions were identified. The interventions identified were predominantly aimed at the prevention of delirium. The training of nurses and wider clinical team in preventing and identifying this syndrome is crucial.

Minimising the number of cancellations at the time of a severe lack of postanesthesia care unit beds or nurses

A deficiency of the postanesthesia care unit (PACU) beds or nurses may cause delays in the operating rooms (ORs) and increase the number of cancellations. Some disruptions like the COVID-19 pandemic may cause this deficiency. This paper investigates two integrated OR and PACU scheduling problems; one with few PACU beds, and the other with few PACU nurses. For each problem, a mathematical model and a matheuristic are proposed for minimising the number of cancellations. To the best of our knowledge, it is the first study that investigates the implications of a severe lack of the PACU beds or nurses on the number of cancellations. The matheuristics hybridise the decomposition of each instance into some small-sized sub-instances with a variable neighbourhood search algorithm. The main advantages of these methods are their flexibility to incorporate many problem details (such as a step-wise demand for the PACU nurses) and to solve any large-scale problem. Numerical results for a data set with 22 ORs show that with an increasingly severe lack of PACU capacity there is progressively greater benefit of the matheuristics than their initial solutions. Moreover, these results show the influence of the overtime and the recovery in ORs on improving the situation.

Instrument Development: Knowledge, Attitudes, and Confidence in Palliative Care Concepts Held by Trauma and Neuroscience Intensive Care Nurses.

Unique pressures impact trauma intensive care unit (TICU) nurses in their provision of care for severely injured patients. When it becomes clinically obvious that these patients may not survive, TICU nurses must continue life-saving measures while at the same time consider a palliative care consultation. In order to facilitate this referral, TICU nurses need to have the appropriate knowledge, attitude, and confidence in doing so. The purpose of this study is to refine an instrument that aims to support this process. A convenience sample of 42 respondents completed the Knowledge, Attitudinal, and Experiential Survey on Advance Directive (KAESAD). Domains with the highest Cronbach's alpha value were "professional attitudes" (α = .995) and "clinical experiences" (α = .999). Reliability assessments suggest that most domains of the instrument have strong internal consistency, and with a larger sample size, future studies may elucidate how nurse educators can use this instrument to target areas for continuing education.

Intensivpflege in Zeiten der COVID-19 Pandemie: Zur Frage des Verh\xe4ltnisses von F\xfcrsorge und Selbstsorge

ZusammenfassungDie COVID-19 Pandemie stellt eine beträchtliche Herausforderung für die Kapazität und Funktionalität der Intensivversorgung dar. Dies betrifft nicht nur Ressourcen, sondern vor allem auch die körperlichen und psychischen Grenzen von Pflegefachpersonen. Der Frage, wie sich Fürsorge und Selbstsorge von Pflegefachpersonen auf Intensivstationen im Rahmen der COVID-19 Pandemie zueinander verhalten, wurde bislang im öffentlichen und wissenschaftlichen Diskurs keine Aufmerksamkeit geschenkt. Der vorliegende Beitrag reflektiert dieses Verhältnis mit Hilfe des Ethikkodex des International Council of Nurses, unter besonderer Berücksichtigung der Prinzipienethik und der Care-Ethik nach Joan Tronto und zeigt einen Korridor ethisch vertretbarer Pflege auf.Die Arbeit leistet damit einen wichtigen Beitrag zu einer differenzierten ethischen Betrachtung der Rechte und Verantwortlichkeiten von Pflegefachpersonen als moralischen Akteuren innerhalb des Pandemiegeschehens in Deutschland. Damit schafft er eine erste Voraussetzung für einen breiten gesellschaftlichen und politischen Diskurs, der über die Pandemie hinaus dringend notwendig ist, um die Situation der Pflegefachpersonen und der Gepflegten nachhaltig zu verbessern.

Etični konflikti med medicinskimi sestrami v intenzivni enoti

Uvod: Etični konflikt predstavlja resen poklicni problem. Medicinske sestre so na svojem delovnem mestu čedalje bolj obremenjene, še posebej v intenzivnih enotah, kjer je delo z življenjsko ogroženimi pacienti posebej težavno in je pojavnost etičnih konfliktov toliko večja. Namen je bil ugotoviti stopnjo izpostavljenosti etičnim konfliktom med medicinskimi sestrami v intenzivni enoti.Metode: Izvedena je bila kvantitativna presečna opazovalna raziskava. Vanjo je bilo vključenih 97 medicinskih sester, zaposlenih v intenzivnih enotah ene bolnišnice. Podatki so bili zbrani s pomočjo vprašalnika in statistično obdelani z uporabo univariatne statistike.Rezultati: Raziskava je pokazala, da so medicinske sestre v intenzivnih enotah zmerno izpostavljene etičnim konfliktom (x = 138,85, s = 89,08). Najpogosteje doživljajo etični konflikt ob situacijah, ko primanjkuje virov,ki zdravstvenemu timu omogočajo upoštevanje etičnih smernic za delovanje (x = 14,11, s = 9,13). Najmanj so izpostavljene etičnim konfliktom, ko je treba v okviru kliničnega preizkušanja dajati zdravila in pri tem ne prejmejo vseh potrebnih informacij, ki so po njihovem mnenju potrebne za izvajanje te naloge (x = 2,25, s = 3,70).Diskusija in zaključek: Pri pojavnosti stopnje etičnega konflikta pri medicinskih sestrah je bilo ugotovljeno, da se srečujejo s konflikti, ki so rahlo problematični in precej problematični. Največji etični konflikt se pojavi pri pomanjkanju časa za upoštevanje etičnih smernic za delovanje, kar kaže, da je medicinskih sester premalo za kakovostno oskrbo pacientov.

Exploring Professional Nurses’ Use of the Nursing Process at Selected Public Hospitals in Limpopo, South Africa

The nursing process is regarded as a systematic approach in which professional nurses use problem-solving to identify, plan and implement nursing interventions to enhance quality patient care. The steps of the nursing process include assessment, nursing diagnosis, planning, implementation and evaluation. However, nurses’ effective and efficient use of the nursing process steps in public hospitals still poses a challenge due to several factors. The current study is significant to nursing practice as the nursing process remains a standard of nursing practice for hospitals to provide quality patient care. The purpose of the study was to explore professional nurses’ use of the assessment and implementation steps of the nursing process at selected public hospitals in the Vhembe District, Limpopo, South Africa. A qualitative, phenomenological, descriptive, explorative and contextual research design was used. Non-probability purposive sampling was used to select professional nurses who were using the nursing process in the nursing care units until data saturation was reached with 13 participants. The data was collected using semi-structured one-on-one interviews using an interview guide, and then analysed using Tesch’s open-coding method where themes and sub-themes emerged. The study found that the participants’ initial assessment of patients during admission is an achievable performance, while implementation of the set nursing intervention is still a challenge on different levels. It is recommended that a problem-based learning approach be made a compulsory practical teaching method in order to equip nurses with the knowledge of the nursing process at all nursing education institutions.

MEASURING KNOWLEDGE OF JORDANIAN NURSES WORKING IN CRITICAL CARE UNITS TOWARD STROKE PATIENTS

Background. Stroke is a devastating disease. It is a major cause for the neurological admission to hospitals all over the world. Limited knowledge among the critical care nurses about stroke in general and specifically about the risk factors, signs, and symptoms of stroke usually is a main source of delayed prompt stroke management and non-compliance with follow-up rehabilitation. Therefore, there is a need for a study that examines the impact of these factors in order to promote stroke management and improve nursing care outcomes.Aim. This study aimed at measuring the knowledge of Jordanian nurses working in critical care units toward stroke patients.Methods. This cross-sectional study used the descriptive approach in order to measure the knowledge of the Jordanian nurses working in critical care units regarding stroke patients in the Jordanian hospitals. Data were collected from Jordanian critical care units' nurses from seven hospitals; five private and two public hospitals. Critical care units’ nurses were selected conveniently based on specific inclusion criteria. Eligible participants were required to complete self–reported questionnaires about knowledge in addition to completing demographic questionnaires. The descriptive and inferential statistics were conducted using the SPSS software. Results. A total of (200) Critical care units’ nurses from public and private hospitals participated in the study. The nurses in this study exhibited poor knowledge on the study scales. There were statistically significant differences among nurses according to the type of hospital on the one scales (P&lt; .05). There is a negative relationship between the knowledge and years of nursing practice in ER or ICU (P= .013).Conclusions. The measures of knowledge among the nurses in critical care units in the Jordanian hospitals towards stroke patients seem to be highly poor. Nurses in critical care units seem to have acceptable information, but inadequate to correctly enhance stroke awareness. There is a gap that should be stuffed via planning and implementation of educational and instructional programs focused on hospital nurses as well as community sectors in order to improve the stoke focus and experience and avoid the delay in accessing the medical help which would, in return, improve stroke management and reduce its effect in Jordan.

Development of a high-risk neonatal infection control competency scale.

The purpose of this study was to develop a scale to measure high-risk neonatal infection control competency among nurses in neonatal intensive care units (NICUs). The study participants included 251 nurses working in NICUs at seven hospitals in South Korea. The scale was designed to measure high-risk neonatal infection control competency in terms of performance (HirNICCS_P) and knowledge (HirNICCS_K), which were addressed by 42 and 54 items, respectively, and was initially subjected to content validity testing by a panel of experts in neonatal nursing. The items were divided into five factors: (1) basic care, (2) skin care, (3) feeding management, (4) medication and invasive procedures, and (5) environment management. After participant completion of the scale, construct validity was examined using exploratory and confirmatory factor analyses, and internal reliability tests were also performed. The total variance of validity was 53.7% for HirNICCS_P, and the Cronbach's alpha values for reliability were .95 for HirNICCS_P and .67 for HirNICCS_K based on a Kuder-Richardson-20 test. We concluded that this scale can be used to assess the performance and knowledge with regard to infection control competency among neonatal nurses and that its application can support strengthening of infection control education for nurses who score low on performance and knowledge competency.

Assessment and Management of Delirium in Critically Ill Veterans.

Delirium is a complex syndrome prevalent in the intensive care unit. It has been associated with significant morbidity including distress, longer hospital stays, prolonged cognitive impairment, and increased mortality. To describe a nurse-led interdisciplinary quality improvement initiative to increase nurses' knowledge of delirium, documentation of delirium assessment, and patient mobility. Sixty-seven nurses in medical and surgical intensive care units were required to attend an interactive education program on delirium assessment and management. Scores on tests taken before and after the education program were used to evaluate knowledge. Medical records and bedside rounds were used to validate Confusion Assessment Method for the Intensive Care Unit documentation and interventions. Descriptive statistics were used to describe changes over time. A delirium resource team composed of nurses, physicians, and therapists provided didactic education paired with simulation training and bedside coaching. Mobility screening tests and computer templates guided assessments and interventions. Documentation of the Confusion Assessment Method improved from less than 50% to consistently 99%. Mobilization in the surgical intensive care unit increased from 90% to 98% after intervention. Days of delirium significantly decreased from 51% before intervention to 31% after intervention (χ12=7.01, P = .008). The success of this quality improvement project to enhance recognition of delirium and increase mobility (critical components of the pain assessment, breathing, sedation choice, delirium, early mobility, and family education bundle) was contingent on nursing leaders hip, interdisciplinary team collaboration, and interactive education.
