Low Birth Weight and Related Metabolic Risk Factors, Cardio-Respiratory Fitness and Physical Activity in Adolescents

Background/Objectives: The associations between low birth weight (LBW) and the aggregation of metabolic risk factors (MRF) in youth remain ambiguous. Thus, this study analysed the interrelationship among MRF, LBW, and behavioural factors in adolescents. Methods: The sample of the present cross-sectional study comprised 491 youth (229 males, 262 females) aged 14–17 years. Height, weight, and BMI were assessed. Cardiorespiratory fitness (CRF) was measured using the PACER test. Physical activity (PA) was evaluated using a 3-day diary. A MRF risk score was calculated using the Z-scores of the five MRF criteria (HDL-C, triglycerides, BP, insulin, and blood glucose). Results: The average values for height, weight, and systolic and diastolic BP were significantly higher in males (p < 0.01); in contrast, females exhibited higher HDL cholesterol and insulin levels (p < 0.01). Boys revealed higher levels of PA (p < 0.01), and they achieved better CRF scores than girls (p < 0.01). BMI emerged as a significant predictor of clustered metabolic risk for both males (β = 0.26; 95% CI, 0.16–0.36) and females (β = 0.02; 95% CI, 0.14–0.30); additionally, the results indicated that more physically active girls had a notably lower metabolic risk (β = −0.01; 95% CI, −0.10–−0.01) than their male peers. Conclusions: LBW was found to be independently correlated with the aggregated MRF (β = −0.01; 95% CI, −0.01–0.00) among boys aged 14–17 years.

Proteomic analysis of cardiorespiratory fitness for prediction of mortality and multisystem disease risks

Despite the wide effects of cardiorespiratory fitness (CRF) on metabolic, cardiovascular, pulmonary and neurological health, challenges in the feasibility and reproducibility of CRF measurements have impeded its use for clinical decision-making. Here we link proteomic profiles to CRF in 14,145 individuals across four international cohorts with diverse CRF ascertainment methods to establish, validate and characterize a proteomic CRF score. In a cohort of around 22,000 individuals in the UK Biobank, a proteomic CRF score was associated with a reduced risk of all-cause mortality (unadjusted hazard ratio 0.50 (95% confidence interval 0.48–0.52) per 1 s.d. increase). The proteomic CRF score was also associated with multisystem disease risk and provided risk reclassification and discrimination beyond clinical risk factors, as well as modulating high polygenic risk of certain diseases. Finally, we observed dynamicity of the proteomic CRF score in individuals who undertook a 20-week exercise training program and an association of the score with the degree of the effect of training on CRF, suggesting potential use of the score for personalization of exercise recommendations. These results indicate that population-based proteomics provides biologically relevant molecular readouts of CRF that are additive to genetic risk, potentially modifiable and clinically translatable.

Adolescent mental health and cardiorespiratory fitness: A comparison of two cohorts 12 years apart.

The aim of the study was to compare the mental health and cardiorespiratory fitness (CRF) of adolescents in two cross-sectional cohorts, one measured in 2003 and the other in 2015, both at age 15 and across sexes. The study also sought to estimate the association between mental health and CRF in the two cohorts and examine the relationship between the level of CRF and mental health in each cohort overall and by sex. Data from 443 participants born in 1988 (228 males, 215 females) and 303 participants born in 1999 (126 males, 177 females) were analyzed. Mental health was assessed using self-reports of body image, self-esteem, and symptoms of depression and anxiety. CRF was estimated using a maximal cycle ergometer test. From 2003 to 2015, body image scores improved (p = .043), self-esteem remained stable, and CRF declined significantly (p < .001). No self-esteem differences were observed between sexes in any cohort. Males had higher CRF and body image scores than females in both cohorts (p < .001 for all comparisons). Higher CRF correlated with fewer depressive symptoms across sexes and cohorts. Specifically, higher CRF was associated with anxiety in females and improved body image in males (2003) and both sexes (2015). Increased CRF was linked to higher self-esteem in females but not in males. Overall, higher CRF levels were associated with better mental health outcomes for both sexes. These results highlight the potential of improving adolescent mental health through increased physical fitness.

Health-related physical fitness, physical activity and its correlates among school going adolescents in hilly state in north India: a cross sectional survey

IntroductionHealth-related physical fitness, which includes body composition, cardiorespiratory fitness, muscular endurance, flexibility, power, and strength are associated with risks of chronic diseases and promote good health and wellness. There have been reports of increasing levels of physical inactivity among children and adolescents, leading to increasing rates of obesity and decreased physical fitness. The present study was conducted among school going adolescents to estimate the levels and correlates of PF for timely intervention.MethodologySchool based cross-sectional study was done among students of class 8-11th in Government schools of Garhwal division of Uttarakhand. Multistage stratified random sampling was applied for recruitment of study participants. We recruited a final sample size of 634 students. Validated questionnaires and standard methods for assessment of physical fitness, physical activity levels and other variables such as waist circumference, hip circumference, BMI and hemoglobin estimation were done.ResultsAverage and above average cardiorespiratory fitness score as per Harvard step test among boys (54.3%) was significantly higher as compared to girls (21.3%) (χ2 = 88.93, p < 0.001). There was a significant association between gender and dominant handgrip strength (χ2 = 8.02, p = 0.01) as well as between gender and Shoulder stretch test (SST) of dominant (χ2 = 17.5, p < 0.05) as well as nondominant arm (χ2 = 13.5, p < 0.05). Sit and reach test results also showed a significant association with gender (χ2 = 27.17, p < 0.001). Gender, hemoglobin level, BMI and PAL scores significantly predicted cardiorespiratory fitness scores (R2 = 0.188, F value of the model = 37.69, p =< 0.001)).ConclusionPhysical fitness of school going adolescents in Garhwal division of Uttarakhand was better than other parts of India, with significant gender differences. Physical activity levels (PAL) were poor and are also a significant predictor of physical fitness. More emphasis needs to be paid on the health and fitness of girl students. School based policies to increase PAL among students through innovation and rewards may go a long way in improving the long-term health of the students.

Mediation role of cardiorespiratory fitness on association of physical activity and physical literacy among 8-12 years old children: the PAK-IPPL cross-sectional study.

The effect of cardiorespiratory fitness (CRF) on the relationship of physical activity (PA), and physical literacy (PL) in 8-12 Pakistani children are largely unknown. Therefore, this study aims to examine the mediating role of CRF in the relationship between PA and PL in this demographic. This cross-sectional study involved 1,360 children aged 8-12 (mean age = 10.00, SD = 1.41 years) from 85 higher secondary schools in South Punjab, Pakistan. Data were collected during the 2020-2021 academic year using the Canadian Assessment of Physical Literacy-2 (CAPL-2) protocol to assess physical activity (PA), Body Mass Index (BMI), and PL levels. CRF was measured using the PACER (Progressive Aerobic Cardiovascular Endurance Run) test. Descriptive statistics, independent samples t-tests, Chi-squared tests, and Generalized Additive Models for Location, Scale, and Shape (GAMLSS) were used for age and sex-specific CAPL-2 scoring. Associations among components were evaluated through Pearson's correlation, multivariate logistic regression, and mediation analyses. The study revealed that boys had significantly higher scores in CRF, PA, and PL across all age groups compared to girls (p < 0.001), with boys' scores being 20%, 10%, and 14% higher, respectively, than those of girls. Conversely, overweight children showed significantly reduced PA and PL levels (p < 0.001). An inverse correlation was found between BMI and CRF (r 2 linear = 0.022; quadratic = 0.028). CRF scores had significant negative correlations BMI (r = -0.143) and positive associations with MVPA, PA, DB, and PL (r ranging from 0.241 to 0.624). CRF was observed to partially mediate the association between MVPA and PL. The direct impact of PA on PL was significant and meaningful (β = 0.002, p < .001). Additionally, the indirect effect of PA on PL through CRF was also significant (β = 0.001, p < .001), indicating that CRF serves as an important mediator in this relationship. The combined total effect of PA on PL, which includes both direct and mediated pathways, was robust and highly significant (β = 0.003, p < .001). The study revealed a strong positive correlation between CRF, PA, and PL, but a negative one with BMI in South Punjab children aged 8-12. Notably, CRF and PA emerged as significant predictors of PL levels in this population. Consequently, interventions that are both systematic and targeted towards improving these factors should be implemented as strategies to enhance children's PL levels and promote physically active behaviors.

Can We Predict Cardiorespiratory Fitness From Body Composition and Core Symptoms in People With Schizophrenia?

<h3>Research Objectives</h3> Cardiorespiratory fitness (CRF) can be directly measured and assessed by the cardiopulmonary exercise test (CPET) or estimated from different field tests such as the Modified Shuttle Walking Test (MSWT). The CRF in population with schizophrenia (SP) may be altered due to sex, age, body composition, and core symptoms variables. The objective was to analyze the predictive value of body composition and core symptoms in SP for CRF. <h3>Design</h3> Prospective, randomized, single-blinded (by psychiatrists to the patient group) comparison of SP patients. <h3>Setting</h3> Hospital and clinic records. A psychiatric hospital and a mental health network. <h3>Participants</h3> Participants (N = 122, 41.7 ± 10.3 yr old) adults with SP. <h3>Interventions</h3> Baseline data pre-intervention. <h3>Main Outcome Measures</h3> Body composition (body mass index and fat percentage), CRF (direct by cardiopulmonary exercise test with ramp protocol on a bicycle ergometer, and the MSWT), and positive and negative symptoms of the disease (Positive and Negative Syndrome Scale, PANSS; and "The Brief Negative Symptom Scale", BNSS). Stepwise Multiple Regression analyses were performed to determine which variables predict CRF scores. <h3>Results</h3> Lower negative symptoms (P< 0.001) and positive PANSS (P=0.035) predicted peak oxygen uptake (V̇O2peak) (L·min−1) (R2=28.3%). Lower negative symptoms (P< 0.001), positive PANSS (P=0.006), and fat body mass (P< 0.001) explained V̇O2peak (mL·kg−1·min−1) (R2=46.5%). MSWT was predicted (R2=58.9%) by lower negative symptoms (P=0.001), body mass (P< 0.001), and total PANSS (P=0.004). <h3>Conclusions</h3> In people with SP significantly higher CRF was detected in those with lower negative and positive symptoms, as well as lower body mass. Exercise interventions for improving CRF should be promoted in this population for better control of core symptoms. <h3>Author(s) Disclosures</h3> Absence of conflicts of interest.

Effect of Excessive Screen Time on Cardiorespiratory Fitness in Children: A Longitudinal Study.

Background: This study investigated the effect of different components of screen time (mobile phone use, TV/video viewing, and video gaming) on cardiorespiratory fitness (CRF) development in children aged 9–12 years. Methods: This was a two-year longitudinal study conducted with 175 children (49.7% girls, mean age = 9.5) in Japan. CRF was assessed using a 20 m shuttle run test conducted at baseline and again at follow-up. Children were categorized as “Good” or “Poor” based on the change in CRF scores for each gender. Screen time was assessed using a self-reported questionnaire at baseline and termed as “high” if children reported ≥ 2 h/day. Univariate and multivariate logistic regression analyses were performed after adjusting for gender, physical activity, and time of data collection. Results: Children scoring “high” on mobile phone use had lower odds of being categorized as “Good” in CRF change (crude odds ratio (OR): 0.34; 95% confidence interval (CI): 0.15–0.90 (adjusted OR: 0.33; 95% CI: 0.12–0.91)). There were no significant effects of TV/video viewing (crude OR: 1.54; 95% CI: 0.84–2.81) and video gaming (crude OR: 0.98; 95% CI: 0.48–1.97) on changes in CRF. Conclusions: Limiting excessive mobile phone usage might be important for ensuring healthy development of CRF in children.

Moderate-to-Vigorous Physical Activity Is Associated With Cardiorespiratory Fitness Among Primary Schoolchildren Living in Côte d'Ivoire, South Africa, and Tanzania.

Background: Physical inactivity and low cardiorespiratory fitness (CRF) are independent cardiovascular risk factors among children, but have rarely been investigated concurrently in sub-Saharan Africa. The purpose of this study was to compare physical activity (PA) and CRF of primary schoolchildren living in Côte d'Ivoire (CI), South Africa (ZA), and Tanzania (TZ), to test sex- and age-related differences, and to examine whether PA and CRF are associated with each other.Methods: Baseline data from an ongoing cluster-randomized controlled trial were used, including 499 children from CI (Taabo, 49% girls, M = 8.0 ± 1.6 years), 1,074 children from ZA (Gqeberha, 49% girls, M = 8.3 ± 1.4 years), and 593 children from TZ (Ifakara, 51% girls, M = 9.4 ± 1.7 years). PA was assessed by accelerometry and CRF by a 20 m shuttle-run test. The data were analyzed using multi-/univariate analyses of variance and mixed linear models.Results: Most children met recommendations put forward by the World Health Organization for moderate-to-vigorous PA (MVPA) and achieved high CRF scores. In CI, 89.6% of the children met MVPA recommendations (boys: 91.7%, girls: 87.4%), whereas this rate was 76.9% in ZA (boys: 91.0%, girls: 62.4%), and 93.8% in TZ (boys: 95.5%, girls: 92.0%). Children from TZ had the highest CRF and MVPA levels, followed by children from CI and ZA. Boys had higher MVPA levels than girls, whereas girls engaged in more sedentary behavior. Sex differences were strongest in ZA. Sedentary behavior and MVPA were higher among older schoolchildren compared to their younger peers. Higher MVPA, but not sedentary behavior, was associated with better CRF.Conclusions: In all three settings, higher levels of MVPA were associated with higher CRF scores. Nevertheless, children living in the most urbanized setting (such as observed in ZA) were physically less active and had lower CRF than peers living in more rural areas (such as observed in CI and TZ). Particularly for girls, urbanization might increase the risk for insufficient MVPA, which may have negative effects on their CRF, thus negatively influencing health and well-being at later age.

The Association between Cardiorespiratory Fitness and Reported Physical Activity with Sleep Quality in Apparently Healthy Adults: A Cross-Sectional Study.

Background: Recently, poor cardiorespiratory fitness (CRF) has been postulated as an adverse health outcome related to poor sleep quality. However, studies investigating the relationship between CRF and a subjective sleep quality index are scarce. Thus, the current study aimed to investigate the association between CRF and the Pittsburgh Sleep Quality Index (PSQI) in apparently healthy people. The secondary aim was to investigate the association between reported physical activity (PA) and PSQI. Methods: Thirty-three healthy male participants volunteered to participate. CRF (VO2PEAK) was measured via cardiopulmonary exercise testing on a treadmill. A short form of the International Physical Activity Questionnaire (IPAQ) was used to measure PA, and PSQI was used for the sleep quality index. Results: There was no correlation between CRF and PSQI total score or any component of the PSQI. There was a significant inverse correlation between IPAQ and PSQI total score (r = −0.36, p = 0.04). Categorical data analysis of the two questionnaires revealed that 42.4% of the participants who reported low physical activity also had poor sleep quality. Conclusions: The current study showed no association between CRF and the subjective sleep quality index but demonstrated a moderate inverse association between reported PA and subjective sleep quality index. The findings suggest that the more reported PA, the better the overall sleep quality.

Reciprocal Longitudinal Relationship Between Fitness, Fatness, and Metabolic Syndrome in Brazilian Children and Adolescents: A 3-Year Longitudinal Study.

To verify the reciprocal longitudinal relationships between cardiorespiratory fitness (CRF), percentage body fat (%body fat), and metabolic syndrome in Brazilian primary school students. This longitudinal study involved 420 children and adolescents followed for 3 years (2011-2014). The continuous Metabolic Syndrome (cMetSyn) score was calculated by summing adjusted z scores of glucose, systolic blood pressure, total cholesterol/high-density lipoprotein cholesterol ratio, triglycerides, and waist circumference. The CRF was assessed using running/walking tests, and %body fat was assessed through sex-specific 2-site skinfold thickness. Cross-lagged panel models were used to analyze longitudinal reciprocal relationships between CRF and %body fat with cMetSyn. Results indicated that 2011 %body fat significantly predicted both 2014 CRF scores and 2014 cMetSyn scores (P < .001); however, 2011 CRF only predicted 2014 %body fat (P < .001) but not 2014 cMetSyn (P = .103). Furthermore, 2011 cMetSyn predicted 2014 %body fat (P = .002). The model explained 36%, 48%, and 37% of the variance in 2014 CRF, %body fat, and cMetSyn, respectively. The results suggest a reciprocal inverse relationship between %body fat and metabolic syndrome risk and that %body fat may play a more important role in the risk of developing metabolic syndrome compared with CRF.

Diverging trends for onset of acute myocardial infarction, heart failure, stroke and mortality in young males: role of changes in obesity and fitness.

As opposed to the decreasing overall rates of coronary heart disease (CHD) incidence and overall cardiovascular disease (CVD) mortality, heart failure (HF) and stroke incidence are increasing in young people, potentially due to rising rates of obesity and reduced cardiorespiratory fitness (CRF). We investigated trends in early major CVD outcomes in a large cohort of young men. Successive cohorts of Swedish military conscripts from 1971 to 1995 (N=1,258,432; mean age, 18.3years) were followed, using data from the National Inpatient and Cause of Death registries. Cox proportional hazard models were used to analyse changes in 21-year CVD event rates. 21-year CVD and all-cause mortality and incidence of acute myocardial infarction (AMI) decreased progressively. Compared with the cohort conscripted in 1971-1975 (reference), the hazard ratios (HRs) for the last 1991-1995 cohort were 0.50 [95% confidence interval (CI) 0.42-0.59] for CVD mortality; 0.57 (95% CI 0.54-0.60) for all-cause mortality; and 0.63 (95% CI 0.53-0.75) for AMI. In contrast, the incidence of ischaemic stroke, intracerebral haemorrhage and HF increased with HRs of 1.43 (95% CI 1.17-1.75), 1.30 (95% CI 1.01-1.68) and 1.84 (95% CI 1.47-2.30), respectively. During the period, rates of obesity increased from 1.04% to 2.61%, whilst CRF scores decreased slightly. Adjustment for these factors influenced these secular trends only moderately. Secular trends of young-onset CVD events demonstrated a marked shift from AMI and CVD mortality to HF and stroke incidence. Trends were significantly, though moderately, influenced by changing baseline BMI and CRF.

Cardiorespiratory effects of project energize: a whole-of region primary school nutrition and physical activity programme in New Zealand in 2011 and 2015

BackgroundSince 2004, Sport Waikato has delivered Project Energize, a through-school nutrition and physical activity program to primary schools in the Waikato. As part of the program’s continued assessment and quality control, the programme was evaluated in 2011 and 2015. This paper’s aim was to compare the cardiorespiratory fitness (time to run 550 m (T550)) levels of children participating in Project Energize in 2011 and 2015.MethodsIn the 2011 evaluation of Project Energize, gender specific- T550-for-age Z scores (T550AZ) were derived from the T550 of 4832 Waikato children (2527 girls; 2305 boys; 36% Māori) aged between 6 and 12 years. In 2015, T550 was measured for 4798 (2361 girls; 2437 boys; 32% Māori) children, representative of age, gender and school socioeconomic status (SES). The T550AZ for every child in the 2015 study and 2011 evaluation were derived and differences in T550 between 2015 and 2011 by gender, SES and age were determined using independent t-tests. Multiple regression analysis predicted T550 Z score and run time, using year of measurement, gender, ethnicity, age and school SES.Results With and without adjustment, children in 2015 ran 550 m faster than in 2011 (adjusted Z score 0.06, time 11 s). Specifically, girls ran at a similar speed in 2015 as 2011 but boys were faster than in 2011 (Z score comparison P < 0.001, mean difference 0.18 95%CI 0.12, 0.25). Regression analysis showed time taken to run 550 m was 11 s less in 2015 compared with 2011. Boys ran it 13 s faster than girls (Z score 0.07) and for each 1 year age increase, children were 8 s slower (Z score 0.006). For each 10% decrease in SES, children were 3 s slower (Z score 0.004) and Māori children were 5 s slower than Non-Māori children (Z score 0.15).ConclusionsThe findings from this study support the continuation of the delivery of Project Energize in the Waikato region of New Zealand, as cardiorespiratory fitness scores in 2015, compared to 2011, were improved, particularly for lower SES schools and for Māori children. Ethnically diverse populations, schools with higher deprivation and girls, continue to warrant further attention to help achieve equity.

S87. IDEAL CARDIOVASCULAR HEALTH IN RACIALLY AND ETHNICALLY DIVERSE PEOPLE WITH SERIOUS MENTAL ILLNESS

BackgroundCardiovascular disease (CVD) is a primary contributor to premature death among people with serious mental illness (SMI). This study examined the prevalence and correlates of the American Heart Association (AHA) metric of ideal cardiovascular health (ICVH) in racially and ethnically diverse people with serious mental illness living in supportive housing.MethodsOur study used baseline data from an NIMH-funded trial testing the effectiveness of a peer-led healthy lifestyle intervention, for overweight/obese participants with SMI in three supportive housing agencies. A total of 314 participants were enrolled in the trial and included in these analyses. Five ICVH metrics (BMI, smoking, diet, physical activity, and blood pressure [BP]) were measured and summed to create a composite ICVH score. Correlates were informed by findings from systematic literature reviews examining ICVH in the general population and studies examining correlates of CVD in people with SMI. Hierarchal regression analysis was used to examine the associations of sample correlates with the composite ICVH score.ResultsThe mean age of participants was 48.7 and 57.3% were male. The most common lifetime mental health diagnoses were depression (75.2%), schizophrenia/schizoaffective disorder (56.7%), and bipolar disorder (46.5%). Approximately 38.5% of participants reported lifetime substance abuse/dependence. The majority (62.7%) were taking an antipsychotic medication. Lifetime physical health diagnoses were high cholesterol (36.3%), diabetes (32.5%), cardiovascular disease (17.2%), and cancer (4.5%). Participants walked on average 318.4 meters during the six-minute walking test (6MWT), a measure of cardiorespiratory fitness (CRF). The prevalence of smoking (64.7%) and obesity (64%) were high, while the prevalence of ideal physical activity (37.6%), healthy diet status (2.2%), and ideal BP (23.6%) were low. The mean ICVH composite score was 3.15 (range 0 – 8). After controlling for all covariates, women, racial/ethnic minorities, use of antipsychotic medications, lifetime cancer diagnosis, and poor CRF were significantly (p < .05) related to low ICVH scores.DiscussionIn our racially/ethnically diverse sample of people with SMI in supportive housing, the prevalence of ICVH was low. Our findings suggest that there are specific subgroups that may benefit from targeted screening and interventions to improve cardiovascular health. The association between CRF and ICVH scores highlights the importance of increasing physical activity and reducing sedentary behavior. The AHA ICVH metric can be a useful tool for tracking and improving the cardiovascular health of people with SMI.

Normative-referenced percentile values for physical fitness among Canadians.

This study developed age- and sex-specific normative-referenced percentile values for five physical fitness tests across a wide age range of Canadians, using a nationally representative sample. The data are from 5,188 Canadians (50.1% female) and were collected as part of cycle 5 of the Canadian Health Measures Survey (2016 to 2017). Males had slightly better cardiorespiratory fitness and substantially better grip strength, jumping height and jumping power scores than females, whereas females had better sit-and-reach flexibility. Among females, there were pronounced increases in jumping height (P50: 25%) and jumping power (P50: 58%) between ages 8 and 13, and in grip strength (P50: 193%) between ages 6 and 19. Performance gradually declined with age, beginning in adolescence for jumping ability and at approximately age 35 for grip strength. Among males, there were pronounced increases in jumping height (P50: 69%) and jumping power (P50: 233%) between ages 8 and 20, and in grip strength (P50: 365%) between ages 6 and 20. Performance gradually declined with age, beginning immediately after adolescence for jumping ability and at approximately age 30 for grip strength. Sit-and-reach flexibility remained relatively stable with age in both sexes. Cardiorespiratory fitness scores in both sexes declined steadily with age beginning (generally) at age 8, with a larger decline evident in females until age 18. These normative-referenced values for physical fitness could be useful for screening in public health and clinical practice.

The impact of environmental tobacco smoke exposure on cardiorespiratory fitness in children: A pilot study

Environmental tobacco smoke (ETS) in indoor air is a substantial risk factor for many health issues. Children are particularly susceptible to ETS with increased risk of asthma attacks, respiratory infections, and sudden infant death syndrome. The health effects of ETS are well researched in adults, but few studies examine the impact on children’s cardiorespiratory fitness (CRF). CRF has been shown to be a useful biomarker for monitoring health effects which would normally be too subtle to identify at rest. In adults, ETS has been shown to reduce CRF, and children may be at greater risk due to high respiration rates and developing organs. This preliminary research tests the hypothesis that ETS has a detrimental impact on CRF in children. Twenty-five children (9-11 yrs) from one Merseyside primary school were recruited. ETS exposure was determined by parental surveys and coupled with children’s exhaled carbon monoxide concentration. CRF was determined using a VO2peak test, with lung function assessed using standard spirometry, and fractional exhaled nitric oxide (FeNO) provided an indication of lung inflammation. Initial results show children exposed to ETS had statically lower CRF scores (p = 0.048) and were more likely to be classified as ‘unfit’ compared to children not exposed. A negative correlation was found between the number of cigarettes smoked at home and children’s CRF (r = - 0.526, p = 0.008), suggesting a possible dose-response relationship. Spirometry and FeNO values were not statistically different between groups. Results indicate that ETS exposure is likely to be detrimental to children’s CRF. They highlight the need for further work, on a larger dataset that will allow more robust analysis with greater statistical power. To the author’s knowledge, this study is the first of its kind to use laboratory-based fitness measurements to explore associations between ETS and CRF in children.

Test method for health-related physical fitness of college students in mobile internet environment

Improving health-related physical fitness plays an important role in enhancing the comprehensive physical fitness of college students. The traditional test of health-related physical fitness is characterized by complicated operations and low efficiency. A set of test methods for health-related physical fitness of college students based on an intelligent mobile terminal is designed in this work. The intelligent test method calculates the health-related physical fitness level of users through image acquisition and analysis by combining intelligent terminal software and test items of key elements. To verify the validity of the proposed method, a total of 116 college students (59 males and 57 females) aged between 18 and 22 are chosen as test respondents. The health-related physical fitness of respondents is tested and compared by using the traditional and proposed methods. The traditional test method reports the cardiorespiratory fitness scores of 69.6 ± 9.5 (M) and 77.1 ± 9.9 (F), and the proposed method reports 70.9 ± 9.7 (M) and 77.7 ± 9.8 (F). Twenty samples are chosen randomly to calculate the correlation coefficients r = 0.944 (M) and 0.965 (F) and significance p = 0.145 (M) and 0.489 (F). The flexibility fitness scores in the traditional method are 74.6 ± 11.9 (M) and 73.3 ± 11.5 (F), and the scores of the proposed method are 74.8 ± 11.5 (M) and 75.0 ± 11.2 (F). Twenty samples are chosen randomly to calculate r = 0.944 (M) and 0.938 (F) and p = 0.941 (M) and 0.098 (F). In the proposed method, muscle strength/muscular endurance fitness and a body composition module are tested by the traditional method, and test data are input into an artificial input system. The experimental results verify the accuracy of the proposed method in evaluating the health-related physical fitness of college students. Application of the proposed method can effectively reduce the cost of physical fitness testing, increase the convenience of testing, and direct substantial attention to the health-related physical fitness of college students.

The Relationship between Cardiorespiratory Fitness and Montreal Cognitive Assessment Scores in Older Adults

Background: Relatively little is known regarding the association between objective measures of physical function such as cardiorespiratory fitness (CRF) and cognitive function tests in healthy older adults. Objective: To evaluate the relationship between CRF and cognitive function in adults aged 55 and older. Methods: Between 2008 and 2017, 4,931 men and women underwent a comprehensive preventive physical exam at the Cooper Clinic in Dallas, Texas. CRF was determined by duration of a maximal treadmill exercise test. Cognitive function was evaluated with the Montreal Cognitive Assessment (MoCA). In a multivariate model, adjusted odds ratios with 95% confidence intervals for MoCA scores < 26 (i.e., cognitive impairment) were determined by using CRF as both a continuous and a categorical variable. Results: The mean age of the sample was 61.0 ± 6.0 years; mean maximal MET values were 10.0 ± 2.2. Mean MoCA scores were 26.9 ± 2.2; 23.4% of the sample had MoCA scores indicative of cognitive impairment. The odds ratio for cognitive impairment was 0.93 (0.88–0.97) per 1-MET increment in CRF. When examined as a categorical variable, and using the lowest CRF quintile as the referent, there was a significantly reduced likelihood for cognitive impairment across the remaining ordered CRF categories (p trend = 0.004). Conclusion: The association between CRF and MoCA score in older adults suggests that meeting or exceeding public health guidelines for physical activity is likely to increase CRF in low fit individuals, maintain CRF in those with a moderate to high level of CRF, and thereby help to maintain cognitive function in healthy older adults.

Role of cardiorespiratory fitness in lifetime cardiovascular risk prediction models: a cross-sectional analysis

BackgroundDespite well-established associations between cardiorespiratory fitness (CRF) and long-term cardiovascular disease risk, CRF remains underused in clinical practice. With a growing emphasis on longer-term risk, developments to UK cardiovascular disease risk equations such as the Joint British Societies' 3 (JBS3) calculator now see the prediction of both lifetime and short-term risk. However, unlike the US LifetimeRisk model, the JBS3 algorithm does not include CRF as a risk variable. The aim of this study was to examine whether differences in CRF values are reflected in long-term cardiovascular disease risk prediction scores using a risk equation that includes CRF in the design (LifetimeRisk) and one that does not (JBS3). MethodsA retrospective, cross-sectional analysis was performed on 81 male steelworkers who participated in the second phase of the Prosiect Sir Gâr workplace-based cardiovascular disease risk assessment initiative in Carmarthenshire, Wales. CRF was calculated with a validated and reliable submaximal protocol based on heart rate responses (Chester Step Test). Between-group CRF differences (excellent or good vs average or below average) were determined by independent sample t test or Mann-Whitney U test, and any association between CRF and individual risk scores investigated by Pearson's correlation. FindingsMen in the lower CRF categories had greater body-mass index scores than men in the higher CRF categories (p=0·001); and less desirable blood lipid profiles with higher total cholesterol (p=0·041) and non-HDL cholesterol (p=0·019) concentrations. The predicted long-term cardiovascular disease risk in the LifetimeRisk model was also greater in the lower fitness group than higher fitness group (mean 13·6% [SD 2·9] vs 7·2 [1·9], p<0·0001), and a negative correlation was evident between individual CRF values and predicted risk scores (r=–0·603, p<0·0001). However, there were no differences between CRF groups in predicted JBS3 lifetime risk (median 79 years [IQR 77–80] vs 79 [78–80], p=0·354). InterpretationThese preliminary observations demonstrate that the LifetimeRisk prediction scores more accurately represent the current literature between CRF and longer-term cardiovascular disease risk in a group of white men. Although, more research is required, this study raises an important question as to whether CRF should be considered in the design of future UK long-term cardiovascular disease risk equations. Abstract 37: The Association of Cardiorespiratory Fitness and Ideal Cardiovascular Health in the Aerobics Center Longitudinal Study

Purpose: To examine the cross-sectional association between cardiorespiratory fitness (CRF) and ideal cardiovascular health (CVH) in middle-aged adults. Methods: The association between CRF and ideal CVH score was examined in 11,590 adults (8,865 men, 2,725 women) from the Aerobics Center Longitudinal Study. CRF was measured as duration in minutes from a maximal treadmill test. The AHA’s ideal CVH score was calculated on a 14 point scale using data on smoking status, BMI, physical activity (MET-min/wk), healthy diet, total cholesterol, blood pressure, and fasting plasma glucose recorded between 1987 and 1999. Participants were grouped into categories of inadequate (0-4), average (5-9), and optimum (10-14) based on their CVH score. Three CRF groups were created from age- and sex-specific quintiles based on the previously established cutpoints of treadmill time: low, moderate, and high CRF. We used general linear and logistic regression models adjusted for age, sex, and year of examination to evaluate the association of CRF with ideal CVH score. Results: The mean CVH score for men was 8.4 ± 2.2 and 9.7 ± 2.0 for women. Approximately 33% of men and 57% of women had optimum CVH, while only a small proportion of participants had inadequate CVH (5.1% M, 1.4% F). Treadmill time was moderately correlated (p&lt;0.0001) with CVH score in both men (r=0.56) and women (r=0.50). CRF explained 16% and sex 18% of the variance in CVH score (both p&lt;0.0001). Our adjusted model found that participants in the optimum CVH category had 20% and 43% higher CRF levels than those in the average and inadequate CVH groups (p&lt;0.0001), respectively ( Figure 1 ). The adjusted odds (95% CI) of having optimum CVH were 14.0 (11.0-17.8) and 3.1 (2.4-4.0) times greater for high CRF and moderate CRF, respectively, compared to low CRF (p&lt;0.0001). Discussion: Higher levels of cardiorespiratory fitness are associated with better cardiovascular health profiles in both men and women. Thus, improving fitness represents a strategy to improve cardiovascular and public health.
