Papillary Thyroid Carcinoma of a Large Thyroglossal Duct Cyst: A Case Report

Background: Thyroglossal duct cyst is the most common congenital anomaly of the thyroid gland. Carcinoma arising from it is rare which composes 1% of total cases, often diagnosed incidentally after surgical excision. Here we are reporting a case of papillary thyroid carcinoma of thyroglossal cyst in 45-year-old female with normal thyroid gland. Case Description: A 45-year-old female presented with 1 year history of swelling over midline of the neck with normal thyroid gland. FNAC suggested a benign cystic lesion, most likely thyroglossal cyst. CT scan neck showed a well-defined lobulated and septated thin-walled cystic lesion of approx. 6×4×5cm in the midline upper infrahyoid region anterosuperior to thyroid, suggestive of thyroglossal cyst. Sistrunk procedure was performed. HPE revealed papillary thyroid carcinoma of thyroglossal cyst. Conclusion: A case of thyroglossal duct cyst with normal thyroid gland and without any clinical or radiological evidence of malignancy, a possibility of underlying malignancy should be kept in mind and can be managed adequately with Sistrunk procedure only, with regular follow up.

Papillary Thyroid Carcinoma within a Thyroglossal Cyst: A Rare Case Report with Review of Literature.

Thyroglossal cyst (TGC) is the most common congenital anomaly of the thyroid gland and is found in approximately 7% of general population. It represents cystic degeneration of a remnant of the thyroglossal duct that failed to involute during gestation. Malignancy occurring in TGC is rare entity, accounting only for 1% of all thyroglossal cysts. We are presenting such a rare case of papillary thyroid carcinoma arising in a thyroglossal cyst.30-year-old female presented with a painless, gradually progressive swelling below the chin extending to left side. On examination, swelling was noted in submental region and measuring 4 × 3 × 1cm, firm in consistency and moves with deglutition and movement of tongue.On radiological investigations, lobulated cystic lesion in midline with thin septations was seen, suggestive of thyroglossal cyst. The thyroid of the patient was normal. Patient underwent Sistrunk procedure. On histopathology, papillary thyroid carcinoma in thyroglossal cyst was seen and it was invading cyst wall and superficial skeletal muscle.The 1st case of thyroglossal duct carcinoma was reported by Brentano in 1911. Till now 300 cases have been reported in literature. Carcinomas occurring in thyroglossal cyst are extremely rare, (< 1% cases). The most frequent histological type is papillary pattern followed by mixed(papillary and follicular), squamous cell, Hürthle cell, follicular and anaplastic variety. The etiology of thyroglossal duct carcinoma is unknown and neither good clinical history nor examination can lead to a preoperative diagnosis. Diagnosis is often incidental on histopathology as in present case.Thyroglossal duct carcinoma is a rare condition that comes as a surprise to both the patient and surgeon and should be considered in patients presenting with cystic midline neck masses.

Synchronous Carcinoma in Thyroglossal Cyst: A Rare Occurrence.

A Thyroglossal cyst is a commonly encountered clinical entity resulting due to the persistence of the thyroglossal duct and the transformation of a few embryonic cells into a cyst. The incidence of malignant change in the thyroglossal cyst is reported as between 1 to 1.8 percent. Here we present a case report of a male who presented with swelling in the neck, on ultrasonography (USG) found to be a thyroglossal cyst, fine needle aspiration cytology (FNAC) suggested a papillary carcinoma within the thyroglossal cyst. Total thyroidectomy with bilateral selective neck dissection, central compartment clearance, and sistrunk operation were done. The histopathological report revealed papillary carcinoma of the thyroid within a thyroglossal cyst with neck nodal metastasis.

Introduction: Thyroglossal duct cysts (TDC) are the most common anomaly occurring during thyroid development. Thyroid carcinoma can be seen in less than 1% of untreated thyroglossal duct cysts. The overall prognosis of primary papillary carcinoma in thyroglossal duct cyst is very good with a five year- survival rate of about 95%.

ABSTRACT Ectopic thyroid tissue is rare, with a reported incidence of 1 in 300,000. A thyroglossal duct cyst is the most common developmental anomaly of the thyroid gland. The probability of carcinoma arising in such tissue is &lt;1%. The incidence of papillary carcinoma of the thyroid is more in 30–50 years. Here, we report a case of an 18-year-old female presenting with anterior neck swelling, found to have a thyroglossal cyst, fine-needle aspiration cytology (FNAC) of which showed a cystic mass with sparse epithelial cells. After the Sistrunk surgery, biopsy showed classic papillary carcinoma of the thyroid. She underwent total thyroidectomy followed by thyroxine replacement. Thyroglossal cyst papillary carcinoma of the thyroid is a rare entity in young whose diagnosis is easily missed on FNAC; a thorough examination of the biopsy specimen is required. Management should be considered based on multiple associated factors. Overall, the prognosis of it is very good, with a 5-year survival rate of 95%.

High-grade mucoepidermoid carcinoma in thyroglossal cyst: Post-surgical histological surprise and dilemmas

Occurrence of malignancy in the TG cyst has been rarely reported, though rare, and papillary carcinoma predominates the common type but squamous cell carcinomas, anaplastic carcinoma, and medullary have been reported rarely. Mucoepidermoid carcinomas are most commonly seen in salivary glands, and as per the available literature, there was only two cases reported in thyroglossal cyst. We are presenting a 67-year-old lady presented with a 6×8 cm hard swelling below symphysis menti with no thyromegaly and moving on protrusion of tongue, and on MRI, it was found to be thyroglossal cyst with infiltration of strap muscles. Cytological investigation revealed it to be a TG cyst malignancy. The patient underwent total thyroidectomy and radical Sistrunk’s operation. Histopathological and immunohistochemistry revealed it to be a histological examination revealed a low-grade mucoepidermoid carcinoma consistent with origin in a thyroglossal duct remnant it invaded the hyoid bone and adjacent strap muscles. Various diagnostic and treatment dilemmas in the treatment of TG cyst malignancy are discussed with reference to mucoepidermoid carcinoma. We are reporting an usual histological surprise in a thyroglossal cyst malignancy being the only second reported case of TG cyst mucoepidermoid carcinoma this case highlights the importance of removal of thyroglossal duct cysts at an early stage and aggressive surgical approach in high-grade tumors.

Papillary Carcinoma of Thyroglossal Cyst with Thyroid Hemiagenesis: Is Conservative Surgical Management Enough?

We describe a case of papillary carcinoma in a thyroglossal cyst with left hemiagenesis of the thyroid gland in a 24-year-old female patient. We opted not to perform a total thyroidectomy as the patient was at low risk for aggressive disease. In this article we review the criteria for selection of thyroidectomy in incidentally discovered papillary carcinomas after thyroglossal cyst excision and management protocol in case of coexisting thyroid hemi-agenesis. To our knowledge, this case represents the second case report of an association between left thyroid hemiagenesis and thyroglossal duct cyst papillary carcinoma.

Synovial sarcoma masquerading as thyroglossal cyst carcinoma

&lt;p class="abstract"&gt;Synovial cell sarcoma is very rare in head and neck. It is usually seen in the extremities in close relation to the bursae and joints. In head and neck, most of the cases reported are in the retrohyoid, parapharyngeal and retropharyngeal areas. Reporting the case of a 18 year old man who presented with signs and symptoms of thyroglossal cyst. Radiological findings were suggestive of a haemorrhagic thyroglossal cyst or a cystic neoplasm. Frozen section revealed a malignant spindle cell neoplasm of mesenchymal origin. A Sistrunk operation was done. Histopathological and immunohistochemical analysis yielded a diagnosis of monophasic synovial sarcoma. Patient underwent intensity modulated radiotherapy according to tumor board decision. Follow up PET Scans were negative. We are reporting this case to highlight the rarity of the condition and the high index of suspicion required to diagnose such a synovial sarcoma in this area. Treatment should be aimed at complete surgical resection of the tumor and appropriate adjuvant radiotherapy.&lt;/p&gt;

Introduction: Carcinomas with thyroglossal cyst localization have a low frequency, about 1% of patients present this disease; the most frequent histology is papillary thyroid carcinoma. We describe a case of papillary carcinoma of thyroglossal cyst. Clinical case: A 54-yearold man with an anterior cervical tumor of six months of evolution, not mobile, not painful, with diameters of 3 × 5 cm. Thyroid ultrasound with right thyroid nodule TIRADS 4c and suspicious adenopathies in level VI. Total thyroidectomy + Sistrunk was performed, with pathological report of multifocal conventional variant papillary carcinoma. It infi ltrated thyroid capsule and adjacent prethyroid muscles; right lobe tumor 2 × 1.5 × 1.3 cm, left lobe tumor 3.5 × 3 × 4 cm; then, I-131 dose was applied. Discussion: There is no consensus regarding the ideal treatment of papillary carcinoma in the thyroglossal cyst, although Sistrunk surgery with total thyroidectomy seems to be the most appropriate one. As this is a rare presentation of well-differentiated thyroid cancer, it could be difficult to draw conclusions with a high level of evidence for its management, but they should be guided by current guidelines of well-differentiated thyroid cancer and decisions should be made in multidisciplinary teams.

The extent of surgery in thyroglossal cyst carcinoma

The optimal management of thyroglossal cyst carcinoma, particularly the extent of surgery required is controversial. The aim of this study was to evaluate the need for routinely adding total thyroidectomy to Sistrunk's operation in the management of this condition. The clinical records of 19 patients with a diagnosis of thyroglossal cyst carcinoma encountered in an 11-year period (2004-2015) were reviewed. All patients underwent total thyroidectomy in addition to Sistrunk's procedure. The rate of concomitant thyroglossal cyst and thyroid carcinomas was calculated and cancers were staged according to the AJCC-TNM staging system. Patients were divided into two groups: those with thyroglossal cyst carcinoma only (group A) and those with a synchronous or metachronous thyroid carcinoma as well (group B). The need for radioactive iodine ablation in group A was assessed. The ability to omit total thyroidectomy based on thyroglossal cancer size and a negative thyroid ultrasound was also evaluated. The rate of concomitant thyroid cancer was 63.2% (12/19). Based on stage, three out of the seven patients in group A required radioactive iodine ablation. Total thyroidectomy was ultimately justifiable in 78.9% (15/19) of cases. Omitting total thyroidectomy in T1 thyroglossal cyst cancers or based on a sonographically normal thyroid was associated with a 43% risk of missing thyroid malignancy. The routine addition of total thyroidectomy to Sistrunk's procedure seems to be appropriate for comprehensive loco-regional control especially that selecting a subset of patients in which it could be omitted is a difficult task.

A case of papillary carcinoma in a thyroglossal cyst without a carcinoma in the thyroid gland

The thyroglossal duct cyst is the most common developmental anomaly of the thyroid gland. We present a patient with a papillary thyroid carcinoma diagnosed after surgical resection of a thyroglossal cyst. The 39-year-old patient reported to our hospital due to a swelling of this throat. The results of the ultrasonography were reported as showing multiple nodules in both thyroid lobes, and an ~3-cm diameter image in the submental area, in keeping with lymphadenomegalia. In the fine-needle aspiration cytology (FNAC) evaluation from the nodules in the thyroid, it was observed that the thyrocites with uniform nuclei made up single-layer groups. FNAC from the thyroglossal cyst, a few histiocytes were observed on a ground with a large number of erythrocytes. The patient was subjected to a total thyroidectomy, pyramidal lobe excision, and total excision of the 3-cm diameter soft mass, starting at the tip of the pyramidal lobe. The case was diagnosed as papillary thyroid carcinoma in thyroglossal duct cyst with histopathological and immunohistochemical findings. If the thyroglossal channel cyst had been diagnosed preoperatively, total thyroidectomy and a Sistrunk operation would have been performed. For this reason, repeated FNAC, particularly, if performed under ultrasound guidance, may improve the diagnostic value of FNAC. Thus, in such patients, throat ultrasonography must be carried out in expert hands, thin-needle aspiration biopsy, computerized tomography, and thyroid scintigraphy must be done if necessary, and the necessary treatment protocols carried out after a definite diagnosis.

Malignancy of thyroglossal cyst is a rare entity. Medical literature has reported only a very few cases till date. Histopathologically papillary carcinoma of thyroglossal cyst is the most common variety encountered in clinical practice. Surgical excision by Sistrunk operation is the standard treatment of choice followed by subtotal or total thyroidectomy whenever indicated. We report here two cases of thyroglossal cyst carcinomas and their respective management.

Papillary carcinoma of the thyroglossal duct cyst in a 15-year-old girl

Summary Thyroglossal duct (TGD) cysts are the most common midline mass found in children. Most of the thyroglossal duct remnants are benign, however, a small percentage of them undergoes neoplastic change. Carcinoma arising in a TGD is rare, with only approximately 250 reported cases. The median age of development of carcinoma in thyroglossal cyst is 40 years old. We report a case of papillary carcinoma in a thyroglossal duct cyst in a 15-year-old girl. She presented to Hospital Universiti Kebangsaan Malaysia (HUKM) with 3 months history of anterior neck swelling. A Sistrunk operation was performed. Intraoperatively the cystic swelling was noted to be adherent to the infrahyoid muscle. Preoperative diagnosis of patients with carcinoma in a TDC is unusual. In a large series report, preoperative thyroid scanning did not seem to be of value in diagnosing carcinoma in TDC. Patients are also usually euthyroid. Therefore, TDC carcinoma is often diagnosed only after surgery. The possibility of second focus of tumor in the thyroid gland was reported as 11–33% of cases, hence thyroidectomy and long term follow-up is warranted.

The thyroglossal cyst is found in 7% of the population and the incidental papillary thyroid carcinoma in thyroglossal cyst is a rare entity with an incidence 1 to 2%. The clinical presentation is indistinguishable from a benign lesion and the histopathological postoperative study defines the diagnosis. Papillary carcinomas have favorable prognosis and cervical or distant metastases are rare. There is now a consensus on the indication of total thyroidectomy, radioablation with iodine and/or suppressive therapy with levothyroxine after being removed surgically [1–3] (Patrucco et al., 2015; Gupta et al., 2014; Choi et al., 2013).46-year-old female patient with an asymptomatic midline neck mass consistent with a thyroglossal cyst. That was excised by Sistrunk's procedure and an intraoperative biopsy that reports papillary carcinoma infiltrating the capsule. It was decide to complete the total thyroidectomy without complications, evolution is consistent and graduated euphonious and no evidence of hypoparathyroidism.Management dilemmas regarding the roles for total thyroidectomy are reviewed in the context of relevant evidence based literature.The initial evaluation of carcinoma of thyroglossal duct cyst includes careful examination, ultrasound and biopsy fine needle aspiration. Sistrunk's procedure is adequate treatment for thyroglossal cyst but find another diagnosis as papillary thyroid carcinoma makes us continue with a total thyroidectomy after discuss the case with experts [4,5] (Tharmabala and Kanthan, 2013; Miranda-Aguirre et al., 2008).

Primary papillary carcinoma in thyroglossal cyst

Introduction: Thyroglossal cyst is the most common congenital midline neck swelling. Most of the cases are benign with rare malignant transformation is approximately in 1-2% cases. Only about 200 cases have been reported from different centers since 1911. Clinical presentation is similar to that of benign cyst and diagnosis is usually made incidentally during histopathological examination of excised cyst. Case report: We report a case of 46-year-old woman who underwent Sistrunk’s operation for thyroglossal cyst. Histopathological examination revealed papillary carcinoma. Thyroxine was added in suppressive dose and patient was on regular follow-up for 2 years. No thyroid abnormality was found preoperative cytology, scintigraphy as well as per-operative procedure and post-operative follow-up period. Conclusion: Sistrunk’s operation followed by suppressive dose of thyroxin and regular follow up is recommended for thyroglossal duct cyst carcinoma in asymptomatic incidental case without the cyst wall invasion.
