Elevated Testosterone and Progestin Concentrations in a Spayed Female Rabbit with an Adrenal Cortical Adenoma

This case was described briefly in a recent book chapter (Lennox AM, Fecteau KA: 2014, Endocrine disease. In: BSAVA Manual of Rabbit Medicine, eds. Meredith A, Lord B, pp 274–276. British Small Animal Veterinary Association, Gloucester, UK). In the previous description, the tumor was described as a pheochromocytoma; however, further evaluation suggested that it more closely resembled an adrenal cortical adenoma. A 10-year-old, spayed female rabbit was presented for a behavior change of 8 months’ duration. The rabbit was inappropriately urinating and defecating, as well as demonstrating aggressive behaviors such as chasing, biting, and mounting various objects. The rabbit had elevated progesterone, 17-hydroxyprogesterone, and testosterone concentrations, and ultrasound examination of the abdomen showed a round, homogenous nodule measuring 1.1 × 0.8 × 0.9 cm in the region of the left adrenal gland. Necropsy revealed a unilateral adrenal cortical adenoma. To the authors’ knowledge, this is the first complete description of a female rabbit with an adrenal cortical adenoma documented in the literature.

Ethical and evidential considerations on the use of novel therapies in veterinary practice

Novel therapies may be necessary both in the interests of the individual animal and in the advancement of veterinary science. However, special consideration is necessary to safeguard the welfare of the animals, ensure fully informed consent from the owner, determine the procedure's value in other cases and appropriately disseminate results. This article reviews ethical and evidential considerations raised when novel therapies are undertaken in veterinary practice, based on a series of committee meetings and intervening discussions held between members of the British Small Animal Veterinary Association Scientific Committee and Officers (2010-2011). The aim of this article is to encourage the advancement of novel veterinary therapies while safeguarding the welfare of animals.

Management of cranial cruciate ligament rupture in small dogs: A questionnaire study

To survey the current management of cranial cruciate ligament rupture in small dogs (<15 kg) by veterinarians. A questionnaire was distributed to veterinarians attending the British Veterinary Orthopaedic Association Spring meeting and British Small Animal Veterinary Association Annual Congress which took place in April 2010. Respondents were surveyed on their management of small dogs with cranial cruciate ligament rupture. Immediate surgical management was chosen by 15.5% of the respondents. Following that, 77.6% of respondents suggested that case management would depend on severity of lameness (81%), age (72.6%), bodyweight (70.4%), degree of instability (64.8%), and duration of lameness (64.0%). Conservative management included non-steroidal anti-inflammatory drugs (91.1%), short leash walks (91.1%), weight loss (89.0%), hydrotherapy (53.6%), physiotherapy (41.9%), and cage rest (24.2%). Where surgical management was chosen, 71.4% would perform the procedure rather than refer it to another practice. Surgical techniques included extra-capsular stabilisation (63.4%), corrective osteotomies (32.9%), and intra-articular stabilisation (6.8%). The demographic characteristics of the responding veterinarians had no effect on their initial management of the case. General practice veterinarians were less likely to perform a corrective osteotomy than to refer the case, but were no less likely to perform an extracapsular technique. Conservative management is still widely used for treatment of cranial cruciate ligament rupture in dogs weighing less than 15 kg; this is in agreement with previous publications on the management of this condition in small dogs.

How to manage the veterinary dispensary

This article provides readers with guidance on the setting up and running of a veterinary dispensary. Following guidance and requirements from relevant bodies (such as the Veterinary Medicines Directorate (VMD), British Small Animal Veterinary Association (BSAVA) and Royal College of Veterinary Surgeons (RCVS)), readers will be taken through some of the required and recommended policies and regulations associated with the management of a veterinary dispensary. It is important for the veterinary nurse to be aware of current requirements for veterinary prescriptions, prescribing cascade, suitably qualified person, waste guidelines and the understanding of stock control which has been included in this article.

The perceived and actual diagnostic utility of veterinary cytological samples

To establish the proportion of cytology samples sent to a commercial veterinary laboratory that yields diagnostically useful information in the context of current use and perceptions of cytology. Nine hundred and forty-five cytology submissions were retrospectively collected and categorised according to diagnostic utility. A survey into the use and perceptions of cytology was distributed at the British Small Animal Veterinary Association Congress 2008. A specific diagnosis was reached in 23.1 per cent of samples and a cytological diagnosis in 35.3 per cent. 22.4 per cent of samples yielded some useful information, but 19.2 per cent were unacceptable. Seventy-four participants in the survey took an average of 3.9 cytological samples per week, of which they examined 27.0 per cent in-house only, 21.6 per cent in-house before sending to an external laboratory and 51.4 per cent were sent externally without prior examination. "To obtain a definitive diagnosis" was the principal reason cited for performing cytology. Results suggest that cytology is underused and may be applied in an inappropriate context in the UK. It is hoped that illustrating the diagnostic outcome of samples received by a commercial laboratory will encourage increased, appropriate use of cytology.

Management of occupational health risks in small-animal veterinary practices

Small-animal work is a major element of veterinary practice in the UK and may be hazardous, with high levels of work-related injuries and ill-health reported in Australia and USA. There are no studies addressing the management of occupational health risks arising from small-animal work in the UK. To investigate the sources of health and safety information used and how health and safety and 12 specific occupational health risks are managed by practices. A cross-sectional postal survey of all small-animal veterinary practices in Hampshire. A response was mandatory as this was a Health & Safety Executive (HSE) inspection activity. A total of 118 (100%) practices responded of which 93 were eligible for inclusion. Of these, 99 and 86%, respectively, were aware of the Royal College of Veterinary Surgeons (RCVS) practice standards and had British Small Animal Veterinary Association (BSAVA) staff members, while only 51% had previous contact with HSE (publications, advice and visit). Ninety per cent had health and safety policies, but only 31% had trained responsible staff in health and safety. Specific health hazards such as occupational allergens and computer use were relatively overlooked both by practices and the RCVS/BSAVA guidance available in 2002. Failings in active health risk management systems could be due to a lack of training to ensure competence in those with responsibilities. Practices rely on guidance produced by their professional bodies. Current RCVS guidance, available since 2005, has remedied some previous omissions, but further improvements are recommended.

FC‐55 Efficacy of low‐dose immunotherapy in the treatment of canine atopic dermatitis: a prospective, double‐blinded study

There are anecdotal reports of increased effectiveness of allergen‐specific immunotherapy (ASIT) with doses of vaccine lower than that recommended by the manufacturers. However, no controlled studies have been carried out. The aim of this prospective, double‐blinded study was to evaluate whether induction and maintenance with low‐dose (LD) ASIT resulted in a different success rate compared with the standard dose (SD). Twenty‐seven dogs with confirmed atopic dermatitis were allotted by block randomisation to two groups. One (n = 13) received SD ASIT; the other (n = 14) received LD ASIT (1/10 of the SD) following the same frequency protocol. Cases were graded at 0, 3, 6 and 9 months for clinical signs using a modified Canine Atopic Dermatitis Extent and Severity Index (mCADESI) and for pruritus using a 0–5 descriptor scale. There were no significant differences between the groups in the pruritus and mCADESI scores (P &gt; 0.155) at the end of the study. Changes in pruritus (P &gt; 0.920) and mCADESI (P &gt; 0.296) scores from the beginning to the end of the study were similar in both groups, with pruritus scores in both groups not changing during the study (P &gt; 0.052). However, significant reductions in mCADESI scores were seen in both groups (P &lt; 0.032). Six dogs achieved a final pruritus score of 0, six dogs achieved a reduction in pruritus score, and 15 did not improve or worsened. There was, therefore, no evidence that LD ASIT was more effective than the standard protocol. Funding: Petsavers, British Small Animal Veterinary Association, Wellcome Trust.
