Back muscle function in adolescent girls treated with a rigid brace for idiopathic scoliosis: no impact of 6-month brace wear on muscle strength or endurance.

The study aimed to determine the impact of 6-month rigid brace on back muscle strength and endurance in adolescents with idiopathic scoliosis. Sixty-one girls, aged 7.0-16.0, were analyzed in two groups: the study group (6-month rigid brace wear) vs. the control group (no brace treatment), recruited consecutively and matched for age, body height, weight, BMI, primary curve location and Cobb angle. All patients underwent clinical and radiological examination, modified Biering-Sorensen test, prone and standing maximum strength and endurance tests. No significant difference between groups in back muscles strength or endurance, both gobal and reported to body weight was found. No relation between the daily brace time and the back muscle strength or endurance was observed. The 6-month use of a rigid brace did not affect the strength or endurance of the back muscles in adolescent girls treated for idiopathic scoliosis.

French validation of the Brace Questionnaire (BrQ)

BackgroundQuality of Life (QoL) scales have to be introduced in the treatment evaluation of our patients with adolescent idiopathic scoliosis.Vasiliadis et. al. created the Brace Questionnaire (BrQ), which is specific for brace-treated adolescents. This tool was developed and validated in Greek.The aim of our study was to undertake the process of cultural adaptation of the Brace Questionnaire (BrQ) into French.MethodsThe BrQ is made of 34 items on Likert scale, divided in eight domains. The questionnaire was developed for self-completion by the children and is adapted for 9 to 18-year-old patients.The process of cultural adaptation of the questionnaire was in accordance with the International Quality of Life Assessment (IQOLA) guidelines.In the first place, descriptive statistics were used to calculate mean scores and standard deviations for a given question and a domain. The second level was comparative, concerning reliability and validity.ResultsThe internal consistency was satisfactory; Cronbach’s alpha coefficient was 0.85. There were no floor or ceiling effects.ConclusionsThe French version of the BrQ (F-BrQ) is reliable and reproducible, and can therefore be used to evaluate the quality of life of children and adolescents treated with a brace for idiopathic scoliosis.

A Cochrane systematic review. To evaluate the efficacy of bracing for adolescents with AIS versus no treatment or other treatments, on quality of life, disability, pulmonary disorders, progression of the curve, psychological, and cosmetic issues. Idiopathic scoliosis is a three-dimensional deformity of the spine. The most common form is diagnosed in adolescence. Although adolescent idiopathic scoliosis (AIS) can progress during growth and cause a surface deformity, it is usually not symptomatic. We searched CENTRAL, MEDLINE, EMBASE, five other databases, and two trials registers up to February 2015. We also checked reference lists and hand searched grey literature. Randomized controlled trials (RCTs) and prospective controlled cohort studies comparing braces with no treatment, other treatment, surgery, and different types of braces for adolescent with AIS. We used standard methodological procedures expected by the Cochrane Collaboration. We included seven studies. Five were planned as RCTs, two as prospective controlled clinical trials. One RCT failed completely, another was continued as an observational study. There was very low quality evidence from one small RCT that quality of life (QoL) during treatment did not differ significantly between rigid bracing and observation. Two studies showed that bracing did not change QoL during treatment, and QoL, back pain psychological and cosmetic issues in the long term (16 years.) All articles showed that bracing prevented curve progression. The high rate of failure of RCTs demonstrates the huge difficulties in performing RCTs in a field where parents reject randomization of their children. 1.

Stress level in adolescents wearing corrective brace for idiopathic scoliosis: study with BSSQ questionnaires

Material and methods The study involved 80 adolescents (71 girls and 9 boys), fulfilling the inclusion criteria: age range between 11.0 and 16.0 years, Cobb angle range between 20 and 45 degrees, wearing Cheneau orthosis for more than 3 months, at least 12 hours per day. The group was analyzed in subgroups according to (1) age: 11.0-13.0 years (n=40) versus 14.016.0 years (n=40), (2) brace wearing time: less than 20h (n=45) versus more than 20h per day (n=35), (3) place of residence: village (n= 34), town (n= 20) or city (n= 26). Maximal angle of trunk rotation (ATR max) was measured using Bunnell scoliometer. Bad Sobernheim Stress Questionnaires (BSSQ), both the Brace and the Deformity versions were used, the former estimates the stress the patients have whilst wearing brace, the latter estimates the stress induced with body deformation.

There is low quality evidence from one RCT with 43 girls that a rigid brace is more successful than an elastic one (SpineCor) at curbing curve progression when measured in Cobb degrees, but there were no significant differences between the two groups in the subjective perception of daily difficulties associated with wearing the brace.Authors' conclusionsThere is very low quality evidence in favour of using braces, making generalization very difficult. Further research could change the actual results and our confidence in them; in the meantime, patients' choices should be informed by multidisciplinary discussion. Future research should focus on short and long‐term patient‐centred outcomes, in addition to measures such as Cobb angles. It is often idiopathic, or having an unknown cause. The most common type of scoliosis is discovered at 10 years of age or older, and is defined as a curve that measures at least 10° (called a Cobb angle; measured on x‐ray). Because of the unknown cause and age of diagnosis, it is called Adolescent idiopathic scoliosis (AIS).While there are usually no symptoms, the appearance of AIS frequently has a negative impact on adolescents. Increased curvature of the spine can present health risks in adulthood and in the elderly. Braces are one intervention that may stop further progression of the curve. They generally need to be worn full time, with treatment lasting for two to four years. However, bracing for this condition is still controversial, and questions remain about how effective it is.This review included two studies; one multicenter international cohort study (a study where treatment groups were defined according to the centre where patients were treated) of 286 girls and a randomised controlled study (an experimental study that randomised the participants to treatment groups) of 43 girls. There is very low quality evidence that braces are more effective than observation (wait‐and‐see) or electrical stimulation in curbing the increases in the curves of the spine. There is low quality evidence that rigid braces are more effective than a soft, elastic one. Adverse effects of braces were not discussed.Limitations of this review include the sparse data and studies available, and the fact that available studies only included girls (even if there is only one male with scoliosis for every seven females), making it very difficult to generalize the results to males. Due to the very low quality of the evidence in favour of bracing, patients and their parents should regard these results with caution and discuss their treatment options with a multi‐professional team.Further research is very likely to change the results and our confidence in them.

LONG-TERM ALTERATIONS OF RESPIRATORY FUNCTION IN ADOLESCENTS WEARING A BRACE FOR IDIOPATHIC SCOLIOSIS

STUDY DESIGN This is a prospective study. OBJECTIVE The authors investigated the effects of continuous bracing for idiopathic scoliosis on lung function variables at three consecutive time points over a 2-year period. SUMMARY OF BACKGROUND DATA Only short-term results regarding lung function impairment caused by bracing exist. METHODS Thirty adolescents (aged 13.6 +/- 1.8 years) with primary idiopathic thoracic scoliosis of 28.7 degrees +/- 4.1 degrees and primary lumbar scoliosis of 26.5 degrees +/- 10.4 degrees were treated with a Boston brace. All patients underwent pulmonary function studies at the beginning of brace treatment and 12 and 24 months after treatment initiation. The examinations were always performed while the patients were sitting, in and out of the brace. Patients removed the brace for 1 hour before the measurements for non-brace-wearing were performed. Vital capacity, forced expiratory volume in 1.0 second, and minute ventilation were determined with a low inertia, low resistance bell spirometer. Lung volume, including total lung capacity and functional residual capacity, was recorded. RESULTS The primary thoracic scoliosis was corrected to 14.5 degrees +/- 4.0 degrees and the primary lumbar scoliosis to 13.0 degrees +/- 6.0 degrees. The values of the following parameters taken while the brace was worn were significantly lower than those taken without the brace at all time points (one-way analysis of variance); vital capacity (P < 0.02), forced vital capacity (P < 0.03), functional residual capacity (P < 0.02), and residual volume (P < 0.05). Furthermore, the predicted negative residual volume and negative functional residual capacity values differed significantly in all time points from negative residual volume and negative functional residual capacity values of patients while wearing the Boston brace (P < 0.01 and P < 0.02, respectively). CONCLUSIONS The results suggest that brace wearing for mild idiopathic scoliosis does not harm adolescent lung function over a 2-year period and is recommended for treatment of idiopathic scoliosis in early adolescence when the generally accepted criteria for bracing are fulfilled.

Curve Progression After Treatment with the Wilmington Brace for Idiopathic Scoliosis

We evaluated curve progression after completion of Wilmington brace treatment for adolescent idiopathic scoliosis in 67 patients. Follow-up ranged from 5 to 13 years. Pretreatment curve magnitudes ranged from 20 to 39 degrees, with a Risser sign of 0 or 1. Sixteen patients (21%) experienced 5-16 degrees of curve progression after brace treatment was discontinued. For nine of these patients, the final curve magnitude was within 5 degrees of the pretreatment Cobb angle, reflecting a loss of correction achieved during brace treatment. The remaining seven patients had final curve magnitudes measuring 8-32 degrees greater than the pretreatment Cobb angle.

A new orthosis for immediate postoperative bracing in idiopathic scoliosis.

The "crutch-type scoliosis brace" and the KSO are leather covered aluminum braces, which have been successfully used in treating scoliosis patients at the Kosair Spinal Deformity Center both nonoperatively and postoperatively. This report defines the measurements, materials, and construction details for effective use of these braces and reports the very encouraging results of the KSO in a group of instrumented idiopathic scoliotic patients.
