Bilateral adrenal histoplasmosis in a Malaysian tertiary hospital: Report of four cases

Rationale: Adrenal histoplasmosis can present as bilateral adrenal enlargement with constitutional symptoms and/or adrenal insufficiency. Often these patients are initially investigated as secondary adrenal metastases before eventually their diagnosis established by histopathological examination of the adrenal tissues. Patients concerns: We report 4 cases of patients aged 55-78 who presented with hypocortisolism and bilateral adrenal masses. Diagnoses: Bilateral adrenal histoplasmosis. Interventions: Three patients received antifungal treatment for at least one year while 1 patient passed away before the initiation of antifungal treatment. Outcomes: Two patients’ adrenal function recovered not requiring any steroid replacement, while 1 patient still requires long-term steroid replacement. Lessons: Adrenal histoplasmosis should be excluded in patients from endemic areas presenting with bilateral adrenal masses and adrenal insufficiency. Usually patients have an exposure to bird droppings, bat guano or soil contaminated with histoplasmosis fungi. Histopathological examination of biopsied adrenal tissues is still the gold standard investigation as serology and molecular diagnostic methods are still not widely available in most centers in Malaysia.

Isolated Bilateral Adrenal Histoplasmosis With Addisonian Crisis in an Immunocompetent Host

Bilateral adrenal histoplasmosis presenting as an adrenal crisis and being the only site of demonstrable disease in an immunocompetent adult is a rare presentation. The adrenal gland is frequently involved in disseminated histoplasmosis but commonly presents as unilateral mass; isolated bilateral involvement in an immunocompetent patient is rare. We describe a patient case of 38-year-old man presenting with epigastric pain, intermittent low-grade fever, nausea and vomiting, skin darkening, generalized weakness, and weight loss for 6 months. Investigations suggested primary adrenal insufficiency. On imaging, patient was found to have bilateral adrenal masses. The histopathological examination revealed Histoplasma species.

P186 Disseminated Histoplasmosis in an immunocompetent patient in a tertiary care center in North India

Poster session 2, September 22, 2022, 12:30 PM - 1:30 PM ObjectivesHistoplasmosis is a geographically restricted dimorphic fungi that causes disseminated infection in immune-competent as well as immunocompromised patients. Adrenal involvement is seen in disseminated disease but sometimes it may be the only site where then infection can be demonstrated. Early diagnosis and treatment are needed to save the patient from fatal adrenal insufficiency. We present a case of bilateral adrenal histoplasmosis in immunocompetent patient.MethodsA 63-year-old male presented to our hospital with a history of insidious onset of decreased appetite and unintentional weight loss for the last 6 months associated with generalized weakness. Patient had a history of mild to moderate intensity epigastric pain and discomfort which was intermittent in nature. Patient originally belongs to Azamgarh, Uttar Pradesh, but he was residing in Kolkata for the last 8 months.Patient had no history of fever, cough, hemoptysis, jaundice, chronic diarrhea, and steatorrhea. No history of orthostatic hypotension, salt craving, hyperpigmentation, headache, visual field disturbances, polyuria, behavioral changes, episodic headache, palpitation, diaphoresis, systemic hypertension with episodic all four-limb weakness. There was no history of abdominal striae, easy bruisability, difficulty in standing from squatting position. There was no history of tuberculosis among family members. On detailed history, it was revealed that he fed pigeons every day in the slum house where he lived in Kolkata.On the CECT abdomen it was found that there is an ill-defined hypodense enhancing lesion (72 × 52 × 77 mm) in right suprarenal region and bulky, necrosed 25 × 26 × 19 mm lesion in left suprarenal gland associated with multiple nonnecrotic paraaortic and aortic caval lymphadenopathy. He also received empirical anti-tubercular therapy for 15 days in the form of ethambutol and levofloxacin.ResultsIn all, 10% KOH wet mount of crushed smear of adrenal biopsy samples showed tissue debris and small narrow neck budding yeasts. Giemsa stain shows few small budding yeasts. Culture was put in SDA at 25°C and 37°C and incubated. On day 12, growth of colony in 25°C appears as white cottony growth with yellowish white reverse. On day 24, colony appears as buff brown with yellowish brown reverse. LPCB was done from the colony showing presence of characteristic tuberculate macroconidia (8–14) μm in diameter formed on short, hyaline, undifferentiated conidiophores and production of plenty round to pyriform microconidia (2–4 μm) in diameter, occurring on short branches and directly on the sides of the hyphae. Based on the direct microscopy and culture characteristics a diagnosis of Histoplasma capsulatum was given.ConclusionSystemic histoplasmosis is typically acquired through inhalation of microconidia or small hyphal elements in soil contaminated with bird and bat droppings leading to primary infection. This patient only manifested bilateral adrenal involvement with nonspecific symptoms.

F-18 FDG PET-CT in a Case of Unsuspected Bilateral Adrenal Histoplasmosis

Histoplasmosis is a common fungal infection in endemic countries including Malaysia. Although it is commonly diagnosed in immunosuppressed individuals, there is a preponderance of immunocompetent individuals among individuals with disseminated disease involving adrenal gland. There is a diagnostic dilemma in most patients as the clinical presentation, biochemical parameters and imaging features are frequently non-specific. On 18F-FDG PETCT, histoplasmosis shows high FDG avidity. However, due to overlapping features with other disease conditions, tissue biopsy remains as the confirmatory test. In this report, we present a case of disseminated histoplasmosis with high FDG uptake in the adrenal glands and lymph nodes which mimic a metastatic cancer.

A Case Report on Bilateral Adrenal Histoplasmosis

Disseminated histoplasmosis affecting the adrenal glands is a very rare infection and an uncommon cause of adrenal insufficiency. We present a case of bilateral adrenal histoplasmosis in a 77 years old immunosuppressed patient who presented with bilateral adrenal masses. Bilateral Adrenal Histoplasmosis in a Diabetic Patient: A Case Report and Review of Cases in India

Disseminated histoplasmosis usually affects immunocompromised and immunosuppressed individuals and may involve various organ systems of our body. The definitive diagnosis of histoplasmosis is very challenging because it usually presents with nonspecific symptoms such as malignancies, tuberculosis, sarcoidosis, and other chronic infections. Hereby, we report a culture-proven case of bilateral adrenal histoplasmosis in a diabetic but immunocompetent patient. The patient showed good response to treatment with antifungal drugs.

Bilateral adrenal histoplasmosis in an immunocompetent man from Texas

Disseminated histoplasmosis affecting the adrenal gland(s) of immunocompetent adults is a very rare infection. Here, we present a case of bilateral adrenal histoplasmosis in an immunocompetent, 62-year-old gentleman from Texas along with a brief review of the published literature. Given the risk of patient decompensation secondary to adrenal insufficiency and the wide availability of effective treatments, adrenal histoplasmosis must be considered even in immunocompetent adults who acquire adrenal masses.

Histoplasmosis is an opportunistic infection caused by a dimorphic fungus Histoplasma capsulatum. Histoplasmosis causing bilateral adrenal involvement is rare and usually occurs only in immunocompromised individuals with disseminated disease. Here we report a case of bilateral adrenal histoplasmosis with disseminated disease in an immunocompetent patient.

18F-FDG PET as a Monitoring Tool to Assess Treatment Response in Bilateral Adrenal Histoplasmosis

A 60-year-old woman with clinical and biochemical features suggestive of adrenal insufficiency was found to have bilateral adrenal masses on CT scan and was subjected to F-FDG PET scan. The scan showed hypermetabolic mediastinal nodes in addition to intense tracer uptake in bilateral adrenal masses. CT-guided adrenal biopsy grew Histoplasma capsulatum on Sabouraud dextrose agar culture. A second F-FDG PET scan after 6 months of appropriate antifungal therapy showed a significant decrease in intensity of uptake consistent with the clinical outcome.

Bilateral Adrenal Histoplasmosis in an Immunocompetent Man

Histoplasmosis is a deep fungal infection caused by Histoplasma capsulatum. It presents commonly with transient pulmonary infection but disseminated form can involve any system in the body. Adrenal gland is affected frequently in histoplasmosis but bilateral involvement is unusual. Adrenal histoplasmosis is also common in immunocompromised host. We report a case of bilateral adrenal histoplasmosis in an immunocompetent patient without any features of adrenal insufficiency. Histoplasmosis was diagnosed by fine needle aspiration cytology (FNAC) from adrenal gland. Patient was treated with itraconazole with good response and is currently under follow-up.

A case of clinically inapparent bilateral adrenal histoplasmosis in an Immunocompetent person

Histoplasmosis presenting as bilateral large adrenal masses is rare. We describe the case of a 35-year-old male with bilateral adrenal masses which turned out to be histoplasmosis on biopsy, so the masses were treated with antifungal agents. We describe the case for its rarity.

High 18F-fluorodeoxyglocose uptake in adrenal histoplasmosis; a case report

Adrenal histoplasmosis is one of the most common adrenal granulomatous infections in endemic areas. Although CT or MRI findings of adrenal histoplasmosis have been documented, there are no reports regarding 18F-fluorodeoxyglocose (FDG) positron emission tomography (PET) findings. We report a case of bilateral adrenal histoplasmosis showing a significantly high uptake of 18F-fluorodeoxyglocose on PET study. Adrenal histoplasmosis should be considered as one of the differential diagnoses in cases of adrenal tumors with intense FDG uptake, even in non-endemic areas.
