Atlas of Cancer Surgery is a beautifully illustrated atlas that emphasizes normal anatomy and key principles in the surgical resection of solid tumors. Hugh A. Thomas penned all of the superb illustrations. The book is dedicated to Edward J. Beattie, M.D., who died during the final stages of the preparation of the atlas. Dr. Beattie’s remarkable career included highly successful tenures at George Washington University, the University of Illinois, Memorial Sloan-Kettering Cancer Center, and the Beth Israel Medical Center.

Integrating services for schizophrenia and substance abuse.

Over the past decade, several studies have attempted to determine whether integrating psychiatric and substance abuse treatment leads to better outcome for patients with comorbid schizophrenia and substance use disorders. A recent (1999) Cochrane Review (1) analyzed the effectiveness of prospective randomized studies of integrated treatment approaches, and concluded that there was no clear evidence for superiority of integrated treatment. This paper describes one such integrated treatment approach, in Beth Israel Medical Center's COPAD (Combined Psychiatric and Addictive Disorders) program. We summarize findings from an initial outcome study and a recent replication study; and describe clinical and research issues relevant to this population. Our data suggests the benefits of integrated treatment for patients with addictive disorders and schizophrenia, at least with regard to treatment retention. Clinical issues for such patients include identification of patients at risk, proper assessment and treatment planning, decision-making about mainstreaming vs. referral to specialized programs, and the importance of initial engagement and ongoing reengagement in successful treatment.

Dilemma of angiogenesis.

An old specter haunted the 72nd annual sessions of the American Heart Association in Atlanta last November—the awful nothingness of the placebo effect. No one could prove whether it was or it was not there, but its presence cast a pall over some sessions that dealt with one of the most promising therapies to date—angiogenesis. The prospect of growing a new network of blood vessels to feed starving heart muscles energized the American Heart Association meetings 2 years ago. However, there appears to be a dichotomy. Patients report improvement; scientists cannot prove it exists. “What are we trying to do?” asked Michael Simons, MD, director of the Angiogenesis Research Center at Beth Israel Medical Center in Boston. He identified various types of blood vessel growth as angiogenesis, arteriogenesis, and vasculogenesis. “Which of these processes are we trying to induce? One of them or all of them?” “How does the process work?” he asked. “What are the appropriate targets for the therapy?” The discussion at the meeting convened by the Angiogenesis Foundation, a nonprofit organization based in Cambridge, MA, dealt with the following new therapies thought to enhance angiogenesis: gene therapy, application of growth factors to stimulate growth, and transmyocardial or direct myocardial revascularization using lasers to punch holes in the myocardium. Founded in 1994, the Foundation has participated in activities involving the therapeutic potential of antiangiogenesis for the past 5 years. Comparison of three methods of presurgical risk stratification in cardiac surgery

Background: During the past decade several models were derived to predict the outcome of heart surgery on the basis of clinical and angiographic preoperative data in adult patients with acquired heart disease. The purpose of our study was to verify and compare the predictive accuracy of hospital mortality according to the following 3 models: Parsonnet (Newark Beth Israel Medical Center, NBI score), Higgins (Cleveland Clinic Foundation, CCF score), and Roques (French Multicenter study, French Score), in a setting independent from the one where the models were derived. Methods: For each of the 516 patients undergoing heart surgery in our institution between January 1992 and December 1993, we calculated the presurgical risk according to the 3 models. Then we divided the patients into 3 tertiles of the same dimension, after having ordered the patients on the basis of the mean score obtained by the 3 models. We then compared the predicted with the observed mortality by means of the accuracy index and the overestimation histogram. Results: Overall observed mortality (1.5%) was similar to the mortality predicted by the NBI score (1.5±2.1%, p=n.s.), by the CCF score (1.7±2.0%, p=n.s.), and by the French score (1.9±2.5%, p=n.s.). All 3 models identify a significantly increasing risk from the first to the third tertile. The accuracy of the prediction of global mortality is close to 1 and equal with the 3 models. The accuracy index decreases from the first to the third tertile, showing the loss of the predicting power in the patients at high risk. The accuracy index in the 8 patients who died is similarly low (NBI score: 0.06±0.06; CCF score: 0.13±0.07; French score: 0.10±0.07; p=n.s.). Conclusion: The global predicted mortality obtained by the 3 models is not significantly different from the observed mortality, and therefore, the global accuracy is similar and high. The predictive accuracy is reliable for patients who will survive but is poor for those who will die. The models for presurgical risk stratification are useful to compare the results among different institutions or different surgeons, or to monitor the results over time in the same institution, but they cannot be used to predict accurately the individual risk of hospital mortality.
