Infecções relacionadas à assistência a saúde baseada em critérios internacionais, realizada em unidade neonatal de cuidados progressivos de referência de Belo Horizonte, MG

To describe occurrence of Healthcare Related Infections in a neonatal unit of public reference service in Belo Horizonte-MG, based on international criteria. This is a descriptive study, performed by active searching, in the Progressive Care Unit Neonatal Hospital das Clinicas, Federal University of Minas Gerais (HC / UFMG), from 2008 to 2009. Notification of infections was based on National Healthcare Safety Network (NHSN) criteria. The database and analysis were performed in a internal program. A total of 325 episodes of infection in newborns were notified and overall incidence density of infections was 22.8/1,000 patient-days, with a rate of 36.7% of newborns. Sepsis was the main infection (62.5%) reported. The incidence density of infections was higher in neonates weighing lower than 750g (42.4/1,000 patient-days). There were 18.15 episodes of central venous catheter related sepsis/1,000 central venous catheter-day and 19.29 umbilical catheter related sepsis /1,000 umbilical catheter-days. Microorganisms were isolated in 122 (37.5%) cases of reported infections, mainly defined as Staphylococcus coagulase negative and Staphylococcus aureus (51 cases). Mortality and lethality rates were 4.3% and 17,12%, respectively. The use of standardized criteria for reporting infections is necessary for the construction of indicators in neonatology, which are scarce in the country and highlight the need for evaluation of national criteria proposed by National Agency of Sanitary Surveillance (ANVISA).

Acute diarrhea associated with Salmonella enterica in Belo Horizonte-MG: prevalence and characterization of isolates

Introduction: Acute infectious diarrhea is still regarded as a public health problem associated with a wide range of etiologic agents, from which Salmonella enterica is particularly worth mentioning inasmuch as it is a major cause of inflammatory diarrhea in both developed and developing countries. Objective: To assess the distribution of S. enterica among children with acute diarrhea in Belo Horizonte and to characterize bacterium isolates. Material and methods: The study group comprised a total of 157 children from low socioeconomic background. Stool samples were collected for leukocyte analysis and Salmonella bacterial culture. The isolates were serotyped and evaluated as to antimicrobial susceptibility profile, extended-spectrum β-lactamases (ESBL) production, and presence of virulence markers (invA, iroB, and spvC). RESULTS: A total of 5/3.2% children were infected by S. enterica, 3/60% by S. enterica Typhimurium, 1/20% by S. enterica Enteritidis and 1/20% S. enterica subsp. enterica serotype 8.20:z4,z23:-. Fecal leucocytes were detected in two out of five fecal specimens positive for S. enterica. Isolates from three children were resistant to nalidixic acid, nalidixic acid + chloramphenicol, and nalidixic acid + chloramphenicol + ampicillin. ESBL production was not detected. All samples presented invA and iroB genes. spvC marker was observed in isolates from two children infected by S. Typhimurium and S. Enteritidis. Conclusion: The results demonstrate that S. enterica infection is uncommon among children from our region. Furthermore, they indicate the need for periodic monitoring of bacterial antimicrobial susceptibility profile in order to establish suitable antimicrobial therapy when required.

Aplicação da TRI em uma medida de avaliação da compreensão de leitura

Este trabalho objetivou verificar os parâmetros dos itens e dos sujeitos, por meio da Teoria de Resposta ao Item (TRI), em uma medida de avaliacao da compreensao de leitura, englobando analises quantitativas e qualitativas do mapa de itens, assim como investigar a presenca de funcionamento diferencial dos itens (DIF). Participaram 518 criancas do 3o, 4o e 5o anos do ensino fundamental, com idades entre 6 e 16 anos, de escolas particular e publica de Belo Horizonte. Utilizou-se um texto elaborado de acordo com a tecnica de Cloze. Foi confirmada a unidimensionalidade do instrumento; verificou-se media de theta maior que a media de dificuldade dos itens; e, a presenca de DIF foi observada em alguns itens de acordo com os anos de ensino. Tais resultados demonstraram evidencias de validade para o instrumento e sao discutidos no trabalho.

Abstract P3-07-12: Risk Factors for Breast Cancer in Women Served in Odete Valadares Maternity, Belo Horizonte-MG

Abstract With changes in lifestyle of the population and changes in women reproductive patterns in the last century, the profile of mortality among women has changed, today the prevailing non-infectious chronic diseases, like cancer, including breast cancer. This paper is a case-control population-based, with the objective of evaluating the risk factors and their association with breast cancer in a population of women seen in Odete Valadares Maternity, in the Hospital Foundation of the State of Minas Gerais, in Belo Horizonte, between January and July 2006. Inclusion criteria were age over 18 years and referral for mammography service. Women with abnormal mammography and confirmation of malignancy of the breast were invited to participate in the study, and those with normal mammography were invited to compose the control group. We evaluated 710 women and 456 selected to participate in the study, 261 with breast cancer and 195 healthy controls. The participants were interviewed, answered a questionnaire on reproductive aspects and lifestyle habits (smoking, alcohol consumption, physical activity, children, age at menarche and menopause, oral contraceptive use and hormone replacement therapy and breast-feeding) and was measured anthropometric data. The association between several independent variables and the presence of breast cancer was evaluated by Student's t test, Mann-Whitney and chi square test. We estimated odds ratio and their respective confidence intervals. This study found a positive association between physical inactivity, menopause, nulliparity, lack of breastfeeding and family history of breast cancer and the diagnosis of breast cancer, and alcohol conferred a protective effect in this population. Significado atribuído por trabalhadores da saúde de Belo Horizonte-MG ao princípio da resolutividade nas ações cotidianas

Trata-se de estudo qualitativo, fundamentado na sociologia compreensiva, cujo objetivo foi o de compreender o significado atribuído pelos profissionais da rede hierarquizada de saúde de Belo Horizonte-MG, Brasil, ao princípio da resolutividade nas ações cotidianas de saúde. Trinta e dois trabalhadores foram os informantes desta pesquisa. Foi utilizada a entrevista semiestruturada e a técnica de análise de conteúdo. Profissionais descrevem o que é resolutividade e fatores que dificultam sua concretização. Percebem, também, que os pacientes nem sempre saem das Unidades com a resposta que desejam; e que o serviço é resolutivo no que corresponde ao seu nível de atenção, embora para a população e alguns gerentes seja o inverso. Conclui-se que se torna premente a reorganização de aspectos relacionados a este princípio doutrinário do SUS.

Prevalência baixa de adenovírus em crianças com diarreia em Belo Horizonte-MG

INTRODUCTION: Several microorganisms, among them enteric adenovirus, are widely recognized as etiological agents of acute diarrhea. The association between adenovirus and the disease varies among geographical regions and is poorly known in Brazil. OBJECTIVES: To investigate the presence of adenovirus in stool samples from children with and without diarrhea in Belo Horizonte-MG. To study factors associated with enteric adenovirus infection. MATERIAL AND METHOD: A qualitative immunochromatographic assay (kit VIKIA® Rota-Adeno, bioMerieux) was performed to detect adenovirus antigens in stool samples from 268 children with acute diarrhea and 124 without diarrhea at Hospital Infantil Joao Paulo II from January/2005 to December/2006. Clinical, laboratory, and epidemiologic data were recorded in SPSS database (SPSS Statistical package, IBM). RESULTS: Adenovirus was detected in 16 samples (4.1%): 12 of them (4.5%) were from children with acute diarrhea and 4 of them (3.2%) from children without diarrhea. The viral infection was more prevalent among girls and the age distribution was homogenous. Among the 16 children infected by adenovirus,11 (68.8%) were younger than 12 months old. However, no significant statistical difference was observed within the analyzed parameters. There was no seasonal distribution of adenovirus infection. CONCLUSION: Our data demonstrate that the prevalence of enteric adenovirus infection is low among children with diarrhea in Belo Horizonte-MG.

Nutritional Status and Associated Factors in Institutionalized Elderly

Peculiar situations due to physiological changes of aging, the diseases present and psychosocial factors and dietary factors can influence the nutritional status of the elderly. The purpose was to identify nutritional status and associated factors among elderly residents of a long-term institution for the elderly in Belo Horizonte-MG. Sectional study conducted using a representative random sample. Socioeconomic data, nutrient intake and anthropometry were collected and a Mini Nutritional Assessment was conducted. The analysis employed multinomial logistic regression and decision trees. There was high prevalence of overweight (46.1%) among subjects, according to body mass index, as well as risk of malnutrition, according to the Mini Nutritional Assessment (67.3%), and inadequate intake of nutrients. In the decision tree analysis, it was found that the more independent elderly, who received visits and contributed financially less to the institution, had better nutritional status. Inadequate nutritional status associated with social conditions and mobility indicates the need to promote healthy eating habits by a nutrition team in conjunction with nursing staff and other professionals providing comprehensive health care for the elderly.

Disinfection of domestic effluents by gamma radiation: Effects on the inactivation of Ascaris lumbricoides eggs

This work investigated the inactivation of Ascaris lumbricoides eggs in domestic effluents by gamma radiation from a 60Co source. Domestic wastewater was treated in a compact demo-scale system consisting of a UASB reactor and a trickling filter; treatment was carried out at the Center for Research and Training on Sanitation (CePTS), Federal University of Minas Gerais, in Belo Horizonte-MG, Brazil. One-liter of treated wastewater samples was artificially contaminated with an average of 1000 non-embryonated Ascaris lumbricoides eggs from human feces; samples were then irradiated in a multiple-purpose irradiator at different doses (0.5–5 kGy). Eggs were recovered from the wastewater and the viability of these irradiated eggs was evaluated; the description of the egg developmental phases with each dose of gamma radiation was recorded. Radiation doses of 3.5 kGy effectively disinfected effluents with lower concentrations of A. lumbricoides eggs; higher radiation doses of 5 kGy were necessary to disinfect effluents with higher eggs concentrations.

Clinical experience with a novel bovine collagen dura mater substitute

Dural substitutes are used to achieve watertight closure of the dura mater when adequate closure is not possible. The purpose of this study was to evaluate the efficacy and safety of a new collagen matrix dural substitute (Duradry, Technodry, Belo Horizonte MG) in the repair or expansion of cranial and spinal dura mater. Thirty patients, operated on between March and September, 2008, were studied. Surgical records were reviewed for sex, age, location of graft, technique, and presence of fistula or infection. The patients were followed up for at least 3 months, and presence of complications, such as cerebrospinal fluid leakage, infection, aseptic meningitis hydrocephalus, pseudomeningocele, was analyzed. Only one patient presented cerebrospinal fluid fistula. No patients presented wound infections, hydrocephalus, pseudomenigocele, meningitis, brain abscesses or signs of toxicity related to the dural substitute. The new dural substitute used in this study is effective and safe, and the initial results are similar to those of other dural substitutes reported in the literature.

In this article it is proposed a method and its supporting tool for stress and performance test generation of web based systems, in order to facilitate the execution of non-functional testing. The stress and performance test generation takes advantage of functional tests. Experimental studies are also presented showing that the method and the tool can generate overall effort reduction for these tests

A Influência de fatores sociais, individuais e lingüísticos no desempenho de crianças na leitura em voz alta de palavras isoladas

O estudo avalia a interferencia de fatores sociais, individuais e linguisticos no desempenho de leitura em voz alta de 333 criancas de Belo Horizonte cursando 1a a 4a serie do Ensino Fundamental. Medidas de tempo de reacao e precisao foram coletadas. Uma analise de regressao hierarquica (metodo de Estimacao de Equacao Generalizada) mostrou a existencia dos efeitos de regularidade grafema-fonema e fonema-grafema na leitura e um impacto do numero de categorias de regularidade utilizadas na emergencia desse efeito. Dos fatores sociais, apenas a escolaridade da mae foi importante para a precisao (na direcao inversa a esperada). Nao houve efeito de genero. Outros fatores alem dos tradicionais tambem mostraram-se relevantes, como a idade de aquisicao da leitura e a compreensao verbal. O estudo contribuiu com questoes teoricas relevantes para a avaliacao cognitiva da leitura no portugues.

WISC-III e WAIS-III na avaliação da inteligência de cegos

Owing to the almost lack of a national research on psychological testing for the evaluation of blind people, a psychometric study has been developed with the WISC-III and WAIS-III verbal scales. After adaptations in stimuli and instructions, scales were applied to 120 children and 52 adults in Belo Horizonte MG Brazil. Results show that modified verbal scales had a good internal consistency (a > 0.80) and the factorial analysis clearly indicated the presence of a single principal component. Actually it explains a total of 81% and 64% for WISC III and WAIS III respectively. Since adaptations do not affect the factorial structure of the above-mentioned scales, professionals may use the modified scales to measure the intelligence of blind people.

Equity and Resolution: Of Theory to Construction in the Daily of Health Professionals. A comprehensive study

It is a qualitative study, based on the comprehensive sociology that aimed to understand the health workers' conceptions on fairness and resolutivity and how those principles are inserted in their daily practices. Interviews were accomplished with 16 health professionals from the Basic Units of Belo Horizonte-MG. The data were analyzed and structured in three categories: 1) Fairness: the attention to the health founded in the differences; 2) Resolutivity: small answers making the difference; and 3) Family’s Health Program as reinvention of the health care: reality or utopia? The conceptions of fairness go from the access subject, right to the service, but it should also be recognized the social differences. The data show that the professionals look for solutions for the problems and users' needs, and they believe that the resolutivity should happen starting from an interdisciplinary practice. It is noticed that the strategy of the family health is a process in construction, stuffed with contradictions, because the learning of a differentiated way of attending is happening in the daily routines of the services, without a previous preparation, however they are learning in that daily practice to make the “health of the population” happen.

A abordagem e a clínica no atendimento aos moradores de rua portadores de sofrimento psíquico

A exposição da miséria nos espaços públicos, fruto do aumento das desigualdades sociais no mundo capitalista e globalizado, é um fenômeno urbano que se amplia nas grandes metrópoles brasileiras. As atividades desenvolvidas por grupos que vão buscar nas ruas das cidades modernas recursos para a sua sobrevivência tornaram-se uma demonstração exemplar de como a rua se transformou em espaço de construção de fronteiras econômicas, políticas e culturais. Pretendo fazer, neste artigo, um recorte nesse contingente que se amplia no espaço urbano, caracterizando um personagem, um antigo habitante das cidades, o portador de sofrimento psíquico, o denominado louco de rua. De forma singular, essa figura excêntrica e bizarra que faz parte da paisagem urbana, geralmente isolada do conjunto da população pauperizada que vive nas ruas, faz também, do espaço público, um lugar de moradia e sobrevivência. Um trabalho de abordagem, pesquisa e atendimento a essas pessoas que se encontram fora dos censos oficiais e das políticas públicas de assistência tem sido realizado desde 1996, por profissionais de saúde mental, em parceria com outros setores governamentais e ONGS, no Município de Belo Horizonte

Qualidade do serviço oftalmológico prestado aos pacientes ambulatoriais do Sistema Único de Saúde - SUS

To identify the perception of the ophthalmic service quality provided for outpatients of the public healthcare system as well as to detect which actions should be considered necessary and priority in order to improve its quality. A quantitative descriptive study was carried out on 100 outpatients of the public healthcare system which were submitted to ophthalmic tests at Fundação Hilton Rocha--Belo Horizonte-MG, from July 1st-July 30th 2004. Individual interviews were carried out by giving the interviewees two structured questionnaires adapted from the modified SERVQUAL. This scale is in agreement with the reality of the studied institute. The adapted SERVQUAL scale was submitted to statistical validation and it showed a suitable internal consistency index. In general terms, a slight general dissatisfaction was detected regarding ophthalmological service quality. The interviewees cared more about safety and reliability. A higher degree of dissatisfaction was detected mainly concerning fulfillment of procedures at scheduled appointments related to the execution of services within due time-limits. The institute is supposed to plan as well as carry out actions which lead to a general improvement in the patient's satisfaction regarding service quality and mainly reliability. Service quality monitoring through periodic use of the SERVQUAL scale will not only make it possible to plan highly precise and effective intervention strategies in these and in other healthcare services but it will also allow monitoring the responses to these actions. All these actions will contribute to the improvement of the service in the system as a whole.

WISC-III e WAIS-III: alterações nas versões originais americanas decorrentes das adaptações para uso no Brasil

O presente trabalho apresenta alterações efetuadas entre as versões americanas dos testes WISC-III e WAIS-III e as adaptadas para uso no Brasil. As análises empíricas realizadas com base nas amostras de adaptação, validação e normatização do WISC-III (801 crianças residentes na zona urbana da cidade de Pelotas/RS) e do WAIS-III (788 participantes adolescentes e adultos residentes na região metropolitana de Belo Horizonte/MG) acarretaram em alterações nas versões originais dos testes, no que se refere: aos conteúdos de alguns ítens dos subtestes verbais; à ordem de apresentação dos itens; aos tempos limites e à concessão de bônus; aos critérios de início e suspensão da aplicação e; aos grupos etários para os quais foram estabelecidas normas. Os resultados reforçam a importância de se proceder amplos estudos com instrumentos criados em contextos diferentes daqueles em que serão utilizados.
