Alleviating effect of the geriatric care system integrating physical and psychological on the common mental problems of the elderly

BackgroundWith the growth of age, various functions of the human body enter the aging stage. During this period, immunity and physique decline rapidly, making the elderly prone to various mental problems. The mental problems of the elderly mainly include schizophrenia, manic depression, etc. These mental problems may become worse after a period of time, leading to dementia, which seriously endangers the physical and mental health of the elderly. At the same time, the mental problems of the elderly will also lead to the difficulty in taking care of themselves, and they cannot independently complete simple life skills such as eating and dressing. In this case, some elderly people may also have such behaviors as excluding contact with others and not cooperating with medical personnel in treatment and nursing, which in turn makes it difficult to alleviate mental problems. Therefore, the corresponding nursing measures should be implemented from both psychological and physiological aspects to explore the effect of relieving the mental problems of the elderly.Subjects and Methods100 elderly patients with mental problems admitted in our hospital from January 2021 to January 2022 were selected as the research objects, and they were randomly divided into the observation group and the control group, with 50 cases in each group. The experiment was conducted with the consent of patients or their families to ensure the smooth development of the study. The elderly patients in the control group were given routine nursing care, while the patients in the observation group were given targeted psychological counseling on the basis of routine nursing care, including encouraging patients to actively express their emotions and actively communicating with patients. After four weeks, the relief effect of the elderly care system integrating physiology and psychology was illustrated by statistics of the self-management ability of the two groups of elderly patients.ResultsThe self-management ability of the elderly in the two groups after four weeks is shown in Table 1. The results showed that the self-management abilities of the elderly patients with mental disorders in the observation group, such as self-living ability and psychological state expression, were stronger than those in the control group after the integration of physiological and psychological elderly care measures, i.e., the elderly under conventional care, with a statistically significant difference (P < 0.05).ConclusionsThe mental problems of the elderly have become an important factor hindering the improvement of their quality of life. The study integrated physiological and psychological nursing measures when treating the elderly with mental disorders. Biofeedback therapy combined with Baduanjin on quality of life and gastrointestinal hormone level in patients with colorectal cancer.

BACKGROUNDWith the change in people’s lifestyles, the incidence of colorectal cancer (CRC) is increasing. It is essential to study the efficacy of various treatment methods for CRC patients to prevent and treat CRC.AIMTo investigate the efficacy of biofeedback therapy combined with Baduanjin in improving the quality of life and gastrointestinal hormone levels of patients with CRC.METHODSA total of 120 patients with CRC who were admitted to our hospital from June 2020 to June 2021 were included in the study. They were randomly divided into four groups (n = 30): the control group (group A), the biofeedback therapy intervention group (group B), the Baduanjin exercise intervention group (group C), and the combination group (group D). Patients in group A adopted the standard nursing mode and necessary health education. Patients in group B were treated with biofeedback therapy based on routine nursing care. Patients in group C were given Baduanjin intervention for 12 wk based on conventional drug treatment and care. Patients in group D were treated with biofeedback therapy and Baduanjin exercise. In this study, patients’ quality of life, gastrointestinal hormone levels, and clinical efficacy in the four groups were observed at baseline and 12 wk after intervention. Meanwhile, the correlation between gastrointestinal hormone levels and various functional areas of quality of life was analyzed. By comparing the observed indicators of patients in the four groups, the efficacy of biofeedback therapy combined with Baduanjin in improving the quality of life and gastrointestinal hormone levels of patients with CRC was explored.RESULTSAt baseline, there were no significant differences in quality of life, gastrointestinal hormone levels, or clinical efficacy among the four groups (P > 0.05). Twelve weeks after the intervention, the combination group’s quality of life, gastrointestinal hormone levels, and clinical effectiveness were better than those of the three other groups.CONCLUSIONOn the basis of routine nursing care, patients with CRC combined with biofeedback therapy and Baduanjin exercise can improve the quality of life of patients with CRC and the efficacy of gastrointestinal hormone levels.

Effects of multidisciplinary collaborative comprehensive nursing on uncertainty in illness and quality of life in patients with percutaneous coronary intervention treatment

Objective To explore the effects of multidisciplinary cooperative comprehensive nursing on uncertainty in illness and quality of life in patients with percutaneous coronary intervention(PCI) treatment. Methods From February to November 2017, 69 cases of coronary heart disease patients admitted to the department of cardiac surgery were randomly divided into the control group (34 cases) and the observation group (35 cases). The control group was given routine care of cardiac surgery.On the basis of routine nursing care, the observation group was given the multidisciplinary comprehensive nursing care composed of doctors, nurses, psychologists, nutritionists from admission to 1 day before discharge.Before and after the intervention, the Mishel disease uncertainty scale and the concise health questionnaire(SF-36)were used to evaluate the intervention effect of the two groups. Results After intervention, the total score of MUIS-FM in the observation group was(74.57±9.33)points, the sense of disease uncertainty was at a low level, and the total score of SF-36 was(59.55±10.93)points.The total score of MUIS-FM in the control group was (101.46±10.50)points, the sense of disease uncertainty was in the middle level, and the total score of SF-36 was(54.56±9.31)points.The differences were statistically significant(t=8.636, 8.472, P<0.05). MUIS-FM and SF-36 of the two groups were compared in all dimensions, and the differences were statistically significant(P<0.05). Conclusions The implementation of multidisciplinary comprehensive nursing for patients with coronary heart disease PCI can effectively control the uncertainty of disease and improve the quality of life of patients. Application of comprehensive nursing approach based on psychological intervention in patients with liver cancer undergoing enhanced CT scan

Objective To explore the application effect of comprehensive nursing based on psychological intervention in enhanced CT examination of patients with liver cancer. Methods By convenience sampling, a total of 100 patients with primary liver cancer who underwent enhanced CT examination in Imaging Department of Baoji Central Hospital from July 2016 to April 2018 were selected and randomly divided into control group and observation group, 50 cases in each group. The control group was given routine nursing care, while the observation group was given comprehensive nursing based on psychological intervention on the basis of routine nursing care. Self-rating Anxiety Scale (SAS) was used to evaluate the degree of nervousness and anxiety in the two groups, and the nursing satisfaction of the two groups was recorded. Results After intervention, the SAS score of the observation group was (33.92±6.67) , lower than that of the control group (47.04±8.32) , the difference was statistically significant (P<0.05) ; the nursing satisfaction of the intervention group was 88%, higher than 68% of the control group, the difference was statistically significant (P<0.05) . Conclusions Comprehensive nursing based on psychological intervention can effectively improve the patients' nervousness and anxiety undergoing enhanced CT scan, and improve the nursing satisfaction. Effect of cognitive behavioral therapy on health literacy of patients with permanent urinary stoma

Objective To explore the effect of cognitive behavioral intervention on improving the health literacy of patients with bladder cancer undergoing total cystectomy and ureterodermatostomy. Methods Totally eighty patients with bladder cancer after radical cystectomy and ureterodermatostomy from December 2016 to June 2018 in a Class Ⅲ Grade A hospital in Harbin were selected and randomly divided into the control group (n=40) and experimental group (n=40) . The patients in the control group received routine nursing care whereas the experimental group were treated with cognitive behavioral therapy on the basis of routine nursing care. The effects of intervention in the two groups was compared by the use of Health Literacy Self-rating Scale before and 1 month after intervention. Results One month after intervention, the total score of health literacy in the experimental group was (101.82±9.43) and that in the control group was (87.87±10.96) . The difference was statistically significant (t=6.102, P < 0.05) . Conclusions The cognitive behavioral therapy is an effective way to improve the health literacy of patients with bladder cancer after radical cystectomy and ureterodermatostomy. Effect of optimized health education on regulating use of antimicrobial drugs for outpatient patients in infection department

Objective To explore the role of optimizing health education in regulating the use of antimicrobial agents for outpatient patients in infection department. Methods Using convenience sampling method, 400 patients with moderate infection who needed intravenous antibiotics in the infectious Outpatient Department of Zhongnan Hospital of Wuhan University from January to December 2017 were selected and divided into experimental group and control group with 200 cases in each group according to random number table method. The patients in the control group were given routine nursing care, while the patients in the experimental group were given optimized health education intervention on the basis of routine nursing care. After the treatment, the knowledge, compliance and long-term efficacy of antimicrobial drugs were compared between the two groups. Results Before intervention, there was no significant difference in the knowledge of antimicrobial drugs between the two groups (P>0.05) ; after intervention, the knowledge level of antimicrobial drugs in the two groups was improved, and the scores of the dosage of antimicrobial drugs, the principle of drug use, adverse reactions, emergency measures and the total score of knowledge in the experimental group were higher than those in the control group with statistically significant differences (P<0.01) . Compliance rates of drug use on time, reasonable diet, timely re-visit, smoking cessation and alcohol cessation in the experimental group were 95.0%, 93.5%, 97.0% and 96.0%, respectively, which were higher than those in the control group (80.0%, 85.0%, 78.0% and 84.0%, respectively) with statistically significant differences (P<0.01) . After 6 months of intervention, the recurrence rate in the observation group was 4%, lower than the control group with 10%, and the difference was statistically significant (P<0.05) . Conclusions Optimized health education can effectively improve the knowledge level of antibiotics in outpatients of infectious department, improve the compliance of patients and reduce the recurrence rate, which is worthy of clinical application. Effect of collaborative nursing model on self-care ability, quality of life and complications of hemodialysis patients with diabetic nephropathy

Objective To explore the effect of collaborative nursing model on self-care ability, quality of life and complications of hemodialysis patients with diabetic nephropathy. Methods A total of 110 cases of diabetic nephropathy patients who received hemodialysis treatment in this hospital from June 2011 to June 2014 were selected as the research objects, and were randomly divided into control group and observation group, with 55 cases each.The control group was given routine nursing care during hemodialysis treatment, and the observation group was given collaborative nursing mode on the basis of routine nursing care in the control group.The ability of self-care was assessed by the Exercise of Self-care Agency Scale (ESCA), and the quality of life was assessed by the Quality of Life Scale (SF-36). Meanwhile, the complications of patients during hemodialysis were analyzed and compared. Results The quality of life score of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.01). The self-nursing ability score of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.01). The incidence of infection, heart failure, hypotension and other complications during hemodialysis treatment in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.01-0.05). Conclusion The implementation of collaborative nursing mode during hemodialysis treatment of diabetic nephropathy patients can enhance the self-care ability of patients, improve the quality of life of patients, and help reduce the incidence of complications. Effect of early combined functional exercise on lower limb rehabilitation of vessel bridge taken in coronary artery bypass surgery

Objective To investigate the effect of early combined functional exercise on lower limb rehabilitation of vessel bridge taken in coronary artery bypass surgery. Methods From January to October 2017, 90 patients undergoing coronary artery bypass surgery were randomly divided into control group and experimental group by coin toss, with 45 patients in each group. The control group was given routine nursing care, and the experimental group was given early combined functional exercise (foot dorsiflexion, ring rotation exercise and straight leg raising exercise) on the basis of routine nursing care. The difference of correlation index between two groups was compared. Results In the experimental group, the number of swelling in the thigh, crus and ankle were 20, 25 and 35 respectively; the control group was 32, 35, 43, the difference was statistically significant(χ2=6.55, 5.00, 8.38, P<0.05). The remission of limb swelling in the experimental group was also better than that in the control group(P<0.01). The incidence of limb wound complications: 6 cases in the experimental group, 18 cases in the control group (χ2=11.79, P<0.01)); limb muscle strength grade Lovett: experimental group 0 case with grade 3, 19 cases with grade 4, 26 cases with grade 5, control group 9 cases with grade 3, 26 cases with grade 4, 10 cases with grade 5, there were significant differences between two groups (χ2=17.20, P<0.05). The limb comfort score, postoperative bed time, postoperative hospital stay was (7.11±1.15) points, (5.28±1.07) d, (11.04±2.01) d in the experimental group and (5.18±1.13) points, (6.97±1.21) d, (12.67±1.59) d in the control group, the differences were significant(t=-8.02, 6.97, 4.23, P<0.01). Conclusions To carry out early functional exercise can not only take angiogenesis effectively reduce limb swelling rate after coronary artery bypass grafting, relieve the severity of limb swelling, but also can reduce the incidence of incision complications, promote the recovery of limb muscle strength, shorten the postoperative recovery time and postoperative hospitalization time, is worthy of promotion and use in clinical nursing work Key words: Functional exercise; Early rehabilitation; Vascular bridge; Lower limb; Coronary artery bypass grafting

Clinical observation of early rehabilitation nursing on the prognosis of patients with acute myocardial infarction effect

To explore the effect of early rehabilitation nursing on the quality of life of patients with acute myocardial infarction. Methods 41 cases of patients with acute myocardial infarction were selected according to the diagnostic criteria of acute myocardial infarction, stable vital signs, and no obvious complications. On the basis of routine nursing care, the system of early rehabilitation nursing care, the development of daily rehabilitation nursing plan. The cardiac function, clinical symptoms, quality of life and average length of stay were observed in 41 patients. Results During hospitalization, 41 patients had no deaths and all patients clinical symptoms significantly reduced; length of hospital stay was significantly decreased; heart function recovered well; the quality of life of patients significantly improved. Conclusion Early rehabilitation nursing care for patients with acute myocardial infarction is conducive to improving the condition, shorten the hospital stay, can greatly improve the quality of life of patients, it is worthy of clinical promotion.

Influence of CCM on postoperative function training effect of anal sphincter in patients with anus preserving surgery for low rectal cancer

Objective To discuss influence of continuous collaboration care model (CCM) on postoperative function training effect of anal sphincter in patients with anus preserving surgery for low rectal cancer. Methods 86 cases with low rectal cancer treated by anus preserving surgery from July to December in 2013 were selected as control group, to whom routine nursing care was carried out. 88 patients with low rectal cancer treated by anus preserving surgery from July to December in 2014 were selected as observation group, to whom continuous CCM was implemented on the basis of routine nursing care in the control group. Defecation function and quality of life of patients in the two groups were compared 3 months and 6 months after the surgery. Results After 3 months of nursing intervention, 81.8% of patients in the observation group had fine anal function, higher than the number in the control group, 47.7%; after 6 months, the proportions of the two groups were 90.9% and 67.4% (χ2=8.632, 7.523; P<0.05). Physiological function, psychological function, social function, self-awareness of health and score of quality of life of patients in the observation group were all higher than those of the control group (t=5.264, 4.635, 4.265, 4.145, 3.958; P<0.01). Conclusions Implementation of continuous collaborative family care management can promote recovery of patients′ defecation function and improve their quality of life. Effects of measurement of gastric residual volume on the prevention from aspiration pneumonia in patients with ischemic stroke

Objective To explore the effects of measurement of gastric residual volume on the prevention from aspiration pneumo- nia in ischemic stroke patients. Methods A total of 63 cases of ischemic stroke patients with dysphagia and indwelling gastric tube were se-lected as control group and 76 cases as observation group, who were admitted from March 2011 to August 2011 and from September 2011 to February 2012 respectively. Measurement of gastric residual volume in the basis of routine nursing care in the observation group was adopted. The aspiration pneumonia incidence rate, indwelling time of gastric tube and hospitalization time between the two groups were compared be-tween two groups. Results Compared with the control group, in the observation group the aspiration pneumonia incidence rate was lower （23.3% vs 39.7%, P =0. 039）, indwelling time of gastric tube was shorter [ （ 10. 88 ±4. 77） d vs （ 13.05 ±6. 54） d, P =0. 0271, hospitalization time was shorter [ （ 16.38 ± 5.08 ） d vs （ 18.86 ± 4. 75 ） d, P = 0. 004]. Conclusions The measurement of gastric resid-ual volume can reduce the occurrence of aspiration pneumonia and shorten the indwelling time gastric tube and hospitalization time in ische-mic stoke patients with dysphagia. Nursing of emotional disorder in patients with alcohol dependence

Objective To analyze the nursing effect of emotional disorder in patients with alcohol dependence.Methods From January 2009 to February 2010,81 patients in our hospital with emotional disorder of alcohol dependence were randomly divided into the observation group (41 cases)and the control group (40 cases).The control group was given routine nursing care,and the observation group was given enhanced care for emotional disorders on the basis of routine nursing care,the clinical results of two groups were compared.Results In the observation group,the re-drinking rate and readmission rate were19.5 ％ and 7.3％,significantly lower than 40.0％ and 27.5％ of the control group,the time interval of re-drinking of the observation group was significantly longer than the control group,the anxiety and depression scores were significantly better than the control group.Conclusions To give enhanced care on the basis of routine nursing care for patients with emotional disorders because of alcohol dependence has better clinical effect,it contributes to the patients′ rehabilitation. Effects of music therapy on depression and duration of hospital stay of breast cancer patients after radical mastectomy.

Breast cancer remains the most important cancer among women worldwide. The disease itself and treatment may have a profound impact on the patients' psychological well being and quality of life. Depression is common in breast cancer patients and affects the therapeutic effects as well as prolongs the duration of hospital stay. However, few studies reported the effectiveness of music therapy on depression and duration of hospital stay of female patients with breast cancer after radical mastectomy. One hundred and twenty subjects were recruited to this clinical trial and randomly allocated to two groups. The experimental group (n = 60) received music therapy on the basis of routine nursing care, whereas the control group (n = 60) only received the routine nursing care. The whole intervention time was from the first day after radical mastectomy to the third time of admission to hospital for chemotherapy. Data of demographic characteristics and depression were collected by using the General Questionnaire and Chinese version of Zung Self-rating Depression Scale (ZSDS) respectively. One pre-test (the day before radical mastectomy) and three post-tests (the day before discharge from hospital, the second and third admission to hospital for chemotherapy) were utilized. Duration of hospital stay was calculated from the first day after radical mastectomy to the day of discharged from hospital. The mean depression score of all subjects was 37.19 ± 6.30. Thirty-six cases (30%) suffered from depression symptoms, with 26 (72.2%) mild depression cases, 9 (25.0%) moderate depression cases, and 1 (2.8%) severe depression case. After music therapy, depression scores of the experimental group were lower than that of the control group in the three post-tests, with significant differences (F = 39.13, P < 0.001; F = 82.09, P < 0.001). Duration of hospital stay after radical mastectomy of the experimental group ((13.62 ± 2.04) days) was shorter than that of the control group ((15.53 ± 2.75) days) with significant difference (t = -4.34, P < 0.001). Music therapy has positive effects on improving depression of female patients with breast cancer, and duration of hospital stay after radical mastectomy can be reduced. It is worthy of applying music therapy as an alternative way of nursing intervention in clinical nursing process of caring female patients with breast cancer.
