Spiral time-of-flight magnetic resonance angiography for intracranial vascular imaging: performance compared to conventional Cartesian angiogram.

Computed tomography angiography (CTA) and digital subtraction angiography (DSA) usually raise the risk of potential malignancies with cumulative radiation doses. Current time-of-flight magnetic resonance angiography (TOF-MRA) (dubbed as cTOF), which is based on Cartesian sampling mode, may show limited diagnostic conspicuity at sinuous or branching regions. It is also prone to relatively high false positive diagnoses and undesirable display of distal intracranial vessels. This study aimed to use spiral TOF-MRA (sTOF) as a noninvasive alternative to explore possible improvement, such that the application of magnetic resonance angiography (MRA) can be extended to facilitate clinical examination or cerebrovascular disease diagnosis and follow-up studies. Initially, 37 patients with symptoms of dizziness or transient ischemic attack were consecutively recruited for suspected intracranial vascular disease examination from Zhongshan Hospital of Xiamen University between July 2020 and April 2021 in this cross-sectional prospective study. After excluding 1 patient with severe scanning artifacts, 1 patient whose scanning scope did not meet the requirement, and 1 patient with confounding tumor lesions, a total of 34 participants were included according to the inclusion and exclusion criteria. Each participant underwent intracranial vascular imaging with both sTOF and cTOF sequences on a 3.0 T MR scanner with a conventional head-neck coil of 16 channels. Contrast CTA or DSA was also performed for 15 patients showing pathology. Qualitative comparisons in terms of image quality and diagnostic efficacy ratings, quantitative comparisons in terms of signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), vessel length, and sharpness were evaluated. Pair-wise Wilcoxon test was performed to evaluate the imaging quality derived from cTOF and sTOF acquisitions and weighted Cohen's Kappa was conducted to assess the rating consistency between different physicians. Compared to cTOF, sTOF showed better performance with fewer artifacts. It can effectively alleviate false positives of normal vessels being misdiagnosed as aneurysm or stenosis. Improved conspicuity was observed in cerebral distal regions with more clearly identifiable vasculature at finer scales. Quantitative comparisons in selected regions revealed significant improvement of sTOF in SNR (P<0.01 or P<0.001), CNR (P<0.001), vessel length (P<0.001), and sharpness (P<0.001) as compared to cTOF. Besides, sTOF can depict details of M1 and M2 segments of middle cerebral artery (MCA) at metallic implant region, showing its resistance to magnetic susceptibility. The sTOF shows higher imaging quality and lesion detectability with reduced artifacts and false positives, representing a potentially feasible surrogate in intracranial vascular imaging for future clinic routines.

Simple Host-Guest Assembly for High-Resolution Magnetic Resonance Imaging of Microvasculature.

Magnetic resonance angiography (MRA) is an important imaging technique that can be used to identify and characterize various types of vascular diseases. However, currently used molecular contrast agents are unsuitable for MRA due to the short intravascular retention time, the whole-body distribution, and the relatively low contrast effect. In this study, we developed a vascular analysis contrast agent (i.e., VasCA) for MRA, which is a simple and biocompatible 1:1 host-guest assembly of PEGylated β-cyclodextrin and gadolinium chelate with renal clearable size and high relaxivity (r1 = 9.27 mM-1 s-1). Its biocompatibility was confirmed by in vivo animal studies as well as in vitro 3D cell culture. In a tumor-bearing rat model, VasCA circulated in the blood vessels much longer (4.3-fold increase) than gadoterate meglumine (Dotarem) and was mainly excreted by the renal system after intravenous injection. This feature of VasCA allows characterization of tumor microvasculature (e.g., feeding and draining vessels) as well as visualization of small vessels in the brain and body organs. Furthermore, after treatment with an angiogenesis inhibitor (i.e., sorafenib), VasCA revealed the vessel normalization process and allowed the assessment of viable and necrotic tumor regions. Our study provides a useful tool for diverse MRA applications, including tumor characterization, early-stage evaluation of drug efficacy, and treatment planning, as well as diagnosis of cardiovascular diseases.

VSION as high field MRI T1 contrast agent: evidence of their potential as positive contrast agent for magnetic resonance angiography

Because of their outstanding magnetic properties, iron oxide nanoparticles have already been the subject of numerous studies in the biomedical field, in particular as a negative contrast agent for T2-weighted nuclear magnetic resonance imaging, or as therapeutic agents in hyperthermia experiments. Recent studies have shown that below a given particle size (i.e. 5 nm), iron oxide may be used to provide a significant positive (brightening) effect on T1-weighted MRI. In such an application, not only the size of the crystal, but also the control of the coating process is essential to ensure optimal properties, especially at a very high field (> 3 T). In this work, we focused on the development of very small iron oxide nanoparticles as a potential platform for high field T1 magnetic resonance angiography (MRA) applications. The feasibility has been evaluated in vivo at 9.4 T, demonstrating the usefulness of the developed system for MRA applications.

Micro‐vascular imaging experiences of time‐of‐flight MRA at 7T for cerebrovascular diseases

ABSTRACTSurgical or endovascular approaches have proved effective for large‐vessel diseases over the past decade. However, approaches for small vessel diseases are unlikely to be accomplished by those for large vessels and only few have been applied, because it is hard to access to those small vessels and one could not directly delineate the affected small vessels due to a lack of detection modalities. This study is to examine patients with vascular diseases using ultra‐high field 7T MRI with conventional time‐of‐flight (TOF) sequence, 3D fast low‐angle shot (FLASH) gradient‐echo. We have evaluated several radio‐frequency (RF) coils to find the optimal one for 7T magnetic resonance angiography (MRA), especially for micro‐vascular imaging. We have conducted several comparison studies with vascular disease patients. The results showed that micro‐vessels such as lenticulostriate arteries in the subjects with risk factors like hypertension or stroke patients were significantly less than in the healthy subjects. 7T MRA images in steno‐occlusive patients also showed clearly numerous collateral vessels not visible by 1.5T or 3T MRA. Furthermore, 7T MRA images were comparable to those obtained by digital subtraction angiography (DSA), particularly for micro‐vascular imaging. In this article, we would like to share the clinical experiences on 7T MRA that vascular images of 7T MRA were superior to conventional angiography images including 1.5T and 3T MRA, and even comparable to DSA. We also expect that further technical development and clinical applications of 7T MRA would be a clinically important diagnostic tool, in terms of an early detection of the stroke in a totally non‐invasive manner. © 2014 Wiley Periodicals, Inc. Vascular Imaging of the Head and Neck

Pathologies of the cranial and cervical vasculature are often devastating and require rapid and accurate anatomic delineation. The clinical scenario will often dictate the most appropriate imaging modality warranted. In this review, we will discuss the application of magnetic resonance angiography (MRA), computed tomographic angiography (CTA), and digital subtraction angiography (DSA) in the evaluation of cerebrovascular diseases with an emphasis on ischemic and hemorrhagic stroke. Noninvasive modalities such as MRA and CTA are becoming widely available and increasingly sensitive for most pathologies, but DSA continues to be the gold standard in cases of diagnostic ambiguity and is increasingly utilized with advances in endovascular treatments.

Magnetic Resonance Angiography Using Fresh Blood Imaging in Oral and Maxillofacial Regions

The present paper provides general dentists with an introduction to the clinical applications and significance of magnetic resonance angiography (MRA) in the oral and maxillofacial regions. Specifically, the method and characteristics of MRA are first explained using the relevant MR sequences. Next, clinical applications to the oral and maxillofacial regions, such as identification of hemangiomas and surrounding vessels by MRA, are discussed. Moreover, the clinical significance of MRA for other regions is presented to elucidate future clinical applications of MRA in the oral and maxillofacial regions.

Non-contrast-enhanced MR angiography of the abdomen

Non-contrast-enhanced magnetic resonance angiography (MRA) techniques have experienced a resurgence of interest in the MR community, in part because of safety concerns related to the possible link between gadolinium-based contrast agents and nephrogenic systemic fibrosis (NSF). In abdominal MRA, NSF is of particular concern, given that many of the patients may have renal disease. After introducing various non-contrast-enhanced MRA techniques, this article focuses on MRA applications in the abdomen, specifically the renal arteries and portal and hepatic veins. Developments on the horizon are discussed, including techniques that provide standardization of renal artery imaging, challenges in imaging of the hepatic arteries, and further advancement at 3 T.

Magnetic Resonance Angiography With Midsagittal Saturation for the Assessment of Blood Flow From Superficial Temporal Artery-to-Middle Cerebral Artery Bypass

The purpose of this study was to evaluate the feasibility of magnetic resonance angiography (MRA) with midsagittal saturation for depiction of superficial temporal artery-to-middle cerebral artery bypass flow. Eleven patients undergoing bypass surgery in 13 regions were examined by external carotid artery angiography (ECAG) and 3-dimensional time-of-flight MRA with saturation pulse applied to the midsagittal plane covering the internal carotid arteries. We classified the extent of visualization of bypass flow into 4 types and examined the agreement of findings between them. The MRA revealed types of bypass flow in agreement with those observed on ECAG in 10 of the 13 regions. The MRA underestimated bypass flow in 2 regions and overestimated it in 1 region. The MRA with midsagittal saturation demonstrated bypass flow in agreement with ECAG in most cases. Application of MRA with midsagittal saturation permits noninvasive assessment of physiological flow from superficial temporal artery-to-middle cerebral artery bypass for a postoperative follow-up.

Coronary Artery Magnetic Resonance Angiography (MRA): A Comparison between the Whole-Heart and Volume-Targeted Methods Using a T 2 -Prepared SSFP Sequence

In this study, coronary MRA was performed on 10 healthy volunteers using the whole-heart and volume-targeted scans with comparable imaging parameters. Similar results in the SNR, CNR, and vessel diameter were observed. The depicted length of coronary arteries was longer using the whole-heart scan (whole-heart: RCA/LAD = 13.4 +/- 3.9/10.5 +/- 1.6 cm; volume-targeted: RCA/LAD = 11.0 +/- 2.6/8.7 +/- 1.8 cm). Imaging times for the RCA- (3.8 +/- 1.4 minutes) and LAD-targeted (3.6 +/- 1.3 minutes) are similar, while the time required for one whole-heart scan is significantly longer (12.2 +/- 4.0 minutes). The measured vessel sharpness was higher using the volume-targeted method (whole-heart: RCA/LAD = 0.65 +/- 0.18/0.78 +/- 0.16; volume-targeted: RCA/LAD = 0.84 +/- 0.22/0.90 +/- 0.20). Combination of the whole-heart and volume-targeted methods could be potentially useful in clinical applications of coronary MRA.

Three Types of Physical Measurements Needed to Characterize Iron Oxide Nanoparticles for MRI and MRA: Magnetization, Relaxometry, and Light Scattering

Iron oxide nanoparticles, with monocrystalline cores (generally magnetite or maghemite) and coated with organic polymer to increase chemical stability and solubility, have generated widespread interest as negative contrast (susceptibility) agents for magnetic resonance imaging (MRI) (1,2). For negative agents, the requisite T2 shortening depends on the geometry of the physiologic structures that take up the agent (2) but is relatively insensitive to the size and structure of the nanoparticles themselves. Nanoparticles also have a potential as positive agents for magnetic resonance angiography (MRA) (3–6), an application that depends on T1 shortening in the vasculature by individual nanoparticles, provided that T2 is not too short (7). To optimize their suitability for MRA, the agents must be synthesized and characterized reproducibly, the size of the magnetic core must be carefully controlled, and the interaction of the organic coating with nearby solvent molecules must be understood (8). Here we describe a set of physical techniques for measuring the parameters that determine MRA efficacy: the magnetic moment and diameter d of the iron oxide cores, and the interaction of organic coating with solvent. We study magnetization (8) as a function of magnetic field B0; l/Tl nuclear magnetic relaxation dispersion (NMRD) profiles (9) for B0 from 0.24 mT to 1.2 T (0.01–50 MHz) and 1/T2 at 0.47 T (20 MHz); and solute translational diffusivity by photon correlation spectroscopy (PCS) (10). As demonstrated earlier (8), magnetization data, being thermodynamic, yield and the saturation magnetization of the particles, Msat. Combined with total iron content and the known density of the core, one obtains d. When these results are combined with NMRD data—which depend on and d and yield certain correlation times—one can detect any water-opaque organic “rind” and measure the diffusivity D of outer sphere solvent, a measure of viscous interactions between solvent and polymer. These interactions can be quantitated by PCS, which yields the effective radius of gyration of the polymers (11), including contributions from both polymer and viscously entrained water molecules. This trio of measurements lets one characterize the physical properties of solute nanoparticles self-consistently, deduce their behavior in MRA applications, and guide the development of improved agents. We report on two preparations of coated iron oxide nanoparticles: (a) CLARISCAN, a Nycomed Amersham product in clinical trials (8); and (b) MION-46L, a laboratory preparation (11) of MION-46 obtained from Massachusetts General Hospital.

Magnetic resonance angiography of the brain and neck: Techniques and applications

Magnetic resonance angiography has become a widely used and versatile technique for evaluating many neurologic diseases. Even with the current widespread use of magnetic resonance angiography, it continues to evolve as older techniques are optimized and new methods are developed. A general description of magnetic resonance angiography techniques will be provided, and the strengths and weaknesses of the techniques will be highlighted. The article focuses on time-of-flight and gadolinium-enhanced magnetic resonance angiography methods. The application of magnetic resonance angiography in the evaluation of arterial dissections, cerebral aneurysms, arteriovenous malformations, and carotid stenosis will be described.

Magnetic resonance angiography of peripheral vascular disease.

Due to rapid developments in gradient and software technology, magnetic resonance angiography has become a routine clinical tool for imaging of blood vessels in the body. The introduction of contrast-enhanced 3D gradient echo techniques in particular has brought about a turning-point in the application of magnetic resonance angiography in daily clinical practice. This paper presents a brief overview of the various approaches to the application of magnetic resonance angiography for imaging of the peripheral vasculature, not only of the arteries, but also of the veins.

Contrast-enhanced three-dimensional magnetic resonance angiography of the mesenteric vasculature.

The diagnosis and treatment of mesenteric occlusive disease is important due to the high morbidity and mortality associated with ischemia of the bowel. This article describes the application of magnetic resonance angiography (MRA) as a minimally invasive tool for diagnosing mesenteric and portal vascular disease. The techniques for three-dimensional (3D) contrast-enhanced MRA and flow measurement of the mesenteric circulation are described. Excellent image quality is obtained using sagittal contrast-enhanced 3D MRA of the aorta and proximal mesenteric vessels. Delineation of the small distal mesenteric branch vessels is still limited due to the finite spatial resolution of MRA. The application of MRA techniques to the diagnosis of mesenteric ischemia, revascularization, transplantation, and portal hypertension are demonstrated. Finally, the merits of various acquisition techniques and future contrast agents are discussed. MR angiography using intravenously administered contrast agents provides the clinician with a powerful, minimally invasive method for diagnosing mesenteric vascular disease.

Novel paramagnetic macromolecular complexes derived from the linkage of a macrocyclic Gd(III) complex to polyamino acids through a squaric acid moiety.

Macromolecular Gd(III) complexes may find useful application as contrast agents for magnetic resonance angiography (MRA). Herein two novel systems are reported, namely Gd(DO3ASQ)3-lys16 and Gd(DO3ASQ)30-orn114. Their syntheses are based on the ability of the squaric acid moiety to act as a linker between the DO3A (1,4,7, 10-tetraazacyclododecane-1,4,7-triacetic acid) chelate moiety and the polyamino acidic chain. Moreover, the squaric acid participates in the coordination cage of the Gd(III) ion. The investigation of 1H and 17O NMR relaxation processes of solvent water nuclei allowed a detailed characterization of the systems under study. Gd(DO3ASQ)30-orn114 displays a remarkable ability to enhance the water proton relaxation rate of its solutions, and it may be considered as potential contrast agent for MRA applications.

Three-dimensional contrast-enhanced magnetic resonance angiography of the thoracic vasculature.

Magnetic resonance angiography (MRA) has become a useful non-invasive imaging technique for the assessment of vascular disease. Due to limitations such as respiratory motion artefacts, saturations problems, and long acquisition times, applications of MRA in the thorax have largely been restricted to imaging of the aorta. The recent introduction of breath-hold three-dimensional (3D) contrast-enhanced MRA promises not only to enhance conventional MR protocols for aortic imaging, but to extend the clinical indications of MRI to diseases affecting other vascular structures of the thorax, most notably the pulmonary arteries. This article describes the technical aspects of contrast-enhanced 3D MRA and reviews existing and potential future clinical applications.

Three-dimensional time-of-flight MR angiography with variable TE (VARIETE) for fat signal reduction.

Uniform fat saturation over a large region of interest remains a problem in time-of-flight (TOF) magnetic resonance angiography applications. We demonstrate that a variable echo time with an opposed phase value at low spatial slice select frequencies can effectively reduce most of the fat signal in an otherwise standard three-dimensional TOF acquisition. We evaluated this method at 1.5 T using a short TE = 5.3 ms and a long TE = 6.75 ms for different values of the slice encoding gradient (i.e., different kz values). Shorter echo time (TE = 5.3 msec) was used at higher spatial slice select frequencies, but all echoes have the same gradient structures. By keeping the number of slice encoding steps with longer echoes to a minimum, field inhomogeneity effects on flow compensation remained small. A magnetization transfer saturation pulse was used to suppress signal of brain parenchyma. Overall, highly uniform and selective fat signal reduction was obtained while maintaining superior flow compensation in all volunteer studies.

Selected indications for and applications of magnetic resonance angiography in children.

The purpose of this study was to investigate the efficacy and practicality of magnetic resonance angiography (MRA) in evaluating pediatric cerebrovascular disorders. A retrospective evaluation was performed of MR angiograms in 20 pediatric patients with cerebrovascular pathology. When appropriate, comparisons were made with duplex ultrasonography or conventional catheter angiography. MRA accurately assessed the patency of carotid reanastomoses in 8 babies who had previously undergone extracorporeal membrane oxygenation (ECMO). In 6 patients with moyamoya syndrome, MRA accurately evaluated stenotic intracranial carotid and circle of Willis arteries and progressive enlargement of the superficial temporal and middle cerebral arteries after revascularization procedures, and thus obviated the need for sequential angiograms. Thrombi and emboli were identified in 4 of 5 patients with symptoms and imaging evidence of an acute stroke. Two-dimensional time-of-flight MR venograms, acquired in both axial and coronal planes, were useful for preoperative venous mapping in a patient with an occipital encephalocele and detecting venoocclusive disease. MRA provided diagnostically useful information in a spectrum of pediatric cerebrovascular disorders. It can be used as the initial vascular imaging modality for patients with imaging evidence of acute cerebrovascular event, to evaluate progression of chronic vasoocclusive disease, to evaluate vessel patency following intracranial revascularization surgery, and for visualization of the venous circulation.

Magnetic resonance angiography. II. Applications.

The objective of this paper is to describe clinical applications of magnetic resonance angiography. The data used were obtained from recent articles on vascular and flow magnetic resonance imaging retrieved from Index Medicus 1988-91. Other contributions were from the 19th Annual Meeting of the American Society of Neuroradiology held in Washington DC in June 1991 and the 10th Annual Conference of the Society of Magnetic Resonance in Medicine in August 1991. The data are presented in such a way as to give the reader unfamiliar with magnetic resonance a basic insight into some of the new imaging techniques. Magnetic resonance imaging is advancing at a rapid rate. Magnetic resonance angiography is already finding many areas of clinical use with concomitant reduction in conventional angiographic and duplex Doppler ultrasound procedures.

Magnetic resonance angiography in pediatric neuroradiology

Magnetic resonance angiography (MRA) is a novel technique that uses gradient echo pulse sequences and computer postprocessing technology to create vascular-flow images. The technique is easy to apply, noninvasive, and frequently yields information not available by other noninvasive means. The utility and benefits of MRA are amply documented by clinical studies in the neuroimaging literature. This article reviews the techniques of MRA and the current literature in the application of MRA in pediatric imaging. Illustrative cases demonstrating the use of MRA in pediatric neuroradiology are presented. The utilization of MRA in congenital lesions such as aberrant internal carotid artery and arteriovenous malformation and in differing causes of ischemic disease is highlighted. There is an ever-expanding number of applications of MRA in pediatric neuroimaging. Despite the lack of standardized protocols, with a thorough knowledge of the basic principles, MRA can be successfully applied in many imaging situations. The ease of use and noninvasive nature of MRA make it an ideal tool in the evaluation of the pediatric neurovasculature.
