Individuals progressive tension protocol in the application of elastic bandage in nonspecific chronic low back pain: a case study

Introduction: Low back pain is a disorder of musculoskeletal origin of multifactorial character, which can become chronic when it persists for more than 3 months. It is expected that 80% of the world's population will present with low back pain at some stage of life, characterizing it as a serious public health condition that requires specific interventions for treatment. Given the context, elastic banding arises, a resource that can be useful to these patients, however, its effectiveness requires that the bandage be of quality that the therapist has experience and that its application is standardized, the latter being scarce in the current literature. Thus, the present study sought to observe what would be the application of a progressive tension protocol with elastic bandage in an individual with nonspecific chronic low back pain. Methods: A 47-year-old female, caregiver of the elderly and diagnosed with nonspecific chronic low back pain participated in the study. The treatment protocol lasted 10 weeks, where the elastic bandage was applied to the participant and in the first and last session, in addition to the report of the level of pain obtained through the Numerical Pain Scale (END), the following questionnaires were used to evaluate: Oswestry Disability Index (ODI), Survey of Pain Attitude (SOPA-B) and Fear Avoidance Beliefs Questionnaire (FABQ). Conclusion: The findings suggest that the application of elastic bandage following the progressive tension protocol promoted a reduction in disability index, pain and fear of movement, such outcomes can be considered clinically relevant, given the constant search for therapeutic techniques that provide improvement in the quality of life of patients with chronic low back pain.

Static Baropodometry Immediately After Application of Elastic bandage on the Ankle

AbstractElastic bandage is able to promote support or external support, aiding the normal function of the tissues and in the proprioception. Baropodometry is used to identify abnormalities, classify the step variations, compare results, so as to obtain the effectiveness of treatments. The objective of the study was the analysis of the immediate effects of elastic bandage in ankle immediately after static baropodometry. Type of study: Experimental: clinical trial. 25 individuals were evaluated (18 women and 7 men) by means of static tests (1 without elastic bandage; 2. after 10 min with the elastic bandage on bipodal stance, at the baropodometer. It was applied at the ankle of the dominant lower limb the elastic bandage (Therapy Taping ® method),from one malleolus to another, passing by the plantar surface of the foot, at maximum tension in order to promote greater joint stability. Variables: middle pressure (PM) and maximum (PMa) (Kpa) and surface area (SA) (cm²), in addition to the pressures in the retro foot and fore foot were analyzed. Data were normalized by the Shapiro Wilk test, followed by the t-student test for a p≤0.05 in comparing pre and post immediate intervention in SPSS. Mean height was 1.67 ± 0.87, the weight was 65.08±11.32 and BMI 23.16±02.96. Among the variables analyzed, there was significant difference in PMa in the left foot (which received no intervention) (p=0.000) and in the fore right foot (which received intervention) (p=0.025), in the other items there was no difference. Elastic bandage for the ankle stabilization is able to change the distribution of plantar pressure immediately after its application. Keywords: Ankle. Postural Balance. Athletic Tape. A baropodometria e utilizada para identificar anormalidades, classificar as variacoes de pisada, comparar resultados, para assim, obter a efetividade de tratamentos. O objetivo do estudo foi a analise dos efeitos imediatos da bandagem elastica em tornozelo imediatamente apos baropodometria estatica. Tipo de estudo: Experimental: ensaio clinico. Foram avaliados 25 individuos (18 mulheres e 7 homens) por meio de testes estaticos (1. sem bandagem elastica; 2. apos 10min com a bandagem elastica) em postura bipodal, no baropodometro. Aplicou-se no tornozelo do membro inferior dominante a bandagem elastica (Therapy Taping® method), de um maleolo a outro, passando pela superficie plantar do pe, em tensao maxima a fim de promover maior estabilidade articular. Variaveis: pressao media (PM) e maxima (PMa) (Kpa) e area de superficie (AS) (cm²), alem das pressoes no retro pe e ante pe foram analisadas. Dados foram normalizados pelo teste de Shapiro Wilk, seguidos do teste t de student para um p≤0,05 na comparacao pre e pos intervencao imediata no SPSS. Media de estatura foi de 1,67±0,87, o peso foi de 65,08±11,32 e o IMC 23,16±02,96. Dentre as variaveis analisadas houve diferenca significativa na PMa no pe esquerdo (que nao recebeu intervencao) (p=0,000) e no ante pe direito (que recebeu intervencao) (p=0,025), nos demais itens nao houve diferenca. Bandagem elastica para estabilizacao de tornozelo e capaz de alterar distribuicao de pressao plantar imediatamente apos sua aplicacao. Palavras-chave: Tornozelo. Equilibria Postural. Fita Atletica.

Assessment of Splints Applied for Pediatric Fractures in an Emergency Department/Urgent Care Environment.

Fractures are common in the pediatric population. The initial evaluation is rarely by an orthopaedic surgeon, but commonly an emergency room or urgent care center physician/extender. This typically involves splint application by a nonorthopaedist to immobilize the extremity and provide stabilization. Iatrogenic injuries from inappropriate splint placement are a potential public health and legal concern that can lead to complications. The primary purpose of this study was to prospectively evaluate the adequacy of all splints placed on patients who presented to a pediatric orthopaedic office; secondary outcomes included assessing prevalence and types of complications that were associated with inadequate splints. Patients aged 0 to 18 years who presented with a splint were prospectively enrolled. Information was obtained regarding demographics of the patient and splint placement. Splints were evaluated for functional position, appropriate length, and presence of elastic bandage on the skin. Photographs were taken of each splint, and the extremity was examined for any soft tissue complications. Splints were not removed in 31 patients who had undergone fracture reduction. In total, 275 patients were prospectively enrolled. Splints were improperly placed in 93%, with application of elastic bandage directly to the skin accounting for 77%. Improper positioning was observed in 59%, and inappropriate splint length was present in 52%. Skin and soft tissue complications were observed in 40%. The most common iatrogenic splint-related complication was excessive edema, seen in 28%. Direct injury to the skin and soft tissue was seen in 6%. Many practitioners incorrectly apply splints, potentially leading to suboptimal results or causing injury. Complications of poor splint placement include excessive swelling, skin breakdown, and poor immobilization. Health care workers who treat pediatric fractures may benefit from more extensive education regarding proper splinting techniques. Level 2-therapeutic study.

Acute effect of elastic bandage technique on middle deltoid muscle force and activation in healthy men

Abstract Introduction: Elastic bandages (EB), such as Kinesio taping, have been widely used in sports or daily life activities with the aim of preventing or reducing musculoskeletal injuries. It has been suggested that Kinesio Taping is capable of altering muscle activation through neurophysiological mechanisms, but the evidences about this are controversial. Objective: To verify the acute effect of EB on maximum voluntary isometric force (MVIF) and muscle activation of the middle deltoid muscle during muscle contraction. Method: Twenty-four healthy male (24 ± 4 years, 73.2 ± 13.9kg, 1.80 ± 0.10m) were randomly assigned to a group with elastic bandage activated at 100% (AEB n = 8); with tensionless elastic bandage (NEB n = 8), and the control group (CG n = 9). The volunteers were instructed to perform 5s of maximal isometric contraction at 90° of shoulder abduction while the MIVF and EMGrms records were registered. Results: One-way ANOVA was unable to identify significant difference (α = 0.05) in MIVF and EMGrms of the middle deltoid at 90° of shoulder abduction. Conclusion: The application of elastic bandage was not able to alter the production of maximal isometric voluntary contraction and activation of the middle deltoid muscle of healthy individuals and, therefore, its use is not justified for these purposes.

Comparison of the application of elastic bandage and medical tape in pain reduction in primary and secondary teachers

ABSTRACT BACKGROUND AND OBJECTIVES: The convolutions generated on the patient’s skin with the application of the elastic bandage reduce the pressure on the mechanoreceptors and thus, the nociceptive stimulus. The objective of this study was to compare the effect of the elastic bandage application with the application of the medical tape in myofascial pain in the region of the upper fibers of the trapezius muscle in teachers. METHODS: Participants were assessed using the McGill-Melzack Pain questionnaire and the numeric pain rating scale, palpation for the detection of trigger points, goniometry for shoulder abduction and lateral neck flexion, and the upper trapezius muscle strength test. Participants were randomly divided into two groups. In the first moment, the participants of group A received an application of elastic bandage, with the “Y” technique, and those belonging to group B received the application of the same technique, however, using the medical tape. Both groups were reassessed after teaching class and after 24 hours. Two weeks later, there was the inversion of the materials used. RESULTS: The sample consisted of 16 teachers. Group A had a significant statistical pain reduction, according to the numeric pain rating scale, between the initial assessment and post-application at the first moment (p=0.00) and at the second moment (p=0.02). A similar result was found in group B, according to the numeric pain rating scale, both at the first moment (p=0.01) and at the second moment (p=0.03). In both groups, there was pain attenuation with no significance on the effect of the elastic bandage or the medical tape. CONCLUSION: The application of elastic bandage has the same effect that the medical tape in reducing pain.

Atuação fonoaudiológica em disfunção temporomandibular em dois casos: análise comparativa dos efeitos da terapia tradicional e o uso da bandagem terapêutica associada

Objetivo: verificar e comparar os efeitos da terapia fonoaudiológica tradicional e o uso associado da bandagem terapêutica no tratamento de disfunções temporomandibulares musculares em dois casos. Descrição e histórico de procedimentos: O estudo foi composto por dois indivíduos do sexo feminino, ambos com trinta e três anos de idade, diagnosticados com disfunção temporomandibular através do Research Diagnostic Criteria for Temporomandibular Disorders. O primeiro indivíduo denominado como TT foi submetido à terapia fonoaudiológica tradicional em disfunção temporomandibular e o segundo indivíduo denominado como TB, além desses mesmos recursos contou com a utilização de bandagem elástica no músculo masseter bilateral. Os seguintes parâmetros foram verificados de forma qualitativa e descritiva: sinais e sintomas, intensidade de dor e amplitude dos movimentos funcionais mandibulares. Resultados: Os sinais e sintomas presentes no início da terapia foram cessados em ambas as terapêuticas. O paciente TB teve o quadro álgico reduzido em um número reduzido de sessões em relação ao seu par. Ambas as terapêuticas mostram-se efetivas quanto ao aumento de excursões mandibulares. Conclusão: A fonoterapia utilizando-se de técnicas tradicionais e a aplicação da bandagem elástica associada a estas demonstram benefícios terapêuticos em sujeitos com disfunção temporomandibular. O uso da bandagem elástica aparenta ter promovido eficiência terapêutica em menor tempo.

Early experience of transilluminated cryosurgery for varicose vein with saphenofemoral reflux: review of 84 patients (131 limbs).

PurposeVaricose veins with great saphenous vein (GSV) insufficiency is prevalent. Among various surgical treatments for varicose veins, cryosurgery of GSV is an alternative minimally invasive technique that could leave fewer scars and replace conventional stripping with ambulatory phlebectomy. This study sought to assess outcomes including efficacy and safety of cryosurgery for varicose veins.MethodsOne hundred thirty-one limbs in 84 patients (37 male, 47 female; mean age, 53.3 years) with varicose veins were treated with cryosurgery over a 2-year period. The patients were followed for postoperative complication with surveillance of recurrence. The analyzed variables included age, sex, symptom, operative time (from skin incision to the application of elastic bandages on the legs for compression purposes), and postoperative complications including bruising, hematoma, superficial thrombophlebitis, cellulitis, seroma, cutaneous nerve damage, and wound complication.ResultsThe mean operative time was 64.7 minutes for both limbs and 44 minutes for single limbs. The median number of incisions was 2. For stripping of GSV, one incision at the groin was necessary. Postoperative complications included an episode of hematoma in 3 limbs (2.3%), cutaneous nerve damage in 3 limbs (2.3%), cellulitis in 2 patients (1.5%), and seroma in 1 limb (0.8%). No wound infection, thrombophlebitis or deep venous thrombosis was observed. During follow-up (mean follow-up period, 13.5 months; range, 3–28 months), there was no recurrence.ConclusionCryosurgery to treat varicose veins is an effective and safe treatment modality in terms of postoperative complication, cosmetic result, and recurrence.

Application of elastic bandages in alleviating limb swelling of patients with closed fracture of forearm

Objective To study the effect of elastic bandage on alleviating limb swelling of patients with closed fracture of forearm.Methods Eighty-six cases of patients with closed fracture of forearm who had received manipulative reduction were randomly divided into the observation group and the control group,each with 43 cases.The observation group used elastic bandage to bind up and small splint for fixation after manipulative reduction,while the control group directly used small splint for fixation.Swelling degree and tension blister incidence were observed and compared between two groups.Results On the seventh day after manipulative reduction,patients with degree Ⅰ swelling or no swelling were 83.72％ in the observation group and 83.72％ in the control group.And there was statistically significant difference of swelling degree between two groups (x2 =0.7675,P ＜ 0.01).There was two cases of tension blister in the observation group and 15 in the control group,and the difference was statistically significant (x2 =11.918,P ＜ 0.01).Conclusions Elastic bandage is helpful to reduce limb swelling and tension blister in patients with fractures. Outcome of transilluminated powered phlebectomy for varicose vein: review of 299 patients (447 limbs)

Varicose vein incompetence in the legs is very prevalent in the Korean population. New technologies and improvements in established methods have changed the treatment of varicose veins. Transilluminated powered phlebectomy is an alternative surgical technique that combines endoscopic powered vein resection and ablation of superficial varicosities with tumescent anesthesia and irrigated illumination. The present study sought to determine the clinical efficacy and safety of transilluminated powered phlebectomy from clinical data. Four hundred and forty-seven limbs in 299 patients (157 male, 142 female; mean age 50.6 years) with varicose veins were treated with transilluminated powered phlebectomy over a 7-year period. The patients were followed for 1 year postoperatively. The analyzed data included sex, age, body mass index, operative time (from skin incision to the application of elastic bandages on the legs for compression purposes), and postoperative complications. The mean operative time was 87.2 min for both limbs and 57.3 min for single limbs. The median number of incisions was five. Postoperative complications included an episode of cellulitis in 10 patients (2.2%), wound abscess in two patients (0.4%), hematoma in 15 patients (3.4%), residual veins in five patients (1.1%), cutaneous nerve damage in 10 patients (2.2%), and seroma in 13 patients (2.9%). No skin perforation and deep venous thrombosis were observed at the 1-year follow-up. Transilluminated powered phlebectomy is an effective and safe method for the excision of varicosities.
