Annular elastolytic giant cell granuloma after COVID-19 vaccination

The pandemic of SARS-CoV-2 during the first years of the 2020s led to a great commitment to develop effective vaccines. Despite of the good safety and tolerability profile, vaccines may trigger a broad spectrum of cutaneous side effects. Granulomatous dermatitis has been rarely reported after SARS-CoV-2 mRNA vaccines, but no cases of annular elastolytic giant cell granuloma have been already described. Moreover, in our case, it was also associated with a central area of mid-dermal elastolysis, confirming the strong association between these two diseases already reported in literature. The observation of occasional eosinophils within the infiltrate and the presentation of the cutaneous eruption few days after the administration of the second dose of Pfizer/BioNTech (BNT162b2) vaccine are highly suggestive of a drug-related eruption. To our knowledge, this is the first report in literature of an annular elastolytic giant cell granuloma as an adverse effect of SARS-CoV-2 vaccination.

Annular elastolytic giant cell granuloma in a patient with Ehlers–Danlos syndrome

The article presents a clinical case of annular elastolytic giant cell granuloma (AEGCG) in a young patient with a vascular type of EhlersDanlos syndrome. The first clinical manifestations of AEGCG appeared on the skin in the right subclavian area about two years ago. Subsequently, new rashes appeared on the skin of the upper and lower extremities up to four new foci per year. The patient underwent ambulatory therapy as a solution of calcium gluconate 10% 5.0 ml No 10 i/v in every other day; a solution of chloropyramine hydrochloride 1.0 ml No 10 i/m every other day; betamethasone + salicylic acid ointment applied to the affected areas of the skin 2 times a day for 2 weeks. The treatment was ineffective, the rashes did not regress. External therapy with tacrolimus was carried out next, 0.1% ointment 2 times a day in the form of applications for 24 weeks but also without effect. The patient by herself started to take a dietary supplement containing 400 mg of collagen in 1 tablet; 3 types of amino acids 20 mg; vitamins B2 1.1 mg; B6 1.5 mg; calcium pantothenate 5 mg 2 tablets a day during meals. A month after the start of the application, she noticed a slight paling of the rashes. At the moment, the patient is under follow up.

An 83-year-old man with a history of monoclonal gammopathy presented with a 3-month history of diffuse annular erythematous to violaceous plaques with a prominent border starting on his upper extremities and face and then progressing to involve his back, upper chest, and anterior thighs. He admitted to intense pruritus and some pain of these lesions. He denied fever, chills, night sweats, or any new medications. A punch biopsy from the newest plaque on the right anterior thigh demonstrated a granulomatous histiocytic infiltrate composed of multinucleated giant cells exhibiting elastophagocytosis.

Papular variant of actinic granuloma: a case report

Annular elastolytic giant cell granuloma (AEGCG) is an uncommon granulomatous skin disease characterized by giant cells in the dermis phagocytosing damaged elastin fragments, found mostly in areas that are more commonly exposed to sunlight. Asymptomatic annular plaques is the most common clinical presentation, although rare reports of a papular variant of AEGCG exist [1, 2]. We report a case of annular elastolytic giant cell granuloma papular variant with a history of allergies and intensive photodamage.

Few challenging cases of AEGCG have been reported in the literature. We describe a rare clinical presentation of AEGCG mimicking cutaneous sarcoidosis.

A Retrospective Study of 105 Patients with Elastolytic Giant Cell Granuloma and a Proposal for a New Clinical Classification.

Elastolytic giant cell granuloma, an idiopathic granulomatous dermatosis, is characterized by annular plaques on sun-exposed areas, and has been termed actinic granuloma or annular elastolytic giant cell granuloma. Many atypical clinical manifestations and lesions involving sun-protected areas have been reported. The aims of this retrospective study of 105 patients were to summarize the clinical and histological features of patients with this condition; to provide evidence for the viewpoint that elastolytic giant cell granuloma is a better term to include all clinical morphological types presenting with elastolysis, elastophagocytosis, and an infiltrate of multinucleated giant cells histologically; and to establish a new clinical classification. The varying clinical manifestations were further categorized into annular, papular, giant, mixed and generalized forms. The pathological manifestations were classified into giant cell, necrobiotic, histiocytic, sarcoidal and mixed patterns. Diabetes mellitus or impaired glucose tolerance were the most commonly identified comorbidities. Oral low-dose corticosteroid may be an effective treatment.

A clinicopathological analysis of non-infectious granulomatous dermatoses

Non-infectious granulomatous dermatoses involves a broad group of distinct reactive inflammatory conditions. They have overlapping morphological and clinical features and hence pose a diagnostic challenge to differentiate from other granulomatous dermatoses and affect the management of the patient. To study the histopathological patterns of non-infectious granulomatous dermatoses and to correlate with clinical features. It is a retrospective study for a period of 5 years at a tertiary care centre in central Karnataka.In the present study, out of 7273 skin biopsies there were 34 cases of non-infectious granulomatous dermatoses. Out of these 34 cases, granuloma annulare (50%) was the most common non-infectious granulomatous lesion followed by annular elastolytic giant cell granuloma (14%), foreign body granuloma (9%), tattoo granuloma (9%) perforating folliculitis (9%), xanthoma(6%) and xanthelasma (3%). Non-infectious granulomatous disorders of skin encompass a large group of disorders with overlapping features. Thus adequate clinical data and subtle histological findings are important for specific diagnosis which provides better patient management.

Treatment of granuloma annulare and related granulomatous diseases with sulphasalazine: a series of 16 cases.

Granuloma annulare (GA) and the related annular elastolytic giant cell granuloma (AEGCG) and interstitial granulomatous dermatitis (IGD) are idiopathic histiocytic inflammatory disorders, which are frequently recalcitrant to treatment. Evaluate the efficacy of sulphasalazine in treating GA, AEGCG and IGD. Sixteen patients were identified with granulomatous disease who were treated with sulphasalazine between September 2015 and September 2019. Outcomes were based on patients' and providers' subjective evaluations. Sixteen patients were included in the study (ages 56-89, four male and twelve female). Previous treatments were attempted in fifteen patients. Clinical improvement was seen in fourteen patients (87.5%). Initial improvement was noted within a mean (SD) of 66.4 (35.1) days after starting therapy, with increasing benefits over time. Ten patients (62.5%) reported complete or near-complete clearance, three patients (18.8%) reported significant improvement, and one (6.3%) reported partial improvement. Twelve patients elected to stop or reduce therapy, resulting in relapse or worsening in five patients. Sulphasalazine may be considered as treatment for GA and GA-related conditions.

Clinico-etiological analysis of annular skin eruptions: a cross-sectional study from Western Maharashtra

<p class="abstract"><strong>Background:</strong> Annular skin eruptions commonly present in dermatology outpatient department. The most common cause of annular lesions encountered among the adult population in developing nations is dermatophytosis. The aim of the study was to identify risk factors and determine etiology of annular skin lesions after correlation of clinical, pathological, serological and mycological features and to analyze the clinical and laboratory features.</p><p class="abstract"><strong>Methods:</strong> A cross-sectional study was conducted on 103 patients presenting with annular skin eruptions to the Dermatology outpatient/inpatient department of BJ Government Medical College and Sassoon Hospital, Pune during a two year period (2015 to 2017).<strong></strong></p><p class="abstract"><strong>Results:</strong> During a two year period from 2015 to 2017, 103 patients with annular skin eruptions were enrolled. Of these, 67% (65.05%) had superficial dermatophytosis while 16 (15.5%) had Hansens disease. Other groups of sporadic cases found were chronic plaque psoriasis (54.85%), pityriasis rosea (43.9%), pityriasis versicolor (21.9%), secondary syphilis (21.9%), granuloma annulare (21.9%), annular elastolytic giant cell granuloma (11%), Bowens disease (11%), erythema multiforme (11%), porokeratosis (11%) and sarcoidosis (11%). Statistically significant association was found between number, color, site and symptoms in superficial dermatophytosis.</p><p><strong>Conclusions:</strong> This was a novel study that attempted a comprehensive clinico-etiological compilation of annular skin eruptions analyzing a large number of clinical variables and risk factors. Based on the observations, an algorithmic clinical approach has been suggested to facilitate early diagnosis and treatment initiation.</p>

Annular elastolytic giant cell granuloma: An atypical presentation

Annular elastolytic giant cell granuloma is a rare granulomatous disease with unknown pathogenesis. It clinically presents as single or multiple well-defined erythematous annular arciform serpiginous polycyclic plaque with indurated elevated borders and central atrophy in sun-exposed areas. It is found to have a causal association with conditions such as diabetes mellitus, T-cell leukemia, CD4 lymphoma, cutaneous amyloidosis, squamous cell carcinoma lung, sarcoidosis, and monoclonal gammopathy. On histopathology, characteristic “elastophagocytosis” of dermal elastic fibers by the multinucleated giant cells is seen. The presence of horizontally oriented fragmented elastic fibers is characteristic. There is no necrobiosis or mucin deposition, thus differentiating from granuloma annulare. We herein report a case of elderly male with type 2 diabetes mellitus who presented with asymptomatic multiple discrete erythematous shiny dome-shaped papules over anterior chest, abdomen, and back of 2-month duration. The diagnosis was confirmed by biopsy and thorough workup for associations was done. This case is reported due to the rarity of the disease, the atypical presentation, and the characteristic histopathology which lead to the diagnosis.

Annular elastolytic giant cell granuloma (AEGCG), a rare entity first described in 1979, is very similar to O'Brien actinic granuloma (AG), first described in 1975. Since then, many cases have been published under one or other of the two names. We performed a single-centre histopathology study to identify the distinguishing features and determine whether there was any objective difference between AEGCG and AG. Cases classed as AEGCG or AG at the dermatopathology laboratory in Strasbourg were included and analysed using haematoxylin-eosin, orcein and Alcian blue staining. The diagnosis was made in the event of granuloma rich in multi-nucleated giant cells and reduction or disappearance of elastic tissue. Clinical data were collected from the analysis requests and clinical files. We identified 73cases: 12classed as AEGCG and 61classed as AG. Mean age was 60.5years with a sex ratio of 0.55. The duration of the disease ranged from 8days to 17years. A single lesion was seen in 52% of cases with multiple lesions in the remaining cases. Lesions measured between 0.3 and 10cm and exhibited a predilection for photo-exposed areas, chiefly on the head, neck and upper limbs. In most cases, an annular erythematous edge was seen together with a light centre, and slow centrifugal spread. The diagnosis was made by a clinician in only 5.5% of cases. These granulomas were chiefly in the superficial and mid dermis and only rarely deep, and contained numerous giant cells with a constant contingent of lymphocytes, but plasma cells were also seen in half of the cases. Orcein staining revealed marked decrease or total disappearance of elastic tissue within the granulomatous area together with elastophagocytosis in practically all images. More rarely, there was evidence of necrobiosis, palisading granuloma, vascular involvement or orcein-stained asteroid bodies. There were no notable clinical or histological differences between the cases initially classed as AEGCG or AG. AEGCG is a separate entity from annular granuloma that must be assimilated with O'Brien actinic granuloma. The central role of elastic fibres seems clear, but that of ultraviolet radiation remains more controversial. The term annular elastolytic giant cell granuloma seems more appropriate since it is more descriptive and carries no physiopathological implications. There appears to be a significant association with sarcoidosis and with diabetes in a minority of cases, thus warranting screening for these diseases.

Spectrum and clinical variants of giant cell elastolytic granuloma.

Giant cell elastolytic granuloma, also known as annular elastolytic giant cell granuloma or actinic granuloma, is histologically characterized by elastophagocytosis. Recent studies have revealed various clinical presentations in both sun-exposed and non-sun-exposed areas. To clarify clinical characteristics based on case series observation. We retrospectively reviewed patients who fulfilled the pathological diagnosis criteria for giant cell elastolytic granuloma seen at Mackay Memorial Hospital from 2000 to 2014. Patient characteristics, clinical presentation, duration, associated diseases, treatment, and prognosis were analyzed. A total of 22 patients were analyzed and categorized into three major variants. Eight patients with the "annular form" showed large annular lesions, which were usually associated with sun exposure. Six patients with the "papular form" presented with small papules. Eight patients in the "mixed form" group exhibited both papules and smaller annular plaques. The papular form had the youngest age of onset and shortest disease duration. The known consequences in 19 patients were resolved in seven, improved in three, recurred in four, and persisted in five patients. The term "giant cell elastolytic granuloma" is more appropriate because these were not completely related to actinic changes and may be nonannular. The papular form is not easily recognizable without a biopsy.
