TREPONEMES OR GLASS?

To the Editor. —In reference to the article entitled Uveitis and Intraocular Treponemes by Randolph Whitfield, MD, and Emil Wirostko, MD, ( Arch Ophthal 84 :12-15, 1970), I would like to point out the similarity between the treponemes shown in their photomicrograph and what we have termed in our article Treponema-like Forms and Artifacts by Ernesto N. Rios Montenegro, MD, William G. Nicol, MD, and J. Lawton Smith, MD, in the American Journal of Ophthalmology ( 68 :197-205, 1969). The small-corkscrew-like form in the upper right hand corner of their photomicrograph on page 14 is what I consider a typical glass shaving. This can be most easily seen if their photomicrograph is compared carefully to Fig 4 to 6 in our article. Glass shavings do not show true fluorescence in a fluorescent antibody darkfield preparation, but their mild bluish or greenish color can be confused easily with true fluorescence. Whitfield

EFFERENT INNERVATION

<h3>To the Editor.</h3> —In their recent paper on "Efferent Innervation of the Retina" in theArchives(<b>80</b>:98-103 [July] 1968), F. M. Honrubia, MD, and J. H. Elliott, MD, stated that "... Wolter described fibers ending in the wall of small retinal vessels which he felt were centrifugal in orientation; however, with his techniques he was not able to conclusively demonstrate the intraretinal course or pattern which these fibers take prior to their termination." The authors have failed to observe that seven years ago I demonstrated the same type of centrifugal nerves in the human retina, which they now rediscovered. Figure 13 of my paper on "Diabetic Retinopathy" in the<i>American Journal of Ophthalmology</i>(<b>51</b>:1123 [May] 1961) shows an example of such a centrifugal nerve and its branchings away from the disc. <h3>To the Editor:</h3> —The readers of theArchiveswho do more and more cataract surgery, need accurate papers and lectures in helping them to understand both the transient and permanent problems of the spectacle-corrected aphake. Therefore, would you please allow me to call attention to several misleading inaccuracies and very important omissions regarding the so-called "ring scotoma" of high-plus (aphakic) spectacle lenses in the recent paper on "Spectacle Correction of Aphakia" by Dr. Paul Boeder in the December, 1962, issue of theArchives of Ophthalmology?¹ This blind area in the field of view does not "cover approximately 10°" as Dr. Boeder stated for a +12.00 D 44 mm diameter lens. Perhaps Dr. Boeder and those interested in the subject would benefit from a recent paper in the July, 1961, issue of the<i>American Journal of Ophthalmology</i>, entitled "The Roving Ring Scotoma with Its Jack-in-the-Box Phenomenon of Strong-Plus (Aphakic) Spectacle

OCULAR TENSION AFTER CATARACT EXTRACTION

To the Editor: —I have read with great pleasure the very interesting paper Intraocular Pressure Following Cataract Extraction by Drs. Galin, Baras, and Perry (Arch. Ophthal. 66:80, 1961). It seems to be the first work to be published on such early tensional measurements after cataract surgery and would seem to do away with the ciliary body shock concept prevailing up to now. I would like, however, to take exception to the authors' fears about using a Schiotz tonometer for the measurement of the early postoperative tension after cataract extraction, as well as to their assumption that these readings would be misleading because of a supposedly proved diminution in scleral rigidity after cataract surgery. In a paper read in November, 1960, at the Quintas Jornadas Chilenas de Oftalmologia (to be published in the American Journal of Ophthalmology ), I reported on the Schiotz tonometric readings in 50 cataract patients, carried out

All-acrylic anterior chamber implants. New developments and uses.

Introduction The story of artificial intraocular lenses and implants started with Harold Ridley's invention of the posterior chamber lens in 1950. 1 From this brilliant and courageous concept arose Strampelli's anterior chamber implant of 1953. When I started work on this problem in 1956 my first efforts were directed toward making Strampelli's technique as safe as possible. This involved making several changes in his original design. Two years later I was able to report to the Ophthalmological Society of the United Kingdom 2 my experience with the first 60 cases of uniocular aphakia treated with a modified Strampelli implant. The American Journal of Ophthalmology in its March, 1960 number, published an account of my first 111 cases of uniocular aphakia. 3 This paper gave details of preoperative management, operative technique, postoperative care, and complications. RETINAL DETACHMENT SURGERY: AN ENFOLDING OF THE SCLERA

<h3>To the Editor:</h3> —In the August, 1958, issue of the A. M. A.Archives of Ophthalmology(60:247, 1958), Dr. James S. Shipman, in his article entitled Retinal Detachment Surgery: An Enfolding of the Sclera," described a modification of scleral shortening. He states as follows: "In my modification two incisions are made instead of one...." He then goes on to say, "The incisions are now carried deeper through the outer three-fourths of the scleral coat. The anterior and posterior edges of the two incisions are then undermined for about 1 mm., leaving the strip of full thickness of sclera between." Actually, this is the procedure of "Lamellar Undermining," described by me together with my late colleague, Dr. Karl Rubner, in the<i>American Journal of Ophthalmology</i>(41:633 [April] 1956). CERTIFICATE IN OPHTHALMIC SURGERY

To the Editor: —The creation of a Certificate of Ophthalmic Surgery by the American Board of Ophthalmology has been announced in the American Journal of Ophthalmology and in the A. M. A.Archives of Ophthalmology(March, 1957, page 476). The following are some thoughts concerning this announcement. The proposal to create a certification in Ophthalmic Surgery contains several unfavorable aspects. First, it will lower the prestige of the American Board of Ophthalmology. Second, it will brand as surgically incompetent all ophthalmologists who fail to possess this certificate. The requirements for certification in ophthalmic surgery will be extremely difficult to obtain in areas of smaller population. There are many specialists certified by the Board practicing today in areas in which neither the surgical volume nor the necessary certified colleagues are easily available. This, of necessity, may prevent them from obtaining certification and will indirectly and unjustly brand these men as incompetent

Marcus Gunn phenomenon following squint and ptosis surgery; definition and review.

Marcus Gunn, 1 in 1883, published in the Transactions of the Ophthalmological Society of the United Kingdom , a case of Congenital ptosis with peculiar associated movements of the affected lid. Earlier summaries of case reports were made by Sinclair, 2 in 1895 (32 cases) and Villard, 3 in 1925 (93 cases). Grant 4 reviewed the literature and published the 101st case in 1936. Falls, Kruse, and Cotterman 5 published a good summary in the American Journal of Ophthalmology in 1949. They collected 115 published cases. Spaeth, 6 in 1947, found 2 cases in 100 consecutive ptosis cases; to other authors this has appeared an unusually high incidence. Kanter, 7 in 1955, reports eight cases, of which six were typical. The Marcus Gunn phenomenon is often referred to as the jaw-winking phenomenon. This is inaccurate, because it is not a wink but a lid retraction. Clinical Picture The typical case is

To the Editor: —Weimar and Leopold, in a very constructive article in the November, 1954, issue of theArchives, point out that acetate injected subconjunctivally did not penetrate the eye in high levels, with the test employed. In my article in the American Journal of Ophthalmology in April, 1954, it was pointed out that hydrocortisone acetate was inferior to the free-alcohol form of in the treatment of eye disease by subconjunctival injection. A combination of the acetate and free-alcohol form of has proven very satisfactory. Experience with systemic indicates a superiority over cortisone in intraocular inflammations. Peculiarly enough, acetate has a marked superiority over the virtually inactive cortisone acetate when employed as an ointment in the treatment of dermatological conditions of the skin or mucous membranes. BEGINNING with the July 1950 issue, theArchiveswill institute a new department—Annual Reviews—and will eliminate the abstracts from Current Literature. The coverage of abstracts in ophthalmology in the English language is now more than adequate, both in this country and abroad. Section XII of<i>Excerpta medica and Ophthalmic Literature</i>, both relatively recent publications, extend further the excellent coverage of the field by the<i>American Journal of Ophthalmology</i>. It has seemed advisable, therefore, to the Editorial Board of theArchivesto eliminate duplication and to use the space, which is now fast becoming a scarce commodity, for other valuable material. The colossal growth of all phases of ophthalmology during the last twenty years in both the clinical and the experimental field makes it impossible for the reader to keep abreast of the times without some process of condensation. Harrington, published in the July 1949 issue of theArchives, I wish to call attention to a slight error. He credits Horner with the introduction of lid sutures in the cataract operation. Evidently he had overlooked my paper read at the meeting of the Chicago Ophthalmological Society in January 1934, and published in the American Journal of Ophthalmology in September 1934, from which the following paragraph is quoted: Upper-lid sutures (fig. 1). Two are used, spaced in the middle of each half of the upper lid. They include the skin only and are placed just above the lash line.
