Abstract

Objective To investigate the effect of Nasal high-flow oxygen therapy combined with noninvasive positive pressure ventilation on COPD patients with respiratory failure and the effects on the levels of PaO2, PaCO2 and SaO2. Methods A total of 126 patients with COPD complicated with respiratory failure who were admitted from January 2017 to June 2019, including 69 males and 57 females, aged(58.94±5.89)years and ranging in age from 45 to 76 years, were randomly divided into NPPV group and HFNC group by random number table method, with 63 cases in each group.Patients in the NPPV group were given NPPV, while HFNC group was given HFNC on the basis of NPPV group.The clinical treatment effect and complications of the two groups were compared 48 h after treatment, and the changes in blood oxygen partial pressure, blood carbon dioxide partial pressure, blood oxygen saturation level, heart rate, respiratory rate and sputum viscosity of the two groups before and after treatment were compared. Results After treatment, the effective rate of HFNC group[95.2%(60/63)]was higher than that of NPPV group[82.6%(52/63)]. HFNC group had higher blood oxygen partial pressure[(87.87±9.45)mmHg, 1 mmHg=0.133 kPa]and blood oxygen saturation[(89.29±8.99)%]than NPPV group[(78.43±8.69)mmHg and(82.21±8.20)%], and lower blood carbon dioxide partial pressure[(47.95±4.85)mmHg]than NPPV group[(56.72±5.19)mmHg]. HFNC group had lower heart rate[(85.45±4.38)times/minute]and respiratory rate[(18.39±2.03)times/minute]than NPPV group[(92.87±4.72)times/minute, [22.49±2.52)times/minute]than NPPV group, with statistically significant differences(P<0.05). Conclusion HFNC combined with NPPV in the treatment of COPD patients with respiratory failure has a good clinical treatment effect, which can improve the levels of PaO2, PaCO2 and SaO2 and reduce the viscosity of sputum, which is worthy of clinical application.
