Abstract

Objective To analyze the surgical characteristics and skills of the various orbital tumors,so to improve the surgical rate of success,and to reduce the complications. Methods A total of 112 cases (2-75 years old,an average of 30.1 years; male:70 cases,female:42 cases) of orbital lesions were analyzed,including 26 cases of cavernous hemangioma,17 schwannomas,14 inflammatory pseudotumor,11 dermoid cyst,10 pleomorphic adenoma of the lacrimal glan,9 lymphoma,8venous angioma,7 adenoidcysticcarcinom,4 osteom,3 vascular tumors,and 3 meningioma. Results The diagnosis of quality and location were made according to preoperative imaging examination,the relation between lesions and normal tissues were understood,and the surgical risks and the complications were appraised.The surgical approach:anterior orbitotomy (transderma and transconjuctiva) 76cases,lateral orbitotomy 29 cases,and transcranial orbital 7 cases.The process of operation:for cavernous hemangioma,the lesion was punctured and incised to reduce the size of tumor.The stripping off within the capsule was for schwannomas.As for dermoid cysts,cystic walls were cut off after absorbing the contents within the cyst.For venous angioma,the lesion was cut off tightly along the edge of lesion after reducing the size of tumor.For inflammatory pseudotumor,the lesion was cut off partly in case of injuring normal tissues.After operation,visual acuity improved in 37 cases,unchanged 56,decreased 19; 16 cases with ptosis,15 cases of abnormal oculomotor,and 10 cases of enophthalmos. Conclusion To be familiar with the imaging characteristics and local anatomy is the key for surgery.For most orbital tumors,the qualitative diagnosis can be made before operation which have great value for surgical plan and appraisal of the complications so as to improve the surgical effect and life quality for patients.
