Abstract

Objective:To investigate the effect of individual nursing care mode on Bi-level positive airway pressure (BiPAP)ventilation used in patients with chronic obstructive pulmonary disease at acute exacerbation phase (AECOPD) with type2 Ⅱ respiratory failure.Methods:A total of 40 cases of AECOPD patients with Type2 Ⅱ respiratory failure were divided into routine nursing care group and individual nursing care group according the order in hospitalization.The routine nursing care group received Bi2level positive airway pressure ventilation and conventional nursing care while individual nursing care group were given individual nursing care on base of the former.Arterial blood gases (pH,PaCO2,PaO2) and the score of accessory respiratory muscle were used to evaluate the effects of the two different nursing care modes.Results:The value of pH,PaO2 and PaCO2 were improved significantly in both group after 24 h BiPAP treatment (P <0.05),and the change in individual nursing care group were more significant than that in the routine nursing care (P < 0.05) ; The scores of accessory respiratory was lower in both group after treatment for 48 h,but it was more obviously in individual nursing care group,the difference of two groups was statistically significant (P < 0.05).Conclusions:BiPAP ventilation can improve arterial blood gases and relieve clinical symptoms in chronic obstructive pulmonary disease with Type2 Ⅱ respiratory failure.Individual nursing care intervention was superior on winning patients' cooperation,improving therapeutic effect and preventing combination,it should be widely used in clinic.
