Abstract

Introduction/Objective Acute myocardial infarction (AMI) and acute ischemic cerebral infarction (AICI) are both leading causes of death around the world. A simultaneous occurrence of these conditions is called a cardio-cerebral infarction (CCI). We are presenting a case of CCI detected in the prehospital setting as a rarity. Case report: The patient presented is a 78-year-old woman with simultaneous development of AMI and AICI. The diagnosis of a suspected CCI was made in the prehospital setting based on hetero anamnestic data, physical examination findings and ST-segment elevation. At the hospital, additional testing (MDCT of the endocranium, echocardiogram and high troponin levels) confirmed both diagnoses. The patient was treated with fibrinolytic therapy but developed cardiac arrest. After extensive CPR efforts, the patient was pronounced dead. Conclusion: It is necessary to create national guidelines for the diagnosis and treatment of this emergency condition as soon as possible. In the meantime, an individualized approach to each patient is the best policy.

KARDIOCEREBRALNA INFARKCIJA Jelena STOJANOVIĆ

Uvod/ Cilj Akutni infarkt miokarda (AMI) i akutni ishemijski moždani udar (AIMU), vodeći su uzroci smrti širom sveta. Istovremena pojava ovih stanja, naziva se kardiocerebralna infarkcija (CCI). Ključne reči: akutni infarkt miokarda, akutni moždani udar, CCI h oboljenja. Akutni infarkt miokarda (AIM) i akutni ishemijski moždani udar (AIMU), vodeći su uzroci smrti širom sveta [1]. Godine preporučuju alteplazu kod bolesnika koji su ispoljili simptome, ili posljednji put bili bez njih unutar 4,5 sata [4]. Na ovo stanje treba posumnjati, ukoliko je kod bolesnika prisutan akutni fokalni neurološki deficit, povezan sa bolom u grudima, ili dokazom o infarktu miokarda (promene na elektrokardiograma i povišen nivo srčanih enzima) [1]. Chin i saradnici su prijavili učestalost metahrone kardiocerebralne infarkcije od 12,7% kod starijih bolesnika, hospitalizovanih zbog AMI, kod kojih je u roku od 72 h došlo do pojave simtoma AIMU. Iz baze podataka od ukupno 555 pacijenata sa AIMU između 2009. i 2014. godine, Yeo i saradnici su identifikovali pet slučajeva sa CCI (incidenca 0,009%) [8]
