Abstract

PurposeTo observe if exist any kind of variation in levels of fetal cortisol in umbilical cord blood, between spontaneous labors and those that are induce with prostaglandines Methods And MaterialWe studied 73 pregnant women that consulted in our institution between 7/1/99 and 10/30/99. Most of them were primigravida, primipare, with 39 ± 1 weeks of gestation, and with an age average of 27,3 ± 5,2 years. All of them underwent a normal pregnancyThe data that was registered were: hours of labor, medication, induction (specifying doses of misoprostol), frequency and last of the uterine contractions, complications, sex, apgar, hour of birth. The control group consisted of pregnant women with spontaneous labor and the study group those with induced labor, comparing cortisol levels in fetal umbilical cord blood between both groups ResultsWe observed in the study group a larger maternal age, less proportion of primigest, less average of gestation weeks, a high number of pregnancy with more than 40 weeks, longer labors, less use of oxitocine and hioscine bromure, more episodes of polissistolia and hiperdinamia and more umbilical cord circular. All this variables are different in both groups, for that reason we made an adjustment between them, with statistic procedure and, as a result of that, we found a small non significant difference in the variable hours of labor, and weeks of gestation. There was no significant difference in the variable induction. We found also that the levels of cortisol in the variable parity, primigravida had significantly elevated levels, and in the variable sex of the fetus, female had significantly elevated levels, also; taking both groups as one with the adjustment process ConclusionsThere is no significant difference between induced labors and spontaneous labors. With the adjustment of the variables we observed a high level of cortisol in female fetuses and in primigest
