Abstract

Objective To analyze the clinical effects of variable frequency of hybrid artificial kidney or hemodialysis (HD) and hemoperfusion (HP) combined therapy on disorder of calcium and phosphorus metabolism and clearance of parathyroid hormone(PTH). Methods 60 maintenance hemodialysis (MHD) patients treated in our hospital from June, 2015 to June, 2018 were enrolled in this observational study. All the patients were assigned to observation group A, B, C (n1=7, n2=18, n3=5) and control group (n=30) randomly. Hybrid artificial kidney therapy was prescribed in addition to HD. Group A received HD+ HP once a week, while the frequency of which in group B and C were once every 2 weeks and once every 4 weeks respectively. The control group had HD 3 times a week only. The level of serum calcium, phosphate and PTH were compared among the groups in order to analyze the clinical effects of variable frequency of hybrid artificial kidney therapy on disorder of calcium and phosphorus metabolism and clearance of PTH in MHD patients. Results After 24 weeks, there was no significant difference in serum calcium level between the observation groups and the control group (P>0.05), but the level of serum phosphate and PTH was lower in the observation groups (P<0.05). Patients in group A who had hybrid artificial kidney therapy once a week were considered to have better clinical effects on clearance of serum phosphate and PTH than patients in group B and C. Conclusion Receiving hybrid artificial kidney therapy once a week had positive effects on improving disorder of calcium and phosphorus metabolism as well as preventing secondary hyperparathyroidism and cardiovascular complications, decreasing the level of uremic toxins, ameliorating skin pruritus. Regular treatment of hybrid artificial kidney could improve the life quality of uremic patients and was suggested to apply in blood purification therapy. Key words: Hemodialysis; Hemoperfusion; Disorder of calcium and phosphorus metabolism; Secondary hyperparathyroidism
