Abstract

In a rapidly changing healthcare landscape, the need for innovative programs and solutions to address significant system gaps has grown. Community paramedicine is a rapidly expanding model that is able to meet this need by utilizing prehospital providers outside of emergency roles to improve the outcomes of high-risk populations. This is accomplished by performing home visits for such patients where comprehensive clinical and social assessments are performed to identify risk factors that can be rectified. The community paramedicine intervention is able to collaborate with allied health elements in a unique way to successfully reduce inappropriate resource utilization, hospital readmission, or other negative outcomes. The model is highly versatile, able to adapt to specific needs of varying regions and populations.
