Abstract

Normal sexual functioning of both men and women, being a very complex process, is affected by numerous issues besides aging. Environmental factors play a very significant role in addition to many medical comorbidities that are usually associated with aging. Many drugs, chemicals, stress, smoking, and psychological issues affect the sexual performance and quality of life of the young, as well as aged. Erectile dysfunction (ED) is one of the most common disorders in male besides other sexual concerns such as premature ejaculation, performance anxiety, Peyronie's disease, priapism, and hypogonadism. Many of these factors affect the structural organization and function of penile erectile components such as smooth muscle cell and vascular architecture. These modifications affect penile hemodynamics by impairing cavernosal smooth muscle cell relaxation, reducing penile elasticity, compliance, and promoting fibrosis. Age is not a limiting factor for ED medical management and it is never too late to treat. Internet and social media exploit and promotes many sexual enhancers, recreational agents, and street drugs. Many mood-enhancing and sexual performance–enhancing drugs (e.g., Viagra, Cialis, testosterone) have now been approved to help these men with sexual issues.
