Abstract

Objective To explore the effects of case management model in patients with coronary heart disease combined with diabetes mellitus. Methods A total of 66 in-patitents met the inclusion criteria in Cardiovascular Surgery at Affiliated Hospital of Nantong University were selected as subjects by convenience sampling from January 2016 to January 2017. They were divided into observation group (n=33, conventional nursing and follow-up) and control group (n=33, nursing and follow-up with the model of case management) with the method of the random number table. The specific function status of coronary heart disease, score of quality of life, self-management capability and blood glucose of both groups before and after intervention were compared. Results Before intervention, there were no statistically significant differences in the specific function status of coronary heart disease, score of quality of life, self-management capability and blood glucose of both groups (P>0.05) . After intervention, the scores of the specific function status of coronary heart disease, quality of life and self-management in observation group were significantly higher than those in control group (P<0.05) . The fasting plasma glucose (FPG) and 2-hour postprandial blood glucose (2hPG) during hospitalization and after discharge in observation group were lower than those in control group with significant differences (P<0.05) . Conclusions During the treatment and follow-up period among patients with coronary heart disease combined with diabetes mellitus, the application of case management model can prevent the adverse cardiac events, control blood glucose and improve the self-management capability as well as quality of life. Accordingly, the case management model is worth to be used widely in patients with coronary heart disease combined with diabetes mellitus.
