Abstract

Objective To study the value of 24 h dynamic electrocardiogram in the diagnosis of heart rate turbulence in patients with chronic congestive heart failure. Methods Sixty-five patients with chronic congestive heart failure were selected and compared with 40 cases in the control group in the same period. The two groups were all detected with 24 h dynamic ECG and BNP were measured by echocardiography, LVEF level and LVEDD level of two groups were detected and analyzed. Results The levels of TO, BNP, LVDED[(1.33±1.40)%, (625.25±512.32)pg/ml, (62.58±8.35)mm] of the study group were significantly higher than those of the control group[(-0.83±1.02)%, (59.36±31.28)pg/ml, (50.06±8.68)mm], the differences were significant(P<0.05), the levels of TS, LVEF[(1.45±1.40)ms/RRI, (40.53±9.36)%] were significantly lower than those of the control group [(7.65±5.60)ms/RRI, (65.63±6.35)%], the differences were significant(P<0.05). With the cardiac function increased, the levels of TO, BNP and LVDED significantly increased (P<0.05), and the levels of TS and LVEF significantly reduced (P<0.05). The levels of LVEF in patients with normal TO and TS levels were significantly higher than those in patients with abnormal levels of TO and TS(P<0.05), and the level of BNP in patients with normal levels of TO and TS was significantly lower than that in the patients with abnormal levels of TO and TS. TO level was positively correlated with LVEDD level and BNP level (r=0.32, 0.13, P<0.05), and negatively correlated with LVEF level (r=-0.24, P<0.05), and the TS level was negatively correlated with LVEF, LVEDD and BNP levels (r=-0.72, -0.04, -0.27, P<0.05). Conclusions The heart rate turbulence of chronic congestive heart failure is closely correlated with heart function, which may indicate the severity of cardiac function in patients with injury, as the index for judge the prognosis of patients with chronic congestive heart failure.
