Abstract

The inverted-T scar technique has been used to correct breast ptosis for many years, often using an inferior pedicle. In this chapter, the authors review their preferred techniques for the inverted-T technique using either the superior or inferior pedicle. Patient selection, preoperative planning, and markings with a detailed description of these preferred surgical approaches are reviewed. The superior pedicle provides an opportunity to unload the lower pole volume through either resection of the lower pole tissue or relocating the volume to the upper pole through auto-augmentation. The skin provides only secondary support. The inferior pedicle is the most versatile and is used when the ptosis is too great to allow safe elevation on a superior pedicle. The use of mesh reinforcement and fat grafting are discussed as ancillary additions to these procedures that may provide some additional improvement in the final aesthetic outcomes.
