Abstract
INTRODUCTION: The article considers an issue of one of poorly studied mechanisms of initiation, development and treatment of ischemic stroke resulting from dissection of the internal carotid artery. To date, there is no clear sequence of actions to determine indications for surgical treatment of dissections, and the available algorithms are controversial. Absence of universally accepted approaches to the treatment of patients with this pathology stimulates a detailed investigation of the problem and ways to solve it. A clinical example of diagnosis and treatment of ischemic stroke in the acute period, which occurred in result of dissection and thrombosis of the internal carotid artery, is given. On this example, the sequence of diagnostic process and its role in selection of the pharmaceutical and surgical treatment is considered. The role of the cerebral angiography in the diagnosis of dissection is evaluated. CONCLUSIONS: The operative and coordinated work of the multidisciplinary team in the examination of a neurologic patient permits to rapidly and fully examine a patient and make a decision on the tactics of his treatment. A timely comprehensive treatment of acute ischemic stroke accompanied by dissection of a cerebral vessel, in turn, permits to reduce the ischemic focus and improve the prognosis of the disease.
