The Mediating Role of Resilience and Living in Care on Psychosocial Outcomes

Purpose This study examined the mediating role of resilience and living in care experiences between risk exposure (victimization by community and adverse childhood experiences) and psychosocial outcomes (housing instability, delinquency, and post-traumatic stress reactions) for youth receiving child welfare and community services. Method Two hundred and fifty-one youths (14–19 years old) who are receiving interventions from the Department of Community Services in Nova Scotia, Canada, were recruited. Multiple measures were administered to the youth. Results and conclusion Resilience and experiences of living in care significantly mediated the relationship between risk factors and outcomes, with youth demographic characteristics influencing the pattern of these relationships. Findings suggested that resilience and living in care experiences could reduce the effects of risk factors on psychosocial outcomes. These findings demonstrate a role for Child Welfare agencies in promoting youth access to the resilience-enabling resources necessary to sustain well-being.

Foundry: Early learnings from the implementation of an integrated youth service network.

AIMSTo provide the first profile of the demographic and service characteristics of young people (aged 12–24 years) who access Foundry, a provincial network of integrated youth health and social service centres in British Columbia, Canada and to share early learnings about implementation and service innovation.METHODSUsing a retrospective chart review, we conducted a census of all young people accessing a Foundry centre in a ‘proof of concept’ phase. Six centres were assessed between October 2015 and March 2018. Data included demographics, mental health service access history, service type the youth was seeking, and information about how they found out about the centre.RESULTSA total of 4783 young people presented during this proof of concept period, for a total number of 35 791 visits. The most frequently accessed category of service was mental health/substance use (57%) followed by physical health (25%). Young people were most likely to be female, aged 15–19, and White. Youth demographic characteristics showed an over‐representation of Indigenous and LGBTQ2 youth and under‐representation of males and youth aged 20–24. Youth were most likely to learn about Foundry from a friend (44%) or family member (22%). Most youth (58%) reported that they would have gone ‘nowhere’ if not for Foundry.CONCLUSIONSFoundry is a model of integrated health and social services delivery, focused on early intervention, prevention and accessibility, driven by the needs and priorities of young people and their families. Leveraging international integrated youth health service evidence, the model addresses urgent priorities in Canadian health service delivery.

Youth Demographic Characteristics and Risk Perception of Using Alternative Tobacco Products: An Analysis of the 2014-2015 Canadian Student Tobacco, Alcohol, and Drugs Survey (CSTADS).

BackgroundThere is a growing attraction by youth to alternative tobacco products (ATPs) such as e-cigarettes and hookahs. This study investigated risk perceptions and demographic characteristics associated with ATP use in grade 8–10 students.MethodsData were drawn from the 2014/15 cycle of the CSTADS. The analytic sample included 1819 students from a total pool of 42 094 students who completed the survey. Logistic regression models were used to examine factors (demographic characteristics and risk perception) associated with ATP use in the past 30 days.Results12% of students in grade 8–10 self-identified as having used ATPs in the past 30-days, with a majority of students in grade 10 (56%). Male students had higher odds of reporting ATP use when compared to females. Although a lesser proportion of Indigenous students reported ATP use in comparison to White students (31% vs 61%), Indigenous students were 2.42 (1.49, 3.93) times as likely to use ATPs as White students. Students who perceived smoking hookah once in a while as “no to slight risk” were 1.58 (1.09, 2.28) times more likely to report ATP use than students who perceived “moderate to great risk.” Also, students who perceived using e-cigarettes on a regular basis as “no to slight risk” were 2.21 (1.53, 3.21) times more likely to report ATP use as students who perceived “moderate-great risk.”ConclusionA significant number of grade 8–10 students use ATPs, especially e-cigarettes, with the misconception of minimal health risks. There remains the need to do more to counteract the rise in social and epidemiological alternative tobacco use trends among the youth.

Longitudinal examination of youth readmission to mental health inpatient units.

Significant barriers exist for youth in obtaining mental health services. These barriers are exacerbated by growing demand, attributed partially to children and adolescents who have repeat hospital admissions. The purpose of this study was to identify demographic, socioeconomic and clinical predictors of readmission to inpatient psychiatric services in New Brunswick, Canada. Key demographic, support and clinical predictors of readmission were identified. The New Brunswick Discharge Abstract Database (DAD) was used to compile a cohort of all children and adolescents ages 3-19years with psychiatric hospital admissions between 1 April 2003 and 31 March 2014 (N=3825). Primary analyses consisted of Kaplan-Meier survival methods with log-rank tests to assess time-to-readmission variability, and Cox regression to identify significant predictors of readmission. In total, 27.8% of admitted children and adolescents experienced at least one readmission within the 10-year period, with 57.3% readmitted to hospital within 90days following discharge. Bivariate results indicated that male, upper-middle socioeconomic status (SES) youths aged 11-15years from nonrural communities were most likely to be readmitted. Notable predictors of increased readmission likelihood were older age, being male, higher SES, referral to care by medical practitioner, discharge to another health facility, psychosis, and previous psychiatric admission. A significant portion of the variance in readmission was accounted for by youth demographic characteristics (i.e. age, SES, geographic location) and various support structures, including referrals to inpatient care and aftercare support services. Readmission to inpatient psychiatric care among youth is affected by a number of multifaceted risk factors across individual, environmental and clinical domains. This study used provincial population-scale longitudinal administrative data to demonstrate the influence of various individual and demographic factors on likelihood of readmission, which is notably absent from the majority of studies that make use of smaller, short-term data samples. Ensuring that multiple factors outside of the clinical context are considered when examining readmission among youth may contribute to a more thorough understanding of youth hospitalization patterns.

Associations between Walkability and Youth Obesity: Differences by Urbanicity.

Background: Attributes of the built environment, such as neighborhood walkability, have been linked to increased physical activity and reduced obesity risk. This relationship, however, has primarily been documented in adults; less is known about neighborhood walkability and youth obesity, as limited prior research has produced mixed findings. The purpose of this study was to examine the association between neighborhood walkability and youth obesity, including differences by urbanicity. Methods: Data were collected in 2013 from youth aged 7-14 years (n = 13,469) in a Southeastern county school district. Height and weight were objectively measured and utilized to calculate body mass index (BMI) z-scores. Youth demographic characteristics and addresses were obtained, and a Walk Score® was gathered for each youth's home address. Multilevel linear regression analysis, accounting for nesting within census block groups, was conducted to examine the association between Walk Score and BMI z-score and to test for the moderating effect of urbanicity. Separate multilevel analyses examined Walk Score and BMI z-score among urban, urban-rural mixed, and rural youth subsamples. Results: Overall, as Walk Score increased, youth BMI z-score decreased. Walk Score was positively associated with BMI z-score among urban youth and negatively associated with BMI z-score among rural youth; no relationship was observed between Walk Score and youth in urban-rural mixed areas. Conclusions: Neighborhood walkability may impact youth differently across geographic areas. Further study is warranted about how youth utilize a walkable environment and mechanisms through which walkability influences youth physical activity and obesity risk.

The Revised Youth Asset Survey (YAS-R).

The study's purpose was to improve the psychometric properties of the Youth Asset Survey (YAS). Longitudinal cohort study with youth and parents recruited via door-to-door canvassing to participate in a 5-wave, 4-year study that assessed prospective associations among youth assets and youth health-related behaviors. Additional test items were added in the last 2 waves of a study to improve the YAS. Households in randomly selected census tracts located in the Oklahoma City metropolitan area. Youth and their parents (N = 1111 parent/child dyads). Youth demographic characteristics at wave 4 were mean age = 17.3 (standard deviation = 1.62) years; 53% female, 41% white, 28% Hispanic, 24% African American, and 6% other. Sixty-eight items assessing 17 youth asset constructs and 8 single items assessing youth risk behaviors. Confirmatory factor analysis and generalized linear models were conducted to assess construct reliability and predictive validity, respectively. Cronbach α for the revised asset constructs ranged from .72 to .82, predictive validity was strong, and all revised asset constructs were assessed via 4 items. Researchers and practitioners have an improved 68-item YAS-Revised, freely available for their use, which measures 17 youth assets with good reliability, validity, and functionality.

Medical Marijuana Legalization and Marijuana Use Among Youth in Oregon.

While the legalization of marijuana for medical and recreational use has raised concerns about potential influences on marijuana use and beliefs among youth, few empirical studies have addressed this issue. We examined the association between medical marijuana patients and licensed growers per 1000 population in 32 Oregon counties from 2006 to 2015, and marijuana use among youth over the same period. We obtained data on registered medical marijuana patients and licensed growers from the Oregon Medical Marijuana Program and we obtained data on youth marijuana use, perceived parental disapproval, and demographic characteristics from the Oregon Healthy Teens Survey. Across 32 Oregon counties, the mean rate of marijuana patients per 1000 population increased from 2.9 in 2006 to 18.3 in 2015, whereas the grower rate increased from 3.8 to 11.9. Results of multi-level analyses indicated significant positive associations between rates of marijuana patients and growers per 1000 population and the prevalence of past 30-day marijuana use, controlling for youth demographic characteristics. The marijuana patient and grower rates were also inversely associated with parental disapproval of marijuana use, which decreased from 2006 to 2015 and acted as a mediator. These findings suggest that a greater number of registered marijuana patients and growers per 1000 population in Oregon counties was associated with a higher prevalence of marijuana use among youth from 2006 to 2015, and that this relationship was partially attributable to perceived norms favorable towards marijuana use.

Mentoring Relationship Quality Profiles and Their Association With Urban, Low-Income Youth’s Academic Outcomes

This study aimed to (a) identify mentoring quality profiles based on characteristics of informal mentoring relationships, (b) examine how mentor and youth demographic characteristics were related to the profiles, and (c) investigate whether the profiles were related to youth’s academic outcomes. Participants were 411 ninth-grade urban, low-income students. Mentors were comprised of older siblings, extended family members, and non-familial adults. Using cluster analysis, we identified two mentoring quality relationship profiles: (a) less close and growth oriented and (b) closer and more growth oriented. Boys were more likely to have less close and growth-oriented relationship profiles or to be in the non-mentored group compared with girls. Univariate tests showed differences among relationship profile groups and non-mentored groups on intrinsic motivation, educational aspirations and expectations, perceived economic benefits, and limitations of education and grade point average (GPA). The study reveals the importance of taking a within-group, person-centered approach when examining mentoring relationships.

Evaluation of Youth's Health Risk Behaviors in Baghdad City


 
 
 
 
 Objective(s): To evaluate youth's health risk behaviors in Baghdad City and to determine the relationship between such behaviors and the youth's demographic characteristics of age, gender and grade. Methodology: A descriptive study, using the evaluation approach, is carried out to evaluate youth's health risk behaviors in Baghdad City for the period of January 26th 2016 to May 20th 2016. A non-probability "purposive" sample of (160) University students is selected for the purpose of the study from four groups of colleges (medical, engineering, sciences, and education) and it is equally distributed of (40) student from group of colleges. The sample is consisted of (50%) males and (50%) females. A questionnaire is constructed for the purpose of the study. It is comprised of eight parts which deal with youth's health risk behaviors of unintentional injuries, tobacco use, drinking alcohol, drug use, sexual behavior, unhealthy dietary behavior, inadequate physical activity, and health related behavior. Internal consistency reliability and content validity are determined for the study instrument through pilot study. Data are collected through the utilization of the questionnaire and the application of the structured interview technique as means of data collection. Data are analyzed through the application of the descriptive statistical data analysis approach which includes frequency, percent, total score and Cronbach alpha correlation coefficient. Results: The study depicts that most of the students have experienced health risk behaviors of unhealthy dietary behavior, inadequate physical activity and health related behavior regardless of their colleges' classification. Male students have been exposed to health risk behaviors of unintentional injuries, tobacco use, drinking alcohol, drug use and sexual behavior more than female students. Recommendations: The study recommends that Youth Health Risk Behaviors Program should be designed and implemented to present these students with all the information for the prevention and control of such behaviors. Further research is needed to determine whether low grades lead to health risk behaviors or health risk behaviors lead to low grades, or some other factors lead to both of these problems.
 
 
 
 
 
 
 

Prevalence of Weight Problems among Youth with High-Incidence Disabilities in Residential Care

The prevalence of weight problems among youth in general and youth in out-of-home care has been well documented; however, the prevalence of obesity/overweight among youth with high-incidence disabilities in more restrictive settings, such as residential care, has not been assessed. The purpose of the present study was to evaluate the prevalence of weight problems in 245 youth with learning disabilities or emotional disturbance at entrance to residential care. Youth intake files were examined to evaluate the association between youth demographic characteristics, medical conditions, prior placements and placement settings, medication usage, and internalizing and externalizing behavioral problems and clinical weight problems. Findings revealed that over one half of the youth were overweight or obese (50.6%), and weight problems were associated with gender, medical conditions, and age. Study limitations, future research efforts, and treatment implications are discussed.

The Covariates of Parent and Youth Reporting Differences on Youth Secondary Exposure to Community Violence

Survey data for studying youth's secondary exposure to community violence (i.e., witnessing or hearing violence in the community) come from both parents and their children. There are benefits of considering multiple informants in psychosocial assessments, but parents and youths often disagree about comparable information. These reporting differences present challenges for both researchers and clinicians. To shed new light on the individual, family, and neighborhood factors that contribute to parent and youth reporting differences regarding youth's secondary exposure to community violence, this study analyzed hierarchical item response models on a sample of youth respondents from the Project on Human Development in Chicago Neighborhoods. Participants were aged approximately 9, 12, and 15 years (trimodal distribution; mean age = 12.0 years) at baseline (N = 2,344; 49.6% female). Descriptive analyses indicated that parents significantly underestimated their children's exposure to community violence. Logistic hierarchical item response models indicated that absolute discrepancies between parent and youth reports were a function of youth demographic characteristics (male, Hispanic or African American as compared to white, age, 3rd as compared to 1st generation immigrant), individual difference factors (lower levels of self-control, higher levels of violent peer exposure), and family factors (lower household socioeconomic status). Parental under-reporting of youth's exposure to violence was associated with youth demographic characteristics (male, age, 2nd as compared to 3rd generation immigrant), family factors (lower levels of parental supervision), and neighborhood characteristics (higher levels of violence, less access to youth services). The results suggest that a constellation of individual and contextual factors may contribute to the understanding of parent and youth reporting differences. The findings speak to the utility of examining parent and youth reporting differences from a hierarchical lens.

Effects of the local alcohol environment on adolescents' drinking behaviors and beliefs

To examine relationships between characteristics of the local alcohol environment and adolescent alcohol use and beliefs in 50 California cities. The study used longitudinal survey data collected from adolescents; city-level measures of local alcohol policy comprehensiveness, policy enforcement, adult drinking and bar density; and multi-level modeling with three levels (city, individual, time), allowing for random effects. Models included interaction terms (time × alcohol environment characteristics) and main effects, controlling for city and youth demographic characteristics. Analyses also examined possible mediating effects of alcohol-related beliefs. Fifty California cities (50000-500000 population). Random samples of 1478 adolescents and 8553 adults. Past-year alcohol use and heavy drinking, and alcohol-related beliefs (e.g. perceived alcohol availability) among adolescents; past 28-day alcohol use among adults; ratings of local alcohol control policies; funding for enforcement activities; bars per roadway mile. Local alcohol policy comprehensiveness and enforcement were associated with lower levels of past-year alcohol use (betas=-0.003 and -0.085, P<0.05). Bar density was associated with a higher level of past-year alcohol use (beta=1.086, P<0.01). A greater increase in past-year alcohol use and heavy drinking over time was observed among adolescents living in cities with higher levels of adult drinking (betas=0.224 and 0.108, P<0.01). Effects of bar density appeared to be mediated through perceived alcohol availability and perceived approval of alcohol use. Adolescent alcohol use and heavy drinking are related to characteristics of the local alcohol environment, including alcohol control policies, enforcement, adult drinking and bar density. Change in adolescents' drinking appears to be influenced by community-level adult drinking. Bar density effects appear to be mediated through perceived alcohol availability and approval of alcohol use.

Examining Youth and Program Predictors of Engagement in Out-of-School Time Programs

Prior research suggests that youths' engagement in out-of-school time programs may be a crucial factor linking program participation to positive outcomes during adolescence. Guided by the theoretical concept of flow and by stage-environment fit theory, the present study explored correlates of engagement in youth programs. Engagement was conceptualized as the extent to which youth found the program activities enjoyable, interesting, and challenging. The current study examined how program content, monetary incentives, and youth demographic characteristics were linked to youth engagement among a sample of primarily low-income middle and high school youth attending 30 out-of-school programs (n = 435, 51 % female). Results from multilevel models suggested that program content and staff quality were strongly associated with youth engagement. Youth who reported learning new skills, learning about college, and learning about jobs through activities in the program were more engaged, as were youth who found the staff caring and competent. Results demonstrated that the link between learning content for the future and engagement was stronger for older youth than younger youth. In addition, there was a trend suggesting that providing a monetary incentive was associated negatively with youth engagement. Taken as a whole, these findings have important implications for researchers, practitioners, and policymakers interested in understanding the characteristics of out-of-school time programs that engage older youth.

Relationships between local enforcement, alcohol availability, drinking norms, and adolescent alcohol use in 50 California cities.

This study investigated relationships between local alcohol policies, enforcement, alcohol outlet density, adult alcohol use, and underage drinking in 50 California cities. Eight local alcohol policies (e.g., conditional use permit, social host ordinance, window/billboard advertising) were rated for each city based on their comprehensiveness. Local alcohol enforcement was based on grants received from the California Alcoholic Beverage Control agency for enforcement of underage drinking laws. Outlet density was based on the number of on- and off-premise outlets per roadway mile. Level of adult alcohol use was ascertained from a survey of 8,553 adults and underage drinking (frequency of past-year alcohol use and heavy drinking) from surveys of 1,312 adolescents in 2009 and 2010. Multilevel regression analyses were conducted to examine the effects of policies, enforcement, and other community-level variables on adolescent drinking, controlling for youth demographic characteristics. Mediating effects of adolescents' perceived ease of obtaining alcohol, perceived enforcement, and perceived acceptability of alcohol use also were examined. None of the eight local alcohol-policy ratings were associated with adolescent drinking. Funding for underage drinking enforcement activities was inversely related to frequency of past-year alcohol use, whereas outlet density and adult drinking were positively related to both past-year alcohol use and heavy drinking. These relationships were attenuated when controlling for perceived ease of obtaining alcohol, enforcement, and acceptability of alcohol use, providing evidence formediation. Adolescent alcohol use and heavy drinking appear to be influenced by enforcement of underage drinking laws, alcohol outlet density, and adult alcohol use. These community-level influences may be at least partially mediated through adolescents' perceptions of alcohol availability, acceptability of alcohol use, and perceived likelihood of getting in trouble with local police.

Acceptability and Feasibility of Rapid HIV Testing in an Adolescent Clinic Setting: Youth Testing Attitudes, Knowledge, and Behaviors

To assess youths' attitudes, knowledge, and behaviors regarding rapid HIV testing (RHT) and measure acceptability and feasibility of RHT in an adolescent clinic setting. A 2007-2008 project introduced free RHT at an urban, hospital-based adolescent and young adult clinic in Boston, MA. Patients and HIV testing clients were offered either free nonrapid tests or fingerstick RHT. An anonymous questionnaire assessed youths' testing attitudes, knowledge, and behaviors (N = 127). Ordinal logistic regression model was used to determine associations with youth demographic characteristics and testing experience. Most participants valued rapid results. A minority desired confidentiality from parents and insurance providers. Older youth were more likely to know about testing methods (OR: 1.25; CI: 1.04-1.51) and plan for follow-up (OR: 1.43; CI: 1.14-1.81). Age, gender, and race were unrelated to testing facilitators such as rapidity, confidentiality, and cost, although younger clients were more likely to prefer noninvasive methods. Individuals with previous testing experience were more likely to say that they would contribute to expenses and value rapidity over cost. There was strong support for RHT among youth receiving HIV testing. Offering RHT to youth may facilitate routine testing. Future research should focus on increasing RHT access among diverse populations of youth.

Profiles and portfolios of adolescent school-based extracurricular activity participation

The current study presented a new description of adolescent school-based activity participation, in the form of mutually exclusive activity portfolios, and described the kinds of youth that participate in each portfolio. These portfolios included (1) Sports Only, (2) Academics Only, (3) School Only, (4) Performance Only, (5) Multiple Activities, and (6) Non-Participation. Findings indicated that youth demographic characteristics and school size differentiated between different kinds of activity participation as well as nonparticipation. More detailed activity portfolios were also identified that were complex and demonstrate the difficulty of examining participation beyond larger, more inclusive groupings. The Multiple Activity portfolio emerged as a unique group worthy of further examination. Characteristics of non-participators included: lower socioeconomic status, lower grades, and attended larger schools. Hispanic adolescents were also less likely to participate in school-based extracurricular activities. Findings from this study inform ecological models of adolescent development as well as school and social policy.

Urban Adolescents' Out-of-School Activity Profiles: Associations with Youth, Family, and School Transition Characteristics

This study applied individual growth trajectory analyses and person-oriented analysis to identify common profiles of out-of-school activity engagement trajectories among racially and ethnically diverse inner city teens (N = 1,430). On average, teens exhibited declining trajectories of participation in school-based and team sports activities and increasing trajectories of religious activity participation. A wide variety of patterns of participation, however, characterized individual teens' involvement across domains. Cluster analysis revealed 10 unique profiles that reflected varying levels of participation in school-based, religious, and team sports activities. Youth demographic characteristics, parental employment status, and school transition timing predicted activity profile membership. Unemployment predicted membership in profiles characterized by low or decreasing activity participation. Later school transitions were associated with membership in high participation profiles.

Multisystemic Therapy Effects on Attempted Suicide by Youths Presenting Psychiatric Emergencies

To evaluate the efficacy of multisystemic therapy (MST) in reducing attempted suicide among predominantly African American youths referred for emergency psychiatric hospitalization. Youths presenting psychiatric emergencies were randomly assigned to MST or hospitalization. Indices of attempted suicide, suicidal ideation, depressive affect, and parental control were assessed before treatment, at 4 months after recruitment, and at the 1-year posttreatment follow-up. Based on youth report, MST was significantly more effective than emergency hospitalization at decreasing rates of attempted suicide at 1-year follow-up; also, the rate of symptom reduction over time was greater for youths receiving MST. Also, treatment differences in patterns of change in attempted suicide (caregiver report) varied as a function of ethnicity, gender, and age. Moreover, treatment effects were found for caregiver-rated parental control but not for youth depressive affect, hopelessness, or suicidal ideation. Results generally support MST's effectiveness at reducing attempted suicide in psychiatrically disturbed youngsters, whereas the effects of hospitalization varied based on informant and youth demographic characteristics.
