MRI findings of prepatellar Morel-Lavallée effusions

To describe MR imaging characteristics and treatment options for prepatellar closed degloving injuries or Morel-Lavallée effusions. Imaging features are discussed that may aid in the distinguishing of this entity from "housemaid's knee" or prepatellar bursitis. MR images of four young wrestlers were reviewed by two attending radiologists and one fellow, and correlative clinical data were collected using the electronic medical database. MR images in all cases showed a unilocular, T2 hyperintense prepatellar collection extending beyond the normal boundaries of the prepatellar bursa. No necrotic fat or blood products were identified in the collections. Surgical pathology proving the absence of synovial tissue was available for one case, and differentiation of the collection from the adjacent bursa was confirmed visually by the surgeon in a second case. Although prepatellar hemorrhagic bursitis and Morel-Lavallée effusions share many imaging features, making a specific diagnosis in most cases is not necessary, as treatment is often similar for both entities.

Introduction The goal of this study was to verify if cortisol could be used as an indicator of work load during a mixed aerobic and anaerobic training. Results Throughout a 15-week training period in 12 young wrestlers, salivary cortisol was assayed every three weeks and the explosive stength was evaluated on weeks 4 and 15. The results show that the wrestlers who have the highest mean levels of cortisol are those which progress less in explosive force ( r = −0.72, P < 0.01). Conclusion It seems that the follow-up of cortisol should be an indicator of the load of training.

Saliva cortisol, testosterone and T/C ratio variations during a wrestling competition and during the post-competitive recovery period.

This study presents saliva cortisol (C) and testosterone (T) levels in a group of 15 young wrestlers (national and international) during a two-days competition. Values are compared to references established on a resting day (3 weeks before the competition). Post-competition recovery was studied by recording evening hormonal levels (5.30 pm) for 8 days. C levels increased sharply (about 2.5 fold resting levels) throughout the competition with no further changes in T levels. The rise in C appeared before entering the competition, traducing a striking phenomenon of anticipation. At the end of the competition, C levels fell very quickly (within 1.5 h) to basal value when T rose significantly, resulting in a very high T/C ratio. During the recovery period, C levels corresponded to basal ones and T remained high, resulting in a high T/C ratio (>30% than basal one) till the 5th day. Competition resulted in a low T/C ratio, considered as a catabolic phase. The recovery which is associated with a feeling of tiredness and an incapacity to train strenuously recognized by the trainers, paradoxically corresponded to a high T/C ratio (so-called anabolic phase).

Isokinetic Peak Torque in Young Wrestlers

The purpose of the present study was to examine age-related changes in isokinetic leg flexion and extension peak torque (PT), PT/body weight (PT/BW), and PT/fat-free weight (PT/FFW) in young wrestlers. Male wrestlers (N = 108; age M ± SD = 11.3 ± 1.5 years) volunteered to be measured for peak torque at 30, 180, and 300° · s−1. In addition, underwater weighing was performed to determine body composition characteristics. The sample was divided into six age groups (8.1−8.9, n = 10; 9.0−9.9, n= 11; 10.0−10.9, n = 25; 11.0−11.9, n = 22; 12.0−12.9, n = 28; 13.0−13.9, n = 12), and repeated measures ANOVAs with Tukey post hoc comparisons showed increases across age for PT, PT/BW, and PT/FFW. The results of this study indicated that there were age-related increases in peak torque that could not be accounted for by changes in BW or FFW. It is possible that either an increase in muscle mass per unit of FFW, neural maturation, or both, contributes to the increase in strength across age in young male athletes.

Avulsion fracture of the medial humerus epicondyle in young wrestlers

We report 2 cases of an avulsion fracture of the medial humerus epicondyle in teenage wrestlers. Two youngsters (13 and 14 years of age) who both competed in the Danish national championship in wrestling avulsed their medial humerus epicondyle. Both were operated on, and the medial humerus epicondyle was reinserted and fixed. The postoperative results were good, as both patients achieved a normal range of motion. To prevent injuries, we recommended that children who begin to wrestle be taught correct falling techniques. Open reduction and internal fixation of the avulsed medial humerus epicondyle is mandatory to guarantee a good final result without permanent disability, and the early and correct diagnosis k therefore of importance.

When Wrestlers Slim to Win.

In brief High school wrestlers often select a weight class below their normal weight without considering how much weight they can safely lose. Body composition assessments provide objective estimates of minimum weight for competition. However, even if athletes follow medically accepted guidelines for minimum weight, losing weight may still affect their health and performance. Using a variety of techniques, primary care physicians can determine and interpret minimum weight estimates for young wrestlers and offer advice on safe weight loss.

A young wrestler with antecedent both-knee Osgood Schlatter??s disease and JOCD

A 15-yr-old high school wrestler with an antecedent history of bilateral knee Osgood Schlatter's disease and juvenile osteochondritis dissecans (JOCD) presented with a 6-month complaint of episodic bilateral knee pain. History and physical examination results were consistent with a diagnosis of patello-femoral dysfunction secondary to prior intraarticular knee pathology. Radiographic evaluation demonstrated resolution of the previous osteochondral defects in both femoral condyles. After participating in a short course of supervised rehabilitation, the athlete was able to return to full activity. Review of the patient's record since initial diagnosis of bilateral Osgood Schlatter's disease and JOCD suggested a delay in diagnosis secondary to previously existing knee pathology. Informed consent was obtained from the subject and respective guardian prior to preparation of this case review.

Avulsion Fracture of the Olecranon Process.

In brief A young wrestler with an avulsion fracture of the olecranon process was treated conservatively without success. Internal fixation was done, but when no improvement was demonstrated at follow-up, a removable cast was used so that he could do range-of-motion exercises to facilitate healing. This case demonstrates that young patients who complain of elbow pain should be followed carefully because of possible damage to the epiphysis.

Head Over Heels with Success: The Relationship between Self-Efficacy and Performance in Competitive Youth Gymnastics

Research on children involved in youth sport competition has been a popular area in sport psychology. Since the publication of Gould's (1982) paper on the status and future directions of youth sport research, several investigations have emerged, including competitive anxiety patterns of young wrestlers (Gould, Horn, & Spreeman, 1983a, 1983b) reasons for participation and attrition in swimming (Gould, Feltz, Mom, & Weiss, 1982; Gould, Feltz, & Weiss, 1985), and participation motives of gymnasts (Klint & Weiss, 1986, 1987). One topic that has not received much attention in youth sport research is self-efficacy. The construct of self-efficacy (Bandura, 1977, 1986) has been a predominant psychological factor in motor and sport performance (see Feltz, 1988). Self-efficacy is defined as the extent to which an individual believes he or she can execute the behaviors (e.g., sport performance) needed to produce a certain outcome (e.g., succeed in sport). Despite the important and consistent finding that self-efficacy is a strong predictor of performance, most of this research has been conducted with adults. Thus the present study was designed to replicate and extend studies by Gould et al. (1983a) and Scanlan, Lewthwaite, and Jackson (1984) by examining the role of self-efficacy as well as other psychological factors in youth gymnastics. Gould et al. (1983a) studied the competitive anxiety patterns of 13to 19-year-old boys participating in a national wrestling tournament. Sudden Death of a Young Wrestler During Competition

The circumstances of the sudden death of a young healthy wrestler in the course of a wrestling match are described. The death occurred due to acute ischemia of the brainstem as a result of the acute interruption of blood flow to his vertebral-basal system, as a consequence of the injury or rupture of vertebral arteries, after injury of the neck. This interruption of the irrigation of the brainstem with blood provoked the sudden death of the athlete. The cause of the injury to the neck of the wrestler was the wrong position of his head (it was trapped between the mat and his own forearm), at the moment when the athlete was in the inferior disadvantaged position and his opponent was exerting a powerful but permissible effort to overthrow him. The combination of the wrong position of the head of the injured athlete and the forceful action of the wrestler in the offensive resulted in the overflexion and rotation of the head of the former and the (inevitable) damaging of his neck. Various predisposing factors effectively contribute to the injury, the main ones being the lack of warming up of the athlete, the premature fatigue of the athlete, minor injuries at the cervical segment of the vertebral column during the phases preceding the main injury, and the confrontation with an opponent of greater training age and superior fighting level. The only way to deal with similar, very rare indeed, incidents is to implement the set of preventive measures described.

Weight loss through dehydration in amateur wrestling

The desire of the intense, highly competitive athlete to alter body weight without medical supervision commonly results in ineffective, hazardous, and counterproductive abuses that may often endanger health and may affect final growth potential in young wrestlers. Corrective nutrition practices are critical for the endurance athlete to train, compete, and avoid injuries effectively. The best preventive measure may be the education of wrestlers, parents, and coaches about the consequences of rapid and extreme weight loss and the significant role nutrition plays in successful training and competition for the endurance athlete, fluid replacement being a key concern. For as long as wrestlers are required to compete in different weight categories, the popular practice of competing at the lowest possible weight will probably continue. The best course of action nutrition professionals can take is to become acutely aware of the unique nutritional concerns of these athletes in order to make this practice as safe as possible.
