Measurement of Continuous Ambulatory Peritoneal Dialysis Prescription Adherence Using a Novel Approach

The purpose of the study was to test a novel approach to monitoring the adherence of continuous ambulatory peritoneal dialysis (CAPD) patients to their dialysis prescription. A descriptive observational study was done in which exchange behaviors were monitored over a 2-week period of time. Patients were recruited from an outpatient dialysis center. A convenience sample of patients undergoing CAPD at Piedmont Dialysis Center in Winston-Salem, North Carolina was recruited for the study. Of 31 CAPD patients, 20 (64.5%) agreed to participate. Adherence of CAPD patients to their dialysis prescription was monitored using daily logs and an electronic monitoring device (the Medication Event Monitoring System, or MEMS; APREX, Menlo Park, California, U.S.A.). Patients recorded in their logs their exchange activities during the 2-week observation period. Concurrently, patients were instructed to deposit the pull tab from their dialysate bag into a MEMS bottle immediately after performing each exchange. The MEMS bottle was closed with a cap containing a computer chip that recorded the date and time each time the bottle was opened. One individual's MEMS device malfunctioned and thus the data presented in this report are based upon the remaining 19 patients. A significant discrepancy was found between log data and MEMS data, with MEMS data indicating a greater number and percentage of missed exchanges. MEMS data indicated that some patients concentrated their exchange activities during the day, with shortened dwell times between exchanges. Three indices were developed for this study: a measure of the average time spent in noncompliance, and indices of consistency in the timing of exchanges within and between days. Patients who were defined as consistent had lower scores on the noncompliance index compared to patients defined as inconsistent (p = 0.015). This study describes a methodology that may be useful in assessing adherence to the peritoneal dialysis regimen. Of particular significance is the ability to assess the timing of exchanges over the course of a day. Clinical implications are limited due to issues of data reliability and validity, the short-term nature of the study, the small sample, and the fact that clinical outcomes were not considered in this methodology study. Additional research is needed to further develop this data-collection approach.

Marketing and Promoting Intramurals: A Comparison of Four University Approaches

Intramurals has a long history in recreational sports. In managing intramural sports programs, the programmer needs a variety of methods to “get the word out. “It is important to consider promotion, publicity, advertising, sponsorship, and public relations. While these are important to all aspects of recreational sports, intramurals is typically the “backbone” of programming. With this in mind, it is essential that the intramural programmer be concerned with reaching all the students possible. This article discusses and compares the methods utilized by four different universities. They are Middle Tennessee State University in Murfreesboro, Tennessee, Wake Forest University in Winston Salem, North Carolina, Southern Methodist University in Dallas, Texas, and Georgia Southern University in Statesboro, Georgia. It is believed that these four universities represent a variety of types of institutions including a small private university in a medium sized town, a combination of traditional and commuter on the edge of a metropolitan area, a traditional private institution in a major metropolitan area, and a traditional state university in a smaller community. It is very important to get the word out, regardless of the type of institution. The greater the variety of approaches used the better the chances to reach all students.

Aggressive Surgical Treatment of Digital Papillary Adenocarcinoma

The case of a 38-year-old white man with digital papillary adenocarcinoma is presented. It is a lesion of the skin often occurring in the digits of the hand or of the feet. This disease is rare and has not been reported in the plastic surgical or orthopedic literature to date. A case report is presented here along with our rationale for aggressive surgical treatment consisting of amputation.

Surgical Correction of Dysphonia due to Bowing of the Vocal Cords

Dr James Koufman, Winston Salem, NC, at the Annual Meeting of the American Laryngological Association in Palm Beach, Fla, reported his experience with laryngoplastic surgical correction of dysphonia associated with bowing of the vocal cords. Dr Koufman reviewed the four major laryngoplasty techniques that have been used for the treatment of bowing of the vocal cords by lengthening and, thus, tightening the vocal ligaments. Koufman's technique is based on the anatomy of Broyle's ligament, which relates to the attachment of the vocal cords at the anterior commissure. Koufman's innovative modification provides for advancement of Broyle's ligament by using a superiorly based flap. This causes less mechanical disruption of Broyle's ligament. The vocal cords are then extended toward the medial aspect by the placement of Silastic implants to augment the lateral aspects of the larynx and vocal cords. The myth of the healing team

There is increasing use of the concept of "the healing team" or "the therapeutic team" in the literature,1 on television, and in professional conferences. I am bothered by the fact that the concept is used as though there actually were such an entity. After twenty-two years of active work in this area, I have serious doubts that such a "team" exists. There is no doubt in my mind that it is possible for such a concept to have reality: in the general hospital, real approximations have been made in that direction by my friend and colleague, Richard K. Young, Th.D., at the North Carolina Baptist Hospital in Winston Salem.2 Indeed, it must come about if the best possible care is to be given to our patients. But right now it is dishonest to talk as though "the team" existed. What is worse, such talk may seriously hinder the concept's ever becoming a reality. It is the purpose of this paper to inquire into the kind of "team" needed; to outline some criteria as to its meaning; to look at the essential contribution of clinical pastoral training in bringing such a con cept into being; and, finally, to point up the specific contribution that the clergyman has to make to this therapeutic operation.
