Western Section Meeting of the Triologic Society

This report concerns the Western Section Meeting of the Triologic Society, which took place at the Sheraton Grande Torrey Pines, La Jolla, Calif, from January 7 through 9, 1994. Paul Ward, MD , Los Angeles, Calif, presented a fascinating prediction of the future of otolaryngology as his guest-of-honor address. With the legislative and administrative branches of government controlled by the Democrats, he predicts the most dramatic changes in society and medicine since the Social Security and Medicare acts of nearly 60 and 30 years ago, respectively. Health care constitutes 14% of the gross domestic product and increases 1% every 35 months. The cost of health insurance accounts for over $750 of each automobile manufactured. By 2004, we will need 5000 otolaryngologists vs 8000 now. Residencies must be reduced from the present 260 to fewer than 170. Dr Ward also

The following reports the proceedings of the Western Section Meeting of the Triologic Society, which took place at the Four Seasons Olympic Hotel in Seattle, Wash, from January 8 through 10, 1993. Intermaxillary fixation, using cortical bone screws with intermaxillary wires instead of traditional arch bars, was described by Richard F. Busch, MD, Bakersfield, Calif. The technique has been used in 67 cases with reduced operative time and reduced risk of skin puncture by wires. The technique can be applied in tandem with plating in unstable fractures. (1) Placement of a Blom-Singer prosthesis resulted in effective voicing for a patient who had undergone a laryngotracheal separation procedure. This case was reported by David H. Acoustic Neuroma Surgery: A Hearing Conservation Approach

With refinements in early diagnosis of small acoustic neuromas, increasing effort is being directed toward hearing preservation in the resection of small acoustic neuromas, according to a recent report presented at the Western Section Meeting of the Triological Society. Kevin McKennan, MD, of Sacramento, Calif, reviewed his experience with 47 patients who had acoustic neuromas smaller than 2 cm in diameter and who had salvageable hearing. Fifteen of these patients retained useful hearing after acoustic neuroma removal. Useful hearing was described as pure tone levels better than 50 dB and speech discrimination scores of 50% or better. Dr McKennan had greater success in hearing preservation with the suboccipital approach than he did with the middle fossa approach. He reviewed multiple factors that could possibly predict the success of hearing preservation efforts. He found that hearing preservation was more likely if the tumor size was less than 1 cm and if

Radiotherapy for Acoustic Tumors

At the Western Section Meeting of the Triological Society, Douglas A. Chen, MD, Frank M. Rizer, MD, and Derald E. Brackmann, MD, reviewed their experience at the Otologic Medical Group, Los Angeles, with radiation therapy for acoustic tumors. They were able to find only five patients in the records of the Otologic Medical Group who had undergone radiation therapy for acoustic neurinoma. They observed that tumor growth had been arrested in four of the five patients, and that one patient required surgery because of progressive tumor growth. Hearing was lost after radiation therapy in two of the five patients. None of these five patients had useful hearing. Two patients also demonstrated multiple cranial nerve neuropathies. The authors concluded that most of the reports favoring radiotherapy for acoustic neuromas currently come from Scandinavia, and that the present available information suggests that surgery remains the treatment of choice for resectable acoustic neuromas.—

Flap Design for Cochlear Implantation: Avoidance of a Potential Complication

Jeffrey P. Harris, MD, and Robert Cuevo, MD, of the University of California at San Diego, speaking in Los Angeles in January, reported that as more cochlear implants are performed, patients will be implanted who have had previous postauricular incisions. They emphasized that it is important in designing the skin flap for the cochlear implant to give careful consideration to potential blood supply and drainage. In patients who have previously undergone tympanomastoid surgery with postauricular incisions, it was advised that an inferiorly based postauricular flap be designed to assure adequate postsurgical flap viability. The stimulus for this paper, which was presented at the Western Section Meeting of the Triological Society, resulted from Harris and Cuevo's experience with one patient who experienced ischemia of an anteriorly based postauricular flap.—James L. Retrolabyrinthine Vestibular Nerve Section: Limitations and Clinical Experience

Finding the correct surgical approach for the vertiginous patient continues to be a challenge. At the Western Section Meeting of the Triological Society, Carol A. Bowman, MD, and Charles A. Mangham, Jr, MD, of the Virginia Mason Clinic, Seattle, reviewed their experience with the retrolabyrinthine vestibular nerve section. It was pointed out that even though the control rate for vertigo is reportedly high with this procedure, many patients still have postsurgical vestibular symptoms. Bowman and Mangham reviewed 36 cases and reported control of the rotatory vertigo in 94% of the patients involved. In questioning their patients, they found that the majority continued to have symptoms of vestibular disturbance and balance disturbance even though the rotatory vertigo was controlled. In spite of the limitations of the procedure, the authors still feel that it is efficacious for the vertiginous patient.—James L. Parkin, MD, MS, Salt Lake City

Arc furnace steel production, Kansas City Works-Armco Steel Corp.

In this paper we have reviewed some of the factors which, in our experience, influence production rates and production costs of electric furnace operations. We have not mentioned many of the things that any steel producing shop must do. A sound electric and mechanical preventive maintenance program is certainly an important factor in the operation of an electric furnace. An excellent paper on preventive maintenance in an electric furnace shop was presented in the Association of Iron and Steel Engineers’ 1962 Western Section Meeting by Dennis Miner, Supt. of Mechanical and Electrical Departments for Bethlehem Steel Corp., Seattle, Wash.4

The Return of Native Perennial Bunchgrass Following the Removal of Klamath Weed (Hypericum Perforatum L.) by Imported Beetles

During the relatively short period of 4 years since release of imported beetles which feed on it, Klamath weed, HypericUri perforatiun, a serious weed on perhaps a million or more acres of Western range lands, has been brought under strikingly successful control in limited areas by this tool of applied ecology. Though the range in queston has not yet been put under normal grazing, the rapidity and vigor with which the climax hunchgrass, Dantho nia californica, entered the space vacated by the removal of Klamath weed by beetles of the genus Chrysomuiela (= Chrysolina), promises much for the more favorable locations in Humboldt and possibly Menclocino, Trinity and Siskiyou counties in California. The initiation of quantitative studies designed to follow the pattern of vegetational changes and the correlated beetle populations was reported by Iluff aker and Holloway (1949). A brief review of an important survey of these counties by Davy (1902) indicates: 1) The climax grass cover in this area probably consisted of Danithonia californica as the most important component. Other bunchgrasses such as Stipa, Melica, Elymus, Poa, and Festuca were represented, as were important associated forage species such as Trifoliuml spp. (annual and perennial clovers), Lath yrus spp. (wild pea vines), and Wyethia sp. (wild sunflower). 2) There were important early changes, apparently beginning with invasion of the region by such introduced annuals as Erodiumt cicutarpum (filaree) and Avenia fatua (wild oats), later followed, as a result of still heavier stocking, by Hordewun gussoneanum (foxtail), Festuca myuros (rat-tail fescue), and Browrus hlordeaceus (soft chess), and still later by the appearance of weedy arrivals during the last years of the past century. such as Erodium moschatum (musky filaree), Brownus rigidus (broncho grass) Hordeum murinumn (foxtail), Centaurea melitensis (tacaloti), Hypochoeris glabra (hawkbit), and Medicago hispida (burclover). The latter species and the filaree are considered of definite forage value. Some comparison of Davy's survey with the present quantitative results may be worthwhile. For example, Hypochoeris glabra was at that time making its appearance, but now is a definite pest.
