Intracordal cyst in a neonate

Vocal cord cysts are rare, and typically present with symptoms of dysphonia in older children and adults. There is often a background of voice abuse. We present the first recorded and photographed case of an intracordal cyst presenting in a neonate. The patient presented with symptoms of episodic stridor, respiratory distress, and feeding difficulties, which resolved rapidly after simple incision of the cyst. Theories for the aetiology of intracordal cysts are discussed. The authors believe that this case lends support to the theory that intracordal cysts are congenital in origin.

Clinicopathologic Study and Classification of Vocal Cord Cysts

Cysts of the true vocal cords are less common than other laryngeal cysts. They are usually easily recognized and managed. Patients present with complaints of hoarseness and/or dyspnea. We report our experience with 41 cases of cysts located in the true vocal cords. Clinical and histological aspects are reviewed and discussed. A new histological classification is proposed: A: cysts lined by columnar epithelium with mucous content; B: lined by columnar epithelium with cilia; C: lined b squamous epithelium without keratinization; D: lined by squamous epithelium with keratinization.

Value of videostroboscopic parameters in differentiating true vocal fold cysts from polyps.

Differentiating between benign true vocal fold (TVF) cysts, polyps, and nodules on the basis of their gross appearance would be advantageous in determining the need for surgical therapy. A retrospective study of 32 patients covering 31 months was done to assess the ability to differentiate these lesions on the basis of stroboscopic parameters. Stroboscopic examinations were evaluated for symmetry, amplitude, periodicity, mucosal wave, and closure. Preoperative diagnoses were validated with operative and histologic confirmation. Mucosal wave was absent or diminished in 100% of TVF cysts and present or increased in 80% of TVF polyps (P < .05). Thirty-one percent of original histopathologic diagnoses were modified after pertinent clinical information was provided to the pathologist. Therefore, stroboscopic evaluation of mucosal wave is helpful in preoperative differentiation of TVF cysts and polyps, and providing clinical information to the pathologist is critical for useful histologic information.

Microlaryngeal surgery on a day case basis

The aim of this study was to assess the clinical safety of performing microlaryngeal surgery (MLS) under general anaesthesia in selected patients in the ambulatory setting. Twenty-two adult patients were scheduled to have tissue specimens of the larynx taken by biopsy (54%), for vocal cord polypectomy (41%) or for vocal cord cyst excision (5%). Twenty-one ASA I and II patients (95%) were discharged home the same day of the procedure. Two of them presented with laryngospasm after extubation of the trachea. One ASA III patient (5%) had to be admitted overnight because of severe laryngospasm and bronchospasm, but was discharged the day after the operation. None of the patients had significant complications after leaving the recovery room (mean stay 85 min). There were no re-admissions to the hospital. Our data suggests that microlaryngeal surgery in selected patients can be safely performed on a day case basis.

Seventy-five patients with vocal fold cyst who visited our clinic between 1981 and 1990were studied. Almost half of the cases were male and 83% were between thirty and seventy years of age. Only two cases were younger than twenty years of age. Surgery was performed for 38 patients and 97% of those patients showed vocal improvement after surgery. Vocal function tests and acoustic analyses revealed significant improvements after surgery. There was no difference in results between the complete removal of the cyst with preservation of vocal fold epithelium and removal of the superficial cyst wall covered by vocal fold epithelium. Recurrence occurred in only one case after surgery.

Contribution to the understanding of the etiology of vocal fold cysts: a functional and histologic study.

The etiological theories of vocal fold cysts can be divided into two basic groups: those of congenital and acquired cysts. In ongoing practice, the authors had noted that the greater number of cysts appeared at the functionally most active segment of the vocal folds which, on the other hand, has the least number of glands. Also, it had been noted that patients with vocal fold cysts tended to have hyperkinetic patterns of voice production. These observations indicated the possibility of a functional aspect in the etiology of vocal fold cysts, and consideration of such a possibility was the aim of this work. In 37 cases, the exact location of the cyst was established. In addition, the muscular activity of the phonatory apparatus was estimated, patient self-descriptions with respect to talkativeness were taken into account, and histological evaluations were made. The cysts were most frequently found in the area of the junction of the anterior and middle thirds of the free edge of the vocal fold. Muscular activity during speech and phonation was increased in study patients. Sixty-five percent of patients had epidermoid cysts and 35% had retention cysts of the vocal fold. According to study results, the functional aspect of cyst genesis has a marked role in the etiology of vocal fold cysts, which points to the great importance of functional care for cyst patients.

Mucosal Blood Volume and Oxygen Saturation in the Human Vocal Fold

With a newly developed spectrophotometric analyzer we were able to measure hemoglobin (Hb) concentration and its oxygen saturation in the human vocal and ventricular folds. Studies were made on 30 subjects including 18 normal controls. In laryngomicrosurgery under general anesthesia, measurements were made with a fiber probe touching the laryngeal mucosa. Results were as follows: In normal mucosa of the vocal fold, the mean value of IHb was 40 and the ISO2 was 55, while in that of the ventricular fold, the mean values were 74 and 56, respectively. Thus, in spite of the influence of general anesthesia, the mucosal blood volume was lower in the vocal fold than in the ventricular fold. The mean value of IHb was 30 in the vocal fold polyps, 122 in the vocal fold nodule, 43 in the polypoid degenerations, 17 in the vocal fold cyst, 62 in the granuloma, 6 in the sulcus vocalis and 19 in the hyperkeratoses. This method is considered useful for objective and quantitative analysis of pathological conditions in the vocal fold.

In the present report, a clinical observation on 29 cases of the vocal cord cyst who were treated in Kitasato University Hospital during the period from August 1971 to April 1983.The results were as follows.1. The chief complaint of these patients was hoarseness which was worse than expected from their laryngeal appearances.2. Histopathological study revealed that the most cyst walls consist of the squamous epithelium.3. Mechanical implantation of the squamous epithelium could be considered as one of the causative factors.

Cysts of the Larynx. A 10-Year Review of 94 Patients

We reviewed the clinical records of successive 94 patients with laryngeal cysts who were examined in the ENT clinic of Kurume University Hospital during the 10 years from 1971 to 1980. Cysts were located in the epiglottis in 48, the vocal fold in 43, and the ventricular fold in 3 patients. Results were summarized as follows: 1) Epiglottis cysts were more frequent in males (32 patients) than in females (16 patients). Age ranged from 33 to 74 years with a high incidences in the fifth decades or older. Twelve patients were surgically treated. 2) Vocal fold cysts were also more frequent in males (26 patients) than in females (17 patients). Age ranged from 8 to 74 years with a mode in the fifth and sixth decades. Twenty-five patients underwent surgical treatments. Of these 25 patients, 22 (88%) recovered normal voice, 2 (8%) had their voice improved and 1 (4%) had no improvement. 3) All of the 3 patients with ventricular fold cysts were females. The youngest age was 3 years and the oldest, 82 years. All patients underwent surgical treatments. Two patients recovered normal voice and the other was lost to follow up.
