Perception of Fragmented Letters by Patients With Pathologically Confirmed Dementia With Lewy Bodies or Alzheimer Disease.

Patients with dementia with Lewy bodies perform worse than those with Alzheimer disease (AD) on tests of visual perception, but the clinical utility of these tests remains unknown because studies often had clinically diagnosed groups that may inadvertently cross-contaminate Lewy body disease (LBD) with pure AD pathology, used experimental tests not easily adaptable for clinical use, and had no way to examine relationships between the severity of LBD pathology and degree of cognitive impairment. Therefore, we sought to determine whether performance on a widely used clinical test of visuoperceptual ability effectively differentiates between patients with autopsy-confirmed LBD or AD and correlates with the severity of LBD pathology. Patients with mild to moderate dementia (n = 42) and cognitively healthy controls (n = 22) performed a Fragmented Letters Test in which they identified letters of the alphabet that were randomly visually degraded by 70% and additional visuospatial and episodic memory tests. At autopsy, dementia cases were confirmed to have LBD (n = 19), all with concomitant AD, or only AD (n = 23). Severity of α-synuclein pathology in the hippocampus and neocortex was rated on an ordinal scale. Patients with LBD performed worse than those with AD (B = -2.80 ± 0.91, p = 0.009) and healthy controls (B = -3.34 ± 1.09, p = 0.01) on the Fragmented Letters Test after adjustment for age, sex, education, Mini-Mental State Examination score, and ability to name intact letters. Patients with AD did not differ from controls (B = -0.55 ± 1.08, p = 0.87). The test effectively distinguished between patients with LBD or AD with 73% sensitivity and 87% specificity, and the area under the curve in receiver operating characteristic analyses was 0.85 (95% CI 0.72-0.95), higher than for standard tests of visuospatial ability (Block Design; 0.72; CI 0.35-0.75) or memory (California Verbal Learning Test, trials 1-5; 0.55; CI 0.57-0.88). Fragmented Letters Test scores were negatively correlated with LBD pathology density ratings in hippocampus and neocortical regions (Spearman rs = -0.53 to -0.69). Fragmented Letters Test performance can effectively differentiate patients with LBD pathology from those with only AD pathology at a mild to moderate stage of dementia, even when LBD occurs with significant concomitant AD pathology, and may also be useful for gauging the severity of cortical α-synuclein pathology in those with LBD.

Role of testosterone: cortisol ratio in age- and sex-specific cortico-hippocampal development and cognitive performance.

Testosterone (T) and cortisol (C) are steroid hormones that have been argued to play opposing roles in shaping physical and behavioral development in humans. While there is evidence linking T and C to different memory processes during adulthood, it remains unclear how the relative levels of T and C (TC ratio) may influence brain and behavioral development, whether they are influenced by sex of the child, and whether or not they occur as a result of stable changes in brain structure (organizational changes), as opposed to transient changes in brain function (activational changes). As such, we tested for associations among TC ratio, cortico-hippocampal structure, and standardized tests of executive, verbal, and visuo-spatial function in a longitudinal sample of typically developing 4-22-year-old children and adolescents. We found greater TC ratios to be associated with greater coordinated growth (i.e. covariance) between the hippocampus and cortical thickness in several areas primarily devoted to visual function. In addition, there was an age-related association between TC ratio and parieto-hippocampal covariance, as well as a sex-specific association between TC ratio and prefrontal-hippocampal covariance. Differences in brain structure related to TC ratio were in turn associated with lower verbal/executive function, as well as greater attention in tests of visuo-spatial abilities. These results support the notion that TC ratio may shift the balance between top-down (cortex to hippocampus) and bottom-up (hippocampus to cortex) processes, impairing more complex, cortical-based tasks and optimizing visuospatial tasks relying primarily on the hippocampus.

Searching for individual determinants of probabilistic cueing in large-scale immersive virtual environments

Large-scale search behaviour is an everyday occurrence, yet its underlying mechanisms are not commonly examined within experimental psychology. Key to efficient search behaviour is the sensitivity to environmental cues that might guide exploration, such as a target appearing with greater regularity in one region than another. Spatial cueing by probability has been examined in visual search paradigms, but the few studies that have addressed its contribution to large-scale search and foraging present contrasting accounts of the conditions under which a cueing effect can be reliably observed. In the present study, participants physically searched a virtual arena by inspecting identical locations until they found the target. The target was always present, although its location was probabilistically defined so that it appeared in the cued hemispace on 80% of trials. In Experiment 1, when participants’ starting positions were stable, a probabilistic cueing effect was observed, with a strong bias towards searching the cued side. In Experiment 2, the starting position changed across the experiment, such that the cued region was defined in allocentric co-ordinates only. In this case, a probabilistic cueing effect was not observed across the sample. Analysis of individual differences in Experiment 2 suggests, however, that some participants may have learned the contingency underpinning the target’s location, although these differences were unrelated to other tests of visuospatial ability. These results suggest that the ability to learn the likelihood of an item’s fixed location when starting from different perspectives is driven by individual differences in other cognitive or perceptual factors.

Psychometric Tests and Spatial Navigation: Data From the Baltimore Longitudinal Study of Aging.

Spatial cognition is the process by which individuals interact with their spatial environment. Spatial cognition encompasses the specific skills of spatial memory, spatial orientation, and spatial navigation. Prior studies have shown an association between psychometric tests of spatial ability and self-reported or virtual measures of spatial navigation. In this study, we examined whether psychometric spatial cognitive tests predict performance on a dynamic spatial navigation task that involves movement through an environment. We recruited 151 community-dwelling adult participants [mean (SD) age 69.7 (13.6), range 24.6–93.2] from the Baltimore Longitudinal Study of Aging (BLSA). Spatial navigation ability was assessed using the triangle completion task (TCT), and two quantities, the angle and distance of deviation, were computed. Visuospatial cognitive ability was assessed primarily using the Card Rotations Test. Additional tests of executive function, memory, and attention were also administered. In multiple linear regression analyses adjusting for age, sex, race, and education, cognitive tests of visuospatial ability, executive function, and perceptual motor speed and integration were significantly associated with spatial navigation, as determined by performance on the TCT. These findings suggest that dynamic spatial navigation ability is related to spatial memory, executive function, and motor processing speed.

Visuospatial skills are better predictors than dexterity for basic ultrasonographic and fluoroscopic skills in veterinary students.

Radiological skills including ultrasonography and fluoroscopy, require a combination of manual dexterity and visuospatial skill to develop competency. The ability to detect veterinary students with an interest in radiology but who are deficient in these skills, may permit more individual adaptations to training programs, allowing for students training in radiology to achieve maximal potential. The objective of this cohort study was to investigate whether innate dexterity and visuospatial skill could be used to predict performance of basic ultrasound and fluoroscopic skills in veterinary students. Fifty veterinary students from the Ontario Veterinary College completed three tests of visuospatial ability, two tests of manual dexterity, a three-dimensional mouse task, an ultrasound skill-testing task, and a fluoroscopic skill-testing task. Students who reported chopstick use completed the non-dominant hand ultrasound task significantly faster than students who did not (P=0.001). There was a significant positive association between scores on the Mental Rotations Test and time to complete the non-dominant hand ultrasound task (P=0.011) and fluoroscopy task (P=0.029). No variables were associated with time to complete the dominant hand ultrasound task. The results of this study suggest that visuospatial skill, as assessed by the Mental Rotations Test, is a better predictor of baseline ultrasound and endovascular fluoroscopy skill than dexterity, with the exception of reported chopstick use, in veterinary students. Visuospatial skills can be developed and may be useful to include in the veterinary curriculum for students that are deficient, or students entering a field such as diagnostic imaging.

Neurocognitive function in paediatric obsessive-compulsive disorder

Objectives: The small body of neuropsychological research in paediatric obsessive-compulsive disorder (OCD) yields inconsistent results. A recent meta-analysis found small effect sizes, concluding that paediatric OCD may not be associated with cognitive impairments, stressing the need for more research. We investigated neuropsychological performance in a large sample of youths with OCD, while assessing potential moderators.Methods: Participants with OCD (n = 102) and matched controls (n = 161) were thoroughly screened and blindly evaluated for comorbidities, and completed a neuropsychological battery assessing processing speed, visuospatial abilities (VSA), working memory (WM), non-verbal memory (NVM), and executive functions (EF).Results: Compared to controls, youths with OCD exhibited underperformance on tasks assessing processing speed. On tests of VSA and WM, underperformance was found only on timed tasks. There were no differences on NVM and EF tasks. Notably, the OCD group’s standardised scores were in the normative range. Test performance was not associated with demographic or clinical variables.Conclusions: Youths with OCD exhibited intact performance on memory and EF tests, but slower processing speed, and underperformance only on timed VSA and WM tasks. While the OCD group performed in the normative range, these findings reveal relative weaknesses that may be overlooked. Such an oversight may be of particular importance in clinical and school settings.

A pilot study of performance among hospitalised elderly patients on a novel test of visuospatial cognition: the letter and shape drawing (LSD) test.

Conventional bedside tests of visuospatial function such as the clock drawing (CDT) and intersecting pentagons tests (IPT) are subject to considerable inconsistency in their delivery and interpretation. We compared performance on a novel test - the letter and shape drawing (LSD) test - with these conventional tests in hospitalised elderly patients. The LSD, IPT, CDT and the Montreal Cognitive Assessment (MoCA) were performed in 40 acute elderly medical inpatients at University Hospital Limerick The correlation between these tests was examined as well as the accuracy of the visuospatial tests to identify significant cognitive impairment on the MoCA. The patients (mean age 81.0±7.71; 21 female) had a median MoCA score of 15.5 (range=1-29). There was a strong, positive correlation between the LSD and both the CDT (r=0.56) and IPT (r=0.71). The correlation between the LSD and MoCA (r=0.91) was greater than for the CDT and IPT (both 0.67). The LSD correlated highly with all MoCA domains (ranging from 0.54 to 0.86) and especially for the domains of orientation (r=0.86), attention (0.81) and visuospatial function (r=0.73). Two or more errors on the LSD identified 90% (26/29) of those patients with MoCA scores of ⩽20, which was substantially higher than for the CDT (59%) and IPT (55%). The LSD is a novel test of visuospatial function that is brief, readily administered and easily interpreted. Performance correlates strongly with other tests of visuospatial ability, with favourable ability to identify patients with significant impairment of general cognition.

Validity and reliability of a "simplified" version of the Taylor Complex Figure Test for the assessment of older adults with low formal education.

ABSTRACTObjective:The assessment of visuospatial abilities and memory using tasks such as the Taylor Complex Figure Task (TCFT) is biased for older adults with low formal education. We devised a "Simplified" version of the TCFT designed to assess older adults with low educational background and provide preliminary evidence of its psychometric properties. Methods:We evaluated a heterogeneous sample of healthy older adults and patients with mild cognitive impairment and Alzheimer's disease dementia using the simplified TCFT and other neuropsychological measures. Results:Our results suggest that the test copy, immediate and delayed recall have high inter-rater agreement and internal consistency, significant correlations with other tests of visuospatial abilities, memory and intelligence, and also detected significant group differences between controls and patients. Conclusion:Our study presents a new measure for assessing low-educated elderly with promising evidence of validity and reliability.

Association Between Visuospatial Ability and Vestibular Function in the Baltimore Longitudinal Study of Aging.

To investigate the relationship between vestibular loss associated with aging and age-related decline in visuospatial function. Cross-sectional analysis within a prospective cohort study. Baltimore Longitudinal Study of Aging (BLSA). Community-dwelling BLSA participants with a mean age of 72 (range 26-91) (N = 183). Vestibular function was measured using vestibular-evoked myogenic potentials. Visuospatial cognitive tests included Card Rotations, Purdue Pegboard, Benton Visual Retention Test, and Trail-Making Test Parts A and B. Tests of executive function, memory, and attention were also considered. Participants underwent vestibular and cognitive function testing. In multiple linear regression analyses, poorer vestibular function was associated with poorer performance on Card Rotations (P = .001), Purdue Pegboard (P = .005), Benton Visual Retention Test (P = 0.008), and Trail-Making Test Part B (P = .04). Performance on tests of executive function and verbal memory were not significantly associated with vestibular function. Exploratory factor analyses in a subgroup of participants who underwent all cognitive tests identified three latent cognitive abilities: visuospatial ability, verbal memory, and working memory and attention. Vestibular loss was significantly associated with lower visuospatial and working memory and attention factor scores. Significant consistent associations between vestibular function and tests of visuospatial ability were observed in a sample of community-dwelling adults. Impairment in visuospatial skills is often one of the first signs of dementia and Alzheimer's disease. Further longitudinal studies are needed to evaluate whether the relationship between vestibular function and visuospatial ability is causal.

The evaluation of visuospatial performance between screen and paper

This study evaluates the effect of presentation media (PC tablet versus pencil and paper) on the performance, level of visual fatigue, and subjective preference of those taking visuospatial tests. Fifty university students participated and performed three visuospatial short-term memory tests and three visuospatial ability tests by using both types of display media. The display medium substantially affected all of the measured variables (p<0.01). On average, the paper–pencil test scores of the visuospatial short-term memory tests were about 10% higher and the answer time was about 20% shorter than those of the PC tablet tests. The average paper–pencil test score of the visuospatial ability tests was about 35% higher than the average test score of the PC tablet test. The visuospatial performance was substantially decreased under the PC tablet condition compared with that under the pencil–paper condition. In addition, visual fatigue was greater when participants used the PC tablet than when they used a pencil and paper.

Evaluation of a Virtual 3D Learning Resource in Neuroanatomy for Undergraduate Medical Students

In contrast to traditional teaching strategies, e‐learning presents the opportunity for development of learner‐centered educational tools, tailored to meet each student's distinct needs; however this has yet to be examined fully in the literature. Our study's cross‐over design divided participants into two groups, each beginning with anatomy knowledge and visuo‐spatial ability tests, followed by access to either the 3D online learning module or the gross anatomy laboratory. Participants were administered a second anatomy knowledge test, prior to switching to the other learning modality. There were no significant differences between groups in their baseline anatomy knowledge or visuo‐spatial abilities. Students who first accessed 3D online resources scored significantly better than students who accessed gross anatomy resources on the first anatomy knowledge test. After learning with both modalities, there were no significant differences between groups. No correlations were found between spatial ability and assessment score. Students responded positively to the 3D module, and their learning outcomes were equivalent or improved compared to when taught in the gross anatomy lab. Larger studies are required to confirm these preliminary results. Results may be used to help establish guiding principles to facilitate the design and implementation of effective and efficient e‐learning curricula.

The effect of childhood trauma on spatial cognition in adults: a possible role of sex

Although animal evidence indicates that early life trauma results in pervasive hippocampal deficits underlying spatial and cognitive impairment, visuo-spatial data from adult humans with early childhood adversity are lacking. We administered 4 tests of visuo-spatial ability from the Cambridge Neuorpsychological Test Automated Battery (CANTAB) to adults with a history of childhood trauma (measured by the Childhood Trauma Questionnaire) and a matched sample of healthy controls (trauma/control = 27/28). We observed a significant effect of trauma history on spatial/pattern learning. These effects could not be accounted for by adverse adult experiences, and were sex-specific, with prior adversity improving performance in men but worsening performance in women, relative to controls. Limitations include the small sample size and reliance of our study design on a retrospective, self report measure. Our results suggest that early adversity can lead to specific and pervasive deficits in adult cognitive function.

Catch-up growth does not associate with cognitive development in Indian school-age children

Stunting is significantly associated with lifetime morbidity and poorer cognitive outcomes in children. Although several studies have examined the relationship between stunting, catch-up growth and cognitive performance in young populations, this relationship has not yet been explored in school-aged children. In this study, we used data from three different nutritional intervention studies conducted over a 4-year period on school-age children in Bangalore, India to assess these relationships. A battery of cognitive tests was conducted before each intervention to determine whether stunting status at baseline was related to cognitive performance across four separate domains, and repeated after a 6-month period to assess whether changes to stunting status is related to cognitive advancement. Results of independent t-tests showed that while stunted children had significantly poorer performance on short-term memory, retrieval ability and visuospatial ability tests (P=0.023, 0.026 and 0.028, respectively), there was no significant difference in the change in cognitive scores following nutritional interventions over a 6-month period between those who remained stunted and those who were no longer stunted (P>0.10). Evidently, stunting remains associated with cognitive ability in school-age children; however, the reversal of these effects in this age group may be quite difficult.

Reliability of Three Benton Judgment of Line Orientation Short Forms in Idiopathic Parkinson’s Disease

Individuals with Parkinson’s disease (PD) often exhibit deficits in visuospatial functioning throughout the course of their disease. These deficits should be carefully assessed as they may have implications for patient safety and disease severity. One of the most commonly administered tests of visuospatial ability, the Benton Judgment of Line Orientation (JLO), consists of 30 pairs of lines requiring the patient to match the orientation of two lines to an array of 11 lines on a separate page. Reliable short forms have been constructed out of the full JLO form, but the reliability of these forms in PD has yet to be examined. Recent functional MRI studies examining the JLO demonstrate right parietal and occipital activation, as well as bilateral frontal activation and PD is known to adversely affect these pathways. We compared the reliability of the original full form to three unique short forms in a sample of 141 non-demented, idiopathic PD patients and 56 age- and education-matched controls. Results indicated that a two-thirds length short form can be used with high reliability and classification accuracy in patients with idiopathic PD. The other short forms performed in a similar, though slightly less reliable manner.

A test of the circumvention-of-limits hypothesis in scientific problem solving: The case of geological bedrock mapping.

Sources of individual differences in scientific problem solving were investigated. Participants representing a wide range of experience in geology completed tests of visuospatial ability and geological knowledge, and performed a geological bedrock mapping task, in which they attempted to infer the geological structure of an area in the Tobacco Root Mountains of Montana. A Visuospatial Ability × Geological Knowledge interaction was found, such that visuospatial ability positively predicted mapping performance at low, but not high, levels of geological knowledge. This finding suggests that high levels of domain knowledge may sometimes enable circumvention of performance limitations associated with cognitive abilities.

Forced Expiratory Volume in 1 Second and Cognitive Aging in Men

To evaluate forced expiratory volume in 1 second (FEV(1) , a measure of overall lung function), long-term average FEV(1) , and rate of decline in FEV(1) in relation to cognition and cognitive decline in older men. Prospective observational study. Community-based population. Eight hundred sixty-four older men from the Normative Aging Study. Starting in 1984, participants underwent triennial clinical evaluations. Lung function assessments provided estimates of FEV(1) . Cognitive assessments entailing tests of several cognitive abilities began in 1993. FEV(1) measured approximately 12 years before baseline cognitive testing, average FEV(1) over the 12-year period, and rate of change in FEV(1) were all evaluated in relation to baseline and change in performance on the cognitive tests. In multivariable-adjusted analyses, associations between FEV(1) and baseline cognitive scores were mixed, although average FEV(1) predicted significantly better performance on tests of visuospatial ability (P=.04) and general cognition (P=.03). Higher FEV(1) was more consistently associated with slower cognitive decline, but only the association between historical FEV(1) and attention was significant (difference per standard deviation in FEV(1) =0.056, P=.05). Rate of FEV(1) decline was not consistently associated with cognitive function or decline. Findings were generally similar or stronger in men who had never smoked. To account for potential bias due to selective attrition, inverse probability of censoring weights were applied to the cognitive decline analyses, yielding slightly larger estimates; the inadequate prognostic power of the censoring models limited this approach. Overall, the data provide limited evidence of an inverse association between FEV(1) and cognitive aging.

How does dementia affect driving in older patients?

Driving is a complex activity that always becomes impaired at some point in older adults with degenerative dementia. Over time, disruption of the visual processing circuits of the brain that link the occipital and prefrontal regions, particularly in the right hemisphere, leads to increasing degrees of driving impairment that ultimately preclude safe driving. Neuropsychological tests of visuospatial ability, executive function and attention that tap into the integrity of these brain regions provide the clinician with important information regarding the need for a formal determination of driving competence. Enhancement of cognitive function in these domains through anti-dementia therapy and exercise may partially mitigate risk; however, all drivers with dementia must ultimately retire from driving when dementia becomes moderately severe, and often in earlier stages of the illness. Future efforts to improve screening tests for hazardous driving and to develop interventions to help prolong the time that drivers with mild dementia can continue to drive safely are needed for our increasingly aged and mobile population.

Longitudinal Study of the Transition From Healthy Aging to Alzheimer Disease

Detection of the earliest cognitive changes signifying Alzheimer disease is difficult. To model the cognitive decline in preclinical Alzheimer disease. Longitudinal archival study comparing individuals who became demented during follow-up and people who remained nondemented on each of 4 cognitive factors: global, verbal memory, visuospatial, and working memory. Alzheimer Disease Research Center, Washington University School of Medicine, St Louis, Missouri. One hundred thirty-four individuals who became demented during follow-up and 310 who remained nondemented. Inflection point in longitudinal cognitive performance. The best-fitting model for each of the 4 factors in the stable group was linear, with a very slight downward trend on all but the Visuospatial factor. In contrast, a piecewise model with accelerated slope after a sharp inflection point provided the best fit for the group that progressed. The optimal inflection point for all 4 factors was prior to diagnosis of dementia: Global, 2 years; Verbal and Working Memory, 1 year; and Visuospatial, 3 years. These results were also obtained when data were limited to the subset (n = 44) with autopsy-confirmed Alzheimer disease. There is a sharp inflection point followed by accelerating decline in multiple domains of cognition, not just memory, in the preclinical period in Alzheimer disease when there is insufficient cognitive decline to warrant clinical diagnosis using conventional criteria. Early change was seen in tests of visuospatial ability, most of which were speeded. Research into early detection of cognitive disorders using only episodic memory tasks may not be sensitive to all of the early manifestations of disease.
