Visual outcomes after cataract surgery in monocular compared to binocular patients: a case-control study.

We aimed to identify ocular comorbidities and reasons of blindness in monocular patients and to compare visual outcomes of cataract surgery between monocular and binocular patients. A single-center case-control study was conducted between November 2011 and May 2019 to compare consecutive series of patients needing cataract surgery in Strasbourg University Hospitals, France. Cases were patients with permanent monocular vision loss. Controls were binocularly sighted patients. All patients underwent cataract surgery using phacoemulsification technique. Chart analysis included demographic data, medical history, and surgical determinants data. Student's t tests and Fisher's exact tests were the main methods used for statistical analysis. Each group included 80 patients. The mean age at the time of surgery was significantly higher in monocular than binocular patients (77 vs. 71years, p < 0.001). Thirty-two monocular patients (40%) had ocular comorbidities, compared to only 19 (23%) in the control group (p < 0.05). The leading cause of monocular status was amblyopia caused by strabismus (22 patients, 27.5%). Age-related macular degeneration, open-angle glaucoma, and diabetic retinopathy were the three main ocular comorbidities that were observed in the monocular group. Monocular patients had significantly lower visual acuity than the control group (p < 0.01) before and after cataract surgery. Conversely, improvement in visual acuity after surgery was not statistically different between groups (p = 0.054). There was no statistically significant difference in the rate of surgical complications between groups (p = 0.622). This study illustrates that cataract surgery in monocular patients is not more complicated than in binocular patients, but that it is significantly delayed.

Complication rates in phacoemulsification surgeries performed by trainee residents

Objectives: Phacoemulsification with a foldable intraocular lens is the gold standard of cataract treatment. Studies have shown that various risk factors affect the visual outcome of cataract surgery. One of these is the availability of well-trained surgeons. Here, we are trying to understand how to achieve good surgical outcomes even if the surgeon has low surgical experience. Materials and Methods: A hospital-based prospective study was done from August 2019 to March 2021. After a detailed examination, each patient was scored based on the risk factors associated and classified into various risk groups. A total of 286 patients were studied, out of which consultants operated on 143 patients and 143 patients were operated on by trainee residents. The intraoperative and postoperative complication rates are compared based on the complexity of cases between trainee residents and consultants. Results: About 82.5% of the patients operated by trainee residents and 78.32% of patients operated by consultants belonged to the no-risk group with complication rates of 2.50% and 1.80%, respectively, which was statistically not significant. In the case of patients with associated preoperative risk factors, 36% of patients operated on by trainee residents had complications, and 9.6% of patients operated on by consultants had statistically significant complications. Conclusion: Preoperative stratification and allocation of cases according to associated risk factors help reduce complications, help trainee residents gain essential surgical skills, and give better surgical outcomes for patients.

Quality of life assessment for nuclear, cortical, posterior subcapsular patients before and after cataract surgery

PurposeThe impact of visual outcomes of cataract surgery can be measured using a questionnaire. The aim of our study was to evaluate how patient quality of life changes after cataract surgery and if there are differences between the responses of patients with nuclear, cortical, and posterior subcapsular cataracts, which has not been studied before. MethodWe studied 210 cataract patients who were divided into 3 cataract groups based on their cataract type: nuclear (n = 80), cortical (n = 70), and posterior subcapsular (PSC) (n = 60). The patients completed the Visual Function Index (VF-14) questionnaire before, 2 weeks and 1 month after bilateral cataract surgery. The results were analysed using one-way ANOVA (significance level 5 %) and were compared over time and between the cataract groups. ResultsBefore the cataract surgery, cortical cataract patients had the lowest questionnaire score compared to nuclear and posterior subcapsular cataract groups (p = 0.08). After cataract surgery, cortical cataract patients experienced the greatest improvement in near distance daily activities, while PSC cataract patients experienced the greatest improvement in far distance daily activities (p = 0.38). Before surgery, nuclear cataract patients had the highest questionnaire scores compared to the other cataract groups (p = 0.08). ConclusionAt the 1 month follow-up, there were no statistically significant differences in questionnaire scores between the cataract groups for any of the questions. Overall, cortical cataract patients showed a trend to experience the greatest subjective improvement in quality of life after cataract removal, followed by nuclear and posterior subcapsular patients.

Visual Outcomes of Cataract Surgery at the Eye Foundation Community Hospital in Nigeria

Aims: To determine the visual outcome of cataract surgeries postoperatively at first day, one and three months. &#x0D; Study Design: Retrospective, case series study.&#x0D; Place and Duration of Study: Eye Foundation Community Hospital, Ilese-Ijebu-Imushin, Ogun state of Nigeria. 3-month duration from June 1st 2019 to August 31st 2019.&#x0D; Methodology: All patients 18 years and above operated for cataract surgery without comorbidity were included in the study. Type of surgery done were manual small incision cataract surgery (MSICS) and phacoemulsification. Patient data were entered into excel spread sheet created for the study. From where it was cleaned up and exported in to statistical package for social sciences 20. Visual outcome was assessed on the first day, first month and at third month post operatively. Snellen visual acuity was converted to log MAR. P value = 0.05 is determined as statistically significant. &#x0D; Results: Two hundred and fifty (250) patients or eyes were analysed in this study. Two hundred and twenty patients (88%) had MSICS, 30 (12%) had phacoemulsification. Preoperatively, majority (150, 60%) presented with visual acuity less than 3/60 followed by patients with visual acuity between 6/18 to 6/60 (76, 30.4%). Nineteen patients (7.6%) had visual acuity of 6/18 or better. Mean preoperative log MAR was 1.6 ± 0.8 (equivalent to count finger). 3-month post op, two hundred and fourteen (85.6%) had uncorrected visual acuity of 6/18 or better while 231 patients (92.4%) had best corrected (with pinhole) visual acuity of 6/18 or better. (P=0.004 and 0.038 respectively). &#x0D; Conclusion: There is an appreciable improvement in the visual outcome following cataract surgery.

Visual Outcomes of Cataract Surgery with Foldable Lens

Objective: To determine the visual outcomes of cataract surgery with foldable lens at tertiary care eye hospital SindhPakistan. Study design and setting: An observational cross-sectional study carried out at Ghulam Muhammad Maher Medical College (GMMMC) Sukkur from February 2020 to January 2022. Methodology: A Non-probability purposive sampling was used. Inclusion were all individuals attending eye OPD with complaint of cloudy vision and diagnosed with cataract. Patients suffering from corneal diseases, chronic anterior uveitis intraocular pressure &gt;22 mmHg, high ametropia and Diabetes Mellitus were excluded from the study. SPSS was used to analyze data. Results: Total of 153 patients were selected for this study. Age of the patients was from 40 to 72 years. 60% of patients were between 40-60 years age whereas 40% were above 60 years. Gender differences observed 73 (48%) female and 80(52%) male. On 1st postoperative day Best Corrected Visual acuity (BCVA) was 6/9 in 90 patients, 6/12 in 40 patients and 6/6 in 23 patients. After 1 week follow-up, BCVA was 6/6 in 100 patients, 6/9 in 33 patients and 6/12 in 20 patients. After 1 month, BCVA was recorded as 6/6 in 130 patients, 6/9 in 20 and 6/12 in 3 patients. Conclusion: Visual acuity outcome with foldable lens after Phacoemulsification was found excellent within one month. Implantation of foldable lens with phacoemulsification gives better results in terms of visual acuity.

Visual Outcomes of Cataract Surgery in Patients With Keratoconus Using Toric and Non-toric Lenses

To compare the accuracy and outcomes of different intraocular lens (IOL) power calculation formulas in eyes with keratoconus undergoing cataract surgery with toric and non-toric IOLs. This was a consecutive retrospective case series study including patients from the Cornea Service at the Department of Ophthalmology and Visual Sciences at the University of British Columbia, Vancouver, Canada, from 2000 to 2020. Keratoconus was diagnosed based on corneal topography and clinician opinion. Patients who underwent topography-guided photorefractive keratectomy, intracorneal ring segments implantation, or corneal transplant were excluded. The manifest spherical equivalent, prediction errors, and median absolute errors were calculated. Descriptive statistics were expressed as mean ± standard deviation. There were 160 eyes from 101 patients; 136 eyes received non-toric lenses and 24 eyes received toric lenses. Most patients had mild disease (< 48.00 diopters [D]) when stratified by steep keratometry values. Patients with severe disease (> 53.00 D) were significantly more hyperopic following surgery (P < .05). The Barrett Universal II (0.26 D, inter-quartile range [IQR] = 0.4), Holladay 2 (0.31, IQR = 1.2), and SRK/T (0.42, IQR = 0.86) formulas had the lowest median absolute error. The postoperative prediction error following toric lens insertion was not significantly different than following non-toric lens insertion, and the mean absolute astigmatism was significantly reduced with toric lenses. The Barrett Universal II, Holladay 2, and SRK/T were the most accurate IOL power calculation formulas in patients with keratoconus undergoing cataract surgery. Hyperopic surprise was increased in severe keratoconus. Toric IOLs may be considered in patients with mild keratoconus. [J Refract Surg. 2023;39(5):319-325.].

Complications and visual outcome of cataract surgery in pseudoexfoliation patients

Pseudoexfoliation syndrome is a multifactorial diffuse disease and is diagnosed clinically on slit lamp examination of the eye by the deposition of white, dandruff like material on different structures in anterior segment i:e corneal endothelium, iris, pupillary margin, anterior lens capsule, zonules and trabecular meshwork. The aim of study was to study demographic features, intraoperative and postoperative complications and the visual outcome of cataract surgery in pseudoexfoliation patients. 93 eyes of 93 patients with pseudoexfoliation posted for cataract surgery were included in the study. 79.56% patients were of more than 60 years of age group.43 patients were male and 50 patients were female. 44 patients had pseudoexfoliation syndrome in both eyes and 49 patients had either right eye or left eye involvement. Intraoperative problems encountered were poor pupillary dilatation, capsular bag prolapse, zonular dehiscence, posterior capsular rent, residual cortical matter and positive vitreous pressure. Postoperative complications reported were corneal edema, raised intraocular pressure, significant anterior chamber cell reaction, left cortical matter and exudative membrane. Best corrected visual acuity was more than 6/18 in 67 eyes, between 6/60-6/24 in 16 eyes, and less than 6/60 in 10 eyes. At 1 month, best corrected visual acuity was more than 6/18 in 92 eyes. Cataract surgery in pseudoexfoliation patients is associated with high risk of complications. Use of iris hooks and capsule tension rings where required during the surgery can decrease the intraoperative complications and thus will give better visual outcome.

A Study of Visual and Anatomical Outcomes of Cataract Surgery with Intraoperative Complications in a Teaching Institute

Introduction: To study the incidence, visual outcomes, andanterior segment anatomical outcomes of intraoperativecomplications of cataract surgery.Materials and Methods: This is a prospective observationalhospital-based study conducted in the department ofOphthalmology SRMS IMS, Bareilly, Uttar Pradesh. Inthis study, 70 patients were studied for 1.5 yrs from 1st ofNovember 2019 to 13th of April 2021. All these patients withintraoperative complications were evaluated preoperatively andpost-operatively for vision and anterior segment anatomicaloutcomes.Results: In this study, the commonest intraoperativecomplication was Injury to Iris/ Iridodialysis (52.9%). Basedon pre-operative vision, the patients had 5/60 and 6/36 visionwith 30.0%, each followed by 6/24 (14.3%) and 3/60 (12.9%).Corneal hazy/edema was reduced to 10.0% at one-monthfollow-up from 52.9% on post-operative day one. Also, DMfolds were reduced to 11.4% from 75.7% at day one.Conclusions: In this study, visual outcome of the cataractsurgery was much better, and there is a significant progressionin the visual acuity. Routine monitoring of the visual outcomeof the cataract surgery at each hospital would go in long-wayto enhance both the quantity and quality of the surgery andthus decrease the substantial amount of burden of blindnesson our country.

Outcome of Cataract Surgery in Patients With Retinitis Pigmentosa

To assess the visual outcome of cataract surgery in patients with retinitis pigmentosa (RP). Retrospective, noncomparative clinical study. Preoperative, intraoperative, and postoperative data of patients with RP who were undergoing cataract surgery were collected from several expertise centers across Europe. In total, 295 eyes of 225 patients were included in the study. The mean age at surgery of the first eye was 56.1 ± 17.9 years. Following surgery, best-corrected visual acuity (BCVA) improved significantly from 1.03 to 0.81 logMAR (ie, 20/214 to 20/129 Snellen) in the first treated eye (-0.22 logMAR; 95% CI=-0.31 to -0.13; P < .001) and from 0.80 to 0.56 logMAR (ie, 20/126 to 20/73 Snellen) in the second treated eye (-0.24 logMAR; 95% CI=-0.32 to -0.15; P < .001). Marked BCVA improvements (postoperative change in BCVA of ≥0.3 logMAR) were observed in 87 of 226 patients (39%). Greater odds for marked visual improvements were observed in patients with moderate visual impairment or worse. The most common complications were zonular dialysis (n=15; 5%) and (exacerbation of) cystoid macular edema (n=14; 5%), respectively. Postoperative posterior capsular opacifications were present in 111 of 295 eyes (38%). Significant improvements in BCVA are observed in most patients with RP following cataract surgery. Baseline BCVA is a predictor of visual outcome. Preoperative evaluation should include the assessment of potential zonular insufficiency and the presence of CME, as they are relatively common and may increase the risk of complications.

Visual Outcome after Cataract Surgery in Phacolytic Glaucoma

A prospective study was conducted between March 2021 and February 2022 in the National Institute of Ophthalmology &amp; Hospital, Dhaka, Bangladesh involving 30 patients who underwent cataract surgery for phacolytic glaucoma. The aim of this study was to see the postoperative visual outcome of cataract surgery in patients with phacolytic glaucoma. Visual acuity and development of any postoperative complications were recorded. The mean age of the patients was 60.7 years. Female to male ratio was 1.3:1. Most of the patients (67%) presented after 1 week of symptoms. On the first visit, mean preoperative intraocular pressure (IOP) was 37.5 mm of Hg. Following surgery, 93.33% had an intraocular pressure less than 21 mm Hg at discharge. Visual acuity (VA) as perception of light (PL) was found doubtful in 2 (6.67%), while PL was positive in 17 (56.66%) and HM &lt;3/60 was in 11 (36.66%) preoperatively. At discharge, 73.33% achieved VA 6/24 or better, 5 (16.67%) had less than 6/24 and 3 (10%) less than 3/60. Cataract surgery is effective to restore visual acuity and lowering the intraocular pressure (IOP) in patients with phacolytic glaucoma.&#x0D; CBMJ 2022 July: vol. 11 no. 02 P: 120-124

Intraoperative complications and visual outcomes of cataract surgery in patients with retinal vein occlusion: multicenter database study.

To compare the visual outcomes and the rate of intraoperative complications in eyes with and without retinal vein occlusion (RVO) after phacoemulsification over a period of 15 years in a real-world clinical setting. 8 independent U.K. National Health Service ophthalmology departments. Retrospective, multicenter cohort study. Eyes were classified based on the presence or absence of RVO. For analyzing visual acuity (VA) and the incidence of postoperative cystoid macular edema, eyes with any copathology, combined ocular surgical procedures, and intraoperative complications, or of diabetic patients were excluded. Of 178 856 eyes, 1796 eyes with RVO preoperatively and 177 060 eyes without RVO were allocated to the RVO group and the reference group, respectively. Cataract surgery in eyes with RVO was associated with an improvement in the mean VA of 0.35 logMAR (3 to 4 Snellen lines), and a substantial gain (≥0.30 logMAR units [3 Snellen lines]) was achieved in 55.10% of eyes at 4 to 12 weeks postoperatively. The mean postoperative VA was worse in eyes with RVO compared with that in eyes without RVO at 4 to 12 weeks (logMAR 0.40 vs 0.12 [20/50 vs 20/25]; P < .0001). The proportion of eyes achieving a visual gain of ≥ 0.3 logMAR (3 Snellen lines) was also lower in eyes with RVO (55.10% vs 64.55%; P = .0076). There was no statistically significant difference in posterior capsule rupture rates between the 2 groups (1.73% vs 1.72%; P = .9741). Although postoperative vision improved significantly in eyes with RVO after cataract surgery, this improvement was worse than that achieved by eyes without RVO.

Visual outcome of cataract surgery from a free outreach camp among rural areas of Southern-Kaduna, Nigeria

Objectives: The objectives of the study were to assess the visual outcome of those who underwent free cataract surgery in southern senatorial zone of Kaduna State between September and November 2018, and to identify the causes of poor visual outcome (Visual acuity [VA] &lt;6/60 Post op), suggest ways of improving it and comparing findings of this study with other studies. Materials and Methods: An observational study of consecutive patients with dense cataracts and VA &gt;6/60 who underwent manual small incision cataract surgery (MSICS) with intraocular lens implant (IOL). All patients had posterior chamber intraocular lens (PCIOL) implant after cataract extraction except 6 who had anterior chamber intraocular lens. A total of 116 eyes of 116 patients who met the inclusion criteria were recruited for the study but only 109 were followed up for 8 weeks postoperatively. Their VA pre-operative was noted. Postoperatively, 1st day, 1,and 8 weeks presenting VA were noted while best corrected VA (BCVA) at 1 week and 8 weeks were also noted. Information collected was analyzed with Statistical Package for the Social Sciences version 19. A 95% significant confidence interval was used, that is, P &lt; 0.05. Results: Age range of patients in this study was 40–80 years with a male to female ratio of 1.02:1. All patients presented pre-operatively with poor vision (VA &gt;6/60) but after surgery 27.3%, 42.2%, and 56.6% of patients had good vision (VA 6/6–6/18) on 1st day, 1 week, and 8 weeks post-operatively, respectively, with a BCVA in 75.6% of study population. Complications such as corneal edema, posterior capsule rent, iris tear, cortical remnants, and uncorrected residual refractive errors contributed to poor visual outcome (VA &lt;6/60) 8 weeks after surgery. Conclusion: MSICS was effective in restoring visual loss from cataract. Even though the WHO standard was not achieved in this study, provision of required technology for pre-operative preparation and post-operative follow-up along with improvement in surgical techniques will improve visual outcome after cataract surgery.

Outcome of Cataract Surgery at Secondary Eye Care Facility in Karachi

Purpose: To find out visual outcome of cataract surgery at secondary eye care hospital in Karachi, Pakistan. Study Design: Surgical audit. Place and Duration of study: Al-Baseer Eye Hospital, Karachi from January 2019 to January 2021. Methods: A total of 1540 cataract surgeries were performed. Patients referred from distant eye camps (outside of city limits) were not included. Data collection was done using hospital medical records on patient’s age, sex, pre-operative best-corrected visual acuity (BCVA), surgical complications, visual acuity(VA) at 1st post-operative day, 1, 3 and 4 – 16 post-operative weeks, BCVA at 4 – 16 post-operative weeks and causes of poor visual outcome (BCVA worse than 6/60) at 4 – 16 post–operative weeks. For data analysis, IBM SPSS Statistics 23 was used. Results: Out of 1540 cataract surgeries, 81% (1248) cases underwent phacoemulsification with intraocular lens implantation. Mean age at the time of surgery was 56.24 ± 11.45 years. Males were 54.9% (845) while 45.1% (695) were females. Rate of intraoperative complications was 0.9% (14 cases). Posterior capsule rupture occurred in 0.8% (13 cases), and zonular disinsertion in 0.1% (1 case). At 4 – 16 weeks post-operatively, follow–up rate was 53.8% (828 patients); and the percentages of patients with good, borderline and poor BCVA were 94.7% (784 cases), 2.3% (19 cases), and 3.0% (25 cases) respectively. Conclusion: Good visual outcome can be obtained after cataract surgery at a secondary eye hospital, provided the surgeons and operating room team are trained, well equipped and follow aseptic measures. <br>Postoperative astigmatism plays an important role in the final visual outcome of cataract surgery. In small-incision cataract surgery, proper tunnel construction is of utmost importance for good structural integrity, leading to sutureless self-sealing wounds with lesser astigmatism. We report a case of postoperative high astigmatism following manual small-incision cataract surgery due to large, irregular, superficial, and more anterior corneal entry that lead to poor tunnel valve effect and wound sagging. This resulted in a poor postoperative unaided visual outcome.<br>

Visual outcome of cataract surgery in a tertiary care teaching hospital.

Objectives:To review the visual outcomes after cataract surgery in a tertiary care teaching hospital, Isra University Hyderabad.Methods:This retrospective clinical study was carried out for six months at Isra University Hyderabad from December 2020-May 2021. Total patients included were 982. The data comprised of patients who underwent cataract surgery from Sept 2018 – Aug 2020. Individuals over 40 years were included and who returned for out-patient appointments. Visual acuity before and six weeks after cataract surgery were noted and categorized according to World Health Organization criteria (i-e Good, Moderate and Poor).. Data analysis was done with the help of SPSS version 22.0.Results:Out of 982 patients who underwent Phacoemulsification at Isra University Hospital, the operated eyes were 483 right and 499 left respectively. Meanwhile, 966 had good visual acuity six weeks after the surgery, while 16 had moderate visual acuity noted after six weeks.Conclusion:Clinical audit of cataract surgeries by measuring visual acuity postoperatively is an excellent approach in improving the outcomes and maintaining the good care facilities at a tertiary care hospital.

Long-term Outcomes of Cataract Surgery in Patients with Retinitis Pigmentosa

To investigate the long-term outcomes of cataract surgery in patients with retinitis pigmentosa (RP). Retrospective, observational study. Sixty-four patients with typical RP (22 men, 42 women; average age, 62.8 ± 10.1 years) who underwent cataract surgery at Kyushu University Hospital between May 2007 and October 2015 and were followed up for ≥3 years after the surgery. Differences between presurgery and postsurgery visual function, including best-corrected visual acuity (BCVA) and parameters in the Humphrey field analyzer (HFA) examination using the central 10-2 program, were investigated. The presurgery conditions of the foveal ellipsoid zone (EZ) were classified into 3 grades (grade 1: invisible; grade 2: abnormal; grade 3: normal) based on OCT findings. BCVA, the retinal sensitivity in the HFA 10-2 test. Cataract surgery was performed in 96 eyes, with an average follow-up period of 5.8 ± 2.4 years. The mean presurgery BCVA was 0.64 ± 0.52 logarithm of the minimum angle of resolution (logMAR), and the final postsurgery BCVA was 0.61 ± 0.67 logMAR (P= 0.57). Significant improvement in the postsurgery BCVA was observed only in eyes with preserved foveal EZ (grade 3) (P < 0.01). In 62 eyes of 45 patients who underwent the HFA 10-2 test, the mean values of deviation, macular sensitivity, and foveal sensitivity at the final visit were significantly decreased compared with preoperative values (P < 0.01), whereas those in grade 3 eyes did not change significantly after the surgery (P= 0.13). In the long-term course after cataract surgery in patients with RP, many patients experienced vision loss with progression of the disease. The preoperative finding of preserved foveal EZ was associated with a better visual prognosis, suggesting that EZ evaluation is useful for predicting the long-term visual outcome of cataract surgery in patients with RP.

BACKGROUND Traumatic cataract is one of the common sequelae following ocular injury. It is one of the leading causes of preventable blindness. Ocular trauma leading to traumatic cataract, if managed early gives good visual outcome. Associated ocular structure involvement has much bearing on the management and prognosis of traumatic cataract. We wanted to study the visual outcome of cataract surgery in traumatic cataract. METHODS All patients included in this prospective study were subjected complete ocular examination, intra ocular lens (IOL) power calculation and small incision cataract surgery (SICS) with IOL implantation. Visual outcome was assessed after 1st week, 2 nd week, 6th week and 6 months. Postoperative visual outcome and post-operative complications were assessed. RESULTS Out of 50 cases studied, 56 % of cases were in the age group of less than 20 years. Male: Female ratio was 2.57 : 1. Traumatic cataract was higher in patients of rural background as compared to urban background (64 % vs 32 %). In 58 % of cases, penetrating injury was the type of trauma. 60 % of patients had visual acuity of less than 3/60 at presentation. Corneal injuries (34 %), scleral tear in (22 %) and iris related problems (18 %) were the most common associated findings. All the patients were treated with SICS with IOL implantation. Most common intra-operative complication was posterior capsule rupture seen in 14 % of cases, 34 % of patients had anterior uveitis as early post-operative complication and 24 % of patients developed PCO as late post-operative complication; 68 % of the patients achieved visual acuity of above 6/18. CONCLUSIONS Timely management and involvement of other structures in traumatic cataract determines the visual prognosis of cataract surgery in traumatic cataract. Thus, our study showed that traumatic cataract can be successfully managed surgically with intraocular lens implantation with good visual outcome, unless there is associated corneal opacity or posterior segment involvement. More emphasis should be laid on the preventive measures like mass health education and awareness about risk of ocular trauma, and need to adopt safety measures should be focused, especially during playground, and at workplace. Complications and visual outcomes of cataract surgery in patients with pseudoexfoliation.

To compare outcomes of cataract surgery performed by ophthalmic trainees and consultants in patients associated with pseudoexfoliation (PXF) METHODS: Retrospective review of medical records of all patients with PXF who had undergone cataract surgery from January 2016 to December 2018 at L V Prasad Eye Institute, Hyderabad, India, were analysed RESULTS: A total of 914 eyes were included in the study; surgeries on 501 (54.8%) eyes with PXF were performed by trainees, while 413 (45.2%) were performed by consultants. Posterior capsule rupture (PCR) with vitreous loss (VL) occurred in significantly fewer eyes operated on by consultants (n = 8, 1.9%) than those operated on by trainees (n = 23, 4.5%) (p = 0.002). Eyes that underwent small incision cataract surgery (n = 100, 21.2%) had a significantly greater number of complications than those that underwent phacoemulsification (n = 31, 7.1%) (p = 0.00001). Multivariate logistic regression analysis shows that chances of a complication are higher when operated on by a trainee (OR = 1.59, 95% CI: 1.09-2.34, p = 0.02).The mean logMAR corrected distance visual acuity of patients one month post-surgery was significantly higher for patients in the consultant group (0.15 ± 0.3) than for those in the trainee group (0.2 ± 0.4) (p = 0.003). Patients with PXF operated on by trainees for cataract had a relatively higher risk of developing PCR with VL and had poorer visual outcomes than those operated on by consultants.
