Liver impairment status toward workers exposed to vinyl chloride under different techniques

To analyse potential differences towards liver impairment status on vinyl chloride monomer(VCM) exposed population from technique under acetylene hydrochlorination to the one of ethylene oxychlorination respectively and to explore the possible reasons, which will pave the way for occupational health promotion in terms of hazard reduction. a cross-sectional study was initiated between June and September in 2022 towards 2 groups of VCM exposed population from the facility of acetylene hydrochlorination(n=78) and the one of ethylene oxychlorination(n=69) in a PVC petrochemical complex enterprise(abbreviation of H) in Tianjin City. The demographic information concerning age, gender, messages on occupational history, field investigation were inquired through questionnaire interview. Then, venous blood(4 mL/person) and urine(10-50 mL/person) were collected during the physical exam phase and indices of 8-hydroxy-2 deoxyguanosine(8-OHdG) in blood and thiodiglycolic acid(TDGA) in urine were detected through ELISA and solid phase extraction-ion chromatography respectively. The 2 groups of population were matched well in terms of average age distribution and gender composition ratio, with significant differences on population composition ratio were found on variables of working years, alcohol consumption and daily sleeping duration(P<0.01 or P<0.05). It was found that the average content of TDGA in acetylene hydrochlorination group was(0.81±0.05)mg/L while the content in ethylene oxychlorination group reached to(0.83±0.06)mg/L, noteworthy differences were only found among 6 posts in the acetylene hydrochlorination group and 5 others in the ethylene oxychlorination group after classification for specific posts, however, the average concentration of 8-OHdG in acetylene hydrochlorination group(122(78.3, 168.8) μg/m~3) was different from the one in ethylene oxychlorination group(101.7(79.6, 149.7) μg/m~3)(Z=6.82, P<0.05). Moreover, a series of positive correlations in moderate intensity between 8-OHdG concentration and TDGA content were observed among posts of polymerization cleaners(r=0.53), aggregation operators(r=0.47), maintenance repairers(r=0.45), sampling operators(r=0.41) in acetylene hydrochlorination group(P<0.05) and posts of cracking reactants(r=0.64), DCS operators(r=0.51), oxychlorination operators(r=0.50) and chemical loaders(r=0.44) in ethylene oxychlorination group(P<0.05). Liver function indices such as content on ALT(χ~2=15.41, P<0.01), AST(χ~2=9.95, P<0.01) and ALP(χ~2=3.79, P<0.01) were different in the 2 groups population with statistical significance, then proportions on population composition ratio that exceeded normal ranges of indices on ALT, AST, AST/ALT ratio, ALP and Alb/Glb ratio were higher in acetylene hydrochlorination group than ones in ethylene oxychlorination group with great significance(P<0.05), so as to the abnormalities in liver B altrosonography test between groups(χ~2=17.33, P<0.01). Binary logistic regression model indicated that 8-OHdG concentration in blood that exceed 90 μg/m~3, TDGA content in urine that exceed 0.60 mg/L, working years that were over 10a, alcohol consumption, sleeping duration less than 6 h per day and male workers were potential risky factors for liver impairment(P<0.05). The degree on liver impairment status was higher in acetylene hydrochlorination group than ones in in ethylene oxychlorination group under the same PVC factory, which might be associated with the oxidative stress injury induced from the combination of higher VCM concentration at workplaces, longer cumulative exposure time, longer working years, alcohol consumption habits and sleep shortage caused by shift work patterns.

Improvement of protective facilities in vinyl chloride units on liver injury status of occupational exposed group

To comprehensively evaluate the effectiveness of improvement of protective facilities in a vinyl chloride monomer(VCM) on promotion toward health status of occupational exposed group and safety production at poly-vinyl chloride(PVC) by comparing the liver function indicators and inspection result before and after the improvement, and to analyze the possible influential factors. The information collection concerning facilities improvement in 2013 and 2016, identification toward critical controlling points, data or detection result from occupational on-site survey and physical examination were originated in July 2020, and 227 VCM exposed workers and 179 others without chemical reagents exposure history from production factory with calcium carbide process in Tianjin City were selected as objects. The effectiveness toward improvement of protective facilities in critical controlling points that occupational exposed workers usually involve in were evaluated through comparison toward VCM concentration result, thiodiglycolic acid(TDGA) content in urine, indicators on liver function and biochemistry and liver ultrasound. At the mean time, both binary Logistic regression analysis was used to screen the possible factors that contributed to abnormal symptoms and single dependent variable general linear regression model was used to find out the mutual interaction among influential factors. Prior to improvement on protective facilities, the C_(TWA)of VCM exposed by all 8 positions in G had exceeded 1 to 2 folds of occupational exposure limit(OEL=10 mg/m~3) and the position of synthetic operator and field sampling man were ones exposed to 1, 2-DCE with the range from 50% to 1 OEL, others hazard factors were found to meet relative OELs. Next, the content of TDGA in urine of exposed group was found to correlate strongly to the average concentration of VCM(r=0.79, P<0.05), and result of TDGA content, alanine aminotransferase(ALT), γ-glutamyl transpeptidase(GGT) and abnormal rate toward liver ultrasound test in exposed group were much higher than ones in control group with significant differences(P<0.05 or P<0.001). By contrast, after the improvement, the exposure level toward all identified hazard factors were declined to meet OEL levels with significant differences in TDGA content, and result of ALT, GGT and abnormal rates toward symptoms in liver ultrasound test such as liver calcification with thickened liver echo, peripheral fibrosis of the liver, multiple hepatic cysts were markedly lower than ones before improvement and still were higher than ones in control group(P<0.05 or P<0.001). Further, parameters of gender, length of employment, weekly contact time, VCM exposed level, TDGA content were all important contributing factors to abnormal symptoms in liver ultrasound test before and after improvement on protective facilities(P<0.05 or P<0.001), in which a significant interaction effect between gender and length of employment(F=4.028, P=0.044), weekly contact time and TDGA content(F=2.183, P=0.046) in urine were found in contribution analysis to abnormal symptoms in liver ultrasound test(P<0.05). The improvement measure carried out in VCM facilities by this PVC production factories with calcium carbide process effectively reduced the ambient concentration of hazard factors mainly led by VCM, but workers might still be at risk of liver injury even under VCM exposure at relative lower level, which may referred to factors of long weekly contact time, long length of employment and high fat diet.

Reduced adiponectin:leptin ratio associated with inhalation exposure to vinyl chloride monomer

The hepatic toxicity of vinyl chloride monomer (VCM) has often been reported, but few studies have assessed insulin resistance or adipose tissue dysfunction. We analyzed the chronic health effects of moderate exposure to VCM on factory workers in Taiwan. Data were collected from personal air samples, urine samples, and immunohistochemical (IHC) examinations of 122 recruited voluntary participants. Air samples were analyzed to assess personal levels of exposure to VCM and ethylene dichloride (EDC). Urine samples were collected from each worker before they started and after they finished their daily shift. Urinary thiodiglycolic acid (TDGA) levels were analyzed using high-performance liquid chromatography-tandem mass spectrometry (HPLC/MS/MS). IHC examinations included liver function and serum adipokine level tests for insulin resistance. Consequently, the participants included for the final analysis were 113. After confounders had been adjusted for, the airborne VCM concentration significantly (P = 0.043) correlated with pre-shift urinary TDGA levels (β = 0.194). A multivariate analysis showed a significant (P = 0.013) inverse correlation between the adiponectin:leptin ratio and the airborne VCM concentration (β = −0.283), which means that exposure to VCM might increase the risk of insulin resistance and adiponectin abnormalities. We hypothesized that pre-shift urinary TDGA levels can be used as exposure biomarkers for the exposure of workers to VCM.

Exploring a new method for the biological monitoring of plastic workers exposed to the vinyl chloride monomer

Vinyl chloride monomer (VCM) is widely used in the production of polyvinyl chloride (PVC) plastics. VCM is recognized as a confirmed human and animal carcinogenic compound. Recent studies have reported poor health of plastic workers, even having exposure at concentrations below the permissible limit to VCM. There has not been any study regarding exposed workers to VCM in Iran. Similarly, no information exists as to the biological monitoring of such workers. The main purpose of this study was to conduct a thorough occupational and biological monitoring of Iranian plastic workers exposed to VCM.A total of 100 workers from two plastic manufacturing plants (A and B) in Tehran along with 25 unexposed workers as controls were studied. The personal monitoring of all nonsmoking workers exposed to VCM at two plastic manufacturing plants (A and B) was performed in the morning shift (8 a.m. to 4 p.m.) according to the National Institute For Occupational Safety And Health method no. 1007.Biological monitoring of workers was carried out through collection of exhaled breath of all exposed and control workers in Tedlar bags and with a subsequent analysis using gas chromatography-flame ionization detector.Not only the mean occupational exposure of workers to VCM at plant A was higher than the respective threshold limit value but also the statistical significance was higher than workers at plant B. Similarly, VCM concentration in exhaled breath of workers at plant A was also statistically significantly higher than at plant B. Correlation of occupational exposure of all workers to vinyl chloride with its concentration in exhaled breath was statistically significant.This is the first study on biological monitoring for exposed plastic workers to VCM using exhaled breath. On the basis of the results in this study, a novel method of biological monitoring of plastic workers was proposed.

Genotoxicity in vinyl chloride-exposed workers and its implication for occupational exposure limit

Vinyl chloride monomer (VCM) is a colorless gas under room temperature and has been mostly used to produce polyvinyl chloride (PVC) since the 1970s. It is classified by the International Agency of Research on Cancer (IARC) as a known human carcinogen (Group 1). In this study, genetic damage in VCM workers was evaluated in relation to their occupational cumulative exposure to VCM. Cytokinesis-block micronucleus assay was conducted in 229 VCM workers and 138 controls to detect chromosome damage in peripheral blood lymphocytes. The cumulative exposure dose (CED) of VCM was calculated based on the job type and duration of each worker and the workplace VCM concentration. Dose-response relationships between VCM CED and micronucleus frequency or chromosomal damage were evaluated, and benchmark doses (BMDs) estimated. Dose-response relationships between VCM CED and chromosomal damage were obtained. The 95% lower confidence bound of BMD of VCM CED was 2.86 mg/m(3) -year for both genders combined, leading to an estimated exposure limit of 0.072 mg/m(3) assuming a work life of 40 years. VCM exposure may induce chromosomal damage at occupational exposure levels below the Chinese national occupational health standard. Further research is needed to better understand micronuclei as biomarker of VCM genotoxicity. Better dose-response assessment and BMD estimation are desirable in order to improve the quantification of occupational exposure limits for VCM with respect to non-cancer risk.

Persistence of vinyl chloride in ground water at the Woodlawn landfill Superfund site, northeastern Maryland, USA

Since landfill operations ceased in the early 1980s at the Woodlawn landfill Superfund site in northeastern Maryland, USA, the carcinogen vinyl chloride monomer (VCM) has persisted in the site ground water at concentrations in places greater than 100 ppb. Investigations of VCM concentrations from monitoring well data from 1981 through 1991, which were published in 2001, concluded that concentrations were cyclic and were not dissipating. Data from 1991 to 2000 confirm VCM persistence, showing concentration peaks as late as 1997 in the eastern part of the site. These values are significantly above both drinking water standards (>MCL of 2 ppb) and lifetime excess cancer risk (E-6 risk level) from exposure since birth (>0.024 ppb). Sources for VCM are landfill wastes (PVC-sludge) and effluent from an on-site transfer station. In addition to direct release of VCM from wastes, other chlorinated hydrocarbons, trichloroethene and perchloroethene (TCE and PCE) are also present in the ground water, and these compounds can degrade into VCM. Persistence of VCM concentrations is the result of leachate generation, mostly from multi-source wastes in the vadose zone, with subsequent infiltration into the regional aquifer. Increases in VCM concentrations (new leachate generation) alternated with decreases in VCM concentrations (infiltration without leachate) in a cyclic fashion, during this 20-year period.

Liver fibrosis in asymptomatic polyvinyl chloride workers.

This study was designed to determine whether vinyl chloride monomer (VCM) exposure is associated with liver fibrosis. A total of 347 workers with occupational exposure to VCM were systemically examined using liver ultrasonography and routine liver function tests. Vinyl chloride monomer cumulative dose (ppm-month) was estimated by summing the products of air VCM concentration levels and months of employment. Liver fibrosis was defined in subjects with precirrhosis and cirrhosis of liver diagnosed using ultrasonography. Significantly increased risks of developing liver fibrosis were found in workers who had history of high exposure jobs (odds ratio 5.5, 95% confidence interval 1.7-25.4) when compared with workers who did not have history of high exposure jobs. We concluded that there was an increased risk of developing liver fibrosis in PVC workers who had high exposure to VCM.

Effect of UV-Radiation on the Migration of Vinyl Chloride Monomer from Unplasticized PVC Pipes

The effect of direct exposure to UV-radiation on the migration of vinyl chloride monomer (VCM) from unplasticized polyvinyl chloride (uPVC) pipes was investigated using locally manufactured pipes. Specimens 33-cm long were used throughout the research. The investigation was carried out, at different times of exposure to a UV-radiation. The VCM concentration in the water was evaluated using the gas chromatography (GC)/head-space technique. A VCM concentration of 2.3 µg/L (ppb) was detected after 14 days of exposure to the UV-radiation. The initial VCM concentration in the uPVC pipe was calculated and the diffusion coefficient of VCM was expressed as a function of time.

Modeling and Control of Vinyl Chloride in Drinking Water Distribution Systems

In water distribution systems containing PVC pipe manufactured in the “early era” (prior to 1977), vinyl chloride can leach into drinking water resulting in vinyl chloride concentrations exceeding the 2 μg⋅L−1 maximum contaminant level. Field testing of dead-end segments of water distribution systems consisting of early-era PVC pipe was conducted to examine their initial intrapipe vinyl chloride monomer (VCM) concentrations based on a Fickian-diffusion-based leaching model. The experiments showed a wide range of VCM concentrations within early-era PVC pipe ranging from less than 50 to more than 600 mg⋅kg−1. Exposure Assessment of Vinyl Chloride Monomer in the Workplace Air of Lithuanian Industries

In 1998, for the implementation of occupational cancer prevention, Lithuania adopted the regulations on employees’ protection from contact with vinyl chloride monomer (VCM) in accordance with the European Union Council Directives 78/610/EEC and 91/155/EEC. Studies were carried out in the polymer and building/finishing materials industries where PVC is used. Data on the exposure to VCM were obtained by personal air sampling in the worker’s breathing zone and by analysis using gas chromatography. The average VCM concentration found in the workplaces was 0.48 mg/m3. The highest concentrations of VCM were detected in those workplaces where PVC frames (1.50 mg/m3) and finishing tiles (0.51 mg/m3) were produced. These values indicate that there is no risk of exceeding the technical long-term limit value of VCM (8 mg/m3). VCM was not detected in the process of PVC granulation; hydrogen chloride, formaldehyde and toluene were found there, but the maximum allowable concentrations were not exceeded. In Lithuania, industries where PVC is used belong to the group of workplaces with low VCM exposure levels.

Numerical simulations of the pyrolysis of 1, 2 dichloroethane

Numerical simulations of 1, 2 dichloroethane (EDC) pyrolysis are conducted to understand the process in the production of the vinyl chloride monomer (VCM) and by-products. A chemical kinetic mechanism is developed, with the adopted scheme involving 44 gas-phase species and 260 elementary forward and backward reactions. Detailed sensitivity analyses and the rates of production analysis are performed on each of the reactions and the various species, respectively. The concentrations of EDC, VCM, and HCl predicted by this mechanism are in good agreement with those deduced from experiments of commercial and laboratory scale. The mechanism is found to accurately predict the EDC yield and the production of by-products by varying the ranges of pyrolysis temperature, residence time, and pressure which impact on the pyrolysis of 1, 2 dichloroethane. The influence of reactions related to H atom on the relative sensitivity of EDC becomes important as the residence time increases. The pyrolysis of EDC mainly occurs through C2H4Cl2+Cl=CH2ClCHCl+HCl.

Urinary thiodiglycolic acid levels for vinyl chloride monomer-exposed polyvinyl chloride workers.

Thiodiglycolic acid (TdGA) is the major metabolite of vinyl chloride monomer (VCM) detected in human urine. Although urinary TdGA has been reported to be associated with ambient VCM exposure, the relationship between urinary TdGA and a low level of air VCM is not clear. Questionnaires were administered to 16 polyvinyl chloride manufacturing workers to obtain a detailed history of occupation and lifestyle. For each worker, personal air monitoring for VCM was performed and a time-weighted average for VCM exposure was calculated. The urinary TdGA levels at the end of a work shift, and at the commencement of the next shift, were also assessed for each worker. Urine analysis revealed that TdGA levels at the beginning of the next shift were higher than those at the end of that shift. Workers experiencing a VCM exposure greater than 5 ppm in air revealed a urinary TdGA level significantly greater than those experiencing a VCM exposure of less than 5 ppm (P < 0.05). The best fit of regression for urinary TdGA on air VCM was Y = 1.06 + 0.57X for urine collected at the commencement of the following work shift, where X is the air VCM concentration and Y is the urinary TdGA concentration (r2 = 0.65, P < 0.01). We conclude that the urinary TdGA level is best detected at the commencement of the next shift and that it can be used as an exposure marker for polyvinyl chloride workers when the air VCM level to which they are exposed is greater than 5 ppm.

Effect of solar radiation on the migration of vinyl chloride monomer from unplasticized PVC pipes

The effect of direct exposure to solar radiation on the migration of vinyl chloride monomer (VCM) from unplasticized polyvinyl chloride (uPVC) pipes was investigated using locally manufactured and imported pipes. Specimens 33 cm long were used throughout the research. The investigation was carried out, at different times of exposure, by exposing one part of the specimens to the direct sunlight, while keeping the other part in the shade. The VCM concentration in the water was evaluated using the gas chromatography (GC)/head-space technique. A VCM concentration of 2.5 μg/l was detected after 30 days of exposure in direct sunlight. The original initial VCM concentration in the uPVC was predicted, and the diffusion rate of VCM from uPVC pipes was expressed as a function of time.

Vinyl chloride in rural water from pre‐1977 applications of PVC pipe

AbstractIn the early 1990s, vinyl chloride monomer (VCM) was found in a rural water system that included poly(vinyl chloride) (PVC) pipe manufactured prior to 1977. Because VCM, considered a human carcinogen, was not regulated in pipe until 1977, some pre‐1977 pipes could contain it at significant levels. Manufacturing processes adopted in 1975 and in place by 1977 reduced VCM levels by as much as a factor of 1000. Pipe manufactured by using the newer resin technology does not show detectable VCM migration under any reasonable installation circumstances. Migration of VCM is a function of pipe size, VCM concentration at time of installation, water temperature, and flow rate. History, scientific investigations, and conclusions drawn from field investigations are summarized. Tools for the determination of possible candidate systems, potential noncompliance, and remediation, both short‐term and long‐term, are provided. A spreadsheet‐based computer model of diffusion is described.

Liver Angiosarcoma and Hemangiopericytoma after Occupational Exposure to Vinyl Chloride Monomer

Various malignant tumors of the liver, especially liver angiosarcoma, have been described after occupational exposure to vinyl chloride monomer. We present the case records and pathologic findings of two plastic industry workers who had been exposed to high concentrations of vinyl chloride. These workers developed hepatic neoplasms, angiosarcoma, or hemangiopericytoma. We discuss the histogenesis of these tumors; the common vascular origin and the mutual transformation of these two tumors suggest that the hemangiopericytoma may also have developed during occupational exposure to high concentrations of vinyl chloride monomer.

Effect of Water Quality Parameters on the Migration of Vinyl Chloride Monomer from Unplasticized PVC Pipes

The migration of vinyl chloride monomer (VCM) from unplasticizedpolyvinyl chloride (uPVC) pipes was investigated using locallymanufactured pipes. Specimens of 33 cm long were used throughoutthe research. The investigation was carried out under differentconditions of water temperature, pH and total dissolved solidsconcentration and at different durations of exposure. The VCMconcentration in the water was evaluated using the gaschromotography (GC)/head-space technique. A VCM concentration ofmore than 2.5 ppb was detected after 30 days of exposure at45 °C. The initial VCM concentration in the uPVC pipewas predicted using equations derived from Fick's first law ofdiffusion. Water tenperature did not affect the migration ofVCM, unless it was raised to high values (i.e. 45 °C).Total dissolved solids (TDS) and pH of water were found toaffect the release of VCM from uPVC pipes. Diffusion rate of VCMwas predicted as a function of pH or TDS values.

Abnormal Liver Function in Workers Exposed to Low Levels of Ethylene Dichloride and Vinyl Chloride Monomer

We investigated whether exposure to ethylene dichloride (EDC) and vinyl chloride monomer (VCM) resulted in increased risk of liver damage. Epidemiological information, including occupational, medical, smoking, and drinking history, was obtained by interview from 251 male workers. Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), and gamma-glutamyltransferase (GGT) were used as indicators of liver damage. Exposure to moderate or low levels of ECD and VCM resulted in a higher risk of developing abnormal ALT levels than did exposure to lower levels of the chemicals. Results were similar for AST. GGT was not associated with EDC or VCM exposure. Combined exposure to EDC and VCM showed a dose-response relationship in association with abnormal ALT levels. We concluded that relatively low concentrations of VCM and EDC cause liver damage.

Assessment of vinyl chloride-induced DNA damage in lymphocytes of plastic industry workers using a single-cell gel electrophoresis technique

DNA damage and the formation of stable carcinogen–DNA adducts are considered critical events in the initiation of the carcinogenic process. This study was carried out to assess whether exposure of plastics industry workers to the vinyl chloride monomer (VCM) for different periods of time would cause DNA damage, using the single-cell gel electrophoresis (SCGE) technique. Levels of DNA damage was assessed by both extent of DNA migration and numbers of DNA damaged spots in the peripheral blood lymphocytes from 32 plastics workers with different periods of exposure to VCM; they were evaluated by comparison with a group of non-exposed individuals. It was found that plastics workers who were exposed to VCM for different periods of time showed significantly increased levels of DNA damage compared with the non-exposed subjects. There was a significant correlation between the severity of DNA damage and duration of exposure. However, no significant correlation was found between the age of all subjects and DNA damage. Concentrations of VCM in the air inside the factory were found to be significantly higher than values in non-exposed areas, despite being lower than the threshold limit value (TLV). Our results encourage the application of SCGE as a sensitive, simple, fast and useful technique in the regular health screening of workers occupationally exposed to VCM (even at concentrations below the TLV) to assess the possibility of any DNA damage.

Vinyl chloride monomer (VCM) is hepatotoxic as well as carcinogenic in humans. There are reports that exposure to VCM seems to induce abnormal liver function, liver fibrosis, cirrhosis, and angiosarcoma of the liver. In vivo, VCM is metabolized by cytochrome P450 2E1 (CYP2E1) to form chloroethylene oxide (CEO) and chloroactetaldehyde (CAA), both electrophilic metabolites, which may either cause cell damage or are further metabolized and detoxified by glutathione S-transferases (GSTs). This study investigated whether or not the genotypes, CYP2E1, glutathione S-transferase θ (GST T1) and μ (GST M1), correlated the abnormal liver function in vinyl chloride exposed workers. We enrolled 251 workers from 5 polyvinyl chloride plants for this study. The workers who were exposed to VCM were classified into two, high and low exposure groups and the degree of exposure was determined based on their job titles and airborne VCM concentration. The enzyme activities of serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were used as the parameters of liver function. The genotypes, CYP2E1, GST T1 and GST M1, were determined by performing PCR and RFLP on peripheral white blood cell DNA, Other potential risk factors were also ascertained and the confounding effect was adjusted accordingly. Stratified analyses were used to explore the correlation between the alteration of liver function and the genotypes, CYP2E1, GST T1 and GST M1, among the workers exposed to different levels of VCM. The results obtained showed: (1)at low VCM exposure, the odds ratio (OR) of GST T1 non-null genotype on abnormal ALT was 3.8 (95% CI=1.2-14.5) but the CYP2E1 genotype was not associated with abnormal ALT; (2)at high VCM exposure, on the other band, a c2c2 CYP2E1 genotype was associated with increased OR on abnormal ALT (OR=4.8, 95% CI=0.6-34.8) but the GST T1 non-null genotype was not associated with abnormal ALT (OR=0.9, 95% CI=0.2-3,8); and (3)multiple logistic regression also showed strong interactions of the VCM exposure to CYP2E1 as well as to the GST T1 genotype. Thus, These observations suggest that the two genotypes, CYP2E1 and GST T1, may play important roles in the biotransformation of VCM, whose effect leads to liver damage.
