Use of Vicrуl Rapide in Hand and Upper Extremity Surgery

Quick-Absorbable Suture Vicryl Rapide was used at surgical treatment of 112 patients with various injuries and diseases of the hand and upper extremity for wounds closure. Wounds were closed by intradermal (34 patients) and external (78) sutures. Time of ligatures resorption and loss of strength as well as local response of tissues to suture material were analyzed in post-operative period. Complications that developed in 2 (1.8%) cases were not specific to Vicryl Rapide. External knots of intradermal suture were easily removed in all patients on day 14 after surgery. In 72 patients with external sutures complete loss of ligatures strength was noted during the period from 13th to 21st postoperative day (mean in 15.8±2.3 days). No local tissue reaction was observed. In external sutures the time of ligatures resorption and loss of strength was slightly higher than indicated in instruction. Use of Vicrуl Rapide in hand and upper extremity surgery is effective and safe.

Performance Assessment of Suture Type, Water Temperature, and Surgeon Skill in Juvenile Chinook Salmon Surgically Implanted with Acoustic Transmitters

AbstractThis study assessed performance of seven suture types in subyearling Chinook salmon Oncorhynchus tshawytscha implanted with acoustic microtransmitters and held at two water temperatures (12°C and 17°C). Nonabsorbable (Ethilon) and absorbable (Monocryl) monofilament sutures and nonabsorbable (Nurolon and silk) and absorbable (Vicryl, Vicryl Plus, and Vicryl Rapide) braided sutures were used to close incisions in Chinook salmon. When differences existed among suture types, tag and suture retention were generally highest for monofilament sutures. Wound inflammation and ulceration were generally lower for Ethilon and Monocryl than for most of the braided sutures. In this study, Nurolon (braided) often resulted in low wound inflammation and ulceration, although suture retention was poor. Generally, fish held in 12°C water had more desirable postsurgery healing characteristics (i.e., higher tag and suture retention; lower incision openness, wound inflammation, and ulceration) at 7 and 14 d postsurgery than fish held in 17°C water. On days 34 and 63, tag retention remained high among fish in 12°C water, while suture retention decreased dramatically in both water temperatures. We found a significant effect of surgeon on tag and suture retention, wound inflammation and ulceration, and incision openness. Surgeons in this study were initially thought to have similar surgical proficiency based on their extensive previous experience. However, surgeons who had received feedback on their previous surgical technique performed better in this study. Results indicate that surgical training (i.e., feedback) and perhaps aptitude, rather than surgeon experience alone, may be as important as suture type in influencing the retention of sutures and tags. The overall results support the conclusion that Monocryl is the best suture material for closing incisions created during surgical implantation of acoustic microtransmitters in subyearling Chinook salmon. Future research should include testing different suturing patterns and knotting techniques as well as the number of knots required for different incision lengths.

Cleft lip repair without suture removal

The disadvantages of using non-absorbable sutures in cleft lip repair include a need for additional dressings, return to the ward for removal of the sutures under sedation or general anaesthetic and the problem of distressing the child and potentially disrupting the repair. Modern medical adhesives represent an alternative adjunctive technique for skin closure and their use was adopted by this unit in 2005. A few 'key' interrupted sutures of 7/0 Vicryl Rapide followed by layers of a cyanoacrylate adhesive, Dermabond, were used instead of more traditional methods. An audit of the results of cleft lip repairs from this period of change was conducted. Subjective and objective data were collected and are presented to justify the continued use of this technique in the Newcastle Cleft Lip and Palate Unit.

Role of absorbable suture material (vicryl rapide 5-0) for skin closure in infants undergoing cheiloplasty

Aims and objectives: (1) To study the role of absorbable suture material in skin closure in cleft lip surgery; (2) to study wound breakdown, and/or infection when rapide vicryl is used; (3) to study quantity and quality of the scar formation when rapide vicryl is used. Material: The purpose of the study is to analyse the role of the suture material rapid vicryl in skin closure in young children of age 3 months–3 years who underwent surgical repair of cleft lip. Methods: Modified Millard's rotation advancement flap technique under general anaesthesia, preoperative, intraoperative, immediate postoperative and 6 months postop photographs were taken, follow-up i week postop and 6 months postop was done.

The "unsuture" technique originally reported with the use of fast-absorbing gut for the placement of full-thickness skin grafts has provided years of successful full-thickness graft placement without the need for suture removal. The objective was to explore another option for successful graft placement and survival using irradiated polyglactin 910 (Vicryl Rapide, Ethicon Inc, Somerville, NJ, USA), with its longer tensile strength of 7 to 10 days. Irradiated polyglactin 910 was used to suture the edges and place basting sutures in full-thickness skin grafts. In our experience, we have found that the use of irradiated polyglactin 910 for the placement of full-thickness skin grafts provides an alternative to the "unsuture" technique with fast-absorbing gut. It provides excellent graft survival, easy workability, low inflammation, and good long-term cosmesis, without the need for suture removal. Irradiated polyglactin 910 provides another option for the placement of full-thickness skin grafts without the need for suture removal.

Vicryl Rapide® Versus Vicryl® Suture in Skin Closure of the Hand in Children: A Randomized Prospective Study

This randomized prospective clinical study compared the use of two absorbable sutures, Vicryl Rapide versus Vicryl, in elective paediatric hand surgery. Sixty children were randomly allocated into two groups (30 in each group) and all were seen and assessed at 3 and 6 weeks for wound complications and suture reactions. Five problems occurred in the Vicryl treatment group and none in the Vicryl Rapide group (P=0.03). All the problems were related to the delayed absorption of the Vicryl suture material and hence the prolonged presence of the suture in the hands of these children. It is concluded that Vicryl Rapide sutures are more suitable than Vicryl ones in paediatric hand surgery.

Comparison of Standard Eyelid Margin Closure Using Silk with a Modified Repair Using 7/0 Vicryl and a Buried Knot

Aims: To assess the efficacy and complications of eyelid margin closure with 7/0 Vicryl or Vicryl Rapide suture with a buried knot at the grey line and to compare this with standard closure using silk with an external knot. Methods: Analysis of a prospective, non-comparative, interventional case series using 7/0 Vicryl or Vicryl Rapide with a buried grey-line knot (modified lid margin closure). Detailed assessment during the healing period was directed to (1) the lid margin profile, (2) the grey line suture and (3) the tissue reaction. Patients were seen for assessment within two days following surgery, at 14 days and at two months. A retrospective, comparative series of silk with an external knot (standard lid margin closure) and 7/0 Vicryl or Vicryl Rapide (modified lid margin closure) was also studied. An analysis was made of the hospital records of patients who had undergone direct lid margin closure with 7/0 Vicryl, Vicryl Rapide or silk. Short- and long-term complications were noted. Results: The prospective series included 24 patients whose lid margin closure had been performed with the modified technique with 7/0 Vicryl or 7/0 Vicryl Rapide. All lids healed well and there was no difference between these two sutures. The retrospective series included 123 patients. Seventy-one patients had the modified technique using 7/0 Vicryl or Vicryl Rapide. Fifty-two patients had lid margin closure with the standard technique using 6/0 silk. All lids healed well. In summary, in both the prospective and retrospective groups, no significant complications were encountered and the healing was excellent. Conclusions: 7/0 Vicryl and Vicryl Rapide with a buried knot at the grey line allows excellent lid margin healing comparable with standard closure using silk. Vicryl sutures do not need to be removed.

Use of 7/0 Vicryl (coated polyglactin 910) and 7/0 Vicryl-rapide (irradiated polyglactin 910) in skin closure in ophthalmic plastic surgery

AIMS : To assess the safety and efficacy of 7/0 Vicryl and 7/0 Vicryl rapide sutures for skin closure, and without suture removal, in ophthalmic plastic surgery. METHOD : A prospective, comparative study of 40 consecutive procedures in 32 patients. Tissue reactions to the sutures, completeness of wound healing, and number of sutures remaining were observed at first dressing, 2 weeks and 2 months. RESULTS : Wound healing was excellent at 2 weeks in all but one Vicryl rapide patient whose slight wound gape was not attributable to the suture material used. No inflammatory tissue reaction attributable to the suture was seen. No visible suture marks were observed at 2 months. Transient crusting was observed around the sutures in 9/20 (45%) and 10/20 (50%) patients with Vicryl and Vicryl rapide, respectively. Only one suture out of 144 was removed. CONCLUSION : 7-0 Vicryl and 7-0 Vicryl rapide are safe and effective sutures for skin closure in ophthalmic plastic surgery.

The transnasal advancement flap: A technique for medial canthal area defects

AIM . To illustrate an alternative technique for reconstructing defects in the medial canthal area following tumour removal. MATERIALS AND METHODS . Eight consecutive patients who underwent Mohs¹ micrographic surgery for removal of a basal cell carcinoma in the medial canthal area were prospectively recruited. An experienced dermatologist trained in Mohs¹ micrographic surgery removed the tumour, and subsequently an oculoplastic surgeon reconstructed the defect. An incision is made from the superior end of the defect superomedially towards the midline of the dorsum of the nose following one of the natural lines of the skin (spec name). The incision runs in arcuate fashion just medial to the eyebrow and then reaches the dorsum of the nose and extends on the contralateral aspect of the nose towards the medial canthal area. The skin is then undermined to free up the newly formed flap. The flap is advanced towards the lesion and fixed intra-dermally with Vicryl Rapide® sutures to ensure anchoring into the concavity of the medial canthal region. The skin is then sutured with prolene sutures. RESULTS . The defects presented were oval or round in shape with diameters ranging from 0.5 to 2 cm. In all cases the transnasal flap covered the entire area and the defect was successfully reconstructed. Mean follow-up is 10.8 months (±4.5 SD) (range 4–17 months); all patients had an excellent cosmetic result and were subjectively satisfied. CONCLUSIONS . The transnasal advancement flap seems to have some advantages over the usual reconstruction techniques in use to repair medium defects in the medial canthal area. These are the avoidance of vertical scars in the glabellar area, eyebrow hair is not transposed into the medial canthal area, the skin match is excellent, both in colour and thickness, and surgical scars are hidden within the natural lines of the nasal bridge. We advocate the use of this technique for all minor to medium skin defects.

Rate of loss of irradiated polyglactin 910 (Vicryl Rapide) from the mouth: a prospective study

We aimed to find out how long irradiated polyglactin 910 (Vicryl Rapide) remained in the oral tissues and to investigate the effect of chlorhexidine mouthwash on absorption time and bacterial growth on sutures. Twenty-one volunteers each had two Vicryl Rapide sutures placed intraorally under local anaesthesia. Ten of these subjects used chlorhexidine mouthwash twice daily. After 7 days, one suture was removed to be cultured for aerobic and anaerobic bacteria. The other was monitored for time from insertion to loss. The median survival value was 3 days (range 1–13). The median number of days to suture loss was less in the chlorhexidine group (3, range 1–11) than in the control group (4, range 1–13) but this was not significant. There were no significant differences in aerobic and anaerobic counts between the two groups. Vicryl Rapide may be useful for intraoral use.

Slowly and rapidly absorbable sutures and their influence on scar width

In order to compare the possible influence of two absorbable suture materials on the formation of scar tissue, 26 women who had undergone reduction mammaplasty were the subjects of a randomized double blind clinical trial. In 26 women, who underwent a reduction mammaplasty, a prospective randomized double blind clinical trial was undertaken to compare the possible influence of two absorbable suture materials on the formation of scar tissue. The sutures under examination were Polyglecaprone 25 (Monocryl, Ethicon; Johnson & Johnson, Amersfoort, The Netherlands) and Polyglactine 910 (Vicryl Rapide, Ethicon; Johnson & Johnson, Amers-foort, The Netherlands). All scars were examined by one investigator after periods of 2 weeks, 3 months, and 1 year, to judge tissue reaction, redness and scar width. The monofilament Monocryl, which is absorbed more slowly and keeps its tensile strength longer than Vicryl Rapide, gave less tissue reaction initially and after periods of 3 months and 1 year produced significantly narrower scars than Vicryl Rapide.

The Influence of the Interaction of Sutures with the Mucosa on Tumour Formation at Colonic Anastomoses in Rats

In order to explore the respective role of wound healing and suture material on metachronous carcinogenesis at colonic lines in rats, tumour yield was studied after the administration of a chemical carcinogen (azoxymethane) at the moment that in one group of rats most of the suture material was still present and crypt cell proliferation elevated, while in another group, no more suture material was present and crypt cell proliferation rate (CCPR) normalized at the anastomotic site. Azoxymethane (15 mg/kg/week, s.c., during 6 weeks) was administered in male Sprague-Dawley rats (n = 105) 8 weeks after the creation of an anastomosis in the ascending and descending colon with either stainless steel sutures (group A, n = 30) or fast-absorption Vicryl (Vicryl Rapide, group B, n = 30). A control group (group C, n = 30) underwent a sham laparotomy before the administration of azoxymethane, while the animals of a fourth group (group D, n = 15) were not operated upon and received no azoxymethane. Twenty-six weeks after the first injection of azoxymethane there was no significant difference in the total colorectal tumour yield in the three operated groups (A, B, C), but a significantly greater proportion of anastomotic tumours (28/68 vs. 13/88, p < 0.01) and more anastomotic tumours per rat (28/23 vs. 13/28, p = 0.01) as well as more rats with anastomotic tumours (16/23 vs. 11/28, p = 0.04) were found in the steel-sutured group (A), compared with the control group (C).(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of nonabsorbable and rapidly absorbable suture material on the cytokinetics of crypt cells in colonic anastomoses in the rat.

The effect of non-absorbable and rapidly absorbable suture material on the cytokinetics of crypt cells was studied in right and left colonic anastomoses in the rat by using stathmokinetic methods. Forty-five male Sprague-Dawley rats were allocated to three groups. In group A (n = 15), colonic anastomoses were performed in the right and left colon using nonabsorbable, stainless steel sutures. Groups B (n = 15) animals had identically located anastomoses but a rapidly absorbable suture material (Vicryl Rapide) was used. Animals of group C (n = 15) acted as controls. Eight weeks postoperatively, the crypt cell production rate (CCPR) was determined after measuring the slope of the metaphase accumulation line. At this time, no suture material was detected in any of group B (Vicryl Rapide) animals, while in group A rats (stainless steel), 79% of the animals had suture material in place. A steel-sutured anastomosis significantly increased the CCPR in the right colon from 6.2 to 10.6 cells/crypt/h and in the left colon from 5.6 to 9.8 cells/crypt/h compared with controls (p less than 0.05). An anastomosis performed with Vicryl Rapide, whether in the proximal or descending colon, did not influence the CCPR when compared with the control group (right 6.4 vs. 6.2 cells/crypt/h; left 4.2 vs. 5.6 cells/crypt/h). The CCPR at steel-sutured anastomoses was significantly higher than the CCPR at the Vicryl Rapide anastomotic site both in the right (10.6 vs. 6.4 cells/crypt/h) as well as in the left colonic sites (9.8 vs. 4.2 cells/crypt/h) (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)
