Introduction of loop sutures in microsurgical telescoping anastomosis

We have designed a loop suture technique for use in microsurgical telescoping (sleeve) anastomosis to prevent problems from the free floating inserted vessel stump. The patency rate of rat femoral arteries repaired with this technique was 95% and that of interpositional vein grafts, in which both the proximal and distal anastomoses were completed with the same sleeve technique, was 81.0%.

Cranial Base Hemorrhage: A Technique for Controlling the Uncontrollable

In the management of trauma, control of life-threatening hemorrhage must be accomplished expeditiously if morbidity is to be decreased and mortality prevented. Hemorrhage from the inaccessible cranial base, especially when transected vessel stumps are not available for direct control, can be the most demanding, life-threatening, surgical emergency. We present our experience in utilizing an autologous bone grafting technique to successfully control severe hemorrhage from the base of the skull. In our review of the literature to date, we have been unable to ascertain whether this modality has been previously used.

The Omental Sandwich Reconstruction for a Full-Thickness Cheek Defect

We present a case in which a large full-thickness cheek defect was successfully repaired in one stage by the "omental sandwich technique." We transferred the omentum as a free flap, anastomosing the middle omental vessels with the end stumps of the gastroepiploic vessels after they had been brought to the neck through a subcutaneous tunnel. Skin grafts were applied then on both sides of the revascularized omentum.
