EFFECT OF LOW LEVEL LASER OF 650NM ON VIALBILITY OF PREVOTELLA INTERMEDIAWITH AND WITHOUT PHOTOSENSITISERS: AN IN VITRO STUDY

Background: Prevotella intermedia is commonly associated with various forms of periodontal disease. Conventional treatment involves a combination of mechanical therapy and systemic antibiotics administration, which comes with its own set of challenges and disadvantages. An alternative treatment modality is low-level laser therapy with and without photosensitizers. Purpose: This study aimed to evaluate the effectiveness of low-level laser therapy with and without the use of photosensitizers, specifically Methylene blue and Toluidine blue, on the viability of Prevotella intermedia. Methods: The study utilized in vitro experiments to assess the impact of low-level laser therapy alone and in combination with photosensitizers on the viability of Prevotella intermedia. Results: The findings indicate that Methylene blue, when combined with low-level laser therapy, exhibits bactericidal effects similar to or greater than low-level laser therapy alone on Prevotella intermedia. Additionally, the use of Toluidine blue as a photosensitizer in combination with low-level laser therapy also led to a reduction in the viability of Prevotella intermedia, although it was not as effective as Methylene blue. Conclusion: Low-level laser therapy, particularly when combined with Methylene blue as a photosensitizer, shows promise as a potential treatment modality for controlling Prevotella intermedia in periodontal diseases.

Significance of Tissue Inhibitors of Matrix Metalloproteinases Expression in Pathogenesis and Differential Diagnosis of Periodontal Pathology

Background. The balance of metalloproteinases (MMPs) and their tissue inhibitors (TIMPs) is crucial for the manifestation and progression of periodontal diseases and is one of the promising areas of scientific research in terms of developing methods for selective inhibition of MMPs. Aims to determine the significance of TIMP1 and TIMP2 expression parameters in gingival biopsies to identify the type of periodontitis at the stage of disease manifestation. Methods. A prospective study with parallel inclusion of patients with different forms of periodontitis in the compared groups was performed. The object of this study were patients with various forms of periodontitis. Pathological examination with morphometric analysis of TIMPs expression and subsequent statistical data analysis was performed using AperioImageScope v12.4.0.5043, Statistica 10.0, MedCalc19.6. Results. The study included 67 patients with aggressive (AgP, Grade C, n = 19), chronic simplex (CSP, Grade В, n = 10) and chronic complex (CCP, in the presence of occlusal trauma, Grade В, n = 38) periodontitis and 15 conditionally healthy patients (control group). The expression of TIMP1 and TIMP2 was detected with variable intensity both in the epithelium and in the stroma of the gingiva, and was significantly higher in periodontitis groups compared with the control group (MeTIMP1/2 = 32%/70%). Parameters of TIMP1 and TIMP2 expression had no significant differences in the groups with AgP (MeTIMP1/2 = 84%/98%) and CCP (MeTIMP1/2 = 83%/94%), except for higher parameters of the positivity of epithelial expression of TIMP2 (U = 61 372; p 0.05) and lower levels of intensity of its expression in AgP (MeAgP/CCP=180/171; U = 56 491; p 0.001). Correlation analysis revealed an inverse relationship of TIMPs expression parameters with those MMPs, including those most significant for the development of AgP MMP1 ( = 0.40), MMP8 ( = 0.34), and MMP14 ( = 0.24). ROC analysis established acceptable informativeness of all studied parameters of total expression and positivity of epithelial expression of TIMP1, as well as positivity of total expression and intensity of epithelial expression of TIMP2 to distinguish between aggressive and chronic forms of periodontitis at the stage of disease manifestation. Conclusion. Our results show an increase in the expression of TIMP1 and TIMP2 in various forms of periodontitis and an inverse relationship with the MMPs inhibited by them. This complements both the fundamental knowledge of the development and progression of periodontal pathology and may have applied significance in terms of using the studied TIMP1 and TIMP2 expression criteria to establish the aggressive course of periodontitis already at the stage of disease manifestation, which will allow to individualize treatment, prevent or slow down tooth loss in order to preserve and/or improve the quality of life of this patient group.

Total, free, and bioavailable 25-hydroxyvitamin D levels in patients with periodontitis (stage III): a case-control study.

The aim of this study was to investigate the relationship between total, free and bioavailable 25-hydroxyvitamin D levels, and periodontitis. Thirty-nine patients with periodontitis diagnosis and 44 healthy participants were included in this study. 25-Hydroxyvitamin D, vitamin D-binding protein, procalcitonin, and albumin levels were measured from the serum samples obtained. Free and bioavailable 25-hydroxyvitamin D levels were calculated. Total, free, and bioavailable 25-hydroxyvitamin D levels were compared between the healthy control and periodontitis groups. The association of total 25 (OH)D levels with periodontitis was tested through univariate analysis and multivariate logistic regression. Total 25-hydroxyvitamin D, bioavailable 25-hydroxyvitamin D, and free 25-hydroxyvitamin D levels were significantly lower in the periodontitis group than in the healthy control group (p < 0.001). In the logistic regression analysis, a significant correlation was observed between total 25-hydroxyvitamin D and periodontitis (p<0.001). Our study shows that there is a relationship between total, free and bioavailable 25-hydroxyvitamin D levels and periodontitis. In addition, it has been shown that the determination of free and bioavailable 25-hydroxyvitamin D levels does not provide additional advantagesin terms of the relationship between periodontitis and vitamin D. In this case-control study, we examined the relationship between total, free and bioavailable 25-hydroxyvitamin D and periodontitis. We found lower total, free, and bioavailable 25-hydroxyvitamin D levels in periodontitis patients. It was also found that vitamin D-binding protein may be an important biomarker in the identification of various forms of periodontal disease. ClinicalTrials.gov Identifier NCT05343273.

Role of bone morphogenetic proteins in periodontal tissue engineering: Relatively unexplored horizon

Tissue engineering aims to reconstruct the natural target tissue by a combination of three key elements stem/progenitor cells (that will create the new tissue), signaling molecules (that instruct the cells to form the desired tissue) scaffold/extracellular matrix (to hold the cells). Regeneration of the periodontal tissues following destructive episodes of various forms of periodontitis is a formidable challenge to periodontologists. Bone morphogenic proteins have been considered as the most potent growth factors that can promote the bone regeneration. This review will emphasize on the unique nature of the tissue engineered bone morphogenic proteins molecules regarding their structure, classification, signaling mechanism, etc. which will further help in understanding their role and potential advances necessary to facilitate the process of regeneration in the field of periodontics.

Assessment of correlation between presence of red complex bacterias and periodontitis

Context: Oral cavity offers diverse habitats to various species of different microorganisms; some of them under certain conditions cause loss of connective tissue and bone ultimately leading to periodontitis. Once such complex of bacteria often referred to as red complex bacteria consist of Porphyromonas gingivalis, Treponema denticola and Tannerella forsythia have been regarded as the one associated with severe form of periodontitis. With increasing advances in the field of medicine N-benzoyl-DL-arginine-2-naphthylamide (BANA) test, a modern chairside paraclinical method has been designed to detect the presence of one or more such bacteria. Aim: The aim of the study was to detect the presence of red complex bacteria in patients suffering from various forms of periodontitis using BANA test kit. Aims: The aim of this study was to assess the correlation between the presence of red complex bacteria and the severity of periodontal disease. Settings and Design: A total of 150 patients suffering from various forms of periodontitis were selected for the study. Subjects and Methods: These were divided into three groups – Group I: mild periodontitis, Group II: moderate periodontitis, and Group III: severe periodontitis. Microbial parameters were recorded using BANA test. Statistical Analysis Used: Mann–Whitney test and Pearson's Chi-square test were used for statistical analysis. Results: Group III had significantly greater percentage of BANA-positive sites, followed by Group II and Group I. Conclusions: There is a positive correlation between the presence of red complex bacteria and the severity of periodontitis.

Prevalence of different forms of periodontitis in patients visiting Government Dental College and Hospital, Hyderabad, since last decade: A retrospective study.

Context:Periodontitis is a highly prevalent oral disease affecting a wide range of people across all age groups and genders. It is the major cause of loss of teeth among all other oral diseases affecting the people.Aims:The current study aimed to evaluate the prevalence of different forms of periodontitis in patients visiting Government Dental College and Hospital, Hyderabad, over a period of 10 years.Settings and Design:Retrospective study.Materials and Methods:This retrospective analysis was put into execution to evaluate the prevalence of periodontal diseases in 74,414 patients referred to the Department of Periodontics in Government Dental College and Hospital, Hyderabad, from January 2007 to December 2016.Statistical Analysis Used:Descriptive analysis, Chi-square test.Results:Out of the 74,414 patients who were screened, 38,898 were male and 35,516 were female. A total of 67,236 patients (90.35%) had chronic periodontitis and 7178 (9.64%) had aggressive periodontitis. The most commonly found type was chronic localized periodontitis.Conclusion:The results of this study have helped to ascertain the various forms of periodontitis in the population of Hyderabad. This can serve as a basis for future studies with the idea of finding ways to ameliorate oral health in this population.

Prevalence of Periodontal Diseases among Patients Attending the Outpatient Department at the College of Dentistry, King Khalid University, Abha, Saudi Arabia

Aims: A retrospective study was performed involving 2739 patients who were attending the College of Dentistry outpatient clinic department of King Khalid University (KKU), Abha, Kingdom of Saudi Arabia (KSA) during the period of 1st January to 31st December, 2009. Methods: Random data were collected from the patients record files ofthe College of Dentistry who attended from 1st January to 30th December, 2009. Results: Among 2739 patients mean age 32±13 (range 11~82) years, males 86.5% and females 13.5%, were found sufferers of different types of periodontal diseases i.e., gingivitis and periodontitis. Gingivitis cases were found 1722(63.2%), in which mild gingivitis was 49.8%, moderate gingivitis 45.8%, severe gingivitis 1.97%, acute gingivitis 0.52% and puberty gingivitis 1.80%. Total number of patients affected with various forms of periodontitis was 1009 (36.8%), in which mild periodontitis was 57.4%, moderate periodontitis 36.6%, severe periodontitis 4.95% and aggressive periodontitis 0.89%. The prevalence and severity of gingivitis was found increased with age to a peak in the 21~30 years age group, whereas, periodontitis increased significantly (p&lt;0.001) in subjects above the age of 40 years. Conclusion: Using the research results, a greater effort can be made in providing periodontal health to the population of at or around the city of Abha, KSA. Periodontitis and Preterm Low Birth Weight: Is there any Association?

The aim of this paper is to indicate possible etiopathogenetic associations between destructive periodontitis in pregnant women and preterm low birth-weight deliveries. Strong pro-inflammatory potential of chronic periodontitis may attack the whole organism of the future mothers. The destructive inflammatory character of various forms of periodontitis with its bacterial, cytokinal and proteolytic expressions can raise the pro-inflammatory and proteolytic status in the fetoplacental unit and cause premature rupture of chorioplacental membranes. In our study, there are presented possible associations of both states manifested through bacterial, mediatory, genetic, proteolytic and oxidative mechanisms. The majority of other papers report associations of individual ethiopathogenetic mechanisms, by means of which chronic destructive inflammation of periodontium negatively afflicts the feto-placental structures. In spite of wide research in this field, causal associations have not yet been unanimously proven.

Beyond ONJ - A review of the potential uses of bisphosphonates in dentistry.

There is evidence, although limited, for beneficial effects of bisphophonates (BPs) across multiple dental specialties. Within implant dentistry BP coatings have been shown to significantly increase pull out forces and bone density in animal models, and significantly increase implant stability whilst reducing marginal bone loss in humans. Adjunctive topical and systemic application of BPs during conventional periodontal treatment have shown significant improvements in probing depth and clinical attachment level in various forms of periodontal disease. Within orthodontics, BPs have been shown to significantly reduce root resorption and have benefits with respect to anchorage maintenance. Case reports have suggested the use of BPs in the management of diffuse sclerosing osteomylitis. Whilst this review highlights these potential benefits and acknowledges there are no reported cases of osteonecrosis of the jaw associated with locally applied BP, there remains a paucity of human and long-term studies exploring BPs in the context of significant clinical benefit. Further human studies are required to understand the long-term clinical outcomes of these drugs when used as primary therapeutic agents, or adjuncts to conventional treatment.

Role of genetic in periodontal disease

Genetics is the study and understanding of the phenomena of heredity and variation. A large number of genes are associated with many systemic conditions. Periodontitis is inflammatory condition of periodontium. Periodontium consists of gingiva, periodontal ligament, cementum, and alveolar bone. It is considered being a multifactorial disease. Studies of animals and humans support the concept that a large number of genes' factor may be associated with periodontitis and clearly play a role in the predisposition and progression of periodontal diseases. It has been proven that genetic factors impair inflammatory and immune responses during periodontal diseases. Research on identifying specific genes causing periodontitis may improve and prevent the disease progression. The aim of this article is to focus on genetic risk factors and its influence for the various forms of periodontal disease.

Associations between Periodontal Microbiota and Death Rates

It is conceived that specific combinations of periodontal bacteria are associated with risk for the various forms of periodontitis. We hypothesized that such specificity is also related to human cause-specific death rates. We tested this hypothesis in a representative sample of the US population followed for a mean duration of 11 years and found that two specific patterns of 21 serum antibodies against periodontal bacteria were significantly associated with increased all-cause and/or diabetes-related mortalities. These data suggested that specific combinations of periodontal bacteria, even without inducing clinically significant periodontitis, may have a significant impact on human cause-specific death rates. Our findings implied that increased disease and mortality risk could be transmittable via the transfer of oral microbiota, and that developing personalized strategies and maintaining healthy oral microbiota beyond protection against periodontitis would be important to manage the risk.

Relationship between Genetic Polymorphisms with Periodontitis and Peri-Implantitis in the Iranian Population: A Literature Review.

Periodontitis is an infectious disease of the tooth-supporting tissues. Specific periodontal pathogens trigger inflammatory responses and result in progressive bone loss, which eventually leads to exfoliation of teeth. Periimplantitis is the destructive inflammatory process affecting the soft and hard tissues surrounding dental implants. The periodontal pathogens found around failing implants are very similar to those associated with various forms of periodontal diseases. Genetic susceptibility has been known to be an important risk factor for periodontitis and periimplantitis and there have been numerous studies evaluating this in different populations. Many gene polymorphisms may influence individual susceptibility to periodontitis and peri-implantitis. We conducted a review of the literature for gene polymorphisms associated with periodontitis and peri-implantitis susceptibility in the Iranian population. A comprehensive search of the literature on human studies in English, published from January 2004 to February 2015 in PubMed database and Google scholar was performed using the key words "periodontitis," "peri-implantitis," "gene polymorphisms," and "Iranian population." Sixty-six studies were identified; 33 articles were excluded based on review of titles and abstracts. Thirty articles were eligible and had the full text evaluated. Further studies are needed to get more inclusive perception of the influence of gene polymorphisms in periodontitis and peri-implantitis.

Trends in Susceptibility to Aggressive Periodontal Disease.

Aggregatibacter actinomycetemcomitans is a gram-negative microbe involved in periodontitis. Strains with varying degrees of virulence have been identified, in healthy and periodontally compromised individuals alike. Hosts mount differential immune responses to its various serotypes and virulence factors. Studies have explored host immune response in terms of antibody titers, leukocyte responses, and specific inflammatory mediators, questioning the ways in which the infectious microorganism survives. This mini-review will identify the key themes in immune response patterns of individuals both affected by and free from aggressive periodontal disease, thereby using it to understand various forms of periodontitis.

Interaction between periodontal disease and atherosclerotic vascular disease--Fact or fiction?

C-reactive protein (CRP) level is associated with the 10-year risk of an atherosclerotic vascular disease (ASVD), suggesting presence of systemic inflammation probably long before ASVD is present. Where, however, does this systemic inflammation come from? One active area of research has been the study of dental infection and various forms of periodontal disease (PD), both of which are highly prevalent in populations at risk for ASVD. Recent data show that ASVD and PD interact with each other via systemic release of specific pro- and anti-inflammatory cytokines, small signal molecules and enzymes which modulate initiation and progression of the chronic inflammatory reaction involved in both diseases. In addition, periodontal pathogens were identified within atherosclerotic lesions and thrombi isolated from myocardial infarction patients. LDL cholesterol, a strong risk factor for ASVD, is also associated with PD; and statins, used to treat ASVD, are also active to prevent or reduce PD. Finally, there is growing evidence for common genetic susceptibility factors involved in both diseases. These findings support commonalities with respect to the pathogenic mechanisms involved in both inflammatory diseases. Conversely, a causative relationship cannot yet be concluded in the absence of data from large longitudinal cohort and randomized controlled intervention trials.

Association of Fc gamma-receptors IIa, IIIa, and IIIb genetic polymorphism with susceptibility to chronic periodontitis in South Indian population.

Background and Objective:Fc gamma receptors (FcγRs) are the members of the immunoglobulin superfamily and may play a role in the pathogenesis of periodontitis. Genetic variation in these receptors and its link with various forms of periodontitis is being studied in different populations. The aim of the present study is to determine whether specific FcγRIIa, FcγRIIIa, and FcγRIIIb alleles and/or genotypes are associated with risk for susceptibility to generalized chronic periodontitis (GCP) in South Indian population.Materials and Methods:The study population consisted of 120 South Indian subjects; 60 with GCP and 60 periodontally healthy. Deoxyribonucleic acid (DNA) was extracted from samples collected by scrapping buccal epithelium. FcγRIIa and FcγRIIIa genotyping were performed by polymerase chain reaction (PCR) amplification of DNA with allele-specific primers followed by allele-specific restriction digestion of the products. However, FcγRIIIb genotyping was done by allele-specific PCR.Results:No significant difference in the distribution of FcγRIIa H/R and FcγRIIIa NA1/NA2 genotypes or their respective alleles was observed in GCP patients and healthy subjects. For FcγRIIIa F/V genetic polymorphism, the homozygous V/V genotype and V allele were significantly overrepresented in GCP patients while F/F genotype and F allele in controls.Conclusion:The present study demonstrates that FcγRIIIa V/V genotype, as well as V allele, could be a possible risk factor for chronic periodontitis in South Indian population.

Porphyromonas gingivalis:patógeno predominante en la periodontitis crónica

Porphyromonas gingivales es un bacilo gram negativo predominante en la Periodontitis crónica, sus múltiples factores de virulencia la hacen sumamente agresiva. En el surco gingival encuentra las condiciones para su crecimiento, interaccionando con el huésped produciendo una destrucción lenta pero constante de los tejidos del periodonto. Su predominancia ha sido considerada como un factor de riesgos para enfermedades sistémicas inflamatorios, como la del infarto de miocardio. Aunque su susceptibilidad a una diversidad de fármacos hace posible su manejo con antimicrobianos previa remoción mecánica de biofilm subgingival. En conclusión la revisión abarca diversas características de la bacteria, que nos unen al consenso de que Porphyromonas gingivalis es el patógeno de mayor relevancia en la periodontitis crónica, así como su presencia en diversas formas de patologías periodontales.

HIV and Periodontal Disease: Redemption or Resurrection

Human immunodeficiency virus (HIV) infection, a major medical crisis that has hit families, communities & nations across the world, remains a major health concern of unprecedented dimension. Mounting evidence suggests that co-infections and inflammation may result in re-activation of latent virus and blunt the success of Highly Active Anti-retroviral Therapy (HAART). Periodontal diseases are chronic multibacterial infections that can result in significant bacterial & inflammatory load in the body and HIV infection has been found to be associated with various forms of periodontal diseases. Besides Immune Reconstitution Inflammatory Syndrome (IRIS), following HAART, may also lead to increased frequency of periodontal disease among these patients. If co-infections & inflammation can increase the risk of HIV reactivation, can periodontitis in an HIV infected individual lead to HIV re-activation. This question remains unanswered, but initial evidence suggests a probable role of periodontitis in HIV re-activation. The clinical implication of such a role is that, periodontitis being most often preventable and definitely treatable, every attempt should be made to prevent and promptly treat this disease in individuals with HIV infection.

Real-time polymerase chain reaction of “red complex” ( Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola) in periradicular abscesses

The red complex bacteria (Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola) have been implicated, either singly or in combination, in the development of various forms of periodontal diseases. The aim of this study was to investigate the presence of "red complex" in acute periradicular abscesses by real-time polymerase chain reaction (PCR) method. Microbial samples were collected by aspiration from 32 cases diagnosed as acute periradicular abscess. DNA was extracted from the samples by using a QIAamp DNA mini-kit and analyzed with real-time PCR. At least 1 member of the red complex was found in 84% of the cases. In general T. denticola, P. gingivalis, and T. forsythia were detected in 65.6%, 43.7%, and 40.6% of the cases, respectively. Red complex was detected in 15.6% of samples taken from acute periradicular abscesses. Our findings suggest that "red complex" can participate in the pathogenesis of acute periradicular abscesses.

Bains de bouche aux extraits naturels microencapsulés : résultats cliniques sur la plaque dentaire et la gingivite

New mouthwash solutions containing microencapsulated natural extracts have been developed. Besides antiseptic activity, these solutions have antioxidant and immunoregulatory properties on oral tissues. The objective of this preliminary clinical study was to assess the efficiency of a mouthwash containing microencapsulated natural extracts (GingiNat, LoB5 Foundation, Paris, France) on gingivitis. Twenty volunteers (37 + or - 2 years) with significant gingivitis (bleeding when tooth-brushing) used a 6% GingiNat mouthwash solution, two to three times per day (according to oral hygiene habits) for 21 days in addition to their usual oral care. Each volunteer was examined at day 0, 4, 7 and 21. The Löe and Silness Plaque Index, the Russel Periodontal Index, a breath index (halitosis), and oral tissue tolerance were assessed. Finally, each volunteer filled in a daily follow-up form and answered a questionnaire on tolerance and acceptability. A significant decrease of the dental plaque index was observed on day 4, 7, and 21, in respectively 29, 48 and 71% of volunteers. This decrease reached 18% on day 4 (p=0.014), 32% on day 7 (p=0.002), and 47% on day 21 (p<0.001). A significant decrease of the periodental index was observed on day 4, 7 and 21, in respectively 52, 81 and 95% of volunteers. This decrease reached 30% (p=0.001) on day 4, 49% (p<0.001) on day 7, and 78% (p<0.001) on day 21. A significant improvement of the breath index was noted on day 4, 7, and 21, in respectively 43, 52 and 48% of volunteers. This improvement reached 29% (p=0.004) on day 4, 35% (p=0.001) on day 7, and 32% (p=0.002) on day 21. The mouthwash was well tolerated. The patients liked its effectiveness and organoleptic properties. All patients expressed the wish to continue using this solution. The GingiNat mouthwash solution at 6% had a significant efficiency on plaque, gingivitis, and halitosis after 21 days of use. This makes it a good complementary treatment for gingivitis. Tolerance and acceptability were good despite the long and repeated use. Further studies are needed to have a detailed analysis of its efficiency in the long run and on patients presenting with various forms of periodontitis.
