Efficacy and Safety of Switching From Boosted-Protease Inhibitors (bPI) Plus Emtricitabine/Tenofovir Disoproxil Fumarate (F/TDF) Regimens to the Once Daily (QD), Single-Tablet Regimen (STR) of Darunavir/Cobicistat/Emtricitabine/Tenofovir Alafenamide (D/C/F/TAF) in Virologically Suppressed, HIV-1-Infected Adults: Week 96 Results of the Phase 3, Randomized, Non-Inferiority EMERALD Trial

BackgroundThe QD STR D/C/F/TAF 800/150/200/10 mg was noninferior to bPI + F/TDF at 48 weeks in EMERALD. Efficacy and safety of D/C/F/TAF through week 96 are presented.MethodsEMERALD (NCT02269917) is a randomized, active-controlled, open-label, international, multicenter noninferiority trial. Virologically suppressed (VL<50 c/mL for ≥2 months) ART experienced (previous non-DRV VF allowed) HIV-1-infected adults were randomized (2:1) to switch to D/C/F/TAF or continue bPI + F/TDF over 48 weeks. Patients could then continue on D/C/F/TAF or switch from bPI + F/TDF to D/C/F/TAF at week 52 (Late switch, 44 weeks D/C/F/TAF exposure) in a single-arm extension phase until week 96. The percentage of patients with virologic rebound (confirmed VL ≥50 c/mL) cumulative through week 48 and week 96 were primary and secondary endpoints, respectively.ResultsOf 1141 randomized and treated patients (58% had received ≥5 previous ARVs including screening ARVs; 15% had previous non-DRV VF), 1,080 continued in the extension phase (N = 728 D/C/F/TAF; N = 352 late switch). Few patients had virologic rebound cumulative through week 96 in the D/C/F/TAF arm (3.1%, 24/763). Virologic rebound occurred in 2.3% (8/352) in the late switch arm over 44 weeks D/C/F/TAF treatment. Many rebounders (14/24 and 2/8) resuppressed by week 96. At week 96 a high percentage of patients in the D/C/F/TAF arm (90.7%, 692/763) were suppressed (VL<50 c/mL). In the late switch arm, 93.8% (330/352) maintained virologic suppression after 44 weeks of treatment. No DRV, primary PI, TFV, or FTC RAMs were seen post baseline. Few serious AEs and AE related discontinuations occurred in either arm (Table 1). Improvements in renal and bone parameters were maintained in the D/C/F/TAF arm and seen in the late switch arm (week 52–96), with a small change in TC/HDL-C ratio (Table 1).ConclusionSwitching to D/C/F/TAF maintained high virologic suppression rates (>90%) at week 96 with no resistance development, and was well tolerated over 96 weeks with bone, renal, and lipid safety consistent with known TAF and cobicistat profiles. Efficacy and safety results in the late switch arm were consistent with week 48 results in the D/C/F/TAF arm. D/C/F/TAF combines the efficacy and high genetic barrier to resistance of DRV with the safety benefits of TAF, even in patients with a history of non-DRV VF. Disclosures J. Week 48 Results of EMERALD: A Phase 3, Randomized, Non-inferiority Study Evaluating the Efficacy and Safety of Switching from Boosted-protease Inhibitors (bPI) Plus Emtricitabine (FTC)/Tenofovir Disoproxil Fumarate (TDF) Regimens to the Once Daily (QD), Single-tablet Regimen (STR) of Darunavir/Cobicistat/Emtricitabine/Tenofovir Alafenamide (D/C/F/TAF) in Virologically Suppressed, HIV-1-infected Adults

BackgroundEMERALD is evaluating the efficacy and safety of switching from bPI + FTC/TDF regimens (control) to D/C/F/TAF 800/150/200/10 mg in virologically suppressed, HIV-1-infected adults. We present Week 48 primary results.MethodEMERALD (NCT02269917) is a randomized, active-controlled, open-label, international, multicenter, parallel-group, non-inferiority trial. Virologically suppressed (viral load [VL] < 50 c/mL for ≥2 months), HIV-1-infected adults were randomized (2:1) to switch to D/C/F/TAF or continue control. The FDA-stipulated primary endpoint was non-inferiority of D/C/F/TAF vs. control regarding % virologic rebound (confirmed VL ≥ 50 c/mL or premature discontinuations with last VL ≥ 50 c/mL) cumulative through Week 48 (4% margin).Result1141 patients were randomized and treated (N = 763 D/C/F/TAF; N = 378 control); median age 46; 18% women; 76% white; 58% on >2 previous ARVs (prior to screening regimen); 15% with previous non-DRV virologic failure (VF). Virologic rebound through Week 48 was non-inferior for D/C/F/TAF (2.5%; n = 19) vs. control (2.1%; n = 8) (Δ0.4%, 95% CI: –1.5%; 2.2%; P < 0.001). Most rebounders (12/19 [63%] vs. 4/8 [50%]) resuppressed by Week 48 without change in therapy. Week 48 virologic suppression rates (VL < 50 c/mL; FDA Snapshot) were 94.9% vs. 93.7% (Δ1.2%, 95% CI: −1.7%;4.1%) and VF rates (VL ≥ 50 c/mL; Snapshot) were 0.8% vs. 0.5% (Δ0.3%, 95% CI: −0.7%;1.2%), with no discontinuations for VF. No resistance-associated mutations related to any study drug were observed.Adverse events (AEs) were similar between arms: AE-related discontinuations (1.4% vs. 1.3%); grade 3–4 AEs (6.8% vs. 8.2%); serious AEs (4.6% vs. 4.8%); and no deaths. Renal and bone parameters favored D/C/F/TAF vs. control. TC and LDL-C slightly favored control vs. D/C/F/TAF, with no clinically significant difference in TC/HDL-C ratio between arms (Table 1).ConclusionPercentage of virologic rebound after switching to D/C/F/TAF was non-inferior to control cumulative through Week 48, with high suppression rates (94.9%), no resistance development, better bone and renal safety parameters and similar TC/HDL-C ratio. The dataset contains almost 1.2 million plant occurrences of 1,222 native vascular plant species, mostly recorded by volunteers (citizen scientists), mainly since 2008. The occurrences are derived from the database http://www.waarnemingen.be, hosted by Stichting Natuurinformatie and managed by the nature conservation NGO Natuurpunt. Publishing online identification keys in the form of scholarly papers

One of the main deficiencies in publishing and dissemination of online interactive identification keys produced through various software packages, such as DELTA, Lucid, MX and others, is the lack of a permanent scientific record and a proper citation mechanism of these keys. In two earlier papers, we have discussed some models for publishing raw data underpinning interactive keys (Penev et al. 2009; Sharkey et al. 2009). Here we propose a method to incentive authors of online keys to publishing these through the already established model of “Data Paper” (Chavan and Penev 2011, examples: Narwade et al. 2011, Van Landuyt et al. 2012, Schindel et al. 2011, Pierrat et al. 2012, see also Pensoft's Data Publishing Policies and Guidelines). For clarity, we propose a new article type for this format, “Online Identification Key”, to distinguish it from the “Data Paper” in the narrow sense. The model is demonstrated through an exemplar paper of Cerretti et al. (2012) in the current issue of ZooKeys. The paper describes the main features of an interactive key to the Palaearctic genera of the family Tachinidae (Diptera) implemented as an original web application. The authors discuss briefly the advantages of these tools for both taxonomists and general users, and point out the need of shared, standardized protocols for taxa descriptions to keep matrix-based interactive keys easily and timely updated. The format of the “Online Identification Key” paper largely resembles the structure of Data Papers proposed by Chavan and Penev (2011) on the basis of the Ecological Metadata Language (EML) and developed further in Pensoft's Data Publishing Policies and Guidelines. An “Online Identification Key” paper should focus on a formal description of the technical details and content of an online key that is what is often called “metadata”. For example, an “Online Identification Key” paper has a title, author(s), abstract and keywords like any other scientific paper; it should also include in the first place: the URL of an open access version of the online key and possibly also the data underpinning the key, information on the history of and participants in the project, the software used and its technical advantages and constraints, licenses for use, taxonomic and geographic coverage, lists and descriptions of the morphological characters used, and literature references. In contrast to conventional data papers, the “Online Identification Key” papers do not require compulsory publication of raw data files underpinning a key, although such a practice is highly recommended and encouraged. There might be several obstacles in publishing raw data that can be due to copyright issues on either data or source codes. It is mandatory, however, for the online keys published in this way to be freely available for use to anyone, by just clicking the URL address published in the paper. The publication of an online key in the form of a scholarly article is a pragmatic compromise between the dynamic structure of the internet and the static character of scientific articles. The author(s) of the key will be able to continuously update the product, to the benefit of its users. At the same time, the users will have available a citation mechanism for the online key, identical to that used for any other scientific article, to properly credit the authors of the key.

Use of the Thoracodorsal Artery Perforator Flap for Bronchial Reinforcement in Patients With Previous Posterolateral Thoracotomy

The thoracodorsal artery perforator flap (TDAP flap) allows raising the same cutaneous island as in the classical latissimus dorsi musculocutaneous flap without its muscular part. All patients who underwent a completion pneumonectomy with reinforcement of bronchial stump with a TDAP flap from December 2009 to October 2010 were followed prospectively. The 30-day mortality and the procedure-related morbidity as well as bronchial fistula and TDAP flap were analyzed. The TDAP flap was used in 6 cases without failure or fistula formation. At 1 month, all patients were alive, and there was no morbidity (seroma, hematoma, fistula, or shoulder dysfunction). Computed tomography scans were performed at 1 month and 3 months postoperatively and showed viable nonatrophic flap. This type of flap has been described in the field of plastic surgery, and this is the first description of its use in the chest. Deepithelialized fasciocutaneous TDAP flap is safe and reliable. It is available even if the latissimus dorsi has been previously divided. It is now our first-line option to reinforce the bronchial stump.

Fifty healthy women, aged 18-39 years with no known risk of ovarian hyperstimulation were treated with 100 or 150 μg corifollitropin alfa (dependent on body weight) in a long GnRH agonist protocol. At these doses, corifollitropin alfa initiated and supported growth of a large cohort of follicles during the first week of ovarian stimulation.

Poor outcome in oocyte donation after elective transfer of a single cleavage-stage embryo in Turner syndrome patients

In this retrospective, matched study we evaluated the outcome of 31 oocyte donation (OD) cycles performed in 29 Turner syndrome (TS) patients involving the elective transfer of a single, fresh cleavage-stage (day 2 to 3) embryo. Due to the fact that ongoing pregnancy rate was statistically significantly lower (3.2% versus 22.5%) in TS patients when compared with matched, non-TS recipients, other strategies (such as single blastocyst transfer) should be evaluated that could enable better outcomes and at the same time avoid potential complications related to multiple pregnancies in this particularly high obstetric-risk group.

Prospective evaluation of the optimal time for selecting a single embryo for transfer: day 3 versus day 5

To determine the best day for the selection and transfer of a single embryo, a prospective, randomized study was undertaken that compared the ongoing pregnancy rate (PR) after single embryo transfer (SET) on day 3 with that after single blastocyst transfer (SBT) on day 5. Our results show an overall significantly higher PR after SBT (32.8%) compared with SET (23.2%), and a PR of 40.8% after SBT versus 25.6% after excellent-quality embryos became available.

Inelastic scattering of high-energy electrons in a crystal in thermal equilibrium with the environment II. Solution of the equations and applications to concrete cases

The equations governing the dynamical diffraction of high-energy electrons in a crystal in thermal equilibrium with the environment, derived earlier [C. Fanidis, D. Van Dyck and J. Van Landuyt, Ultramicroscopy 41 (1992) 55], have been solved. The solutions for the different wave-function components have been obtained in a closed analytical form. Complex potentials, accounting for absorption of the electrons, have been derived in the solutions. For phonon scattering, analytical expressions of these potentials have been calculated, and for independent vibrations a parametrized form is presented. It is shown that even in the case of no energy transfer the thermal equilibrium will influence the image and the diffraction pattern intensities. For photon scattering, images and diffraction patterns and the complex potentials have been calculated numerically for concrete cases. The inelastic scattering results in diffuse intensity in the diffraction pattern. The images formed from inelastically scattered electrons and the relative intensities in the corresponding diffraction patterns depend on the polarization of the excited vibration modes. From the results it is concluded that thermal equilibrium and inelastic scattering should be taken into account in the calculations.

Raman spectroscopy of incommensurate Ba2NaNb5O15.

We have performed Raman studies of the E-symmetry phonon modes in barium sodium niobate in the tetragonal ferroelectric phase above 582 K and measured carefully their splitting into pairs of ${\mathit{B}}_{1}$ and ${\mathit{B}}_{2}$ symmetry in the incommensurate (IC) phase between 543 and 582 K. For both heating and cooling cycles the data satisfy mean-field predictions with ${\mathit{B}}_{1}$-${\mathit{B}}_{2}$ frequency splitting proportional to the order parameter. The data show that the incommensurate-incommensurate transition at 565 K (heating) does not alter the orthorhombic symmetry; that is, it is not a true 1q-2q transition, and tetragonal symmetry does not set in until the incommensurate-normal transition at 582 K. This is compatible with very recent electron microscopy data (Verweft, Van Tendeloo, and Van Landuyt), which show that the incommmensurate-incommensurate transition near 565 K produces only short-range order in a direction perpendicular to the long-range 1q modulation, and with x-ray studies of this transition (Kiat, Toledano, and Schneck). An analogous situation is observed just below the incommensurate-commensurate transition at 105 K: Here the ${\mathit{C}}_{11}$ and ${\mathit{C}}_{22}$ elastic coefficients have minima at 105 K, signaling a phase transition, but they do not become equal (${\mathit{C}}_{11}$=${\mathit{C}}_{22}$) until approximately 90 K, indicating that there is an incommensurate-incommensurate transition from orthorhombic to tetragonal near 90 K. The 90-K IC-IC transition seems quite analogous to the 565-K IC-IC transition, but electron microscopy shows that these phases are not reentrant.

New results on the phase transformations of In 2Se 3

The various phases and the phase transformations of In 2Se 3 are studied by electron microscopy and electron diffraction methods. Specimens from crystal slices perpendicular to the hexagonal basal plane were used. The results were combined with those reported by J. van Landuyt et al. ( 1) for specimens parallel to the basal plane. It is found that the various α( n)-polytypic forms of In 2Se 3 follow similar transformation paths by heating above room temperature. The ultimate δ-phase is cation disordered with a wurtzite-like average structure. Apart from the γ-phase, the intermediate phases are characterized by highly faulted sequences of structure sheets along the c-axis.
