Vagina is the first organ of mammalian female reproductive system and its secretions are the transudates of vaginal linings which play a dominant role in sperm capacitation 91) and transportation. Studies have been conducted on the effects of contraceptives (2) and intravaginal tampons on the biochemistry of vagina as well as vaginal fluid (3). There is an increase in the amount of vaginal fluid at the time of expected ovulation (4). Kistner (5) reported scanty vaginal secretion rich in progestin due to use of oral contraceptives.

Is there a local production of nitrosamines by the vaginal microflora in anaerobic vaginosis/trichomoniasis?

Nitrosamines have been identified in vaginal secretions of women with trichomoniasis. Both nitrites and amines the elements for nitrosamine synthesis are produced by the abnormal-predominantly anaerobic-vaginal flora of patients with bacterial vaginosis/trichomoniasis. The final formation of nitrosamines could also depend on microbial enzymatic ativity. The confirmation of an endogenous generation of oncogenic nitrosamines in the vaginas of some women could give promising impulses to cancer-research concerning the uterine cervix.

GARDNERELLA VAGINALIS, ANAEROBES, AND VAGINAL DISCHARGE

Quantitative microbiological investigations were carried out on vaginal secretions from 82 selected women. The microbial flora of women with normal white vaginal secretions of pH 4·5 or less consisted mainly of lactobacilli. Anaerobic bacteria and Gardnerella vaginalis were rarely isolated. Similarly, the vaginal secretions of women with vaginal candidiasis usually contained only facultative anaerobes. In contrast, the vaginal secretions of women with trichomoniasis or non-specific vaginal discharge contained predominantly mixed anaerobic bacteria, often accompanied by G. vaginalis, and lactobacilli were rarely isolated. In 3 women classified as normal at vaginal examination and who did not complain of vaginal discharge the vaginal secretions had a mixed anaerobic flora and 2 had Candida albicans. 2 women classified as abnormal at vaginal examination but who did not complain of vaginal discharge had Trichomonas vaginalis. Vaginal pH testing was easy and objective, and could be carried out at vaginal examination. It facilitated the clinical diagnosis of non-specific vaginal discharge. Anaerobic bacteria and G. vaginalis may well act together to cause non-specific vaginal discharge; vaginal pH testing serves to aid accurate diagnosis.

DISCUSSION

Dr. Michael was asked whether the attractant isolated by him was species specific. In reply, he pointed out that this material had been found in several species of primates including the human female. Material isolated from the vaginal secretions of women and baboons acted as an attractant when applied to ovariectomized rhesus females.

The Vaginal Smear in the Diagnosis of Uterine Cancer12

WE ARE presenting evidence that uterine cancer can be diagnosed with accuracy by the vaginal smear. The diagnosis depends upon the identification of cancer cells in the vaginal secretion. Papanicolaou, in 1928, first recognized cancer cells in the vaginal secretion of women with cancer of the uterus. Since 1941 articles have appeared by Papanicolaou (1) and Papanicolaou and Traut (2, 3); and in 1943 a monograph on the diagnosis of uterine cancer by vaginal smear (4) was published by these two authors. In the same year Meigs et al. (5) published results on 220 cases. Since our publication we have studied approximately 600 additional cases and here report on our total group of 813, statistics of which follow. The method is simple. A nurse or the patient herself can easily be taught to take the smear. Trichomonas belongs to the flagellate infusoria.Buetschle¹says that about three species of Trichomonas are known. All are parasitic, living in the intestines of frogs, snails and other lower animals, and in the human vagina. Trichomonas vaginale was found in the vaginal secretions of women byDonné²in 1836. WhileDonné'sobservations, though at first criticised, were finally accepted as true, yet it may be said that the parasites have been deemed of so little importance that they have until very recently received only scant notice.
