Cervical lacerations: A case series

Cervical laceration (CL) after vaginal delivery is one of the uncommon side effects with significant morbidity if undetected and unattended to promptly. This is however one of the common causes for postpartum haemorrhage. There are several documented risk factors for CL such as previous interventions on the cervix, induction of labour, use of Prostaglandins for induction, precipitate labour, operative vaginal deliveries and birth weight of more than 3500gm.In this article we have described a series of eleven women who were detected to have CL following vaginal delivery. One of whom had delivery of the baby through a bucket handle cervical tear with a non-dilated cervical os. Almost all of them needed multiple blood and product transfusions in view of postpartum haemorrhage with one of them developing acute kidney injury subsequently. This necessitates the need for prompt identification of CL and its needed intervention.

A Case Series of Uterine Rupture: Lessons to be Learned for Future Clinical Practice

In this article, we try to discuss risk factors and diagnostic difficulties for uterine rupture. Case series of 12 cases of uterine rupture observed in the Norfolk and Norwich University Hospital in the UK, with an average yearly birth rate of 6,000 deliveries, over a 6-year period. In the present case series, there was no maternal mortality, and uterine rupture was a rare occurrence (12 in 36,000 births). Uterine rupture is associated with clinically significant uterine bleeding, fetal distress, expulsion or protrusion of the fetus, placenta or both into the abdominal cavity, and the need for prompt cesarean delivery and uterine repair or hysterectomy. The risk factors for rupture include previous cesarean sections, multiparity, malpresentation and obstructed labor, uterine anomalies, and use of prostaglandins for induction of labor. Previous cesarean section is, however, the most commonly associated risk factor. The most consistent early indicator of uterine rupture is the onset of a prolonged, persistent, and profound fetal bradycardia. In this case series, we suggest that the signs and symptoms of uterine rupture are typically nonspecific, which makes diagnosis difficult. Delay in definitive therapy causes significant fetal morbidity. The inconsistent signs and the short time in prompting definitive treatment of uterine rupture make it a challenging event. For the best outcome, vaginal birth after previous cesarean section needs to be looked after in an appropriately staffed and equipped unit for an immediate cesarean delivery and advanced neonatal support.

Geburtseinleitung bei Zustand nach Sectio

Angesichts dramatisch ansteigender Sectioraten und einer zunehmenden Frequenz an Geburtseinleitungen wird die Geburtseinleitung nach vorausgegangenem Kaiserschnitt ein immer häufigeres Problem in der Geburtshilfe. Klinisch bedeutend sind die Kontraindikationen und die medizinischen und logistischen Voraussetzungen für eine Geburtseinleitung nach vorausgegangenem Kaiserschnitt. Die Erfolgsrate einer vaginalen Geburt nach Sectio liegt zwischen 60 und 85 %; prädiktive Faktoren für den Erfolg einer Geburtseinleitung sind vor allem: Vorausgegangene vaginale Geburt, Geburtsgewicht unter 4000 g, BMI < 30, spontane Wehentätigkeit und Indikationen bei der vorausgegangenen Sectio nicht Geburtsstillstand/Kopfmissverhältnis. Entscheidend für die mütterliche und neonatale Morbidität und Mortalität ist, ob die Geburtseinleitung nach Sectio erfolgreich ist oder nicht, und dann eine sekundäre Re-Sectio notwendig wird. Andererseits müssen die Langzeitfolgen wiederholter Kaiserschnitte mit dem erhöhten Risiko für perinatale Komplikationen (Placenta accreta, Placenta praevia) in Betracht gezogen werden. Korrespondierend zu der Rate an Uterusrupturen ist die perinatale Mortalität und die Rate an hypoxisch-ischämischen Enzephalopathien nach Geburtseinleitung höher als nach einer elektiven Re-Sectio. Die Rate an Uterusrupturen nach Geburtseinleitung, insbesondere nach Anwendung von Prostaglandinen, ist deutlich erhöht und liegt bei bis zu 2,5 %, ohne Geburtseinleitung unter Wehen zwischen 0,3 und 0,7 %, bei einer elektiven Re-Sectio bei ≤ 0,1 %. Inzwischen wurden verschiedene Risikofaktoren für eine Uterusruptur bei Geburtseinleitung nach Sectio identifiziert. Zur Geburtseinleitung mit Oxytocin und/oder Prostaglandin E2 versus dem Abwarten spontaner Wehen liegen in der Literatur widersprüchliche Ergebnisse vor. Um das tatsächliche Risiko der Uterusruptur bei der einen oder anderen Vorgehensweise zu evaluieren, bedarf es randomisierter Studien mit einer adäquaten Fallzahl; bisher fehlen aber diesbezügliche Empfehlungen mit hoher Evidenzstärke. In Deutschland ist die Anwendung von Prostaglandinen zur Geburtseinleitung durch die soeben publizierten Leitlinien der Deutschen Gesellschaft geregelt; entsprechend den Empfehlungen der ACOG besteht bei Geburtseinleitung mit Prostaglandinen ein erhöhtes Risiko für Uterusrupturen, daher sollte eine Geburtseinleitung mit Prostaglandinen, sofern notwendig, nur nach klar definierter, medizinischer Indikation und nach einem ausführlichen Aufklärungsgespräch durchgeführt werden. Bei entsprechender medizinischer Indikation ist der Patientin als Alternative eine elektive Re-Sectio anzubieten.

Use of prostaglandins for induction of labor

Nurse-midwives are sometimes required to intervene in the normal process of labor. Numerous clinical trials have investigated the use of prostaglandins for induction of labor. Research indicates the use of prostaglandins to be a safe and effective method of induction of labor with favorable results when compared with the more traditional use of oxytocin. Patient acceptance appears to be favorable because of the reduced need for restrictive and invasive methods of monitoring and administration that accompany intravenous oxytocin. Thus, the use of prostaglandins for induction of labor offers nurse-midwives an attractive alternative to the use of oxytocin when induction is medically indicated. This article provides a description of the biochemical effects of prostaglandins on the uterus and cervix, development and clinical use of prostaglandin preparations, indications for use of induction, and potential for utilization in nurse-midwifery practice.

Ruptured uterus: sonographic diagnosis.

Sonography was able to suggest a diagnosis of uterine rupture by demonstrating intra- or extraperitoneal hematoma in the correct clinical setting. A pelvic abscess complicating a uterine rupture was seen in one case. Previous uterine scar, use of prostaglandins for induction of labor, trauma with a currette, and cornual pregnancy were the implicated etiologic factors. Although the rupture site was never directly visualized by sonography, the combination of sonographic and clinical findings was strongly suggestive of the correct diagnosis.
