Technical concepts on blood pool phase SPECT (acquisition, reconstruction).

Guidelines for bone scintigraphy are well established and recommend the use of planar early phase images to investigate a number of clinical indications. With recent advances in gamma camera technology the use of SPECT/CT imaging in the early phases is now possible, offering the potential of improved diagnostic confidence and prognostic value. To date little work has been carried out to optimize the acquisition of early phase bone images using SPECT/CT with most of the available studies acquiring SPECT images after the traditional planar images to allow comparison of the two techniques. Imaging durations of 7 to 10 minutes have been commonly used. However, the use of iterative reconstruction algorithms has been investigated with rapid SPECT imaging to allow imaging durations as low as 4 minutes. The use of CZT based systems with increased sensitivity and improved energy and spatial resolution also offers the potential to reduce imaging times. The optimization of projection measurement order has been investigated as a method of reducing image artefacts as a result of changing tracer distribution during the SPECT acquisition. In this article we consider the current state of early phase SPECT imaging and possible areas for future investigation as well as recommendations for departments looking to adopt blood pool SPECT imaging as part of their routine clinical practice.

Impact of dose reduction and iterative reconstruction algorithm on the detectability of pulmonary nodules by artificial intelligence

PurposeThe purpose of this study was to assess whether the performances of an automated software for lung nodule detection with computed tomography (CT) are affected by radiation dose and the use of iterative reconstruction algorithm. Materials and methodsA chest phantom (Multipurpose Chest Phantom N1; Kyoto Kagaku Co. Ltd, Kyoto, Japan) with 15 pulmonary nodules was scanned with a total of five CT protocol settings with up to 20-fold dose reduction. All CT examinations were reconstructed with iterative reconstruction algorithms ADMIRE 3 and ADMIRE 5 and were then analyzed for the presence of pulmonary nodules with a fully automated computer aided detection software system (InferReadTM CT Lung, Infervision), which is based on deep neural networks. ResultsThe sensitivity of fully automated pulmonary nodule detection for ground-glass nodules at standard dose CT was greater (70.0%; 14/20; 95% CI: 51.6-88.4%) than at 10-fold and 20-fold dose reduction (30.0%; 6/20; 95% CI: 0.0%-62.5%). There were less false positive findings when ADMIRE 5 reconstruction was used (4.0 ± 2.8 [SD]; range: 2–6) instead of ADMIRE 3 reconstruction (25.0 ± 15.6 [SD]; range: 14–36). There was no difference in the sensitivity of detection of solid and subsolid nodules between standard dose (100%; 95% CI: 100–100%) and 10- and 20-fold reduced dose CT (92.5%; 95% CI: 83.8–100.0%). Image noise was significantly greater with ADMIRE 3 (81 ± 2 [SD] [range: 79–84]; 104 ± 3 [SD] [range: 101–107]; 114 ± 5 [SD] [range: 110–119]; 193 ± 10 [SD] [range: 183-203]; 220 ± 16 [SD] [range: 210–238]) compared to ADMIRE 5 (44 ± 2 [SD] [range: 42–46]; 60 ± 2 [SD] [range: 57–61]; 66 ± 1 [SD] [range: 65–67]; 103 ± 4 [SD] [range: 98–106]; 110 ± 1 [SD] [range: 109–111]), respectively in each of the five CT protocols. ConclusionThis phantom study suggests that dose reduction and iterative reconstruction settings have an impact on detectability of pulmonary nodules by artificial intelligence software and we therefore encourage adaption of dose levels and reconstruction methods prior to widespread implementation of fully automatic nodule detection software for lung cancer screening purposes.

Effect of a new deep learning image reconstruction algorithm for abdominal computed tomography imaging on image quality and dose reduction compared with two iterative reconstruction algorithms: a phantom study.

New reconstruction algorithms based on deep learning have been developed to correct the image texture changes related to the use of iterative reconstruction algorithms. The purpose of this study was to evaluate the impact of a new deep learning image reconstruction [Advanced intelligent Clear-IQ Engine (AiCE)] algorithm on image-quality and dose reduction compared to a hybrid iterative reconstruction (AIDR 3D) algorithm and a model-based iterative reconstruction (FIRST) algorithm. Acquisitions were carried out using the ACR 464 phantom (and its body ring) at six dose levels (volume computed tomography dose index 15/10/7.5/5/2.5/1 mGy). Raw data were reconstructed using three levels (Mild/Standard/Strong) of AIDR 3D, of FIRST and AiCE. Noise-power-spectrum (NPS) and task-based transfer function (TTF) were computed. Detectability index was computed to model the detection of a small calcification (1.5-mm diameter and 500 HU) and a large mass in the liver (25-mm diameter and 120 HU). NPS peaks were lower with AiCE than with AIDR 3D (-41%±6% for all levels) or FIRST (-15%±6% for Strong level and -41%±11% for both other levels). The average NPS spatial frequency was lower with AICE than AIDR 3D (-9%±2% using Mild and -3%±2% using Strong) but higher than FIRST for Standard (6%±3%) and Strong (25%±3%) levels. For acrylic insert, values of TTF at 50 percent were higher with AICE than AIDR 3D and FIRST, except for Mild level (-6%±6% and -13%±3%, respectively). For bone insert, values of TTF at 50 percent were higher with AICE than AIDR 3D but lower than FIRST (-19%±14%). For both simulated lesions, detectability index values were higher with AICE than AIDR 3D and FIRST (except for Strong level and for the small feature; -21%±14%). Using the Standard level, dose could be reduced by -79% for the small calcification and -57% for the large mass using AICE compared to AIDR 3D. The new deep learning image reconstruction algorithm AiCE generates an image-quality with less noise and/or less smudged/smooth images and a higher detectability than the AIDR 3D or FIRST algorithms. The outcomes of our phantom study suggest a good potential of dose reduction using AiCE but it should be confirmed clinically in patients.

Influence Of Slice Thickness And Iterative Reconstruction On Coronary Artery Calcification Quantification

Introduction: Reconstruction parameters including iterative reconstruction algorithms and slice thickness influence the quantification of coronary calcification. Moreover, with modern scanner platforms and increasing use of iterative reconstruction algorithms, assessment of coronary calcium quantification will ultimately be affected. We sought to analyze the influence of slice thickness and iterative reconstruction on quantitative parameters of coronary calcification.

Intraindividual variation of dose parameters in oncologic CT imaging.

The objective of this study is to identify essential aspects influencing radiation dose in computed tomography [CT] of the chest, abdomen and pelvis by intraindividual comparison of imaging parameters and patient related factors. All patients receiving at least two consecutive CT examinations for tumor staging or follow-up within a period of 22 months were included in this retrospective study. Different CT dose estimates (computed tomography dose index [CTDIvol], dose length product [DLP], size-specific dose estimate [SSDE]) were correlated with patient's body mass index [BMI], scan length and technical parameters (tube current, tube voltage, pitch, noise level, level of iterative reconstruction). Repeated-measures-analysis was initiated with focus on response variables (CTDIvol, DLP, SSDE) and possible factors (age, BMI, noise, scan length, peak kilovoltage [kVp], tube current, pitch, adaptive statistical iterative reconstruction [ASIR]). A univariate-linear-mixed-model with repeated-measures-analysis followed by Bonferroni adjustments was used to find associations between CT imaging parameters, BMI and dose estimates followed by a subsequent multivariate-mixed-model with repeated-measures-analysis with Bonferroni adjustments for significant parameters. A p-value <0.05 was considered statistically significant. We found all dose estimates in all imaging regions were substantially affected by tube current. The iterative reconstruction significantly influenced all dose estimates in the thoracoabdominopelvic scans as well as DLP and SSDE in chest-CT. Pitch factor affected all dose parameters in the thoracoabdominopelvic CT group. These results provide further evidence that tube current has a pivotal role and potential in radiation dose management. The use of iterative reconstruction algorithms can substantially decrease radiation dose especially in thoracoabdominopelvic and chest-CT-scans. Pitch factor should be kept at a level of ≥1.0 in order to reduce radiation dose.

Given the growing awareness of and concern for potential carcinogenic effects of exposure of children to ionizing radiation at CT, optimizing acquisition parameters is crucial to achieve diagnostically acceptable image quality at the lowest possible radiation dose. Among currently available dose reduction techniques, recent technical innovations have allowed the implementation of low tube voltage scans and iterative reconstruction (IR) techniques into daily clinical practice for pediatric CT. The benefits of lowering tube voltage include a considerable reduction in radiation dose and improved contrast on images, especially when an iodinated contrast medium is used. The increase in noise, which is attributed to decreased photon penetration, is a major drawback but is not as severe as that at adult CT because of the small body size of children. In addition, use of IR algorithms can suppress increased noise, yielding wider applicability for low tube voltage scans. However, a careful implementation strategy and methodologic approach are necessary to maximize the potential for dose reduction while preserving diagnostic image quality under each clinical condition. The potential pitfalls of and topics related to these techniques include (a) the effect of tube voltage on the surface radiation dose, (b) the effect of window settings, (c) accentuation of metallic artifacts, (d) deterioration of low contrast detectability at low-dose settings, (e) interscanner variation of x-ray spectra, and (f) a comparison with the use of a spectral shaping technique. Appropriate use of low tube voltage and IR techniques is helpful for radiation dose reduction in most applications of pediatric CT. Evolution of Electrochemical Cell Designs for In-Situ and Operando 3D Characterization.

Lithium-based rechargeable batteries such as lithium-ion (Li-ion), lithium-sulfur (Li-S), and lithium-air (Li-air) cells typically consist of heterogenous porous electrodes. In recent years, there has been growing interest in the use of in-situ and operando micro-CT to capture their physical and chemical states in 3D. The development of in-situ electrochemical cells along with recent improvements in radiation sources have expanded the capabilities of micro-CT as a technique for longitudinal studies on operating mechanisms and degradation. In this paper, we present an overview of the capabilities of the current state of technology and demonstrate novel tomography cell designs we have developed to push the envelope of spatial and temporal resolution while maintaining good electrochemical performance. A bespoke PEEK in-situ cell was developed, which enabled imaging at a voxel resolution of ca. 230 nm and permitted the identification of sub-micron features within battery electrodes. To further improve the temporal resolution, future work will explore the use of iterative reconstruction algorithms, which require fewer angular projections for a comparable reconstruction.

Effects of Radiation Dose Reduction on Diagnostic Accuracy of Abdominal CT in Young Adults with Suspected Acute Diverticulitis: A Retrospective Intraindividual Analysis

To assess the effects of radiation dose reduction on image quality and diagnostic accuracy of abdominal computed tomography (CT) in young adults with suspected acute diverticulitis. Fifty-four patients ≤40 years who received contrast-enhanced abdominal CT for suspected acute diverticulitis were included. Low-dose CT (LDCT) datasets (25%, 50%, and 75% of the original dose) were generated using sinogram synthesis and quantum noise modeling. A five-point scale was used to assess images qualitatively (overall image quality, noise, artefacts, and sharpness) and for diagnostic confidence (5 being the best possible outcome). Furthermore, the diagnostic accuracy was determined for the presence of acute diverticulitis. Among 54 patients (mean age: 35.2 ± 5.3 years, 77.8% male), the prevalence of acute diverticulitis was high (57.4%). Subjective image quality was highest for original datasets and lowest for LDCT datasets with 25% of the original dose (median [interquartile range]: 5 [5] vs. 3 [2-3], p < 0.001). Diagnostic confidence was high for all datasets down to 50% of the original dose, while 25% LDCT datasets were associated with a significantly decreased diagnostic confidence (p < 0.001). Diagnostic accuracy was high for all LDCT and original datasets (sensitivity: 100%, negative predictive value [NPV]: 100% for 75% and 100% dose levels; sensitivity: 96.8%, NPV: 95.8% for 50% dose level; sensitivity: 93.6%, NPV: 91.7% for 25% dose level, respectively). Inter-rater agreement regarding the detection of diverticulitis was almost perfect at doses ≥50% (kappa: >0.81), while lower for datasets of 25% of the original radiation dose agreement (kappa: 0.67-0.78). Radiation dose reduction down to 50% of the original radiation exposure permits high image quality, diagnostic confidence, and accuracy for the assessment of acute diverticulitis in abdominal CT in young adults without the use of iterative reconstruction algorithms.

Reducing radiation dose and enhancing imaging quality of 4DCT for radiation therapy using iterative reconstruction algorithms

PurposeFour-dimensional computed tomography (CT) images are typically used to quantify the necessary internal target volumes for thoracic and abdominal tumors. However, 4-dimensional CT is typically associated with excessive imaging dose to patients and the situation is exacerbated when using repeat 4-dimensional CT imaging on a weekly or daily basis throughout fractionated therapy. The aim of this work is to evaluate an iterative reconstruction (IR) algorithm that helps reduce the imaging dose to the patient while maintaining imaging quality as quantified by point spread function and contrast-to-noise ratios (CNRs). Methods and materialsAn IR algorithm, SAFIRE, was applied to CT data of a phantom and patients with varying CT doses and reconstruction kernels. Phantom data enable measurements of spatial resolution, contrast, and noise. The impact of SAFIRE on 4-dimensional CT was assessed with patient data acquired at 2 different dose levels during image guided radiation therapy with an in-room CT. ResultsPhantom data demonstrate that IR reduces noise approximately in proportion to the number of iterations indicated by the strength (SAFIRE 1 to SAFIRE 5). Spatial resolution and contrast are conserved independent of dose and reconstruction parameters. The CNR increases with an increase of imaging dose or an increase in the number of iterations. The use of IR on CT sets confirms the results that were derived from phantom scans. The IR significantly enhances single breathing phase CTs in 4-dimensional CT sets as assessed by CT number discrimination. Furthermore, the IR of the low dose 4-dimensional CT features a 45% increase in the CNR in comparison with the standard dose 4-dimensional CT. ConclusionsThe use of IR algorithms reduces noise while preserving spatial resolution and contrast, as evaluated from both phantom and patient CT data sets. For 4-dimensional CT, the IR can significantly improve image quality and reduce imaging dose without compromising image quality.

A Survey of the Use of Iterative Reconstruction Algorithms in Electron Microscopy.

One of the key steps in Electron Microscopy is the tomographic reconstruction of a three-dimensional (3D) map of the specimen being studied from a set of two-dimensional (2D) projections acquired at the microscope. This tomographic reconstruction may be performed with different reconstruction algorithms that can be grouped into several large families: direct Fourier inversion methods, back-projection methods, Radon methods, or iterative algorithms. In this review, we focus on the latter family of algorithms, explaining the mathematical rationale behind the different algorithms in this family as they have been introduced in the field of Electron Microscopy. We cover their use in Single Particle Analysis (SPA) as well as in Electron Tomography (ET).

High-Resolution Computed Tomography Examinations for Chronic Suppurative Lung Disease in Early Childhood: Radiation Exposure and Image Quality Evaluations With Iterative Reconstruction Algorithm Use

PurposeHigh radiosensitivity of children undergoing repetitive computed tomography examinations necessitates the use of iterative reconstruction algorithms in order to achieve a significant radiation dose reduction. The goal of this study is to compare the iDose iterative reconstruction algorithm with filtered backprojection in terms of radiation exposure and image quality in 33 chest high-resolution computed tomography examinations performed in young children with chronic bronchitis. MethodsFourteen patients were scanned using the filtered backprojection protocol while 19 patients using the iDose protocol and reduced milliampere-seconds, both on a 64-detector row computed tomography scanner. The iDose group images were reconstructed with different iDose levels (2, 4, and 6). Radiation exposure quantities were estimated, while subjective and objective image qualities were evaluated. Unpaired t tests were used for data statistical analysis. ResultsThe iDose application allowed significant effective dose reduction (about 80%). Subjective image quality evaluation showed satisfactory results even with iDose level 2, whereas it approached excellent image with iDose level 6. Subjective image noise was comparable between the 2 groups with the use of iDose level 4, while objective noise was comparable between filtered backprojection and iterative reconstruction level 6 images. ConclusionsThe iDose algorithm use in pediatric chest high-resolution computed tomography reduces radiation exposure without compromising image quality. Further evaluation with iterative reconstruction algorithms is needed in order to establish high-resolution computed tomography as the gold standard low-dose method for children suffering from chronic lung diseases.

Implementation of the European multicentre database of healthy controls for [123I]FP-CIT SPECT increases diagnostic accuracy in patients with clinically uncertain parkinsonian syndromes

Even though [(123)I]FP-CIT SPECT provides high accuracy in detecting nigrostriatal cell loss in neurodegenerative parkinsonian syndromes (PS), some patients with an inconclusive diagnosis remain. We investigated whether the diagnostic accuracy in patients with clinically uncertain PS with previously inconclusive findings can be improved by the use of iterative reconstruction algorithms and an improved semiquantitative evaluation which additionally implemented a correction algorithm for patient age and gamma camera dependency (EARL-BRASS; Hermes Medical Solutions, Sweden). We identified 101 patients with inconclusive findings who underwent an [(123)I]FP-CIT SPECT between 2003 and 2010 as part of the diagnostic process of suspected PS at the University of Munich, and re-evaluated these scans using iterative reconstruction algorithms and the new corrected EARL-BRASS. Clinical follow-up was obtained in 62 out of the 101 patients and constituted the gold standard for the re-evaluation to assess the possible improvement in diagnostic accuracy. Clinical follow-up confirmed the diagnosis of PS in 11 of the 62 patients. In patients in whom both visual and semiquantitative analysis showed concordant findings (48 patients), a high negative predictive value (93%), positive predictive value (100%) and accuracy (94%) were found, and thus a correct diagnosis was obtained in 45 of the 48 patients. Among the 14 patients with discordant findings, the additional semiquantitative analysis correctly identified all five of nine patients patients without PS by nonpathological semiquantitative findings in visually pathological or inconclusive scans. In contrast, four of the remaining five patients with decreased semiquantitative values but visually normal scans did not show a PS during follow-up. The age-corrected and camera-corrected mode of evaluation using EARL-BRASS provided a notable improvement in the diagnostic accuracy of [(123)I]FP-CIT SPECT in PS patients with previously inconclusive findings. The gain in accuracy might be achieved by better discrimination between physiological low striatal [(123)I]FP-CIT binding due to age-related loss of the dopamine transporter or pathological loss of binding.

Survey of volume CT dose index in Japan in 2014.

The aims of this study are to propose a new set of Japanese diagnostic reference levels (DRLs) for 2014 and to study the impact of tube voltage and the type of reconstruction algorithm on patient doses. The volume CT dose index (CTDI(vol)) for adult and paediatric patients is assessed and compared with the results of a 2011 national survey and data from other countries. Scanning procedures for the head (non-helical and helical), chest and upper abdomen were examined for adults and 5-year-old children. A questionnaire concerning the following items was sent to 3000 facilities: tube voltage, use of reconstruction algorithms and displayed CTDI(vol). The mean CTDI(vol) values for paediatric examinations using voltages ranging from 80 to 100 kV were significantly lower than those for paediatric examinations using 120 kV. For adult examinations, the use of iterative reconstruction algorithms significantly reduced the mean CTDI(vol) values compared with the use of filtered back projection. Paediatric chest and abdominal scans showed slightly higher mean CTDI(vol) values in 2014 than in 2011. The proposed DRLs for adult head and abdominal scans were higher than those reported in other countries. The results imply that further optimization of CT examination protocols is required for adult head and abdominal scans as well as paediatric chest and abdominal scans. Low-tube-voltage CT may be useful for reducing radiation doses in paediatric patients. The mean CTDI(vol) values for paediatric scans showed little difference that could be attributed to the choice of reconstruction algorithm.

Coronary CT Angiography with Use of Iterative Reconstruction Algorithm in Coronary Stenting: A Systematic Review of Image Quality, Diagnostic Value and Radiation Dose

The aim of this study was to perform a systematic review of the image quality, diagnostic value and radiation dose of coronary CT angiography with use of iterative reconstruction (IR) in the assessment of coronary stents when compared to the standard filtered back-projection (FBP) techniques. A search of medical databases of English literature was performed to identify studies comparing coronary CT angiography with use of IR and FBP techniques in coronary stenting. Qualitative and quantitative assessment of image quality, diagnostic accuracy and radiation dose associated with coronary CT angiography were analyzed and compared between the two reconstruction algorithms. Ten studies met selection criteria and were included in the analysis. In comparison with FBP, coronary CT angiography with use of IR showed improvement in image quality through both qualitative and quantitative analysis. The IR technique resulted in higher diagnostic value than that of FBP with the mean sensitivity, specificity and accuracy being 98%, 77.4% and 80.1% for IR technique, and 90.5%, 68.2%, and 68.6% for FBP approach, respectively, with significant difference reached in the specificity and accuracy ( p< 0� 05). Furthermore, there was significantly lower dose with use of IR compared to FBP technique (4.4 mSv vs. 7.0 mSv). Coronary CT angiography with use of IR algorithm leads to significant improvements in the assessment of coronary stents with much lower radiation dose.

Variability and accuracy of coronary CT angiography including use of iterative reconstruction algorithms for plaque burden assessment as compared with intravascular ultrasound—an ex vivo study

To systematically assess inter-technique and inter-/intra-reader variability of coronary CT angiography (CTA) to measure plaque burden compared with intravascular ultrasound (IVUS) and to determine whether iterative reconstruction algorithms affect variability. IVUS and CTA data were acquired from nine human coronary arteries ex vivo. CT images were reconstructed using filtered back projection (FBPR) and iterative reconstruction algorithms: adaptive-statistical (ASIR) and model-based (MBIR). After co-registration of 284 cross-sections between IVUS and CTA, two readers manually delineated the cross-sectional plaque area in all images presented in random order. Average plaque burden by IVUS was 63.7 ± 10.7% and correlated significantly with all CTA measurements (r = 0.45-0.52; P < 0.001), while CTA overestimated the burden by 10 ± 10%. There were no significant differences among FBPR, ASIR and MBIR (P > 0.05). Increased overestimation was associated with smaller plaques, eccentricity and calcification (P < 0.001). Reproducibility of plaque burden by CTA and IVUS datasets was excellent with a low mean intra-/inter-reader variability of <1/<4% for CTA and <0.5/<1% for IVUS respectively (P < 0.05) with no significant difference between CT reconstruction algorithms (P > 0.05). In ex vivo coronary arteries, plaque burden by coronary CTA had extremely low inter-/intra-reader variability and correlated significantly with IVUS measurements. Accuracy as well as reader reliability were independent of CT image reconstruction algorithm. • IVUS is deemed the gold standard in-vivo coronary plaque assessment • But coronary CT angiography findings correlate strongly with IVUS results • Coronary CT angiography now allows plaque quantification close to IVUS • Iterative image reconstruction algorithms do not alter accuracy or reproducibility • Plaque quantification is more challenging in smaller eccentric calcified lesions.
