Intraoperative navigation system use increases accuracy of glenoid component inclination but not functional outcomes in reverse total shoulder arthroplasty: a prospective comparative study.

While the use of computer-assisted navigation systems in prosthetic implantation is steadily increasing, its utility in reverse shoulder arthroplasty (RSA) remains unclear. The purpose of this study was to evaluate the clinical utility of an intraoperative navigation system in patients undergoing RSA. Patients undergoing navigated or standard RSA at a single institution between September 2020 and December 2021 were prospectively enrolled. Exclusion criteria included noncompliance with study procedures or humeral fracture. Outcome measures included postoperative version and inclination, range of motion (ROM), complications, and patient-reported outcome measurements (PROMs: American Shoulder and Elbow Surgeons score [ASES], Disabilities of the Arm, Shoulder, and Hand score [DASH], Simple Shoulder Test [SST], and Visual Analog Scale [VAS]) at final follow-up. The final cohort contained 16 patients with navigation and 17 with standard RSA at a mean follow-up of 16months (range 12-18months). Average age was 72years (range 66-80years), 8 male (24%) and 25 female (76%). There were no differences in demographics between groups (p > 0.05). At baseline, the navigated group had a greater proportion of Walch B1 and B2 glenoids (p = 0.04). There were no differences between groups regarding baseplate type and native/planned/postoperative glenoid version and inclination. In both groups, planned and postoperative versions were not significantly different (p = 0.76). Patients who did not have navigation demonstrated significant differences between planned and postoperative inclination (p = 0.04), while those with navigation did not (p = 0.09). PROM scores did not differ between groups at final follow-up for SST (p = 0.64), DASH (p = 0.38), ASES (p = 0.77), or VAS (p = 0.1). No difference in final ROM was found between groups (p > 0.05). Over 50% of all screws in both groups were positioned outside the second cortex (p = 0.37), albeit with no complications. There were no statistically significant differences in ROM, PROMs, and satisfaction between patients receiving computer-navigated and standard RSA at a short-term follow-up. Despite more severe preoperative glenoid erosion in the navigated group, all patients were able to achieve an appropriate neutral axis postoperatively. The cost effectiveness and appropriate use of computer-navigated RSA warrant specific investigation in future studies. II, prospective cohort study. 9/1/2020 to 12/31/2021.

ENDOSCOPIC SURGERY OF THE PARANASAL SINUSES AND SKULL BASE NEOPLASMS WITH THE USE OF THE COMPUTER-ASSISTED NAVIGATION SYSTEM (CANS) IN PEDIATRIC PRACTICE

Backgroun d: In the last 10 years, the use of computer-assisted navigation systems (CANS) in endo-nasal endoscopic surgery of the paranasal sinuses and skull base in adults has been well studied. In pediatric practice, isolated cases of the use of this method have been described, with most of patients suffering from various types of sinusitis. Aim: To improve efficacy of the transnasal surgery for sinonasal, nasopharyngeal and skull base neoplasms in pediatric patients. Materials and methods: 77 patients aged from 22 days to 18 years with various nasal, nasopharyngeal, paranasal and skull base neoplasms were divided into two groups depending on the surgical technique. The main group included 40 patients in whom CANS guided surgery was used and the control group comprised 37 patients operated without CANS. Each group was divided into two subgroups based upon the complexity of the intervention (in subgroup 1, it was a biopsy and in subgroup 2, tumor excision). Results: There were no significant differences between 1st subgroups of both groups in the intervention duration, intra-operative blood loss, number of post-operative days in the hospital. Compared to the patients from the subgroup 2 of the control group, in those from the subgroup 2 of the main group the duration of intervention decreased by 60 minutes (p = 0.038, Mann-Whitney U-test), intra-operative blood loss decreased by half (p = 0.044, Mann-Whitney U-test), duration of hospital stay, by 2 days (p = 0.02, Student's t-test). The rates of radical excision was higher in the main group (p = 0.02, Fischer's exact test). Conclusion: The use of CANS in endonasal surgery allows for reduction of the intervention time and anesthesia duration, minimizes blood loss, is associated with earlier post-operative rehabilitation of children.

Navigation-assisted surgery for tumors of the spine

Computer-assisted navigation systems are largely used for pedicle screws positioning in degenerative and traumatic spine surgery. In oncologic spine surgery its use is still developing and could be extended for tumor identification and excision. Aim of this paper is to present our experience. Seven selected patients (5 females, 2 males), mean age 44 years (min 17-max 62) affected by primary benign or malignant tumors of the spine or spine metastases were surgically treated with the use of computer-assisted navigation system from March to October 2011. At 18 months mean F.U. (min 15-max 23), no LR were observed. Revision surgery was necessary only in one case for C1 pedicle screw malpositioning. Navigation system can improve surgical accuracy in screws placement and tumor localization and excision. Learning curve and technical aspects must be considered to avoid potential serious mistakes.

Use of computer-assisted navigation system in reconstruction of unilateral old zygomatic complex fracture

Objective To investigate whether the surgery resorting to computer-assisted navigation system (CANS) can enhance effects in managing unilateral old zygomatic complex (ZMC) fracture in comparison with conventional surgery.Methods Twenty-four patients with unilateral old ZMC fracture for which surgical correction was wanted were divided into study group (n =12,treated with navigational surgery) and control group (n =12,treated with traditional surgery) in the 1 ∶ 1 matched case-control study.Therapeutic effects of CANS were evaluated by postoperative CT measurement,subjective satisfaction questionnaire and clinical examination.Results According to postoperative three-dimensional CT measurement,the mean projection and width differentials of bilateral zygomatic bones were 1.58 mm and 1.47 mm in study group,3.58 mm and 3.63 mm in control group,with significant differences between groups.No complications occurred within three months of follow-up.Moreover,the patients in study group were all satisfied with the treatment,but two patients in control group desired further surgery to ameliorate zygomatic shape.Conclusion Surgery with application of CANS improves efficacy of treatment for unilateral old ZMC fracture. Patellofemoral Instability after Total Knee Arthroplasty

Despite advances in surgical technique and implant design, complications involving the extensor mechanism and patellofemoral joint after total knee arthroplasty (TKA) continue to be the most common cause of pain and the most commonly cited reason for revision TKA surgery. A thorough understanding of the etiologies of patellofemoral instability, careful preoperative planning, and meticulous surgical techniques will optimize clinical outcome. Evaluation of patellofemoral stability should begin in the operating room. Postoperatively, thorough history, physical examination, and dedicated radiographic studies should be obtained. Computed tomography scan is the most accurate and reliable way to assess component positioning. Treatment of patellofemoral instability is directed by its etiology. Revision of one or both components is indicated if malpositioning is present. If the components are determined to be in satisfactory positions, soft tissue procedures can be pursued. Future advancements in prosthetic design and the routine use of computer-assisted navigation systems will minimize patellofemoral instability. Therapeutic study, Level V (expert opinion). Because spinal fractures are so common, especially at the thoracolumbar junction, we thought it worthwhile to perform a statistical workup relating to this region and to prepare a historical outline of their treatment. The development of the surgical treatments by the dorsal approach still used today and of the ventral approaches is traced from the therapeutic nihilism that held sway from 3000 to 100 b.c. through attempts directed exclusively at the correction of gibbus (100 b.c. up to the eighteenth century), through the initial attempts at surgical treatment in the seventeenth to nineteenth centuries and the brief period from 1900 to 1930 when fractures were treated with bedrest and the discussion between Boehler and Magnus. Guttmann’s publications deserve a special mention in connection with the treatment of transverse spinal cord syndrome. The classification system devised by Magerl et al. improved our understanding of spinal syndromes and also the treatment outcomes. The use of computer-assisted navigation systems and the availability of materials for the replacement of vertebral bodies are further new developments. In the future, however, treatment methods must be measured against the long-term results. In deciding on a particular procedure the trauma surgeon must critically review his or her own experiences and those in the literature and take account of these.

Neue Möglichkeiten in der computergestützten Orbitachirurgie

Surgical planning and tumor margins from preoperative image data sets can be transferred to the surgical site by use of computer-assisted navigation systems. Usually registration markers are placed prior to image data acquisition in order to correlate the patient's preoperative image data set with his/her position in the operating room. Pointers are commonly used for navigation, and the position of the pointer within the surgical site is compared to its position within the image data set. With a new navigation system, patient registration can be performed without registration markers. Tumor margins and osteotomy lines can be projected onto the surgical site. With the new navigation system, which has been developed by the Sonderforschungsbereich 414 of the Deutsche Forschungsgemeinschaft, markerless patient registration is performed using the contours of the patient's face. A scanner is mounted above the operating table, which detects the surface of the surgical site intraoperatively. Osteotomy lines and tumor margins are projected onto this surface using an intense beamer. The procedure is illustrated for a patient with a meningeal tumor of the orbit. Patients can be registered with an accuracy of 1 mm. Pointers are not required to transfer the data on the surgical site. Furthermore, monitors are no longer required, since the information is projected directly on the patient's surface. Surgical planning and tumor margins can exactly be visualized within the surgical site. Ergonomics are especially improved for navigated orbitotomies via a coronal approach.
