Urethral meatal stenosis in female children simulating bladder-neck obstruction

In some instances, distal urethral stenosis in female children may be associated withthe roentgenologic appearance of a prominent, collarlike vesical neck, as a reflection of generalized detrusor hypertrophy. This ring has often been confused with bladder-neck obstruction. Clinical response to urethral dilatation and subsequent regression of abnormal roentgenologic findings confirms the importance of distal urethral stenosis as a cause of bladder-neck hypertrophy.

Dr. Arnold, in his discussion of X-Ray Seminar No. 30, makes some valid comments and draws his own conclusions on points considered controversial. It is not intended to imply that residual in the bladder is necessary for a diagnosis of lower urinary tract obstruction, although it represents an important finding in many cases. A minor degree of lower urinary tract obstruction of varying etiology is a controversial subject with regard to diagnosis and treatment, and no unanimous opinion exists. The urethra was excluded as a possible source of obstruction since the cystourethrogram was normal, and no urethral meatal stenosis was found. The urethral meatus can be evaluated by the bougie à boule. This is usually done during the course of a routine physical examination of every patient with a genitourinary problem. On cystography the urethra is dilated proximal to the stenotic urethral meatus. We do not attribute the dilated ureters
