An Unusual Foreign body in Esophagus; Report of a Case

Foreign body (FB) ingestion, a common and serious problem in children, can present with a wide variety of symptoms. This case report describes an unusual esophageal foreign body (EFB), in which the patient presented with dysphagia, odynophagia, vomiting, and regurgitation with history of ingesting her mother’s pin. Plain radiograph of the soft tissue neck revealed the presence of a radio-opaque ingested foreign body (the pin) that located at the lower level of cricopharynx. We performed rigid esophagoscopy under general anesthesia. On esophagoscopy the pin was visualized in lower border of the cricopharynx trapped by the cricopharyngeal sphincter at its base. It was hard metallic in consistency and of approximately 2 cm in size

Management of an Unusual Intranasal Foreign Body Abutting the Cribriform Plate: A Case Report and Review of Literature.

A 35-year-old man with history of schizophrenia presented 3 weeks after placing a screw in his right nostril. Initial imaging showed a screw in the right ethmoid sinus with the tip penetrating the right cribriform plate. On exam, the patient was hemodynamically stable with purulent drainage in the right nasal cavity but no visible foreign body. While most nasal foreign bodies occur in children and are generally removed at the bedside, intranasal foreign bodies in adults tend to require further assessment. The foreign body in this case was concerning for skull base involvement and the patient was brought to the operating room (OR) with neurosurgery for endoscopic sinus surgery (ESS) and removal of foreign body. The screw was removed and the patient recovered with no signs of cerebrospinal fluid (CSF) leak postoperatively. Any concern for skull base or intracranial involvement should call for a full evaluation of the mechanism of injury and intervention in a controlled environment.

Unusual intravesical foreign body management: the Casablanca experience

Self-insertion of foreign bodies in the urethra is most commonly associated with sexual or erotic arousal of adolescents with mental health disorders. It may rarely be practiced by healthy adults for masturbation. A diagnosis is usually made radiologically. Iatrogenic foreign bodies were found to be the most frequent type of insertion encountered. Endoscopic retrieval is usually successful, with minimal morbidity. The aim of this paper was to expose our experience about intravesical foreign bodies. The management of 4 cases with intravesical foreign body is exposed and analyzed.

Rare upper esophageal foreign body and a simple trick to remove it

This is a case report of a 7 year old male child who presented with an unusual foreign body a glass marble in upper esophagus. The child was a case of global developmental delay, seizure disorder, patent ductus arteriosus, congenital hydrocephalus with VP shunt in situ. The case was a challenging one to the entire team of surgeons, anaesthesiologist and paediatrician. Under general anesthesia the foreign body was removed using a foley’s catheter. Spherical foreign bodies like glass marbles are always challenging as the usual instruments cannot grasp such foreign bodies and there is always a chance of pushing the foreign body downwards during the attempt to remove. In this case foley’s catheter was used to retrieve the foreign body which is cheap, easily available and non-traumatic

Unusual foreign body related ultrasound features of breast

Purpose to show the variety of imaging findings of unusual foreign body related Ultrasound features of Breast. Materials & Methods We retrospectively collected the data and analyzed Ultrasound features and Mammography findings. We correlated Ultrasound images with surgical specimen and histo-pathological findings. Results We collected total seven cases of unusual foreign body related breast lesions. Age range was from 38 years to 72 years. Two cases of Sparganosis and two cases of implant rupture, one case of silicone granuloma, one case of oriental herbal metallic component mimicking microcalcification in fiboglandular tissue and one case of charcoal granuloma of breast. The parasite, Sparganosis related breast lesion is very rare. But the feature is very characteristic. Tubular, tortuous hypoechoic lesion is very unique sign of Sparganosis. Silicone granuloma shows hyperechoic mass with fine internal echoes as snowstorm appearnace. Oriental herbal metallic component is mimicking microcalcification. Medical history of breast abscess with treatment using oriental herbal attachment, 25-45 years ago is very useful for diagnosis. The reason hyperdense dots like microcalcification in mammography is that the oriental herb contains Ag++ component. Charcoal granuloma is difficult to be diagnosed pre-operatively. So medical history which got tattooing on breast for operation is very helpful. Increased echogenic nodular lesion involving premammarry layer is seen in Charcoal granuloma. The history of breast augmentation can be given much information to be diagnosed pre-operatively. We may search for the silicone related lesion in the breast. Internal echoes within fluid collected bag is seen in ruptured implant case. Conclusion the Knowledge of various image features of unusual foreign body related pathologic breast lesions can be very useful for accurate diagnosis and guiding treatment.

An unusual foreign body aspiration requiring an unusual retrieval technique

Foreign body aspiration during medical procedures has been reported in the literature. These iatrogenic incidents could be related to instruments malfunction or to accidental occurrences during medical treatment. In this paper, we present a report of a woman coming for a laparoscopic abdominal hysterectomy who developed intraoperative bronchospasm. In an attempt to administer aerosolized albuterol, the resident anesthesia provider fractured the Luer-lock tip of the 60-cc syringe, which he was using to hold the albuterol nebulizer. The plastic tip was dislodged into the endotracheal tube (ETT). On further inspection with a fiberoptic instrument the plastic tip was located loosely adherent to the distal part of the ETT and was held in place by the moisture, which had precipitated in the distal tube. An intraoperative consult with interventional pulmonary medicine was obtained after unsuccessfully attempting to retrieve the foreign body with a grasper. The syringe tip was then removed using a Fogarty balloon catheter that was threaded through the hole of the plastic tip.

Fir branch as rare cause of aspiration pneumonia in adults

The article represents a clinical observation describing the case of mechanical obstruction in the respiratory passages with an unusual foreign body, a fir branch. The diagnostic problem lied in the fact that a patient did not notice the fact of aspiration, the respiratory symptoms were non-specific, and the foreign body was radiotransparent. A detailed case history allowed the detection and endoscopic confirmation of the aspiration of a foreign body, a fir branch, and the consideration of the community acquired pneumonia and secondary aspiration pneumonia that required specialized medical care by the thoracic surgery department.

An Interesting Case of Penetrating Brain Injury by a Self-Hammered Nail

AbstractPenetrating brain injuries by nails are rare. These usually have been reported as work-related accidents such as from falls or nail gun misfires and in cases of assault. The authors present a case of a 38-year-old psychiatric patient who attempted suicide by self-impalement of nail in the vertex near the midline of the head. Surprisingly, he was asymptomatic after a brief loss of consciousness, and with a Glasgow Coma Scale of 15. There was no history of vomiting or seizures. Intraoperative and postoperative periods were uneventful. Many unusual intracranial foreign bodies have been reported in medical literature, but one of a self-inflicted intracranial injury by hammering a nail is rare. In this article, the authors describe their case report and discuss the challenges encountered in the management of a patient with intracranial injuries by nail, along with a brief review of literature.

Use of an image intensifier for the localization and removal of a foreign body in the lower lip

Foreign bodies can be found in the soft tissues of the face and in the maxillary bones in routine radiographic examinations. In the majority they do not present symptomatology. The removal of foreign bodies without the accurate location can cause severe complications. We present a case of 44-year-old male complaining an unusual foreign body in the lower lip region. Radiography did not reveal the exact location, and did not let a resolution at first time. Subsequently, the surgical treatment was effective with the aid of navigation-assisted fluoroscopy that showed the precise location of the foreign body for removal and avoided more discomfort to the patient.Descriptors: Foreign Bodies; Fluoroscopy; Traumatology.

Unusual foreign bodies of the airway: a case series regarding their management

<p>Foreign bodies in the bronchi are a common problem seen by ENT surgeons. Bronchial foreign bodies are common in children because they have difficulty in swallowing hard foodstuffs such as nuts and have an inadequately developed protective respiratory reflexes, which makes them vulnerable to inhalation of foreign bodies This is a case series of four unusual cases of foreign body airway, that presented to our ENT OPD, after taking detailed history and clinical examination, these patients were subjected to radiological studies such as chest X-rays, X-ray soft tissue lateral view, virtual bronchoscopy and CT chest to confirm the foreign body, after taking informed consent rigid bronchoscopy was performed and foreign body extracted.</p>

Multidisciplinary surgical management of an unusual penetrating foreign body of the face

IntroductionFacial injuries by penetrating foreign body are unusual and require specific multidisciplinary surgical management. Case reportThis case report concerns a 20-year-old man who experienced a penetrating injury by a piece of wood to the face and describes the surgical approach to remove the wood and repair the injury. The foreign body had penetrated the infratemporal fossa, with an entry wound situated below the right eye and an exit wound in the neck, in contact with the left internal carotid artery. An adapted surgical strategy was necessary in view of the site of the foreign body. The internal carotid artery was controlled in order to follow the foreign body as far as its entry into the base of the skull. The proximity of the eye and carotid and jugular vessels and the deep penetration of the foreign body required the participation of interventional radiologists, head and neck and vascular surgeons and ophthalmologists. DiscussionThe site of the foreign body, precisely determined preoperatively, justified management by a multidisciplinary team to ensure rapid extraction, while limiting the risk of additional lesions. With a follow-up of 6 months, the patient did not present any sequelae of his facial injury.

Unusual Foreign Bodies in the Lower Genitourinary Tract

Foreign body in the lower genitourinary tract is a relatively rare occurrence. A variety of foreign bodies have been reported in the literature. Commonly reported condition under which foreign bodies are introduced into genitourinary tract is autoeroticism. The highest incidence occurs in homosexuals, lesbians and masochism. But self-introduction of foreign body in the vagina in a 5-year-old girl is an extremely rare occurrence. Tracheobronchial foreign bodies have never been so strange!

In the present study, we aimed to present our experience about retrieval of foreign bodies over a 28-year period. We retrospectively analyzed the files of 22 patients (18 males, 4 females; mean age 34.9 years; range, 9 months to 80 years) who required removal of a foreign body from the tracheobronchial tree between April 1987 and December 2015. A total of 72.7% of the study group were 10 years old or older. There was no history of aspiration in 37% of cases, most often in older patients. Of the 22 unusual foreign bodies, seven (31.8%) were aspirated through permanent tracheostomy. The strangest foreign bodies were grass inflorescences, an acacia thorn, and construction nail. The foreign bodies were removed by rigid bronchoscopy in 18 patients, while thoracotomy was performed in two patients, and pericardiotomy in one patient. No intervention was required in one patient. The elderly and patients with tracheostomies may aspirate unusual foreign bodies. Even if there is no history of aspiration, the differential diagnosis of c ough or dyspnea should include foreign body aspiration.

An unusual foreign body in a cat: a case report

Foreign bodies that affect airways are a well-recognised problem in small-animal practice. Cats in par­ticular, may be especially at risk of aspiration due to their predatory behaviour. Here, we report a case of an insect as a foreign body, namely a beetle in the tracheal bifurcation of a cat. Radiographs suggested a wide range of possibilities for differential diagnosis, but the presence of a foreign body was initially suspected. A final diagnosis was not pursued by the owner and the cat was euthanised. The presence of a summer chafer was then confirmed. To the authors’ knowledge, this is the first time such a foreign body and radiographic findings are reported in the literature.

An Unusual Airway Foreign Body: A Cockroach in the Trachea

We present a case of airway obstruction by an unusual foreign body in a 10 month old male. The child was brought intubated to the emergency room after he was found unconscious and blue by his parents. His older sibling reported a choking episode after he placed a black object in his mouth. Chest x-ray on arrival demonstrated right-sided air trapping but no definite radio-opaque foreign body. Due to difficulty with ventilation and post-obstructive pulmonary edema on initial bronchoscopy, emergent extracorporeal membrane oxygenation (ECMO) was initiated to help stabilize the patient. Subsequent bronchoscopy demonstrated near total obstruction of the mid-trachea by an insect-like object. The object removed appeared to be a common American cockroach. Airway foreign body retrieval is a common part of otolaryngology practice. Most of the time, these foreign bodies are food materials or other inanimate objects. Finding a cockroach in the airway is incredibly rare and only three cases have been reported in the literature. This case also illustrates that ECMO may be considered in a patient too unstable to tolerate temporary withdrawal of ventilatory support for endoscopy. Additionally, rapid diagnosis and treatment is important when caring for a child with suspected foreign body aspiration.
