The aim of this study is to report an unusual case of a variant of odontogenic fibroma with the presence of a central giant cells lesion. A 51-year-old man presented with a nodular and ulcerated lesion in the lower left alveolar ridge, with a firm consistency and bleeding. The adjacent teeth were mobile. Evolution time was 6 months. A panoramic radiograph showed a radiolucent, unilocular area, with irregular margins and radicular resorption. An incisional biopsy was initially performed due to the possibility of malignancy, but the histopathologic diagnosis was a central lesion of giant cells. Excision of the lesion was performed, and microscopic examination of the surgical specimen showed areas of odontogenic epithelium islands associated with a fibrous stroma, in addition to the multinucleated giant cells. The patient is being followed up for 1 year, with no sign of recurrence.
