Complete transposition of mandibular lateral incisor–canine (Mn.I2.C): A rare clinical case

A permanent tooth that grows and emerges from the same location of the dental arch as the other permanent teeth is a rare event. Unilateral transposition has been reported more often than bilateral transposition, with the left side of the body being affected more frequently than the right. With a frequency rate of 0.03 percent, the Mn.I2.C transposition is the rarest of all transpositions. In this unusual case report on a mandibular lateral incisor-canine transposition, dento-alveolar concerns such as arch length disparities and tooth material differences are addressed (Mn.I2.C).

Treatment of maxillary lateral incisor-canine pseudotransposition: A case report

The term transposition is used to refer to an interchange in the position of 2 adjacent teeth within the same quadrant of the dental arch.In complete transposition, both the crowns and the entire root structures of the involved teeth are found parallel in their transposed positions. In incomplete transposition (also called “pseudo” or “partial” transposition) the crowns may be transposed while the root apices remain in their normal positions. Unilateral transposition has been reported more often than bilateral transposition, with the left side being somewhat more frequently involved than the right side. The only dental anomaly that has an apparent association with maxillary canine-lateral incisor transposition is increased third molar agenesis.Treatment of maxillary canine-lateral incisor transposition depends on many factors like the condition of the root of central incisors, shape of lateral incisors, need of extraction because of either severe crowding or profile improvement, etc. Here, we are going to present the management of a rare case report of maxillary right lateral incisor-canine pseudotransposition with minimum available space and agenesis of mandibular third molars.

Canine-premolar Transposition, Family Pedigree and Related Dental Anomalies

Maxillary canine-premolar transposition is a rare and complicated dental anomaly that needs specialconsideration by the orthodontist. The aim of the present study was to investigate the clinical features ofmaxillary canine-premolar transposition and report the family pedigree to determine the mode of inheritance.A cross-sectional study was performed on 39 patients having maxillary canine-premolar transposition inwhich both clinical and radiographic examination were performed. Moreover, 112 family members wereclinically examined looking for canine transposition. Variables such as location, sex and the presence ofhypodontia and peg-shaped lateral incisors were investigated. The study found that unilateral transposition(89.7%) was more common than bilateral occurrence (10.2%) affecting the left side (64.1%) more than theright side (35.9%). Females were affected more than males (74.4% and 25.6% respectively). Congenitallymissing teeth were reported in 12.8% and lower second premolar was the most commonly missing tooth.Peg-shaped maxillary lateral incisor was reported in 7.7% of the sample. Family pedigree confirmed thepresence of a history of transposition or ectopically positioned canine in 15.3% of the sample suggestingan autosomal dominant inheritance of the trait. In conclusion this study suggested an association betweengenetic factors and maxillary canine-premolar transposition. Further studies are required using genetictesting to confirm the findings of the present study.

BREXIT AND PRIVATE INTERNATIONAL LAW: LOOKING FORWARD FROM THE UK BUT ACTUALLY GOING BACKWARD

International judicial cooperation for civil matters between Member States of the EU and the United Kingdom can be severely curtailed as a consequence of Brexit if upon the end of the transitory period, 31 December 2020, no new partnership between the UK and the UE has been approved including agreements on civil judicial cooperation. Other multilateral solutions are possible within the framework of The Hague Conference on Private International Law, such as the accession by the UK in its own right to The Hague Convention 2005 on choice of court agreements or the ratification of The Hague Convention 2007 on child support, or via the UK’s accession to the Lugano II Convention 2007. A unilateral approach (modified unilateral transposition) is also possible with regard to EU legislation retained by the European Union (Withdrawal) Act 2018 determining in each case what piece of the EU legislation is effectively retained, modified or dismissed (pick and choose). In any case, the future outlook of civil judicial cooperation with the UK seems more like a leap back into the past than a step forward into the future. The situation for Gibraltar could be different if a new partnership agreement between the EU and UK includes a separate agreement in relation to Gibraltar where a sufficient repertory of EU cooperation instruments is bilaterally kept in force in order to safeguard the application of EU Private International Law applicable in Gibraltar to date.

Tooth transposition: two case-reports and literature review

Tooth transposition is a rare developmental disorder in which a normal tooth erupts in an inappropriate position. The change in position occurs between two adjacent teeth within the same quadrant. In this article, two cases of tooth transposition involving mandibular molars are described. Patient 1 presented with bilateral transposition between first and second mandibular molars. In addition, giroversion of the first molars was observed. Patient 2 presented with unilateral transposition between the mandibular left molars. Because this tooth disorder does not affect quality of life, both patients declined to seek dental treatment. Out of the 120 cases of tooth transposition found in the literature, 92,5% occurred in the maxilla and 15% were bilateral, 65,8% percent of these cases involved the canine and first premolar; 26.6% involved the canine and lateral incisor; 2,5% involved the canine and central incisor; and only 0.85% involved the first and second molar. The majority of tooth transposition cases occurred in women (66,6%). In literature, tooth transposition is a poorly-documented condition. Consequently, this report seeks to contribute to the existing information in the literature on this disorder.

Unilateral and bilateral dental transpositions in the maxilla-dental and skeletal findings in 63 individuals.

This was to elucidate dental and skeletal findings in individuals with unilateral and bilateral maxillary dental transpositions. The sample comprised of radiographic materials from 63 individuals with maxillary dental transpositions from the Departments of Odontology at the Universities of Copenhagen and Aarhus and by the Danish municipal orthodontic service. The cases were divided into three groups: unilateral transposition of the canine and first premolar (Type 1U), bilateral transposition of canine and first premolar (Type 1B), and unilateral transposition of canine and lateral incisor (Type 2). The dentitions were analysed regarding agenesis and dental morphological anomalies on panoramic radiographs, and craniofacial aspects were cephalometrically analysed on profile images The results were statistically evaluated. All groups demonstrated increased occurrences of agenesis (Type 1U and Type 1B: 31 agenesis in 15 patients; and Type 2 three agenesis in three patients). Taurodontic root morphology was most dominant in Type 1U. Peg-shaped lateral incisors showed an increased occurrence, though not in Type 1U. Skeletally, Type 1B and Type 1U demonstrated maxillary retrognathia (more pronounced in Type 1B). Type 2 showed a significant posterior inclination of the maxilla. Transpositions of maxillary canines involve dental and skeletal deviations. Dental deviations were predominantly taurodontic root morphology and agenesis. Regarding skeletal deviations, bilateral transpositions of the canines and the first premolars are associated with skeletal changes. Unilateral transpositions are possibly a localised deviation with minor or no skeletal involvements. The results indicate a possible difference in the aetiologies of unilateral and bilateral transpositions.

Modified semitendinosus muscle transposition to repair ventral perineal hernia in 14 dogs.

To describe a modified technique of semitendinosus muscle transposition for the repair of ventral perineal hernia. Retrospective review of case records of dogs with ventral perineal hernia that were treated by transposing the medial half of the longitudinally split semitendinosus muscle of one limb. The transposition of the internal obturator muscle was used when uni- or bilateral rectal sacculation was also present in addition to ventral perineal hernia; colopexy and vas deferens pexy were also performed. Fourteen dogs were included. In addition to ventral perineal hernia, unilateral and bilateral perineal hernia was also present in five and six of the dogs, respectively. The mean follow-up time was 890 days. Ventral perineal hernia was successfully managed by the modified semitendinosus muscle transposition with minor complications in all the dogs included in the study. Despite the small number of dogs included, the unilateral transposition of the medial half of the longitudinally split semitendinosus muscle consistently supported the ventral rectal enlargement in perineal hernia without obvious adverse effects.

Case of unilateral transposition of a maxillary lateral incisor and canine treated without extraction

In this article, we report the successful treatment of a patient, aged 11 years 5 months, who presented for treatment with a chief complaint of lack of eruption of his maxillary left canine. Panoramic radiography and computed tomography revealed complete transposition of the maxillary left lateral incisor and canine. The patient was treated with surgical exposure and traction of the impacted canine to an appropriate location, followed by nonextractive treatment with full resolution of the transposition of the lateral incisor and canine. The diagnosis, planning, appliance design and treatment sequences are described. Alternative treatments, including their advantages and disadvantages, are also discussed.

Transposition: an orthodontist's dilemma—a case report

Tooth transposition presents a major challenge in the correction of a malocclusion. A dental transposition is an uncommon disturbance affecting 0.4% of the population. The treatment difficulties make this anomaly an occurrence of orthodontic interest and a source of clinical interest. The treatment requires a great deal of control and carefully applied mechanics. This case report demonstrates the orthodontic management of the unilateral transposition of a maxillary canine with a premolar and missing bilateral incisors that was treated orthodontically without reversing the transposed tooth positions. The case required combined orthodontic and prosthodontic intervention to achieve acceptable morphologic and esthetic occlusion.

Transposition of a maxillary canine and a lateral incisor and use of cone-beam computed tomography for treatment planning

This report describes the orthodontic treatment of a 12-year-old girl with transposition of the maxillary left canine and the lateral incisor. Cone-beam computed tomography was used during treatment planning. The transposed tooth positions were corrected with an unconventional orthodontic approach. Treatment alternatives and their clinical concerns are presented.

Mixed unilateral transposition of a maxillary canine, central incisor, and lateral incisor

A girl, aged 7 years 11 months, came for treatment with the chief complaint of lack of eruption of her maxillary right central and lateral permanent incisors. The radiographic analysis indicated complete transposition of the maxillary right lateral incisor and canine, and a pseudotransposition of the maxillary right central and lateral incisors. This is a clinical condition of mixed transposition in the same hemi-arcade: the combination of a complete transposition and another incomplete or coronary transposition-a situation that has not been reported in the literature. The patient was treated with surgical exposure and ligation of the 3 teeth, the eruption was properly guided, and the correct order of the 3 teeth was restored in the arch. The diagnosis, appliance design, and treatment sequence are described.

Maxillary Canine—First Premolar Transposition

Tooth transpositions present at a relatively low incidence in the world population and primarily affect maxillary canines and premolars. Treatment of this disturbance should take into account aspects such as facial pattern, age, malocclusion, tooth-size discrepancy, stage of eruption, and magnitude of the transposition. Mechanics for correction should be entirely individualized, reducing the risks and adverse effects. Practitioners often select simpler options, indicating extraction of permanent teeth, which is an irreversible procedure that may bring about damages to the patient. This study presents a case report and treatment of unilateral transposition of maxillary canine and premolar with repositioning of affected teeth to their respective normal positions.

Dental transposition as a disorder of genetic origin

A sample of 85 dental transpositions in 75 subjects (27 male, 48 female; mean age at diagnosis 12.25 years) involving both maxillary and mandibular arches was analysed using dental panoramic radiographs and clinical records. Transposition affected the maxillary dentition (76 per cent) more frequently than the mandibular dentition (24 per cent). Unilateral transposition accounted for 88 per cent of cases, with the maxilla being involved more commonly than the mandible. Overall, the most common transposition involved the maxillary canine and first premolar (58 per cent). Considering the jaws in isolation, the canine and first premolar were the most commonly affected teeth in the maxilla (83 per cent) whilst in the mandible, the canine and lateral incisor teeth were most commonly transposed (73 per cent). No significant difference in symmetrical distribution of the unilateral transposition sample occurred. There was evidence of associated hypodontia in 41 per cent of the sample; however, if third molars were excluded, this figure decreased to 25 per cent. Peg-shaped maxillary lateral incisors were judged to be present in 27 per cent of subjects, whilst 41 per cent had retained primary teeth; all of these, except one, were primary canines. Overall, the majority of the sample (76 per cent) demonstrated at least one of the dental anomalies under investigation. Multivariate analysis showed associations between unilateral transposition, gender, and the presence of peg-shaped maxillary lateral incisors; whilst bilateral transposition was more closely associated with gender and the presence of retained primary teeth. There was a poor association between both unilateral and bilateral transposition and hypodontia. Together, these results suggest a mutifactorial aetiology to this disorder, with both genetic and environmental factors playing an important role.

Nerve transposition and implant placement in the atrophic posterior mandibular alveolar ridge

The results obtained with a modified surgical technique for transposition of the inferior alveolar nerve followed by immediate placement of endosseous implants in mandibles with moderate to severe atrophy are presented. Ten transpositions of the inferior alveolar nerve together with the installation of 21 implants were performed in six patients. The mean postoperative follow-up time was 23 months, with a range of 12 to 46 months. All implants with functioning pontics remained stable, with no mobility nor signs or symptoms of pain and infection during the follow-up period. Postoperative radiographic evaluation disclosed no pathologic bone loss around the implants. Neurosensory evaluation was performed using the two-point discrimination test. One patient with unilateral transposition had objective neurosensory dysfunction at 12 months postoperative, although all the nerve function were reported as normal by the patients. Strict patient selection criteria are necessary, with full awareness by the patient of the possibility of long-term or even permanent nerve paresthesia, when this procedure is contem-plated.

Canine transposition

In a study of forty cases of maxillary canine transposition, it was found that unilateral transposition of maxillary canines with premolars on the left side occurred more frequently, transposition was not necessarily complete, and in the absence of pressure from the permanent tooth germ the deciduous tooth was overretained in the oral cavity.
